Ultrasonographic appearance of stomach, pylorus and duodenum in relation to food intake and administration of contrast medium (개에서 사료섭취 및 조영제 투여에 따른 위, 유문부 및 십이지장의 초음파상)

  • The purpose of this study is to determine the changes of ultrasonographic appearance of stomach, pylorus and duodenum in relation to food intake and administration of contrast medium in dogs. Ultrasonograms were obtained with 5.0MHz sector transducer. After placing the dogs in dorsal recumbency, scanning was performed at the 8th intercostal space. The mean inner diameter of stomach immediately after food intake, 2, 4 and 6 hours was 47.9, 51.5, 46.8 and 40.6mm, respectively. The diameter of pylorus immediately after food intake, 2, 4 and 6 hours was 7.4, 9.7, 8.9 and 6.0mm, respectively(p<0.01). The diameter of duodenum immediately after food intake 2, 4 and 6 hours was 12.5, 7.9, 11.9 and 11.8mm, respectively(p<0.01). Before administration of contrast medium, the mean inner diameter of stomach, pylorus and duodenum was 38.6, 7.2 and 9.5mm, respectively. After administration of contrast medium, the diameters of stomach, pylorus and duodenum was 42.8, 7.7 and 9.9mm, respectively. It may be concluded that the ultrasonographic values determined in this study can be used as references for the diagnosis of gastrointestinal disease in dogs.

Electron Microscopic Observations of Endocrine Cells on the Squirrel's Gastrointestinal Tract (다람쥐 소화관의 내분비세포에 관한 전자현미경적 연구)

  • Endocrine cells were observed in the gastrointestinal tract of the squirrels by light microscopy using Grimelius and Masson-Hamperl silver impregnation, and in the stomach and duodenum by electron microscopy. Light microscopically, argyrophil cell decreased gradually in number in the pyloric region and ileum, stomach, duodenum and jejunum and rectum in the order listed. Argentaffin cells were moderately distributed in the stomach, pyloric region and duodenum, while absent in the jejunum, ileum and rectum. Five kinds of cell types, EC, ECL, D, $D_1$ and G cells were identified in the stomach and three kinds of cell types, S, $EC_1$ and $EC_2$ were also identified in the duodenum by electron microscopy. A cilium noted in the cytoplasm of $D_1$-type cell. The species differences in the distribution, size and structure of the secretory granules were discussed.

  • In the present paper, the distribution, relative frequences and cell types of endocrine cells in the gastrointestinal tract of the frog (Rana dybowskii) during the hibernating and the active phase were examined by light and electron microscopy. The results obtained are summarized as follow: The reactive cells for Grimelius were frequently found in the gastrointestinal tract, whereas the reactive cells for Hellman-$Hellerstr{\hat{o}}m$ were found numerous in the fundus and pylorus of stomach, a few in the duodenum and lower small intestine, and very few in the rectum during both phases. No reactive cells for Masson-Fontana were found in the gastrointestinal tract during both phases. Elecron microscopically, 4 types of endocrine cells in the fundus of the stomach, 3 types in the pylorus of the stomach and duodenum, and 1 type in the lower small intestine and rectum, respectively, were identified during the hibernating phase. In the active phase, 3 types of endocrine cell in the fundus of the stomach, 2 types in the pylorus of the stomach and duodenum, and 1 type in the lower small intestine and rectum were observed, respectively. In the hibernating phase, more cytoplasmic granules and various types of endocrine cells were generally found than in the active phase.

  • Recently, the researches on the enteroendocrine cells of vertebrates have made a remarkable advance by the immunocytochemical methods. This study was attempted to investigate the topographical distributions and the shapes of gastrin, glucagon, somatostatin and cholecystokinin-8 immuno-reactive cells in the gastrointestinal tract of the Korean hedgehog, Erinaceus koreanus. For light-microscopical examination of immunocytochemistry, the tissue specimens taken from the various portions(body and pyloric protion of stomach, duodenum, jejunum, ileum and rectum) were fixed in glutaraldehyde-picric acid-acetic acid (GPA) or 10% neutral buffered formalin solutions. For the demonstration of immunoreactive cells, the paraffin sections (6$\mu$m) were immunocytochemically identified by PAP procedure (Sternberger, 1979) with gastrin, glucagon, somatostatin and cholecystokinin-8 antisera. Gastrin-immunoreactive cells were mainly distributed in the pyloric portion of stomach and were a few in the duodenum and jejunum. The shapes of these cells were round or oval in the pyloric portion and pyramidal in the small intestine. Glucagon-immunoreactive cells were sparsely distributed in the only small intestine. The shapes of these cells were mainly pyramidal. Somatostatin-immunoreactive cells were a few in the pyloric portion and duodenum, and were sparsely distributed in the body of stomach and jejunum. The shapes of these cells were round or oval in the stomach and oval or pyramidal in the small intestine. Cholecystokinin-8-immunoreactive cells were sparsely distributed in the only small intestine. The shapes of these cells were mainly oval or pyramidal.

