Purpose: The primary goal of this study was to develop a case management with exercise program for community dwelling elders who live alone, and examine the effects of the program. Method: The design of this research was a one group pre-post test study. The participations were 85 elders diagnosed with hypertension and diabetes mellitus and who lived alone as residents of D city. The case management with exercise program included exercise and counseling as the intervention and was provided for 12 weeks. Data were collected before and after the intervention which lasted from September 3 to November 26, 2009. Collected data were analyzed using descriptive statistics, and paired t-test. Results: There were significant differences in blood pressure (t=-5.24, p<.001, t=-1.94, p= .040), fasting blood sugar (t=-4.41, p<.001), ADL (t=-5.43, p=.022) and cognitive function (t=7.41, p=.008) between pre- and post intervention. Conclusion: These results indicate that the case management program is an important intervention for health promotion for community-dwelling elders, and exercise improves functional status of older persons with diseases. Therefore, now is the time to develop new supportive community-based programs for elders who live alone. However, it is also necessary to do further longitudinal studies to confirm the results of this study.
Nursing as it is practiced and taught, historically has been viewed as natural science. There are new movements to create a paradigm of nursing in the human sciences. A natural science methodologies elicit quantitative data from observable phenomena and reveal causal relationships. Human science methodology is the study of Unitary Man's participative experience in a situation, the simultaneity paradigm. In a theory of nursing rooted in human science assumptions about man and health are synthesized, and the practice of nursing is continuously expanded through research. To find independent nursing interventions especially cognitive nursing intervention models it has been necessary to consult a multitude of journals and text sources for references, and no one nursing textbook can be used in help patients achieve nursing goals. The goals of nursing in the simultaneity paradigm focus on the quality of life from the person's perspective. Cognitive interventions based on the person's perspective were selected from those that colleagues deemed appropriate to nursing and those that were identified in the nursing literature. They were supportive nursing care, reminiscence, meditation, relaxation and imagery. Nurses have been reluctant to make diagnoses, implement actions, and assume responsibility for this actions and this is of concern but can be understood because nurses have had little exposure to action and lack an intervention armamentarium from with which to choose actions to achieve nursing goals. Efforts in nursing education and nursing service are required to remedy this problem. Nurses must be convinced of the challenge and excitement associated with autonomous functioning. It is a characteristic of the true professional. Traditionally, nursing has prescribed one method for handling a situation. Fundamental nursing texts usually only present one way to handle a situation, because alternative interventions to achieve a client goal may be available. Considerably more research is necessary before these can be prescribed. However, unless a first step is taken, progress will not be made. The quality of health care or nursing care is enhanced when nurses transform dilemas into commited action. This is apparent from widespread experiences of nurses.
Purpose: The purpose of this study was to develop a maternal nursing competency reinforcement program for nursing students and assess the program's effectiveness in Korea. Methods: The maternal nursing competency reinforcement program was developed following the ADDIE model. This study employed an explanatory sequential mixed methods design that applied a non-blinded, randomized controlled trial with nursing students (28 experimental, 33 control) followed by open-ended interviews with a subset (n=7). Data were analyzed by both qualitative and quantitative analysis methods. Results: Repeated measures analysis of variance showed that significant differences according to group and time in maternal nursing performance; assessment of and intervention in postpartum uterine involution and vaginal discharge (F=24.04, p<.001), assessment of and intervention in amniotic membrane rupture (F=36.39, p<.001), assessment of and intervention in delivery process through vaginal examination (F=32.42, p<.001), and nursing care of patients undergoing induced labor (F=48.03, p<.001). Group and time improvements were also noted for problem-solving ability (F=9.73, p<.001) and emotional intelligence (F=4.32, p=.016). There were significant differences between groups in self-directed learning ability (F=13.09, p=.001), but not over time. The three main categories derived from content analysis include "learning with a colleague by simulation promotes self-reflection and learning," "improvement in maternal nursing knowledge and performance by learning various countermeasures," and "learning of emotionally supportive care, but being insufficient." Conclusion: The maternal nursing competency reinforcement program can be effectively utilized to improve maternal nursing performance, problem-solving ability, and emotional intelligence for nursing students.
