한약재 특성에 따른 궤양성 대장염 치료과 장내세균 상관관계 연구 (The Study about Relationship between Treatment of Ulcerative Colitis and Intestinal Bacteria According to Characteristic of Herbal Medicine)

  • Many modern people are exposed to chronic inflammatory diseases, such as inflammatory bowel disease (IBD), atopic dermatitis and immune disorder. Among those chronic diseases, the incidence ratio of IBD has been increased. IBD, including Crohn's disease and ulcerative colitis (UC), is known to cause abnormal inflammation in intestinal tissue. UC is accompanied by abdominal pain, bloody stool and diarrhea. Many therapeutic agents, such as sulfasalazine, corticosteroids, immunosuppressive agents, have been used for treating UC. However, those agents have side-effects and temporary effects on UC. The aim of this study was to investigate the effect of herbal medicine on UC and relationship between UC and intestinal bacteria according characteristic of herbal medicine. To determine the effect of herbal medicine on UC, various herbal medicine were chosen within oriental medicine category such as cheongyeol and onyeol medicine. In this study, we found carthami fructus, included in cheongyeol medicine, had stronger effect than onyeol medicine. Also, we determined influence of carthami fructus against lactic acid bacteria. Catthami fructus and lingon berry extracts affected the composition of mice intestinal bacteria in mice fecal. The symptoms of UC could be regulate by using herbal medicine, according to characteristic of herbal medicine. Also, herbal medicine might be change body condition to healthy by controlling intestinal bacteria composition. Herbal medicine characteristic could be a therapeutic agent by revealing relationship between intestinal bacteria and UC.

  • Drug rash with eosinophilia and systemic symptoms (DRESS) syndrome is a rare and potentially fatal condition characterized by skin rash, fever, eosinophilia, and multiorgan involvement. Various drugs may be associated with this syndrome including carbamazepine, allopurinol, and sulfasalazine. Renal involvement in DRESS syndrome most commonly presents as acute kidney injury due to interstitial nephritis. An 11-year-old boy was referred to the Children's Hospital of Pusan National University because of persistent fever, rash, abdominal distension, generalized edema, lymphadenopathy, and eosinophilia. He previously received vancomycin and ceftriaxone for 10 days at another hospital. He developed acute kidney injury with nephrotic range proteinuria and hypocomplementemia. A subsequent renal biopsy indicated the presence of acute tubular necrosis (ATN) and late exudative phase of postinfectious glomerulonephritis (PIGN). Systemic symptoms and renal function improved with corticosteroid therapy after the discontinuation of vancomycin. Here, we describe a biopsy-proven case of severe ATN that manifested as a part of vancomycin-induced DRESS syndrome with coincident PIGN. It is important for clinicians to be aware of this syndrome due to its severity and potentially fatal nature.

  • 약제 유발성 BOOP의 경우는 약을 중단하여도 증상과 방사선학적 소견이 호전되지 않을 수 있고 부신피질스테로이드를 사용한 후 뚜렷한 호전을 보일 수 있기 때문에 BOOP를 일으킬 수 있는 약제에 대한 인지가 진단과 치료에 중요하다. 저자들은 carbamazepine에 의한 BOOP 1예를 경험하였기에 보고하는 바이다.

대추의 궤양성 대장염에 대한 개선 효과 (The improving effect of Zizyphus jujube on dextran sulfate sodium-induced colitis in mice)

  • Objectives : Zizyphus jujube (ZJ) has been used as a traditional medicine for various diseases. However, the inhibitory effect of ZJ on intestinal inflammation has not been fully understood, yet. The aim of this study is to investigate anti-colitis activity of ZJ in dextran sulfate sodium (DSS)-induced colitis mouse model. Methods : To investigate the protective effects of ZJ,the colitis mice were induced by drinking water containing 5% DSS for 7 days. Mice were randomized into groups receiving ZJ (500 mg/kg), sulfasalazine (SFZ) (150 mg/kg) as a positive control, or water as a negative control. We assayed the effects of ZJ on DSS-induced the clinical signs, measuring weight loss, colon length and disease activity index (DAI). Additionally, to find a possible explanation for the anti-inflammatory effects of ZJ, we evaluated the effects of ZJ on the production of prostaglandin $E_2$ ($PGE_2$) and expression of cyclooxygenase (COX)-2 in colitis tissue. Results : The results showed that mice treated with DSS showed considerable clinical signs, including weight loss, and reduced colon length. However, administration of ZJ significantly reduced the weight loss, shortens colon length, and improved DAI as clinical symptoms. Moreover, ZJ inhibited the $PGE_2$ production and COX-2 expression levels in DSS-treated colon tissues. Conclusions : Collectively, the findings of this study provide us with novel insights into the pharmacological actions of ZJ as a potential molecule for use in the treatment of intestinal inflammation including ulcerative colitis.