A Study on the concept of Ulkyuk and Banui (열격${\cdot}$반위의 개념에 대한 소고)

  • Having examined the literatures on the terminologies. UlkyuK and Banui, the following results were obtained. 1. since Ul accompanies the symptom of dysphagia. it is thoughr ro indicate the disease and ailment in the larynx and pharynx. and the esphangus. 2. Kuyk is used in the contextof Kyuksak. Kyukjung. Bikyuk. Ulsakamnd Ulkyuk. and since it also includes the meaning of Ul. it is thought that disease and ailment in the esophagus. stomach and duodenum are included in this category. 3. Although Ul and Kyuk are differentiated. since they appear in conjunction with the other clinically. they can be refferred to as Ulkyuk. 4. Banui represents the phenomenon of vomiting. because it is oresent in a bad case of Ulkyuk. it can be seen to be included in Ulkyuk. Thus. Ulkyuk and Banui can be refferred to as one. 5.Although the name of an ailments in oriental medicine and that in the western medicine can not be in perfect match. Ulkyuk is refferred to as ailments in esophagus such as esophagitis. esophageal ulcer, esophageal deverticulum and esophageal cancer. while Banui is refferred to asailments in stomanch and duodenum such as stomach cancer. chtonic spasmic gastritis. it is reasonable to differentiate the two in general. As seen above, since the concept of the terminologies. Ulkyuk and Banui are somewhat different among Oriental medicine doctors. a consensus is needed to come up with terms that all can agree on.

Immunohistochemical Studv on the Gastrin, Somatostatin and Serotonin Cells in the Gastric and Small Intestinal Mucosa of Rat during Development (발생기 흰쥐 위와 소장점막의 gastrin, somatostatin 및 serotonin세포에 대한 면역조직화학적연구)

  • The developmental changes of three enteroendocrine cells, i.e. gastrln, somatostatin and serotonin, of gastric and small intestinal mucosa in pre- and postnatal rat were examined by peroxidase-antiperoxidase (PAP) method. In the course of development, gastrin cells were obsenred in the pyloric gland region and the whole part of small intestine, while somstostatin and serotonin cells in the whole gastric gland region and small intestine. More entroendocrine cells were detected in the pyloric gland region and duodenum than in the other portion. In the stomach, gastrin, somatostatin and serotonin ceils were first obsenred in the pyloric Bland region on 17, 19 and 19 days of gestation respectively. The small intestinal gastrin and serotonin cells were first appeared in the duodenum and iriunum on 17 and 15 days of gestation respectively, and somatostBtin cells in duodenum on 17 days of gestation. The number of cells examined from the stomach were increased from fetal to weanling period and showed a decrease during adult period: the notable increase was shown at the end of suckling period or at early weanling period. The cells of the small intestine increased from fetal to suckling period, especially, these cells markedly increased at the end of fetal period or at early suckling period, and decreased from weanling period. The shape of these cells was oval or fusiform during fetal period. In the stomach, most of gastrin cells turned out to be oval and open-type from suckling period, while the remaining two tripes of cells were oval and open- or closed-type. In the small intestine, 311%Ves of cells examined were changed to fusiform and open-type from the end of fetal period. Three types of cell were distributed over the stratified epithelium on 15 and 17 days of gestation. In the stomach, these cells were distributed lower gastric pit and gland from the following fetal period, and were detected mainly on the upper part of gland from suckling period, and then obsenred on the whole part of gland. In the small intestine, most of cells distributed over only between epithelium of villi on 19 days of gestation, increased in number on the crypt from following fetal period, and also observed abundantly in the crypt at adult period.

