Objectives : The purpose of this report is to prove the effect of The Daoyin exercise program combined with Korean medicine treatment on supraspinatus tendon tear. Methods : Two patients, diagnosed with Supraspinatus tendon tear by shoulder MRI, were treated with The Daoyin exercise program and Korean medicine treatment. VAS(Visual analog scale), ROM(Range of motion) and Physical examination were used to evaluate patients. Results of application : The Daoyin exercise program combined with Korean medicine treatment reduced shoulder pain and improved shoulder ROM of the two patients significantly. Conclusions : The Daoyin exercise program combined with Korean medicine treatment is proved to be effective treatment for reducing pain and improving shoulder ROM of supraspinatus tendon tear patients.

Objectives : The purpose of this study is to report the improvement of acupotomy in patients with partial tear of supraspinatus tendon. Methods : We treated 4 patients having shoulder pain due to partial tear of supraspinatus tendon with acupotomy combined with oriental medical treatments. We checked visual analog scale(VAS) and range of movement(ROM). Results : There are remarkable improvement in VAS and ROM. Conclusions : This report shows acupotomy has a effectiveness on partial tear of supraspinatus tendon. Follow-up study is needed.

Objectives: The purpose of this study is to report the improvement after oriental medical treatment of partial tear of supraspinatus tendon. Method: We treated 2 patients having shoulder pain due to partial tear of supraspinatus tendon with Oriental medical treatment, including Sa-am acupuncture DaeJang-Jeonggyeok, A-shi point(阿是穴) and herbal medicine. We checked visual analog scale (VAS) score and range of movement (ROM). Results: We treated shoulder pain. Thereafter ROM improved and V AS score dropped to the level of 2 to 4 points from 10 points at onset. Conclusion: This report shows Oriental medical treatment has effectiveness on partial tear of supraspinatus tendon. Follow-up study is needed.

Object: To determine the sensitivity, specificity, and positive and negative predictive values of an empty can test for diagnosing supraspinatus tendon tear. Methods: We reviewed 146 patients who have shoulder pain with limited active range of motion. We evaluated patients according to empty can test. Sensitivity, specificity, and positive and negative predictive values for supraspinatus test was estimated using arthroscopic evaluation. Results: A hundred and twenty-four of 127 cases with positive empty can test had supraspinatus tendon tear. Of 19 samples with negative empty can test, 15 had no supraspinatus tear. Sensitivity, specificity, and positive and negative predictive values for the empty can test were 97.6%, 83.3%, 97.6%, and 78.9%, respectively. Conclusions: Empty can test was found to have a high sensitivity and good positive predictive value in identifying the tear of rotator cuff tendon. We concluded that empty can test of the shoulder is a reliable diagnostic method which could be used for the diagnosis of rotator cuff tear.

목적 : 고식적 견관절 MRI 검사에 사각시상면 영상 추가 시 극상건 파열의 진단적 정확도가 증가하는지 관절경 수술 소견을 대비표준으로 삼아 알아보고자 하였다. 대상 및 방법 : 2011년 1월부터 2012년 12월까지 본원에서 견관절 MRI 검사를 하고 관절경 수술을 받은 121명의 환자를 대상으로 하였다. 두 명의 영상의학과 의사가 독립적으로 관상사면과 시상사면 영상을 이용하여 극상건 파열을 평가하고 4주후 사각시상면 영상을 추가하여 다시 판독하였다. 관절경 수술을 대비표준으로 이용하였다. 건 파열 진단의 민감도와 특이도는 McNemar test로 비교하였고 관찰자 간, 기술 간 일치도는 카파계수로 평가하였다. 결과 : 사각시상면 추가 시 고식적 견관절 MRI만 이용하였을 때 보다 극상건 전층 파열 진단의 민감도가 증가하였고, 부분 파열 진단의 민감도, 특이도, 정확도 모두 증가하였지만 통계학적으로 유의한 차이는 없었다. 관찰자간 일치도는 사각시상면을 추가하거나 하지 않았을 때 모두 높은 일치도를 보였다. 영상 소견과 관절경 수술 소견은 사각시상면 추가 시와 추가하지 않았을 때 모두 중등도의 일치도를 보였다. 결론 : 극상건의 전층 및 부분 파열 평가를 위해 고식적 MRI에 사각시상면 영상을 추가 시 기존의 영상면 만으로 평가하였을 때와 진단적 정확도에 의미 있는 차이는 없었다.