Fatal Balamuthia Amebic Encephalitis in a Healthy Child: A Case Report with Review of Survival Cases

  • Balamuthia mandrillaris is one of the 4 amebas in fresh water and soil that cause diseases in humans. Granulomatous amebic encephalitis (GAE), caused by B. mandrillaris, is a rare but life-threatening condition. A 4-year-old, previously healthy, Thai girl presented with progressive headache and ataxia for over a month. Neuroimaging studies showed an infiltrative mass at the right cerebellar hemisphere mimicking a malignant cerebellar tumor. The pathological finding after total mass removal revealed severe necrotizing inflammation, with presence of scattered amebic trophozoites. Cerebrospinal fluid (CSF) obtained from lumbar puncture showed evidence of non-specific inflammation without identifiable organisms. A combination of pentamidine, sulfasalazine, fluconazole, and clarithromycin had been initiated promptly before PCR confirmed the diagnosis of Balamuthia amebic encephalitis (BAE). The patient showed initial improvement after the surgery and combined medical treatment, but gradually deteriorated and died of multiple organ failure within 46 days upon admission despite early diagnosis and treatment. In addition to the case, 10 survivors of BAE reported in the PubMed database were briefly reviewed in an attempt to identify the possible factors leading to survival of the patients diagnosed with this rare disease.

  • Objectives : Ulcerative colitis (UC) is an inflammatory bowel disease, which is one of chronic gastrointestinal disorders. Orostachys japonicus (OJ) has been used as a traditional medicine for various diseases including gastric cancer, gastric ulcers and intoxication. However, the regulatory effect of OJ on intestinal inflammation has not been fully understood, yet. The aim of this study was to investigate the effect of OJ on dextran sulfate sodium (DSS)-induced colitis in mice. Methods : To ascertain the pharmacological effects of OJ, the colitis mice were induced by drinking water containing 5% DSS for 7 days. Mice were randomized into groups receiving OJ (100 mg/kg), sulfasalazine (150 mg/kg) as a positive control, or water as a negative control. We evaluated the effects of OJ on DSS-induced the clinical signs, measuring weight loss and colon length. In addition, the inhibitory effect of OJ on the tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) level was determined by enzyme-linked immunosorbent assay in colitis tissue. Results : The results indicated that mice treated with DSS showed remarkable clinical signs, including weight loss, and reduced colon length. However, treatment with OJ significantly improved the weight loss and DAI as clinical symptoms. Moreover, OJ reduced the TNF-${\alpha}$ levels in DSS-treated colon tissues. Conclusions : Collectively, the findings of this study provide us with novel insights into the pharmacological actions of OJ as a potential medicine for use in the treatment of ulcerative colitis.

소아기 류마티스모양 관절염 환자에서 etanercept 사용 후 발생한 뇌염 1예 (A case of encephalitis in a juvenile rheumatoid arthritis patient treated with etanercept)

  • $TNF-{\alpha}$ 는 소아기 류마티스모양 관절염의 병태생리학에 관여하는 주요 cytokine 이다. Etanercept 는 $TNF-{\alpha}$ 억제제 중 하나로 소아기 류마티스모양 관절염에 효과적인 약물로 각광받고 있다. Etanercept의 주요 부작용은 면역력 저하에 의한 감염으로, 대게 중등도의 상기도 감염이 대부분으로 알려져 있으나, 최근 중증의 감염도 보고되고 있다. 저자들은 소아기 류마티스모양 관절염 환자가 etanercept를 14개월간 투약 후 발생한 뇌염 1예를 경험하였기에 보고하는 바이다.

해조가 2,4,6-trinitrobenzene-sulfonic acid로 유발된 염증성 장질환 동물모델에 미치는 영향 (Effects of Sargassumpallidum on 2,4,6-Trinitrobenzene Sulfonic Acid-Induced Colitis in Mice)

