The stable isotopic composition of bone collagen plays an important role in reconstructing palaeodiet, nutrition, palaeoenvironment and their lifestyle. This is the first case in extracting palaeodietary information and breastfeeding pattern of Goryeo people using stable isotope analysis due to the lack of human remains in this period. We analyzed human bone collagen excavated from Gyeongju Donggung palace and Wolji pond No.3. The average values of δ13C and δ15N are as follows: (δ13C(‰) = -19.5 ± 0.9‰, δ15N(‰) = 11.1 ± 1.1 ‰, (n = 4). Stable carbon isotope values shows a mainly C3 based diet such as rice and barley. Stable nitrogen isotope results implies the protein sources attributed to terrestrial animals. There are various age groups in this study, which are adult, child and infant. Two individuals within early childhood age ranges (< 3 years) shows more elevated δ15N values than that of adult and this result implies the continuation of breastfeeding in this group until the age of 3. The results provide new insight into the breastfeeding pattern of Goryeo people, where breastfeeding and weaning practices have important implication for fertility, population dynamics, migration pattern and disease.
From August 1992 to July 1996, 63 consecutive patients underwent coronary artery bypass surgery. The mean age of these patient was 57 years(range form 30 to 71years). There were 44 men and 19 women. Preoperative 12 patients had stable angina pectoris and 23 patients were unstable angina pectoris. 8 patients had previous myocardial infarctation history and emergency or urgent myocardial revascularization were performed in 9 cases. In the risk factors of coronary atherosclerosis, 25 patients(40%) were hypercholesterolemia, 38 patients(60%) have smoking history and 19 patients(30%) have hypertension history. In the patterns of disease, 9 patients were single vessel disease, 18 patients were two vessele disease and 33 patients were three vessel disease. We performed total 284 distal anastomosis(mean 3.5 anastomosis per patient) and performed one case of ascending aorta graft interposition, two cases of mitral valve replacement, one case of aortic valve replacement, one case of ventricular septal defect repair and one case of atrial septal defect repair and the mean aortic cross clamp time was 115.3 minutes. The common complications were arrhythmia(7cases), wound infection(5cases), perioperative myocardial infarction(4cases), reoperation for bleeding control(4cases) and stroke(4cases). There were six hospital deaths due to low cardiac output syndrome, ventricular arrhythmia and respiratory failure. In the evaluation of operative risk factors, preoperative intravenous nitroglycerin requirement and prolonged aortic cross clamp tirne(>2hours) were found to be predective factor of morbidity and old age(>65years) was found to be predective factor of mortality.

Background: Gall bladder cancer (GBC) usually presents as unresectable or metastatic disease. We conducted a feasibility study to evaluate the effect of neoadjuvant therapy (NAT) on radiologic downstaging and resectability in unresectable GBC cases. Materials and Methods: Patients with locally advanced disease were treated with chemoradiotherapy [CTRT] ( external radiotherapy (45Gy) along with weekly concurrent cisplatin $35mg/m^2$ and 5-FU 500 mg) and those with positive paraaortic nodes were treated with neoadjuvant chemotherapy [NACT (cisplatin $25mg/m^2$ and gemcitabine $1gm/m^2$ day 1 and 8, 3 weekly for 3 cycles). Radiological assessment was according to RECIST criteria by evaluating downstaging of liver involvement and lymphadenopathy into complete response (CR), partial response (PR), stable disease (SD) and progressive disease (PD). Results: A total of 40 patients were evaluated from January 2012 to December 2014 (CTRT=25, NACT=15). Pretreatment CT scans revealed involvement of hilum (19), liver infiltration (38), duodenum involvement (n=22), colon involvement (n=11), N1 involvement (n=11), N2 disease (n=8), paraaortic LN (n=15), and no lymphadenopathy (n=6). After neoadjuvant therapy, liver involvement showed CR in 11(30%), PR in 4 (10.5%), SD in 15 (39.4%) and lymph node involvement showed CR in 17 (50%), PR in 6 (17.6%), SD in 4 (11.7 %). Six patients (CTRT=2, NACT=4) with 66.6 % and 83% downstaging of liver and lymphnodes respectively underwent extended cholecystectomy. There was 16.6 % and 83.3% rates of histopathological CR of liver and lymph nodes. All resections were R0. Conclusions: Neoadjuvant therapy in unresectable gall bladder cancer results in a 15% resectability rate. This approach has a strong potential in achieving R0 and node negative disease. Radiologic downstaging (CR+PR) of liver involvement is 40.5% and lymphadenopathy is 67.5%. Nodal regression could serve as a predictor of response to neoadjuvant therapy.
