Lung cancer has become a leading cause of cancer death in Korea. Accurate staging of non-small cell lung cancer (NSCLC) is essential to the ability to offer a patient the most effective available treatment and the best estimate of prognosis. PET with F-18 fluorodeoxyglucose (FDG) is indicated for the nodal staging of NSCLC and detection of distant metastases. Use of PET for mediastinal staging should not be relied on as a sole staging modality, and positive findings should be confirmed by mediastinoscopy. FDG PET avoids futile surgery by a more accurate selection of patients, especially by the detection of unexpected distant metastases.
Since histopathological examination was known to be the most sensitive evaluation for testicular toxicity, regulatory authorities have been published the guidelines on practical testicular assay approach. Those guidelines specified details of evaluation including fixation, embedding, stain-ing, histological examination and also seminiferous tubular staging methods. However, there have been confusing understanding among toxicologists and even pathologists on staging theory and its application on industrial testicular toxicity. Guidelines did not intend to conduct quantitative assay with staging but recommended the use of knowledge of staging. To count each tubular stage with statistical analysis is known to be time consuming and labor burdening work but the significance of toxicity has little value. It also has been pointed out that the application of staging theory for longer-term toxicity considered to be lacking of rationale. It could be recommended that qualitative assay with aware-ness of germ cell loss is more efficient method rather than quantitative counting of each tubular stage. Therefore it would be required that comprehensive understanding of testicular toxicity evaluation and the use of testicular staging method.
Purpose: The modification of the cancer classification system aimed to improve the classical anatomy-based tumor, node, metastasis (TNM) staging by considering tumor biology, which is associated with patient prognosis, because such information provides additional precision and flexibility. Materials and Methods: We previously developed an mRNA expression-based single patient classifier (SPC) algorithm that could predict the prognosis of patients with stage II/III gastric cancer. We also validated its utilization in clinical settings. The prognostic single patient classifier (pSPC) differentiates based on 3 prognostic groups (low-, intermediate-, and high-risk), and these groups were considered as independent prognostic factors along with TNM stages. We evaluated whether the modified TNM staging system based on the pSPC has a better prognostic performance than the TNM 8th edition staging system. The data of 652 patients who underwent gastrectomy with curative intent for gastric cancer between 2000 and 2004 were evaluated. Furthermore, 2 other cohorts (n=307 and 625) from a previous study were assessed. Thus, 1,584 patients were included in the analysis. To modify the TNM staging system, one-grade down-staging was applied to low-risk patients according to the pSPC in the TNM 8th edition staging system; for intermediate- and high-risk groups, the modified TNM and TNM 8th edition staging systems were identical. Results: Among the 1,584 patients, 187 (11.8%), 664 (41.9%), and 733 (46.3%) were classified into the low-, intermediate-, and high-risk groups, respectively, according to the pSPC. pSPC prognoses and survival curves of the overall population were well stratified, and the TNM stage-adjusted hazard ratios of the intermediate- and high-risk groups were 1.96 (95% confidence interval [CI], 1.41-2.72; P<0.001) and 2.54 (95% CI, 1.84-3.50; P<0.001), respectively. Using Harrell's C-index, the prognostic performance of the modified TNM system was evaluated, and the results showed that its prognostic performance was better than that of the TNM 8th edition staging system in terms of overall survival (0.635 vs. 0.620, P<0.001). Conclusions: The pSPC-modified TNM staging is an alternative staging system for stage II/III gastric cancer.
Background: Precise staging of lung cancer is usually evaluated by PET-CT and brain MRI. Recently, however, whole-body diffusion-weighted magnetic resonance imaging (WB-DWI) has be applied. The aim of this study is to determine whether the diagnostic performance of lung cancer staging by WB-DWI is superior to that of PET-CT+brain MRI. Materials and Methods: PET-CT + brain MRI and WB-DWI were used for lung cancer staging before surgery with 59 adenocarcinomas, 16 squamous cell carcinomas and 6 other carcinomas. Results: PET-CT + brain MRI correctly identified the pathologic N staging in 67 patients (82.7%), with overstaging in 5 (6.2%) and understaging in 9 (11.1%), giving a staging accuracy of 0.827. WB-DWI correctly identified the pathologic N staging in 72 patients (88.9%), with overstaging in 1 (1.2%) and understaging in 8 patients (9.9%), giving a staging accuracy of 0.889. There were no significant differences in accuracies. PET-CT + brain MRI correctly identified the pathologic stages in 56 patients (69.1%), with overstaging in 7 (8.6%) and understaging in 18 (22.2%), giving a staging accuracy of 0.691. WB-DWI correctly identified the pathologic stages in 61 patients (75.3%), with overstaging in 4 (4.9%) and understagings in16(19.7%), giving a staging accuracy of 0.753. There were no significant difference in accuracies. Conclusions: Diagnostic efficacy of WB-DWI for lung cancer staging is equivalent to that of PET-CT + brain MRI.
The unsteady flow analysis of staging system is conducted. This study focuses on comparing the results of two different governing equations between Euler equations and Navier-Stokes equations. The Chimera grid scheme is applied to moving simulations for unsteady flow analysis with dynamic simulation. As a result, it is certified that inviscid simulation have capabilities enough to analyze the present staging problem.
