Analysis of the knowledge and insurance screening review rates of health insurance claims in the dental hygienist (일부 지역 치과위생사의 치과 건강보험에 관한 지식 및 심사조정률에 미치는 요인)

  • Objectives: The purpose of the study is to investigate the level of knowledge and screening review rates of dental health insurance claims in dental hygienists. This analysis will provide the educational information to the dental hygienists. Methods: A self-reported questionnaire was completed by dental hygienists in Jeonbuk from December 17, 2012 to January 24, 2013. The questionnaire was distributed by ordinary mail or direct visit. Except incomplete answers, 350 data were collected and analyzed. The study instrument was adapted from the structured questionnaire by Hong and Yoo. The questionnaire consisted of education experience of health insurance management, subjective and objective knowledge, insurance screening review, and need for health insurance education. Data were analyzed by SPSS 12.0 program. Cronbach alpha in the objective knowledge on health insurance rate criteria was 0.836 and this was a reliable figure. Results: The subjective knowledge level of dental insurance was higher in the senior dental hygienists. Subgingival curettage was the lowest percentage of correct answers in the objective knowledge. In recent six months, higher review control rate was shown in the higher claim for health insurance and insurance screening review. Conclusions: The majority of the respondents had lower level of knowledge of health insurance claims. The continuous education of dental health insurance will be necessary in the dental hygienists.

Impact of Periodontal Treatment and Demographic and Socioeconomic Factors on Tooth Loss in Persons with Disabilities: An Analysis of Korean National Health Insurance Claims Data

  • Background: This study aimed to analyze the effects of periodontal treatment and individual- and tooth-related factors on tooth extraction in people with disabilities. Methods: The Korea National Health Insurance claims data of individuals with disabilities aged 40~64 years with chronic periodontitis in 2008 were obtained. Of these, data on the disabled who underwent scaling/root plaining, subgingival curettage/periodontal surgery, or non-periodontal treatments, and data on their teeth were selected. The extraction of 716,688 teeth from 39,097 patients was tracked until 2018, and the patient- and tooth-level factors related to tooth loss were identified using a mixed-effect logistic regression analysis. Results: Data from approximately 17% of the teeth were extracted during a follow-up period of approximately 11 years. Among the tooth-level variables, scaling/root planing treatment at baseline and periodontal treatment during the follow-up period were associated with a lower risk of tooth loss (odds ratio=0.692 and 0.769, respectively, p<0.001). Non-vital teeth increased the risk of tooth loss by 3.159 times (p<0.001). Among the patient-level variables, females were less likely to have lost their teeth than males, and those with orthopedic impairment or brain lesions/mental disabilities, a higher age group, lower income level, or residents in medium/small cities or rural areas were more likely to have lost their teeth (p<0.001). Conclusion: Through approximately 11 years of follow-up, scaling or root planing, experience with periodontal treatment at least once, female sex, older age, lower income, smaller residential areas, type of disability, and pulp vitality were found to be associated with tooth loss in individuals with disabilities aged 40~64 years with chronic periodontitis. To prevent tooth loss in individuals with disabilities, it is necessary to establish a dental treatment plan that considers the timing of periodontal treatment and the characteristics of the patient and teeth.

Clinical effect of combined treatment by subgingival curettage and $CO_2$ laser application (치은연하 소파술과 $CO_2$ 레이저의 병용시의 임상적 효과)

  • 최근의 레이저 기술의 발전은 치의화영역에서 많은 가능성을 제시해 주고 있으며, 레이저를 이용한 외과적 치료의 기전과 안전성은 많은 분야에서 입증되어 있다. 근래에는 치주학적 분야에도 레이저를 적용하려는 노력이 계속되어 왔으며, 치석의 제거나 치근면의 세균제거 등에서 효과를 제시한 연구들이 있었다. 본 연구의 목적은 $CO_2$ 레이저를 통상적 치은연하소파술과 병용하였을때의 임상적 효과를 임상 지수의 측정을 통하여 평가하는 것이다. 만성 중둥도-고도의 치주염으로 진단된 12명의 환자가 본 임상연구에 포함되었다. 한 환자에서 각각 2부위의 사분악을 선택하여 임의로 2가지 치료군에 다음과 같이 나누어 포함시켰다: 1) 치은연하 소파술만을 적용한 사분악을 대조군 2) 치은연하 소파술과 0.8W의 에너지 수준을 갖는 $CO_2$ 레이저를 병용하여 적용한 사분악을 Laser 군으로 포함하였다. 치주낭 탐침 깊이, 임상 부착 수준, 치은 퇴축 및 탐침시 출혈풍의 임상지수를 치료전과 술후 각각 1, 3, 6개월 경과시에 측정하여 다음과 같은 결과를 얻었다. 치주낭 탐침 깊이, 임상 부착 수준, 치은 퇴축 및 탐침시 출혈 등의 모든 측정한 임상지수에서 치료전 ${\cdot}$ 후를 비교하였을 때 통계적으로 유의한 개선을 보였다. 그러나 실험군과 대조군간의 비교에서는 치주낭 탐침 깊이, 임상 부착 수준에서는 통계적으로 유의한 차이를 보이지 않았다. 탐침시 출혈은 술후 6개월시에 Laser군에서 대조군에 비하여 통계적으로 유의성있는 차이를 보이며 감소하였다(p<0.05). 결론적으로 $CO_2$ laser를 비외과적 치주 치료에 부가적으로 적용하였을 때 염증 감소에 기여할 가능성이 있을 것으로 사료된다.

  • In order to deliver fixed partial denture to a patient successfully, dentist must take into many considerations beforehand. Patient's occlusal pattern should be fully understood before delivering any type of prosthesis, whether it's canine guidance or group function occlusion. In order to do so, movement of mandible should be recorded precisely. Introduced by Meyers in 1933, functionally generated path technique (FGP) has been successfully used to record mandibular movement with various materials, such as wax, and utilize it in fabricating precise prosthesis. In the following cases, patients showed secondary caries or endodontic-periodontic lesion on maxillary molars. Root canal treatment and subgingival curettage were done for each patient. Since the long term prognosis of each tooth was questionable, lateral force should be minimized. In order to do so, FGP technique was used to record mandibular movements precisely. Instead of using conventional plaster impression, intra-oral scanning of wax tracing was done to fabricate prosthesis efficiently. After delivery of fixed partial denture, favorable outcome was obtained.

Syncope and pneumomediastinum during the maxillary sinus elevation with an air-syringe: a case report (주사기를 이용한 상악동 거상술 시 발생한 실신 및 종격동기종에 관한 증례보고)

  • Pneumomediastinum is a very rare and potentially catastrophic complication of dental procedures. Its common causes are tooth extraction, endodontic treatment, and subgingival curettage using handpieces and high-pressure air/water syringes. We present a case of massive pneumomediastinum with subcutaneous emphysema in a 61-year-old female who underwent bone grafting into the maxilla for pretreatment of dental implantation using a syringe. The patient suffered from abrupt severe odynophagia and loss of consciousness. The patient transferred to emergency department and images work-up revealed a pneumomediastinum and subcutaneous emphysema on the entire face and neck. We performed conservative treatments including prophylactic antibiotics, oxygen inhalation, and fasting meals, and then discharge after 7 days uneventfully. The patient's syncope might be resulting from hypotension and pain shock induced by pneumomediastinum with a sudden chest compression. The pneumomediastinum could be resulting from concurrent perforation and massive air infiltration into the maxillary sinus during bone grafting. We suggest that pneumomediastinum needs prompt diagnosis and management because of the risk of airway obstruction when a patient present syncope in the dental room.