Purpose: Currently lung cancer ranks second in cancer for incidence rate and is a disease that ranks first for a death rate by cancerous growth because it is already advanced at the time of diagnosis. The purpose of this paper was to analyze the factors that affect the effectiveness of and rash occurrence by Epidermal Growth Factor Receptor Tyrosine Kinase Inhibitor (EGFR TKI) in patients with non-small cell lung cancer. Methods: A retrospective chart review of 100 patients, who took EGFR TKI (erlotinib, gefitinib) among patients who were diagnosed with non-small cell lung cancer in a Hospital in Korea between May 2005 and February 2008, was conducted. The drug effectiveness was evaluated by Response Evaluation Criteria In Solid Tumor. Results: EGFR mutation was the only factor associated with drug response (complete response and partial response). When stable disease was added to drug response as the evaluation parameter, ECOG and rash as well as EGFR mutation were found to be important factors. Survival, however, was not affected by EGFR mutation. The factors influenced on survival were older age (${\geq}65$), low ECOG ($1{\sim}2$), adenocarcinoma and rash. In the case of rash, group with EGFR mutation or low ECOG showed significantly higher chance of occurrence. There was no significant difference in rash occurrence between gefitinib and erlotinib groups. Conclusions: Based on the results, EGFR mutation positive and low ECOG ($1{\sim}2$) were significantly important factors for both effectiveness of EGFR TKI and rash occurrence. Also, rash itself was found to be an independently significant factor for the disease control and survival. Therefore, while administering EGFR TKI, patients who have the factors associated with rash occurrence should be closely monitored for effective and safe drug therapy.
The classification of plant diseases by images captured by a camera sensor has been studied over past decades. A method that has gained much interest is to use image segmentation, from which statistical features are derived and analyzed by machine learning. Recently, deep learning has been adopted in this area. However, image segmentation is still a difficult task to achieve stable performance due to a variety of environmental variations. The end-to-end learning in neural network has a demerit that train images may be different from real images acquired in outdoor fields. To solve these problems, we propose superpixel-based disease classification method using end-to-end CNN (convolutional neural network) learning. Based on experiments performed on PlantVillage apple images, the classification accuracy is 98.29% and 92.43% for full-image and superpixel. As well, the multivariate F1-score is (0.98, 0.93). Therefore we validate that the method of using superpixel is comparable to that of full-image.
Antimicrobial activity of Caesalpina sappan L. extract (CS extract) against animal husbandry disease-related bacteria was studied. The CS extracts showed a significant antimicrobial activity against Gram(+) bacteria and this antimicrobial activity was most significant against Staphylococcus epidermidis. Minimum inhibitory concentrations (MIC) of CS was in the range of 0.8∼16 mg/ml and 0.8∼10 mg/ml, in the case of MeOH extracts and EtOH extracts, respectively. In addition, the antimicrobial activity of each solvent fraction was most significant with EtOAc layer. The antimicrobial activities of CS extracts against most microbial strains were stable by either heat treatment or acid treatment, whereas those against S. aureus and Sal. typhimurium were reduced by acid treatment. The inhibitory effect of CS extracts on microbial cell growth was further examined by the addition of 0, 100, 300, and 500 ppm of CS extracts into growth medium. The growth of gram(+) bacteria, S. aureus and S. epidermidis, was inhibited for 72 hours in all ranges of CS extracts added, but the growth of gram(-) bacteria was only inhibited when at least 100 ppm of CS extracts were added. Taken together, the antimicrobial activities of CS extracts were more effective against gram(+) bacteria compared to those against gram(-) bacteria.
The regimen of physical activity of the patient with coronary artery disease requires that he should not overshoot the prescribed heart rate based on his age, health and fuctional status of the heart during his exercise. The step input of work load, however, involves a great danger of overshooting. The purpose of this study was to desigil a system that makes it passible for a subject to check the overshooting. This system shows on tile H.R-meter, the amplified and filtered heart-rate signal of the subject received by the photosensor on his earlobe, puts it in the lead coinpensational circuit where it is conpared with the reference input signal(=the presfribed heart rate). The output of the lead compensational circuit works the aull meter. By means of this null meter, the subject knows whether he is overshooting the prescribed heart rate or not. He can continue the natl meter needle at the'Zero'position through the control of the speed of pedaling of the bicycle ergometer, An experimental test, made on eight men and four women in healthy condition, showed that 91. 7% of them vlaintained the stable heart rate and that the overshooting of the desired heart rate did not exceed $\pm$2BPM. According to the result of this experiment, since the heart rate feedback controller makes it possible for the subject to take the prescribed exercise based not on the work load but on the heart rate which incidentally is inexpensive, it can be made use of as the instrument for the regimen of pflysical activity by the patient with coronary artery disease.