  • Objectives : The aim of the current study was to investigate the effects of Sargassum (Sargassum pallidum (TURN.) C. AG.; SP) on the experimental colitis induced by 2,4,6-trinitrobenzene sulfonic acid (TNBS) in mice. Methods : ICR mice were divided into 7 groups (NOR, CON, $SS50\times5$, $SP20\times3$, $SP50\times3$, $SP20\times5$, $SP50\times5$). TNBS processing was intrarectally applied to all experimental groups on the 3rd experiment day, except the normal group (NOR). For investigating the prophylactic effect, SP at doses of 20 mg/kg ($SP20\times5$) and 50 mg/kg ($SP50\times5$) were orally administered for 5 days. The SP at doses of 20 mg/kg ($SP20\times3$) and 50 mg/kg ($SP50\times3$) were orally administered for 3 days after the colitis induction in order to check the effect of treatment. As a positive control group, sulfasalazine 50 mg/kg ($SS50\times5$) was administrated. Macroscopic findings of epithelial tissue on mice were measured by colon length and macroscopic score. Histologic findings were also checked by crypt cell, epithelial cell, inflammatory cell and edema of submucosa. We measured the ability of SP to inhibit lipid peroxidation and myeloperoxidase activity. We also measured levels of the inflammatory markers, interleukin (IL)-$1\beta$ and cyclooxygenase-2 (COX-2), its transcription factor activation, phospho-NF-${\kappa}B$ (pp65), in the colon by enzyme-linked immunosorbent assay and immunoblot analysis. We measured activation of fecal bacterial enzyme, $\beta$-glucuronidase and degradation activation of fecal glycosaminoglycan (GAG), and hyaluronic acid. Results : Oral administration of SP on mice inhibited TNBS-induced colon shortening and myeloperoxidase activity in the colon of mice as well as IL-$1\beta$ and COX-2 expression. SP also inhibited TNBS-induced lipid peroxidation and pp65 activation in the colon of mice. SP inhibited $\beta$-glucuronidase activation and fecal hyaluronic acid degradation activation as well. Conclusions : SP could be a possible herbal candidate and preventive prebiotic agent for treating inflammatory bowel disease (IBD). Further experiments to differentiate effects of SP on IBD, such as other solutions and extracting times, might be promising.

Effect of DA-6034, a Derivative of Flavonoid, on Experimental Animal Models of Inflammatory Bowel Disease

  • Inflammatory bowel disease (IBD) is a multifactorial disorder with unknown etiology and pathogenesis. DA-6034,$ 7-carboxymethyloxy-3^{l}, 4^{l},$ 5-trimethoxy flavone, is a synthetic flavonoid known to possess anti-inflammatory activity. This study was performed to evaluate the oral therapeutic effect of DA-6034 in three experimental animal models of IBD : two chemical-induced IBD models of rats and the human leukocyte antigen (HLA)-B27 transgenic rat model known to develop spontaneous colitis without the use of exogenous agents. Acute chemical colitis was induced by intracolonic instillation of 1.2 ml of 4% acetic acid solution. Prednisolone (1 mg/kg), sulfasalazine (100 mg/kg) and DA-6034 (0.3~3 mg/kg) were orally administered twice daily for 6 days in these rats. In addition, chronic chemical colitis was induced by intracolonic administration of trinitrobenzene sulfonic acid (TNBS) 30 mg in 50% ethanol and agents were orally administered for 6 or 20 days. In chemical-induced IBD models, all of these agents reduced the severity of colitis and specially, DA-6034 (3 mg/kg) showed more potent effect than other drugs in macroscopic lesion score. In HLA-B27 transgenic rats, DA-6034 (3 mg/kg) and prednisolone (0.5 gm/kg) were treated orally twice daily for 6 weeks. The HLA-B27 transgenic rats showed only mild colitis, compared with the chemical-induced colitis models. DA-6034 ameliorated the loose stool and decreased microscopic damage, which is the important indicator of this model. In conclusion, oral therapy of DA-6034 attenuated the macroscopic and histologic damages of the colon in all three experimental models of IBD, which suggest that DA-6034 could be a promising drug in the treatment of IBD.

Dextran Sulfate Sodium으로 유도된 궤양성 대장염에 대한 작약의 개선 효과 (The Improving Effect of Paeoniae Radix on Dextran Sulfate Sodium-induced Colitis in Mice)

  • 염증성 대장질환에 대한 작약의 효능 연구가 미비하여 본 연구에서는 dextran sulfate sodium로 유도된 궤양성 대장염의 임상증상인 체중감소, 결장단축, 질병활성화 정도에 대한 작약의 효과를 관찰하였다. 또한 대장 조직 내 염증매개인자인 interleukin-6 및 cyclooxygenase-2의 생성에 대한 작약의 효과를 측정하여 다음과 같은 결과를 얻었다. 첫째 dextran sulfate sodium로 유도된 궤양성 대장염에서 체중 감소에 대한 작약의 효과를 관찰한 결과 유의적으로 억제함을 확인하였다. 둘째 DSS로 유도된 궤양성 대장염에서 결장 단축에 대하여 작약의 억제 효과가 유의성 있게 나타났다. 셋째 dextran sulfate sodium로 유도된 궤양성 대장염에서 임상증상인 설사, 잠혈 및 출혈 등의 질병활성화 정도에 대한 작약의 효과를 관찰한 결과 유의적으로 질병활성화 정도를 억제하였다. 넷째 대장 조직내 염증매개인자인 interleukin-6 및 cyclooxygenase-2의 생성증가에 대한 작약의 억제효과를 확인 하였다. 본 연구는 작약의 효능을 과학적으로 입증한 결과로, 작약의 궤양성 대장염의 치료제 개발의 소재로서의 가능성을 시사한다.