Purpose: The purpose of this study was to examine the effect of a discharge education program on maternal role confidence and parenting stress of mothers who delivered premature infants. The program provided them with information on childrearing and supportive educational nursing care. Method: This study was based on pretest-posttest nonequivalent nonsynchronized quasi-experimental design. The participants in this study were 30 mothers who delivered premature infants at a general hospital in G city. The instruments used in this study were Maternity Confidence Inventory, Parenting Stress Index(PSI). Results: The first hypothesis that the mothers in the experimental group would undergo changes in maternal role confidence after the intervention was accepted, as there was a statistically significant gap between the two groups(F=9.386, P=.000). The second hypothesis that mothers in the experimental group would undergo change in parenting stress after the intervention was accepted, as there was a statistically significant gap between the two groups(F=4.425, P=.380). Conclusion: The findings suggest that the Discharge Education Program was an efficient intervention method to boost the maternal confidence of mothers with premature infants and to decrease their parenting stress.
Purpose: The purpose of this study was to examine the effects of Koryo Hand Therapy (KHT) on menopausal symptoms and hormone levels (Follicle Stimulating Hormone [FSH], Luteinizing Hormone [LH], and estradiol [$E_2$]) in climacteric women. Methods: The research design was a nonequivalent control group pretest-posttest design. Data were collected from November 28, 2005 to February 28, 2006. The 45 participants were assigned to either the experimental group (23) or control group (22). KHT was applied three times a week, for a total of 8 weeks to the women in the experimental group. Results: There was a statistically significant decrease in menopausal symptoms (F=42.49, p=.000), FSH level (F=26.98, p=.000) and LH level (F=5.31, p=.026) between the experimental and control groups. There was an increase of the estradiol level in the experimental group but the difference between the two groups was not statistically significant. Conclusion: KHT can be applied as a supportive nursing intervention to climacteric women. KHT is expected to be a complementary alternative intervention for health management of the climacteric women.
Purpose: The purpose of this study is to provide the basic data to develop the effective nursing intervention for the parent who have children with cancer by acquiring the deeper understanding of the mothers' adjustment of caring for their children with cancer. Methods: The ethnographic research method was used to find out the pattern of caring adjustment in Korean cultural context. Informants consisted of 12 mothers who were caring for their children with cancer. The data were collected using in-depth interviews, participant observation, and telephone interviews by maximum variation purposive sampling. The data were analyzed following Spradley's methodology. Results: The mothers' caring adjustment were organized into one cultural theme, four categories, and twelve properties. The cultural theme was 'standing alone as a mother with sin'. The four categories were 'blaming for falling illness', 'overcoming with motherhood', 'desperate struggling with side effects', and 'establishing new network as a dependent'. Conclusion: For the mothers who are caring children with cancer, the supportive nursing intervention based on the deeper understanding of mothers' pattern of caring adjustment for their children and centered on facilitating effective adjustment in each cultural context especially from the very early stage of caring in the hospital ward is extremely required.
Purpose: This study was to confirm the effect of acupressure on the emesis control and the weight change among pediatric cancer patients receiving anti-cancer chemotherapy. Method: Forty pediatric cancer patients, receiving the induction stage of chemotherapy with MTX and vincristine, were divided into control(n=20) and the intervention group(n=20). Both groups received regular anti-emesis medication, but the intervention group was added acupressure maneuver for 5 minutes on P6 point for 3 times a day for 5days: before chemotherapy, lunch and dinner by investigator during the hospitalization and by mother at home. The instruments for this study were Rhode's(1986) Index of nausea, vomiting and retching(INVR), Cas electric scale and pamphlet developed by researcher. Result: Significant differences in the degree of nausea and vomiting were observed between the control and the intervention group as measured by INVR(t=4.73; p=.01). Repeated measures ANOVA also shows that the group effect was significant(F=22.39, P=.01) as was the time effect(F=380.35, P=.01). The group by time interaction was also significant(F=5.27, P=.01). Acupressure maneuver was apparently effective in reducing the degree of chemotherapy-induced nausea and vomiting. There were also statistically significant weight loss noted in the control group than the intervention group(t=5.42, p=.01). Conclusion: Acupressure on P6 point shows an effective adjunct maneuver in reducing the degree of nausea and vomiting and conserving the weight in pediatric cancer patients. Therefore, it is proposed that acupressure should be applied as supportive nursing intervention strategies to relieve chemotherapy induced nausea and vomiting and to prevent weight loss in pediatric cancer patients.
