Trend of Intensive Care Unit Admission in Neurology-Neurosurgery Adult Patients in South Korea : A Nationwide Population-Based Cohort Study

  • Objective : We aimed to examine trends in critically ill neurology-neurosurgery (NNS) patients who were admitted to the intensive care unit (ICU) in South Korea and identify risk factors for in-hospital mortality after ICU admission in NNS patients. Methods : This nationwide population-based retrospective cohort study enrolled adult NNS adult patients admitted to the ICU from 2010 to 2019 extracted from the National Health Insurance Service in South Korea. The critically ill NNS patients were defined as those whose main admission departments were neurology or neurosurgery at ICU admission. The number of ICU admission, age, and total cost for hospitalization from 2010 to 2019 in critically ill NNS patients were examined as trend information. Moreover, multivariable logistic regression modeling was used to identify risk factors for in-hospital mortality among critically ill NNS patients. Results : We included 845474 ICU admission cases for 679376 critically ill NNS patients in South Korea between January 1, 2010 to December 31, 2019. The total number of ICU admissions among NNS patients was 79522 in 2010, which increased to 91502 in 2019. The mean age rose from 62.8 years (standard deviation [SD], 15.6) in 2010 to 66.6 years (SD, 15.2) in 2019, and the average total cost for hospitalization per each patient consistently increased from 6206.1 USD (SD, 5218.5) in 2010 to 10745.4 USD (SD, 10917.4) in 2019. In-hospital mortality occurred in 75455 patients (8.9%). Risk factors strongly associated with increased in-hospital mortality were the usage of mechanical ventilator (adjusted odds ratio [aOR], 19.83; 95% confidence interval [CI], 19.42-20.26; p<0.001), extracorporeal membrane oxygenation (aOR, 3.49; 95% CI, 2.42-5.02; p<0.001), and continuous renal replacement therapy (aOR, 6.47; 95% CI, 6.02-6.96; p<0.001). In addition, direct admission to ICU from the emergency room (aOR, 1.38; 95% CI, 1.36-1.41; p<0.001) and brain cancer as the main diagnosis (aOR, 1.30; 95% CI, 1.22-1.39; p<0.001) are also potential risk factors for increased in-hospital mortality. Conclusion : In South Korea, the number of ICU admissions increased among critically ill NNS patients from 2010 to 2019. The average age and total costs for hospitalization also increased. Some potential risk factors are found to increase in-hospital mortality among critically ill NNS patients.

Time Trends of Nasopharyngeal Carcinoma in Urban Guangzhou over a 12-Year Period (2000-2011): Declines in Both Incidence and Mortality

  • Nasopharyngeal carcinoma (NPC) is an uncommon disease in most countries but occurs with much greater frequency in southern China. This study aimed to examine the secular trends of NPC in urban Guangzhou over the time period of 2000-2011 using data from the Guangzhou Cancer Registry. Age-adjusted annual incidence rates of NPC were calculated by the direct method using the WHO World Standard Population (1960) as the reference. The average annual percentage change (AAPC) was used as an estimate of the trend. A total of 7,532 new cases of NPC and 3,449 related deaths were registered. In both genders, the peak incidence occurred in the 50- to 59-year age group, and this age distribution pattern remained similar throughout. The AAPC in NPC incidence rates was -3.26% (95% CI: -5.4%--1.1) for males and -5.74% (95% CI: -8.9%--2.5) for females, resulting in a total decrease of 39.3% (from 22.14 to 13.44 per 100,000 population) for males and 48.6% (from 10.1 to 5.18 per 100,000 population) for females over this 12-year period. The AAPCs in NPC mortality rates were -4.62% (95%CI: -3.5%--5.7) for males and -6.75% (95% CI: -5.2%--8.3) for females, resulting in a total decrease of -46.1% (from 12.1 to 6.54 per 100,000 population) for males and 51.7% (from 4.14 to 2.00 per 100,000 population) for females. The age-adjusted incidence and mortality rates of NPC declined during 2000-2011 in urban Guangzhou but remained high. Future efforts to improve prevention, early detection and treatment strategies are needed.

  • Relationship between the daily mortality and total suspended particulate(TSP) for the period of 1996-1997 was examined in Seoul, Korea. Generalized additive model was adapted for the analysis to allow the seasonality and long-term time effect in the air pollution and the nonlinear relationships with weather variables such as air temperature and relative humidity. An increase of 100 Ji9/m') TSP corresponds to 0.8-5.2 percent increase of the daily mortality Unidirectional (prospective and retrospective) case-crossover design results in large bias of the estimates. Bidirectional method gives relatively unbiased estimates but the standard errors are larger Case-crossover design should be used with caution in the situation where the data have seasonal trends like air pollution study.

