Background: Chronic subscapularis tendon tear (SBT) is a degenerative disease and a common pathologic cause of shoulder pain. Several potential risk factors for chronic SBT have been reported. Although metabolic abnormalities are common risk factors for degenerative disease, their potential etiological roles in chronic SBT remains unclear. The purpose of this study was to investigate potential risk factors for chronic SBT, with particular attention to metabolic factors. Methods: This study evaluated single shoulders of 939 rural residents. Each subject undertook a questionnaire, physical examinations, blood tests, and simple radiographs and magnetic resonance imaging (MRI) evaluations of bilateral shoulders. Subscapularis tendon integrity was determined by MRI findings based on the thickness of the involved tendons. The association strengths of demographic, physical, social, and radiologic factors, comorbidities, severity of rotator cuff tear (RCT), and serologic parameters for SBT were evaluated using logistic regression analyses. The significance of those analyses was set at p<0.05. Results: The prevalence of SBT was 32.2% (302/939). The prevalence of partial- and full-thickness tears was 23.5% (221/939) and 8.6% (81/939), respectively. The prevalence of isolated SBT was 20.2% (190/939), SBT combined with supraspinatus or infraspinatus tendon tear was 11.9% (112/939). In multivariable logistic regression analysis, dominant side involvement (p<0.001), manual labor (p=0.002), diabetes (p<0.001), metabolic syndrome (p<0.001), retraction degree of Patte tendon (p<0.001), posterosuperior RCT (p=0.010), and biceps tendon injury (p<0.001) were significantly associated with SBT. Conclusions: Metabolic syndrome is a potential risk factor for SBT, as are these factors: overuse activity, diabetes, posterosuperior RCT, increased retraction of posterosuperior rotator cuff tendon, and biceps tendon injury.
Background: Latissimus dorsi (LD) tendon transfer is used as a treatment option for massive irreparable posterosuperior rotator cuff tears, and recently, an arthroscopic-assisted technique was introduced. This study was undertaken to evaluate the clinical and radiological outcomes of arthroscopic-assisted LD tendon transfer for the management of irreparable rotator cuff tears in active middle-aged patients. Methods: The records of five patients (two males) with irreparable tears involving the supraspinatus and infraspinatus tendons managed by arthroscopic-assisted LD tendon transfer were retrospectively reviewed. Clinical outcomes were assessed using the visual analogue scale (VAS) pain scale, American Shoulder and Elbow Surgeon's (ASES) scores, the University of California Los Angeles (UCLA) scale, and ranges of motion. Postoperative integrities of transferred tendon were evaluated by magnetic resonance imaging in 4 patients and by ultrasound in one. Results: Mean patient age was 55 years (range, 48-61 years), and mean follow-up period was 20 months (range, 12.0-27.2 months). Mean VAS score significantly improved from $6.6{\pm}2.6$ preoperatively to $1.8{\pm}2.5$ postoperatively (p=0.009), mean ASES score increased from $67.6{\pm}9.2$ to $84.6{\pm}15.1$, and mean UCLA score from $18.0{\pm}1.4$ to $28.8{\pm}8.5$ (all p<0.001). Postoperative imaging of the transferred LD tendon showed intact repair in 4 patients. The remaining patient experienced LD transfer rupture and a poor outcome. Conclusions: Arthroscopic-assisted LD tendon transfer improved shoulder pain and function in patients with massive, irreparable rotator cuff tears, and may be an option for this condition, especially in physically active patients.
목적 : 고식적인 치료에 반응이 없는 14례의 환자에 대하여 관절경적 치료를 시행하여 이에 대한 치료결과를 평가하고자 한다. 대상 및 방법 : 1996년 3월부터 1998년 6월까지 회전근개의 석회화 건염의 진단하에 6개월이상 고식적 치료에 반응이 없어 관절경적 치료를 받은 14례 환자를 대상으로 하였다. 전례에서 극상근의 원위부에서 병변이 관찰되었으며 그 중 2례에서 극하근에, 1례에서 견갑하근에 병변이 동반되었다. 결과 평가는 UCLA 견관절 평가 지수와 Constant-Murley score를 이용하였다. 결과 : 동통 정도는 Constant-Murley score상 수술 전 평균 3.2점에서 수술 후 평균 8.3점으로 호전되었고 기능상으로는 UCLA점 수상 수술 전 평균 4.5점에서 수술 후 8..3점으로 호전되었다. 수술 전 평균 관절 운동 범위는 굴곡 110도, 외회전 45도, 내회전 제 3요추의 극돌기, 외전 90도로 제한되었으나 수술 후에는 굴곡 170도, 외회전 50도, 내회전 제 12흉추의 극돌기, 외전 140도로 호전되었다. 추시결과 판정은 우수 3례, 양호 9례, 보통 2례였다. 결론 : 보존적 치료에 호전이 없는 석회화 건염에 대하여 관절경적 치료는 효과적인 치료법으로 사료된다.