Purpose: The purpose of this study was to examine the effect of Nei-Guan acupressure on nausea, vomiting, and anorexia in gynecological cancer patients receiving chemotherapy. Methods: Subjects consisted of 40 patients with cancer who were hospitalized and had chemotherapy at a general hospital located in Gyeonggi province, Korea. Control subjects (n=20) received only chemotherapy and intervention subjects (n=20) received Nei-Guan acupressure during their chemotherapy. The data were collected from February to October in 2008. Nausea and vomiting were measured with the Rhode's Index of Nausea, Vomiting, and Retching, and anorexia was measured using a Visual Analog Scale. Data were analyzed by X$^2$-test, t-test, repeated measures ANOVA. Results: Severity of nausea and vomiting in the intervention group was significantly less than in the control group. Repeated measure ANOVA revealed significant group effect (F=28.09, p=.000), time effect (F=321.48, p=.000), and interaction effect (F=7.90, p=.000). Severity of anorexia in the intervention group was significantly less than in the control group. Repeated measure ANOVA revealed significant group effect (F=28.68, p=.000), time effect (F=52.32, p=.000), and interaction effect (F=6.35, p=.000). Conclusions: Acupressure can be applied as a supportive nursing intervention to relieve nausea, vomiting, and anorexia in patients receiving chemotherapy.
Purpose: The purpose of this study was to examine the effects of light therapy on sleep disturbance and depression in climacteric women with menopausal symptoms. Methods: The research design was a nonequivalent control group pre test-post test design. Data were collected from September 29, 2013 to November 11, 2013. Participants included 17 climacteric women in an artificial light therapy group, 17 climacteric women in a sun light therapy group and 16 climacteric women in a control group. Measures consisted of the sleep disturbance, depression, melatonin, and serotonin. Results: There was a statistically significant difference of Korean Sleep Scale A (F=53.87, p<.001), and melatonin (F=31.19, p<.001) among three groups. There was a statistically significant difference of Self-Rating Depression Scale (F=121.86, p<.001), and serotonin (F=102.37, p<.001) among three groups. Conclusion: Artificial and sun light therapy can be applied as a supportive nursing intervention to subjects with sleep disturbance and depression in climacteric women with menopausal symptoms. Artificial and sun light therapy is expected to be a complementary alternative intervention for health management of the subjects with sleep disturbance and depression in climacteric women with menopausal symptoms.
This study was carried out to introduce the Foot reflexo-massage (FRM) as a method of nursing practice based on the health promotion of occupational health. Literature reviews were done to achieve study aims from October in 1999 to August in 2000. The results were as follow ; 1. Theory of the FRM was mainly developed from the reflexology based on function and role of foot. 2. The FRM is focused on the appropriate reflexology of foot with pressures of light-moderate-light or light-strong-light scaled strength for 15 minutes at each feet. The massage should begin with the foot area where it is assumed as a was, of internal to external and upper to lower side. It also should start from left to light side of the feet. 3. The FRM is an alternative therapy increasingly demanded as an self applying method between eastern and western due to the benefit of natural healing power with no side effects. 4. The FRM can be an nursing intervention for health promotion of employees through supportive and educative nursing system in occupational health service. As a conclusion of the study, it suggested that the FRM can be recommended as a nursing intervention to promote health of employees and to prevent disease related to work.