Development of Composite Deprivation Index for Korea: The Correlation with Standardized Mortality Ratio (표준화사망비와 지역결핍지수의 상관관계: 지역사회 통합결핍지수 개발)

  • Objectives : The aims of this paper were to develop the composite deprivation index (CDI) for the sub-district (Eup-Myen-Dong) levels based on the theory of social exclusion and to explore the relationship between the CDI and the standardized mortality ratio (SMR). Methods : The paper calculated the age adjusted SMR and we included five dimensions of social exclusion for CDI; unemployment, poverty, housing, labor and social network. The proxy variables of the five dimensions were the proportion of unemployed males, the percent of recipients receiving National Basic Livelihood Security Act benefits, the proportion of households under the minimum housing standard, the proportion of people with a low social class and the proportion of single-parent household. All the variables were standardized using geometric transformation and then we summed up them for a single index. The paper utilized the 2004-2006 National Death Registry data, the 2003-2006 national residents' registration data, the 2005 Population Census data and the 2005-2006 means-tested benefit recipients' data. Results : The figures were 115.6, 105.8 and 105.1 for the CDI of metropolitan areas (big cities), middle size cities and rural areas, respectively. The distributional variation of the CDI was the highest in metropolitan areas (8.9 - 353.7) and the lowest was in the rural areas (26.8 - 209.7). The extent and relative differences of deprivation increased with urbanization. Compared to the Townsend and Carstairs index, the CDI better represented the characteristics of rural deprivation. The correlation with the SMR was statistically significant and the direction of the CDI effects on the SMR was in accordance with that of the previous studies. Conclusions : The study findings indicated mortality inequalities due to the difference in the CDI. Despite the attempt to improve deprivation measures, further research is warranted for the consensus development of a deprivation index.

  • 대기오염과 사망율과의 관련성을 평가하고자 미국내 23개 SMSA(Standard Metropolitan Statistical Areas) 지역에서 대기오염의 程度를 나타내는 指數인 PSI(Pollutant Standards Index), 氣象學的 變數 및 사회 경제적 변수를 이용하여 多變量 分析을 시도하였다. 그 결과 총사망율과의 관련성은 사회경제적변수가 영향이 큰 반면 PSI는 작은 것으로 示俊되었다. 대기오염과 사망율과의 연관성에 관한 평가는 많은 변수의 復雜性이 내포하고 있어 좀더 具體的이고 세밀한 분석이 기대된다.

  • ${\beta}$-blockers are considered as standard therapy for patients with stable chronic heart failure (CHF) and to prolong survival and reduce hospitalizations. We examined the effects of the ${\beta}$-blocker on mortality, hospitalization and symptoms in patients with CHF and the related factors to the use of ${\beta}$-blockers. Patients in New York Heart Association class II-IV were included if they were treated for heart failure from January 2002 to June 2002. At baseline, 6 months, and 12 months, they were assessed for the change of NYHA class and all deaths and hospital admissions. (omitted)

  • Reproductive performance of gilts and sows in ordinary piggeries was investigated with special reference to litter size, piglets weaned per litter, the cause of preweaning mortality and the incidence of diarrhea. Biochemical properties of Escherichia coli isolated from piglets with diarrhea were also determined. The results obtained are summarised as follows: 1. Of the 2,226 gilts and sows farrowed, average number of piglets born alived and weaned piglets per litter were 8.9 and 7.1, respectively, indicating that both gilts and sows lost more than 20% of all their piglets prior to weaning. 2. The causes of preweaning mortality in order of prevalence were diarrhea (39.3%), pneumonia (20.0%), crushing (13.8%), starvation (11.0%) and born weak (10.3%). 3. Incidence of 3 diarrheal syndromes of piglets, i.e. 1 week diarrhea, 3 week diarrhea and post-weaning diarrhea were 18.4%, 66.1% and 15.5%, respectively, showing that most farms were suffering from so called 'white scours' in piglets 14~28 days old. 4. Biochemical properties of 268 cultures of E. coli isolated from diarrheal piglets were tested and compared with those of standard strains of porcine origin. All those properties of isolates were matched to standard E. coli teated while variable results were obtained with haemolytic capabilities of cultures tested. 5. Of 147 isolates 16 cultures (10.9%) were identified as colicin producers.