Objective: The aim of this retrospective study was to evaluate the feasibility and efficacy of response to continuous-infusion ifosfamide and doxorubicin combination as second-line chemotherapy for patients with recurrent or refractory osteosarcoma. Materials and Methods: Eighteen recurrent or refractory osteosarcoma patients who were treated with continuous-infusion ifosfamide and doxorubicin combination between May 1999 and April 2011 were included in the analysis. Ifosfamide at $12g/m^2$ was administered by intravenous continuous infusion over 3 days, and doxorubicin $60mg/m^2$ was administered as an intravenous bolus injection on day 1. The combination therapy was repeated every 3 weeks. Treatment was continued until evidence of disease progression or unacceptable toxicity. Results: The patients (ages 7-53 years) received a total of 42 cycles of chemotherapy (median: 2 courses; range: 2-5 courses). The overall response rate was 0% and the disease control rate was 22.3%, with four patients having stable disease. The median time to progression and overall survival time were 2 months (range: 2-5 months) and 9 months (range: 3-29 months), respectively. Major severe toxicities were leucopenia 7 (38.9%), nausea and vomiting 3 (16.7%) and alopecia 9 (50%). There were no treatment-related deaths. Conclusions: In our experience, continuous-infusion ifosfamide and doxorubicin combination therapy at this dosage and schedule was found to be well tolerated and moderate effective, which could be considered as salvage therapy for patients with recurrent or refractory osteosarcoma. Further assessment is necessary to confirm the safety and efficacy of this treatment.
Objective: To observe the efficacy and toxicity of nanoparticle albumin bound paclitaxel (nab-paclitaxel) plus platinum agent (cisplatin or carboplatin) as first line treatment for stage III/IV squamous non-small-cell lung cancer (NSCLC). Methods: Forty chemotherapy naive patients with stage III/IV squamous NSCLC received nab-paclitaxel $125mg/m^2$ on day 1 and day 8, cisplatin $75mg/m^2$ on day 1, carboplatin area under the concentration-time curve of 5 (AUC=5) on day 1. One cycle of treatment was 3 weeks, and at least two were completed in each case. Results: Of the 40 patients who participated in the study, 25 achieved partial responses (PR), 12 reached a stage of stable disease (SD), and 3 suffered progressive disease (PD). The overall response rate (ORR) was 62.5% and the disease control rate (DCR) was 92.5%. Of the 20 patients without surgery or radiotherapy, 10 achieved PR, 7 reached a stage of SD, and 3 PD. The ORR was 50.0% and the DCR was 85.0%. The median progression-free survival time (PFS) of patients without surgery or radiotherapy was 5.0 months. Of the 20 patients receiving surgery or radiotherapy, 15 had PR and 5 p had SD, with an ORR of 75.0% and a DCR of 85.0%. Specifically, the DDP arm demonstrated a significantly higher ORR than the CBP arm (100%vs 54.5%, P<0.05). Common treatment related adverse events were myelosuppression, gastrointestinal response, baldness and neurotoxicity, most of which were grade 1 to 2. Conclusion: Nab-paclitaxel plus platinum agent (cisplatin or carboplatin) is effective as a first-line chemotheraphy for stage III/IV squamous NSCLC, and its adverse effects are tolerable.
Background: Bronchioloalveolar carcinoma (BAC) is considered a subtype of adenocarcinoma of the lung. Recently BAC has been variously termed adenocarcinoma in situ, minimally invasive adenocarcinoma, lepidic predominant invasive adenocarcinoma, and invasive mucinous adenocarcinoma. The aim of the study was to analyze and detect prognostic factors of patients with BAC over a 7-year period. Materials and Methods: This retrospective single-center study included 44 patients with BAC. The impact on survival of fifteen variables (gender, age, smoking status, cough, dyspnea, hemoptysis, fever, chest pain, sputum, metastasis number, Karnofsky performance status, pT, pN, TNM stage, cytotoxic chemoterapy) were assessed. Results: Median age was 55 years (38-83). Most patients were male (63.6%) and stage IV (59.1%). Twenty-one patients (47.7%) received cytotoxic chemotherapy (platinum-based regimens) for metastatic disease. Objective response rate was 33.3% (4 partial, 3 complete responses). Stable disease was observed in nine in patients (42.8%). Disease progression was noted in 5 (23.8%). The median OS for all patients was 12 months (95%CI, 2.08-22.9 months). Independent predictors for overall survival were: Karnofsky performance status (HR:3.30, p 0.009), pN (HR:3.81, p 0.018), TNM stage (HR:6.49, p 0.012) and hemoptysis (HR:2.31, p 0.046). Conclusions: Karnofsky performance status, pN, TNM stage and hemoptysis appear to have significant impact on predicting patient survival in cases of BAC.
