EEA Stapler 를 이용한 식도암 수술의 조기 성적 (Early Postoperative results of Esophageal Carcinoma using EEA Stapler)

  • The introduction of end-to-end anastomosis stapler [EEA stapler] into esophageal surgery has reduced the mortality and morbidity associated with esophageal resection mainly owing to a reduction in the incidence of accidence of anastomotic leak. We now report the results of the 37 patients undergoing esophagectomy or esophagogastrectomy with EEA stapler in the department of cardiothoracic Surgery, Kosin Medical Center No leakage was demonstrated in the 37 esophagogastric anastomotic sites, but dehiscence of the TA stapled gastrotomy suture line occurred in two patient One patient was recovered with conservative treatment but the other patient was died due to hepatic metastasis of esophageal carcinoma during conservative treatment. And one severe esophagogastric anastomotic stricture was developed at the 30th postoperative day, but improved after dilatation with balloon dilatation catheter of Swiss Med. Tech. Company. The other complications were postoperative pneumonia[1 case], wound disruption and infection[3 cases], but all of 4 patients were recovered with conservative treatment. We experienced relative good postoperative results with use of EEA stapler in esophageal surgery. On the basis of our experience, we believe that the EEA stapler has definite place in esophageal surgery.

  • A comparative study was taken for pulmonary resection between group stapler used and not used. 1 ] There was no bronchopleural fistula in 22 cases of stapler used group, but in 4.5% in not used group. 2] The postoperative amount of fluid and air leakage through the chest tube were lesser in stapler used group. 3] The operating time was shorter in stapler used group because of lesser dissection of hilar structure.

Meta-analysis and trial sequential analysis of pancreatic stump closure using a hand-sewn or stapler technique in distal pancreatectomy

  • This study aimed to compare outcomes of hand-sewn and stapler closure techniques of pancreatic stump in patients undergoing distal pancreatectomy (DP). Impact of stapler closure reinforcement using mesh on outcomes was also evaluated. Literature search was carried out using multiple data sources to identify studies that compared hand-sewn and stapler closure techniques in management of pancreatic stump following DP. Odds ratio (OR) was determined for clinically relevant postoperative pancreatic fistula (POPF) via random-effects modelling. Subsequently, trial sequential analysis was performed. Thirty-two studies with a total of 4,022 patients undergoing DP with hand-sewn (n = 1,184) or stapler (n = 2,838) closure technique of pancreatic stump were analyzed. Hand-sewn closure significantly increased the risk of clinically relevant POPF compared to stapler closure (OR: 1.56, p = 0.02). When stapler closure was considered, staple line reinforcement significantly reduced formation of such POPF (OR: 0.54, p = 0.002). When only randomized controlled trials were considered, there was no significant difference in clinically relevant POPF between hand-sewn and stapler closure techniques (OR: 1.20, p = 0.64) or between reinforced and standard stapler closure techniques (OR: 0.50, p = 0.08). When observational studies were considered, hand-sewn closure was associated with a significantly higher rate of clinically relevant POPF compared to stapler closure (OR: 1.59, p = 0.03). Moreover, when stapler closure was considered, staple line reinforcement significantly reduced formation of such POPF (OR: 0.55, p = 0.02). Trial sequential analysis detected risk of type 2 error. In conclusion, reinforced stapler closure in DP may reduce risk of clinically relevant POPF compared to hand-sewn closure or stapler closure without reinforcement. Future randomized research is needed to provide stronger evidence.

식도 재건 수술후 발생한 합병증 치험 (Treatment of the complications of the esophageal reconstructive procedures)

  • Post-anastomotic leakage and stenoses remain major complications and are still responsible for many mortalities after esophageal reconstructive procedures. If the hand-suture is used, anastomotic leaks developed frequently than stenoses. But post-anastomotic leakages are more critical than post-anastomotic stenoses. If the stapler is used, anastomotic stenoses will develop frequently and not critical than anastomotic leakages. The stapler suture method is easier and quicker than the hand suture method in the esophageal anastomotic procedures. But the disadvantages of the stapler suture method is that there is not reachable site with anastomotic stapler the thoracic inlet region and the cost is expensive. We have treated 44 cases of the surgical complications after esophageal reconstructive procedure with conservative and surgical treatment for 10 years from January, 1980 to December, 1989. The anastomotic site stenoses were 8 cases in the hand-suture methods and 4 cases in the stapler-suture methods. The anastomotic leaks were 8 cases in hand-suture methods and 5 cases in stapler-suture methods. There were one death in the surgical repair of four post-operative anastomotic stenoses and two deaths in the surgical repair of three post-operative anastomotic leakages. Ever though we consider that there is more anastomotic leakage than stenoses after the hand-suture methods in esophageal reconstructive procedures. the cost with long stay in the intensive care unit to treat anastomotic leakage after the hand-suture, would be more expensive than-the cost of the treatment of the anastomotic stenoses after auto-suture.