목적: 무증상 고교 야구 선수의 양측 견관절 초음파 소견을 알아보고 이를 건강한 대조군과 비교하고자 한다. 대상 및 방법: 총 42명(연령: 1$7.6{\pm}1.2$세)이 본 연구에 참여하였다. 최근 6개월간 견관절 통증이 없었던 남자 고교 야구 선수 중 투수 14명과 야수 18명을 두 그룹으로 분류하고 같은 연령대의 스포츠 활동을 하지 않는 고교 학생 10명을 대조군으로 설정하였다. 초음파를 이용하여 우세수와 비우세수의 상완 이두건 장두와 극상건의 두께를 측정하고 0도 외전 상태에서의 견봉 상완 간격을 측정하였다. 결과: 초음파 검사 상 야구 선수 32명 중 우세수에서 7명, 비우세수에서 2명에서 견봉하 점액낭의 삼출액이 관찰 되었으며 이는 무증상 대조군에서는 관찰되지 않았다. 양 견관절의 상완 이두건 장두와 극상건의 두께, 견봉 상완 간격은 대조군 보다 야구 선수에서 의미 있게 증가되어 있었다(p<0.05). 또한 야구 선수에서 상완 이두건과 극상건의 두께, 견봉 상완 간격은 비우세수에 비해 우세수에서 증가를 보였으나(p<0.05), 투수와 야수와의 차이는 없었으며 대조군 사이에서도 차이는 없었다. 결론: 무증상 고교 야구 선수에서도 초음파 상 견봉하 점액낭의 삼출액이 관찰될 수 있고 정상인보다 야구 선수에서 상완 이두건, 극상건의 두께가 더 두껍고 견봉 상완 간격이 증가되어 있으므로 이는 고교 야구 선수의 견관절 질환의 초음파 진단 시 유용한 참고 자료가 될 것으로 사료된다.

Background: The purposes of the study were to examine rotator cuff tendon degeneration with respect to harvesting location, to determine a rationale for debridement of the torn end, and thus, to determine adequate debridement extent. Methods: Twenty-four patients with a full-thickness rotator cuff tear were included in the study. Tendon specimens were harvested during arthroscopic rotator cuff repair from three locations; from torn ends after minimal regularization of fraying (native end group, NE group), from torn ends after complete freshening of the frayed end (freshened end group, FE group), and from the macroscopically intact portion just distal to the musculotendinous junction (musculotendinous junction group, MTJ group). Control samples were harvested from patients admitted for surgery for proximal humerus fracture. Harvested samples were evaluated using a semi-quantitative grading scale. Results: Mean total degeneration scores in the NE group ($13.3{\pm}3.21$), the FE group ($12.5{\pm}2.30$), and in the MTJ group ($10.8{\pm}3.10$) were significantly higher than those in the normal control group ($5.0{\pm}2.87$; all p<0.001). Mean total degeneration score in the NE group was significantly higher than that in the MTJ group (p=0.012), but was not from that of the FE group. Mean total degeneration score in the FE group was not significantly different from that of the MTJ group. Conclusions: Tendon degeneration exists throughout the entire tendon to the macroscopically intact portion of full-thickness rotator cuff tear. Therefore, aggressive debridement to grossly normal appearing, bleeding tendon is unnecessary for enhancing healing after repair.
Background: Massive rotator cuff tears (RCTs) are complicated by muscle atrophy, fibrosis, and intramuscular fatty degeneration, which are associated with postoperative tendon-to-bone healing failure and poor clinical outcomes. We evaluated muscle and enthesis changes in large tears with or without suprascapular nerve (SN) injury in a rat model. Methods: Sixty-two adult Sprague-Dawley rats were divided into SN injury (+) and SN injury (-) groups (n=31 each), comprising tendon (supraspinatus [SSP]/infraspinatus [ISP]) and nerve resection and tendon resection only cases, respectively. Muscle weight measurement, histological evaluation, and biomechanical testing were performed 4, 8, and 12 weeks postoperatively. Ultrastructural analysis with block face imaging was performed 8 weeks postoperatively. Results: SSP/ISP muscles in the SN injury (+) group appeared atrophic, with increased fatty tissue and decreased muscle weight, compared to those in the control and SN injury (-) groups. Immunoreactivity was only positive in the SN injury (+) group. Myofibril arrangement irregularity and mitochondrial swelling severity, along with number of fatty cells, were higher in the SN injury (+) group than in the SN injury (-) group. The bone-tendon junction enthesis was firm in the SN injury (-) group; this was atrophic and thinner in the SN injury (+) group, with decreased cell density and immature fibrocartilage. Mechanically, the tendon-bone insertion was significantly weaker in the SN injury (+) group than in the control and SN injury (+) groups. Conclusions: In clinical settings, SN injury may cause severe fatty changes and inhibition of postoperative tendon healing in large RCTs.