Water-Immersion-Restraint Stress model in Mongolian gerbil forcomparison of pathoaenicity of Helicobacter pylori strains (Helicobacter pylori의 병원성 비교를 위한 gerbil의 수침구속스트레스 모델)

  • Helicobacter pylori (H. pylori)-infection is an important pathogen of stomach cancer after chronic gastritis and ulceration in the stomach and duodenum. However, the virulences of H. pylori strains have not been well-defined between clinical isolates. This study was designed to establish water-immersion-restraint stress (WIRS) model in mongolian gerbil for comparison of pathogenicity of H. pylori strains. To determine an optimal duration time for WIRS model in gerbil, 5-week-old Mongolian gerbils were divided into different groups by WIRS duration time. After graded duration of WIRS, the macroscopic ulcer index (UI) was measured with the stomach and duodenum of sacrificed animal. There were no significant differences between male and female in same duration group. However, the UI increased significantly in a time-dependent fashion. The group of 6 hours-WIRS animals showed severe hemorrhage and ulceration in their stomach and duodenum. On the other hand, the very mild lesions induced in 2 hours-treated animals. Therefore, we determined an optimal duration time for WIRS model in gerbil as 4 hours. Thereafter, we evaluated whether this WIRS model in gerbil could be used as an useful tool for in vivo comparison of pathogenicity of H. pylori strains by enhancement of pathological severity in H. pylori-infected gerbils. Mongolian gerbils were divided into H. pyloriinfected and PBS-inoculated groups. Thereafter, they were divided again into 4 hours-WIRS and no WIRS subgroups. After treatment, the severity of pathological changes was evaluated in a same manner with previous duration-determining experiment. When the animals were exposed to WIRS, the UI was significantly higher in the infected group than in the uninfected group. These results suggested that the established gerbil-WIRS model in this study enhanced effectively the severity of pathogenic changes in the H. pylori-infected Mongolian gerbils and could be used as an useful tool for in vivo comparison of pathogenicity of H. pylori strains.

  • A partial obstruction of small intestine was diagnosed by physical and radiographic examination in a dog which had signs of anorexia, vomiting, diarrhea and dehydration The dog was treated by surgical method. String-like foreign body was located from stomach to upper jejunum. Diffuse laceration, inflammation and perforation caused by foreign body were observed in intestine. Intestinal resection was carried out to remove the foreign body from upper duodenum to upper jejunum. The dog was convalesced successfully after operation.

  • Annular pancreas is a rare congenital anomaly with the descending duodenum encircled by a ring of pancreatic tissue, which may cause partial or complete obstruction of the duodenum. In newborn, the symptoms can be those of duodenal stasis resulting from partial intestinal obstruction secondary to some degree of duodenal stenosis. A male newborn weighing 2.0 Kg was born by C-section delivery at 37 weeks' gestation to a 27-year-old mother who had a hydramnios. He was in relatively good condition at birth except regurgitation of saliva and intermittent apnea. A plain film of the abdomen showed the double-bubble of gas filled stomach and proximal duodenum, and upper gastrointestinal series showed a dilated proximal duodenum, with a complete obstruction of the descending duodenum. Intraoperative finding revealed encirclement of the duodenal second portion by pancreatic tissue. Duodenojejunostomy was performed. After the operation, he had developed two serious complications, neonatal septicemia by Enterobacter cloacae on postoperative day 12 and systemic candidiasis on postoperative day 19, and been managed with ventilatory support, antibiotics, and antifungal agents with recovery.

Bowel Sound Recording for Structural Deformity in the Pylorus-Duodenum: Report of 3 Cases (각기 다른 유문-십이지장의 기질적 병변 3증례에 대한 장음 측정 보고 : 장음의 주주파수를 중심으로)

  • Objectives: The purpose of this study is to report bowel sound patterns recorded in 3 diseases associated with structural deformity of the pylorus-duodenum. Methods: : Bowel sound recording is a useful non-invasive tool for the evaluation of gastrointestinal motility. However, the difficulty in manual attachment of the electronic stethoscope on the abdominal wall and noise production against bowel sound signals have prohibited its widespread use. Therefore, I developed a new apparatus that eliminates the noise; it contains a sound absorbing device and a holder for the head of stethoscope. Using the new bowel sound recording system, bowel sounds of the patients were recorded repeatedly. The endoscopic and abdominal computed tomography findings were obtained from other hospitals' medical record, and all patients were confirmed to have structural deformity in the pylorus-duodenum. The records of each patient were compared and assessed. Results: : Patients had either duodenal ulcer scar, pyloric stricture, or far advanced malignant stomach cancer, as diagnosed by the gastroscopy. Their dominant frequency of bowel sound obtained from the new system was checked more than 2 times at regular intervals. All 8 recordings in the 2 patients with duodenal ulcer scar (5 times) or pyloric stricture (3 times) showed a decrease in postprandial than fasting dominant frequency. One patient with stomach cancer showed no significant change between postprandial and fasting states at 2 recordings. Conclusions: The analysis of dominant frequency in bowel sounds can indicate the existence of pyloric obstruction that could delay gastric emptying.