Factors Affecting the Number of Stapler Cartridges in Complete Video-Assisted Thoracoscopic Surgery Lobectomy for Non-small Cell Lung Cancer

  • Background: Video-assisted thoracoscopic surgery (VATS) lobectomy has become the major surgical option for the treatment of non-small cell lung cancer (NSCLC). Endoscopic instruments such as stapler cartridges are essential for VATS procedures. In this study, we investigated the factors that affect the number of stapler cartridges used in VATS lobectomy. Methods: A retrospective analysis was conducted of patients who underwent complete VATS lobectomy for NSCLC from January 2013 to December 2015. Results: In total, 596 patients underwent complete VATS lobectomy. The average number of stapler cartridges used for VATS lobectomy was 5.3±1.9. The number of stapler cartridges used for VATS lobectomy was higher in men (5.5±1.9 vs. 5.0±18, p=0.006), those aged older than 70 years (5.5±2.1 vs. 5.1±1.7, p=0.038), those who underwent upper or middle lobectomy procedures (5.7±1.9 vs. 4.1±1.2, p<0.001), those with a higher fissure sum average (p<0.001), and those in whom surgery was performed by a surgeon with a preference for staplers (5.6±2.0 vs. 4.9±1.6, p<0.001). Conclusion: The number of stapler cartridges required to perform VATS lobectomy in NSCLC patients appears to be influenced by sex, age, the location of the tumor, the degree of fissure development, and the surgeon's preference.

식도 재건술 시 흉강경용기계문합기을 이용한 경부식-장문합술 (Cervical Esophago-Enteric Anastomosis with Straight Endostapler)

  • Background: The use of the stapler n esophageal reconstruction after esophageal resection for benign or malignant esophageal diseases has become popular because it has less leakage at the anastomotic site and shorter operation time than manual sutures. However, the use of classic circular stapler has some complications such as stenosis and dysphagia that requires additional treatment. Such complications are closely related to the inner diameter of the anastomotic sites. In this study, the diameter of anastomotic site was compared after the use of circular stapler(EEA) and straight endoscopic stapler(endo GIA). Material and Method: The patients who received esophageal reconstruction by stapler from August 1995 to September 1997 were reviewed. The patients were divided into 2 groups. One group need the circular stapler, and the other group the straight endo GIA(14 cases with endo GIA 30mm, 24 with endo GIA 45mm). After a cervical esophago-enteric anastomosis, the stricture of anastomotic site and the incidence of dysphagia were compared between the 2 groups using an esophagography and the patient's symptoms. The follow-up period was 12months in average. Result: In the former group in which the circular stapler was used, 2 cases of anastomotic stenosis were reported. In comparison, none were reported in the latter group. Dysphagia were reported in 8 cases of the former group, and in 3 cases of the latter group(1 case in endo GIA 30 mm, 2 cases in endo GIA 45 mm). Conclusion: The use of endo GIA in esophago-enteric anastomosis resulted in a wider diameter of the anastomotic site, lesser stricture, and lesser incidence of dysphagia compared to the use of former circular stapler. Therefore, it is thought to be a better method in esophageal reconstructions.

  • Since Steichen and Ravich`s pioneer work in 1972 proved that staples reduced anastomotic leaks and operative time, the use of EEA stapler`s in esophagogastrostomy have gained acceptance and popularity. But overriding these benefits are the high stricture rate, which leads to the reappearance of dysphagia. The mechanism for the development of stricture in stapled anastomosis is likely to be due to the lack of mucosa to mucosa apposition and presence of necrotic tissue between the luminal edge and the rows of the stapler. All strictures were easily dilated. Recently, we encountered a patient who suffered from an esophageal stricture that slowly developed 10 months after an esophagogastric anastomosis with a EEA 425 was performed due to severe muscular hypertrophy of esophagus. Because the stricture failed to respond to the Bougienage, we reoperated using a EEA 28 this time. We feel that this case review helps to show that despite the very low leakage rate in small size EEA stapler, there is also a very high risk of stricture.