Heart Rate Variability and Metabolic Syndrome in Hospitalized Patients with Schizophrenia

  • Purpose: Reduced heart rate variability significantly increases cardiovascular mortality. Metabolic syndrome increases the cardiac autonomic dysfunction. Recently, increasing cardiovascular mortality has been reported in patients with schizophrenia. This study was done to compare heart rate variability between adults with and without schizophrenia and to compare the relationship of heart rate variability to metabolic syndrome in hospitalized patients with schizophrenia. Methods: This was a descriptive and correlational study in which 719 adults without schizophrenia and 308 adults with schizophrenia took part between May and June 2008. We measured the following: five-minute heart rate variability; high-frequency, low-frequency, the ratio of low-frequency to high-frequency, and the Standard Deviation of all the normal RR intervals. Data was also collected on metabolic syndrome, abdominal obesity, triglycerides, HDL cholesterol, blood pressure and fasting glucose. Results: The Standard Deviation of all the normal RR intervals values of heart rate variability indices were $1.53{\pm}0.18$. The low-frequency and high-frequency values of heart rate variability indices were significantly higher in hospitalized patients with schizophrenia ($3.89{\pm}1.36$; $3.80{\pm}1.20$) than those in the healthy participants ($2.20{\pm}0.46$; $2.10{\pm}0.46$). There were no significant differences between the schizophrenic patients with and without metabolic syndrome. Conclusion: The results of this study indicate that schizophrenia patients have significantly lower cardiac autonomic control, but they have significantly higher low-frequency and high-frequency values than those of healthy adults. Use of antipsychotic drug may affect the autonomic nervous system in schizophrenic patients. Metabolic syndrome was not associated with cardiac autonomic control in schizophrenia patients.

Information Engineering and Workflow Design in a Clinical Decision Support System for Colorectal Cancer Screening in Iran

  • Background: Colorectal cancer is a major cause of morbidity and mortality throughout the world. Colorectal cancer screening is an optimal way for reducing of morbidity and mortality and a clinical decision support system (CDSS) plays an important role in predicting success of screening processes. DSS is a computer-based information system that improves the delivery of preventive care services. The aim of this article was to detail engineering of information requirements and work flow design of CDSS for a colorectal cancer screening program. Materials and Methods: In the first stage a screening minimum data set was determined. Developed and developing countries were analyzed for identifying this data set. Then information deficiencies and gaps were determined by check list. The second stage was a qualitative survey with a semi-structured interview as the study tool. A total of 15 users and stakeholders' perspectives about workflow of CDSS were studied. Finally workflow of DSS of control program was designed by standard clinical practice guidelines and perspectives. Results: Screening minimum data set of national colorectal cancer screening program was defined in five sections, including colonoscopy data set, surgery, pathology, genetics and pedigree data set. Deficiencies and information gaps were analyzed. Then we designed a work process standard of screening. Finally workflow of DSS and entry stage were determined. Conclusions: A CDSS facilitates complex decision making for screening and has key roles in designing optimal interactions between colonoscopy, pathology and laboratory departments. Also workflow analysis is useful to identify data reconciliation strategies to address documentation gaps. Following recommendations of CDSS should improve quality of colorectal cancer screening.

Durability of the Low Profile Ionescu-Shiley Valve in the Mitral Position (이오네스큐 단고형 승모판의 내구성)

  • The low profile Ionescu-Shiley pericardial xenograft valve had been initially introduced to have improved hemodynamic performance and expected superior durability, However, subsequent clinical reports and laboratory studies have repeatedly indicated early failure of the valve from mechanical reasons in valve design, A total and consecutive 169 patients underwent isolated MVR with the low profile Ionescu-Shiley valve at Seoul National University Hospital from November 1984 to October 1989 [Group ISLM]. Five patients [3.0%] died within 30 days of surgery, and early survivors were followed up for 734.9 patient-years [Mean$\pm$SD, 4.9$\pm$2.0 years], Late mortality rate was 0.136% /patient-year [pt-yr], Linearized incidences of major complications were: 0.953% embolism /pt-yr; 0.680% bleeding /pt-yr; 0.272% endocarditis /pt-yr; and 0, 408% primary tissue failure [PTF] /pt-yr, The actuarial survival was 99.4$\pm$0.6%, and probabilities of from thromboembolism and from PTF were 90.7$\pm$4.6% and 95.9$\pm$2.6% at 8 years, respectively These results were compared with the clinical data from the 291 patients of isolated MVR with the standard Ionescu-Shiley valve at the same hospital during the period between October 1978 and June 1983 [Group ISUM], Group ISUM included 11% of patients younger than 15 years of age, whereas Group ISLM did no patient of this age. The mean age at the time of surgery was significantly younger in Group ISUM than one in Group ISLM [P<0.001]. Differences were not statistically significant in operative mortality, mean follow-up period and linearized incidences of thromboembolism, bleeding and endocarditis, between two groups, However, the linearized rate of PTF was lower [P<0.001] and probabilities of freedom fro PTF higher [P<0.001] in Group ISLM at least up to postoperative 8 years. While the features of PTF of the standard valve was predominantly degeneration with calcification and stenosis, the ones of the low profile valve was cusp tear and incompetence. It remains, however, to be seen whether the low profile valve fails mostly from mechanical reason, and further follow-up will still be necessary to determine the differences in mode of PTF of two valves.