The objective of this study is to report the improved case of Korean medicine treatment with Aconitum ciliare decaisne pharmacopuncture for supraspinatus tendon full-thickness tear. We used Aconitum ciliare Decaisne pharmacopuncture with other Korean medicine treatments (acupuncture, electroacupuncture, and cupping) for this patient. This case is measured and assessed by the numerical rating scale (NRS), shoulder range of motion (ROM), shoulder pain and disability index (SPADI) and shoulder physical examination. As the result, we found the improvement of NRS, SPADI, ROM, and shoulder physical examination. This study suggested that Korean medicine treatment with Aconitum ciliare Decaisne pharmacopuncture is proved to be helpful to relieve pain and recover function for supraspinatus tendon full-thickness tear. But further clinical studies are needed to clarify the effect of Aconitum ciliare Decaisne pharmacopuncture therapy for patients with supraspinatus tendon full-thickness tear.

The purpose of this study was to investigate the clinical effects of Korean medical treatments combined with Embedding acupuncture on patients with supraspinatus tendon partial tear. We reviewed the medical records of 1 patients with supraspinatus tendon partial tear at Pureunsan oriental medical clinic from Oct, 2017 to November, 2017. The patient received embedding acupuncture therapy and oriental medical therapy. To evaluate the efficacy of the treatments, the patient was asked to complete a Visual analogue scale(VAS) and the Shoulder pain and disability index during treatment period. The VAS of the patient decreased more than the screening period. The SPADI change of the embedding acupuncture group were greater than non treatment period. Embedding therapy combined with Korean medical treatment might be effective in reducing pain and improving the life quality of patients with supraspinatus tendon partial tear. We hope that further studies will be done to produce more clinical data and ensure effective application of these results.

상기 증례에서 저자는 극상근건 부분 파열 환자에게 한방치료 및 ART 치료를 적용하여 견관절 ROM, VAS 평가를 통해 다음과 같은 결론을 얻었다. 1. 극상근건 부분 파열로 견통 및 견관절 ROM 제한 발생한 환자에게 한방치료 및 ART 치료를 적용하여 견관절 ROM, VAS에서 현저한 개선 보인 것을 확인할 수 있었다. 2. 비교적 증례가 많지 않고 한방적 치료 과정이 정립되어 있지 않은 극상근건 부분 파열에 한방 치료와 ART 치료를 병행하는 것이 효과가 있음 을 알 수 있었다.

극상건의 부분 파열(partial thickness tear)이 자기공명영상이나 Middleton, Crass position하의 정적초음파영상에서는 관찰되지 않는 경우가 있다. 저자들은 견관절의 extension position에서 저항 scaption (resisted scaption 또는 scapular plane abduction)을 이용한 동적초음파 검사를 이용하면 다른 영상의학적 도구나 자세에서 진단을 놓칠 수 있는 극상건의 부분 파열도 효과적으로 관찰할 수 있음을 여러 증례를 통하여 경험하였다. 현재까지 극상건 부분 파열을 진단하기 위한 동적 검사 방법은 문헌으로 보고된 적이 없었던 바, 이에 두 증례를 통하여 저항 scaption을 이용한 동적 초음파에 대한 저자들의 술기를 보고하고자 한다.

Background: Our study was to determine the effect on shoulder isokinetic muscle strength and muscle endurance in isolated full-thickness supraspinatus tendon tear and combined other rotator cuff tear. Methods: Total of 81 male patients (mean age $57.8{\pm}7.4$ years) who were diagnosed as a full-thickness supraspinatus tendon tear were included. They were classified into isolated or combined tear. The isokinetic muscle strength and muscle endurance were measured using the Biodex multi-joint system $PRO^{(R)}$ (Biodex Medical Systems, Shirley, NY, USA) in following movements: shoulder abduction, adduction, flexion, extension, external rotation, and internal rotation. Then, the difference in muscle function according to the type of tears were assessed. Fifty-seven patients had isolated supraspinatus tendon (mean age $56.9{\pm}7.3$ years). They were classified into either anteroposterior tear or modified mediolateral tear. The size were measured using T2-weighted magnetic resonance imaging scans in sagittal plane. Results: Between subjects categorized into the type of tear, we found significant inter-categorical differences in isokinetic muscle strength during abduction, adduction, flexion, extension, and internal rotation, and in muscle endurance during flexion, extension, and internal rotation. Anteroposterior diameter tear, we did not show significant differences in either isokinetic muscle strength or muscle endurance during any movements. However, with modified mediolateral diameter, we found significant differences with isokinetic muscle strength during adduction, and in muscle endurance the external rotation and internal rotation. Conclusions: We found that a supraspinatus tendon tear associated with more numbers of rotator cuff tears has lower isokinetic muscle strength and muscle endurance than a tear found alone.