식도암에 있어서 식도 절제술 및 재건술 후 문합 위치와 방법에 따른 문합 부위 누출과 협착 (Anastomotic Leakage and Stricture Relating to Anastomotic Level and Methods in Esophageal Resection and Reconstruction for Esophageal Cancer)

  • 배경: 식도암 수술에 있어서 문합 부위 합병증의 발생은 문합 방법이나 위치와 관계가 있는 것으로 알려져 있다. 이에 저자들은 식도암으로 식도절제 및 식도재건술을 시행 받은 환자에서 문합 방법 및 위치와 누출 또는 협착 발생과의 연관성에 대해 조사하였다. 대상 및 방법: 1993년 8월부터 2003년 5월까지 식도암으로 식도재건술을 시행 받은 321명의 환자를 대상으로 하여 문합 방법, 문합 위치 ,합병증의 발생에 대해 조사하였다. 환자군의 평균 연령은 64.5$\pm$4.9세(37${\~}$94세)였고, 성비는 남자 3001 명($93.5\%$), 여자 21명($6.5\%$)이었다. 결과: 문합부위의 누출은 7예($2.2\%$)에서 발생하였다. 부위별로는 경부 문합에서 3예($4.1\%$), 흉부 문합에서 4예($1.6\%$)로 문합 위치에 따른 차이는 없었다. 문합 방법은 stapler 봉합을 한 경우에서 4예($1.6\%$), semi-stapler 봉합을 한 경우에서 3예($9.1\%$), 수봉합한 경우는 0예($0.0\%$)로 문합 방법에 따른 누출의 차이가 없었다. 문합 부위의 협착은 52예($16.2\%$)에서 발생하였고, 부위별로는 경부 문합에서 2예($2.7\%$), 흉부 문합에서 50예($20.2\%$)로 문합 위치에 따른 차이가 있었다(p<0.001). 문합 방법별로는 stapler봉합이 49예($20.0\%$), semi-stapler봉합 1예($3.0\%$), 수봉합 2예($4.7\%$)로 stapler 봉합에서 유의하게 높았다(p<0.001). 결론: 식도암 환자의 식도재건술에 있어서 문합방법은 문합 부위 누출에 의미 있는 영향을 미치지 않았다. 그러나, 문합 부위의 협착과 관련하여 stapler 봉합은 semi-stapler 봉합 및 수봉합보다 높은 빈도로 협착이 나타났다. 그러므로 stapler 문합시 협착을 줄이기 위해서는 문합 방법의 개선이 필요하다고 생각한다.

개의 장문합술에 있어 봉합 문합술과 Stapler 문합술의 비교 (Comparison of Sutured Intestinal Anastomosis and Stapler Intestinal Anastomosis in Dogs)

  • This study was carried out to investigate the difference between sutured anastomosis and stapler anastomosis (open lumina technique) of jejunum in dogs. Fifteen mongrel-breed female dogs weighting 4 to 6 kgs were allocated to three groups; sutured end-to-end anastomosis (Group I), sutured side-to-side anastomosis (Group II) and stapler anastomosis (Group III), five dogs per each group. All dogs in different anastomosis pattern were compared with time for total operation and suture elapsed for intestines to anastomose, clinical signs, status of feces, complications for 14 days after operation. The total operation time and suture time needed for intestinal anastomosis were significantly(p<0.05) shorter in Group III than Group I and II. All dogs showed no significant difference in vitality, appetite, vomiting between groups for 14 days after operation. All dogs, except one dog in Group II, showed normal vitality and appetite since 7-8 days after operation. Initial return of fecal passage showed in all dogs 8 days after operation and thereafter normal feces were observed in most of the dogs. According to results, all dogs with normal vitality and appetite for 8 days showed good prognosis. In complications after operation, only one dog in Group II showed dehiscence of anastomotic site. There was significant(p<0.05) differences between groups in speed of operation. And all dogs, except one dog in Group II, showed good clinical condition and prognosis. In conclusion, the stapler anastomosis is considered to be more reliable, faster, and precision method compared to the sutured anastomosis for intestinal anastomosis in dogs.

Surgical Resection of Cardiac Hemangiosarcoma Using Articulated Surgical Stapler in a Dog

  • This case report discusses the successful surgical resection of cardiac hemangiosarcoma in a dog using an articulated surgical stapler. Cardiac tumors, particularly hemangiosarcomas, have a poor prognosis. Recommended treatment involves surgical removal of the primary tumor, along with chemotherapy. However, the use of staples for cardiac tumor resection has not yet been extensively documented in the veterinary literature. A 10-year-old dachshund with pericardial effusion underwent surgery to remove a right atrial mass. An articulated linear cutting stapler was used for tumor resection. The patient experienced chylothorax as a complication but recovered well. However, the patient later developed respiratory symptoms and died, most likely due to aspiration pneumonia. The surgical stapler provided stability, convenience, and a shorter surgical time during tumor resection. Surgical resection combined with chemotherapy improves the survival of dogs with cardiac hemangiosarcoma. The stability and adjustability of the stapling device make it advantageous for cardiac tumor resection. Although complications, such as chylothorax, can arise, appropriate management can lead to positive outcomes. This case report demonstrates the feasibility and safety of using an articulated surgical stapler for cardiac tumor resection in dogs. Insights from this case can guide future research and clinical practice.