Purpose: We evaluated prognostic value of the 8th edition of the American Joint Committee on Cancer/International Union for Cancer Control (AJCC/UICC) staging system for nasopharyngeal cancer and investigated whether tumor volume/metabolic information refined prognostication of anatomy based staging system. Materials and Methods: One hundred thirty-three patients with nasopharyngeal cancer who were staged with magnetic resonance imaging (MRI) and treated with intensity-modulated radiotherapy (IMRT) between 2004 and 2013 were reviewed. Multivariate analyses were performed to evaluate prognostic value of the 8th edition of the AJCC/UICC staging system and other factors including gross tumor volume and maximum standardized uptake value of primary tumor (GTV-T and SUV-T). Results: Median follow-up period was 63 months. In multivariate analysis for overall survival (OS), stage group (stage I-II vs. III-IVA) was the only significant prognostic factor. However, 5-year OS rates were not significantly different between stage I and II (100% vs. 96.2%), and between stage III and IVA (80.1% vs. 71.7%). Although SUV-T and GTV-T were not significant prognostic factors in multivariate analysis, those improved prognostication of stage group. The 5-year OS rates were significantly different between stage I-II, III-IV (SUV-T ≤ 16), and III-IV (SUV-T > 16) (97.2% vs. 78% vs. 53.8%), and between stage I, II-IV (GTV-T ≤ 33 mL), and II-IV (GTV-T > 33 mL) (100% vs. 87.3% vs. 66.7%). Conclusion: Current anatomy based staging system has limitations on prognostication for nasopharyngeal cancer despite the most accurate assessment of tumor extent by MRI. Tumor volume/metabolic information seem to improve prognostication of current anatomy based staging system, and further studies are needed to confirm its clinical significance.

Elevated water tanks are considered as important structures due to its post-earthquake requirements. Elevated water tank on reinforced concrete frame staging is widely used in India. Different response reduction factors depending on ductility of frame members are used in seismic design of frame staging. The study on appropriateness of response reduction factor for reinforced concrete tank staging is sparse in literature. In the present paper a systematic study on estimation of key components of response reduction factors is presented. By considering the various combinations of tank capacity, height of staging, seismic design level and design response reduction factors, forty-eight analytical models are developed and designed using relevant Indian codes. The minimum specified design cross section of column as per Indian code is found to be sufficient to accommodate the design steel. The strength factor and ductility factor are estimated using results of nonlinear static pushover analysis. It was observed that for seismic design category 'high' the strength factor has lesser contribution than ductility factor, whereas, opposite trend is observed for seismic design category 'low'. Further, the effects of staging height and tank capacity on strength and ductility factors for two different seismic design categories are studied. For both seismic design categories, the response reduction factors obtained from the nonlinear static analysis is higher than the code specified response reduction factors. The minimum dimension restriction of column is observed as key parameter in achieving the desired performance of the elevated water tank on frame staging.
Cancer of the head and neck is an uncommon disease accounting for 5 % of all cancers. In an anatomic area so readily visible and palpable for examination without special and expensive diagnositic tools, it is unfortunate that many patients still present with advanced diseases. Since the prognosis is so intimately related to stage of disease, it is very important to detect the earliest stage of cancer with a complete head and neck examination. In the evaluation of cancer at any anatomic site, the description of the extent of the lesion is important. Not only does proper staging of the tumor lead to make decision of the most appropriate treatment, it also serves as a guide for the results of treatment. Proper staging demands a careful clinical assesment of the extent of the cancer. The current staging system for head and neck cancer uses the TNM system devised by American Joint Committee for Cancer Staging and End Result Reporting. T represent the primary tumor, N, regional nodal metastases, and M, distant metastases. The detection, diagnosis, and appropriate treatment of eary cancer will result in improved survival.
The aim of this work is to identify application of ultra low NOx and CO combustor. To achieve this, we developed the premixed oxidizer-staging combustor using a cyclone flow. Various factors such as equivalence ratio for the combustion condition and swirl type for secondary air injection have been tested experimentally for flame stability and NOx, CO emission characteristics. Before to do this, we had been tested cyclone premixed combustor in advance. it is similar to first combustor of premixed oxidizer-staging combustor. As a result, cyclone premixed flame shows the very high flame stability and low NOx emission. however, it can be identified that there were some problems such as a little high CO emission and thermal resistance of combustor wall. Cyclone premixed oxidizer-staging combustor can resolve those of problems. In our combustor, we can found out optimal condition that the secondary air injection method is swirl type, swirl direction is co-swirl and equivalence ratio of first combustor is 1.3. Quantitatively, we can achieve 10.8 ppm for NOx and 30.2 ppm for CO emissions respectively. Form this result, we can identified that cyclone premixed oxidizer-staging combustor can apply to ultra low NOx and CO combustor.
Numerical investigations on air staging and fuel staging were carried out with a newly designed coaxial cyclone combustor, which uses the method of two stage coal combustion composed of pre-combustor and main combustor. The pre-combustor with a high air/fuel ratio is designed to supply gas at high temperature to the main combustor. To avoid local high temperature region in this process, secondary air is injected in the downstream. Together with the burned gas supplied from the pre-combustor and the preheated air directly injected into main combustor, coals supplied through the main burner react rapidly at a low air/fuel ratio. Strong swirling motion of cyclone combustor keeps the wall temperature high, which makes slagging combustion possible. Alaska, US coal is used for calculations. Predictions were made for various coal flow rates in the main combustor for fuel staging and for the various flow rate of secondary air in the pre-combustor for air staging. In-scattering angles are also chosen as a variable to increase residence times of coal particles. Temperature fields and particle trajectories for various conditions are described. Predicted temperature variations at the wall of the combustor are compared with corresponding experimental data and show a similar trend. The in-scattering angle of 20° is recommended to increase the combustion efficiency in the main chamber.
