Occlusal splints have been widely used in dentistry for treatment and diagnosis of signs and symptoms related to dental occlusion. The autor studyed the effect of occlusal splint on masticatory movement of young adult men (mean age 23.8 yrs.). Who had worn. occlusal splint for 1 week. The result were as follows: 1. Lateral movement of the mandible during mastication could be increased after wearing occlusal splint for 1 week. 2. Vertical movement of the mandible during mastication was increased after wearing occlusal splint for 1 week. 3. Maximum opening velocity of the mandible during mastication was increased after wearing occlusal splint for 1 week. 4. Mandibular movement during mastication was more affected by chewing of hard foods than of soft foods following occlusal changes. 5. Steepness of the anterior guidance affected several aspects of masticatory movement. 6. Masticatory movement of the mandible became more regular after wearing occlusal splint for 1 week.
2009년부터 2011년까지 부산대학교병원 구강내과에 측두하악관절장애를 주소로 내원한 환자 중 임상검사와 방사선 검사 및 전산화 단층(Cone beam Computed tomographic view)촬영으로 측두하악관절의 골관절염으로 진단된 76명의 환자를 대상으로 초진 시와 교합안정장치 장착 전까지 물리치료 및 약물치료를 시행하고 교합안정장치 장착 시부터 최종 내원 시까지 월 1회 장치조정을 시행하여 시기별 치료결과를 비교하여 다음과 같은 결과를 얻었다. 1. 초진 시와 교합안정장치 장착 시와 최종 내원 사이에 모든 증상의 변화가 유의한 차이가 있게 개선되었다. 2. 교합안정장치 치료 전까지 치료에서는 급성군에서 통증과 관절잡음이 유의하게 개선되었고, 만성군에서 통증, 관절잡음, 개구제한 및 최대편이개구량이 모두 개선되었다. 3. 교합안정장치 장착 후에는 급성군에서 통증, 개구제한과 최대편이개구량이 유의하게 개선되었고, 만성군에서 통증, 관절잡음과 개구제한이 유의하게 개선되었다.
목적: 측두하악장애 환자에게 주로 사용되는 장치 치료의 적응증 및 효과에 대한 연구는 다수 존재하나, 증상 개선에 따라 장치를 테이퍼링하여 종료하는 방법에 대한 연구는 부족하다. 따라서 본 후향적 연구는 측두하악장애 환자를 대상으로, 증상 개선에 따라 장치를 테이퍼링하여 종료하는 과정을 고찰하고자 한다. 연구 재료 및 방법: 턱관절 질환, 근막 통증, 이갈이 및 이악물기 등을 주소로 연세대학교 치과대학병원 구강내과를 내원하여 장치 치료(교합안정장치, 전방위치교합장치)를 시행한 130명의 환자의 차트 리뷰를 통해 장치 치료를 테이퍼링하여 종료하는 과정을 알아보았다. 결과: 평균 장치 장착 기간은 29개월로, 매일 장치를 장착한 기간은 8.4개월, 일주일에 3 - 4일간 장치를 장착한 기간은 9.5개월, 일주일에 1 - 2일간 장치를 장착한 기간은 11.1개월이었다. 결론: 측두하악장애 환자에서 장치 장착 시 처음 6개월 간은 매일 장착, 이후 6개월에서 18개월 동안은 일주일에 3 - 4일간 장착, 18개월 이후부터는 일주일에 1 - 2일간 장착하는 방식으로 테이퍼링하여 장치 치료를 종료할 시 측두하악장애 환자의 증상은 완화시키면서 교합 변화 등의 부작용은 최소화시킬 수 있을 것으로 사료된다.
A conservative treatment approach to temporomandibular disorder (TMD) is recommended as the first line of management, usually with a stabilization splint. Recently, computer-aided design/computer-aided manufacturing and three-dimensional printer has been widely used in the dentistry since several years ago. The authors apply digital dentistry in oral medicine fields to make stabilization splint for TMD treatment.
Various conditions such as pain or effusion of temporomandibular joint, degenerative condylar resorption, and articular disc displacement can be a cause of malocclusion. However, the reasons of occlusal changes are ambiguous in some patients. Unexpected occlusal change in patients with or without temporomandibular disorder (TMD) symptom was mostly caused by masticatory muscular disorders. This article reports two cases of recovery of occlusal relationship in TMDs patients after stabilization splint therapy. Stabilization splint therapy could be useful in certain conditions of occlusal changes in TMD.
Purpose: The objective of this study is to propose an effective management of unstable zygomatic arch fracture. The reduction methods of arch fracture were relatively simple but the maintenance of reduction state is very troublesome. On this, authors introduce an effective management method of unstable zygomatic arch fracture. Methods: Authors experienced 23 cases of unstable segmental zygomatic arch fractures and used Thermo-splint in all cases. All the arch fractures were reduced through Gillies' approach under the general anesthesia. After the reduction, the most effective suspension points were marked on the covering skin of the fractured arch. A needle of heavy nonabsorbable suture material was inserted toward the marking site under the reduced zygomatic arch. And then Therm-splint was dipped in the hot water, and we got the splint pattern of patient face. Reshaped Thermo-splint was trimmed and fixated with previous suspension suture materials. More additive suspension was done if necessary. The splint was applied for in two to three weeks postoperatively. Results: In all the cases, good cosmetic and functional results were observed without severe complications. There were 4 cases of incomplete reductions but they also had no specific problems. There were no facial nerve symptom and scar(stitch mark). Postoperative slight tenderness and trismus were completely subsided after removal of the splint. Conclusion: The Thermo-splint safely protect and maintain the postoperative reduction state. The application, maintenance and removal were easy and simple. It could be reformed to any contour of face and had enough rigidity for supporting. Above all these things, effective prevention of displacement and easy manipulation were significant merit. Authors experienced good results with Thermo-splint, and would introduce it for another method of management of zygomatic arch fracture.
In recent years the relationship between occlusal stability, mandibular position and temporomandibular joint function has been greatly emphasized. Anterior repositiong splint has been used for the purpose of correcting a disk-condyle in coordination, so we have usually wed it in ease of the click of the temporpmandibular joint. We have used anterior repositioning splint in 28 patients who have chief-complain of click in symptoms of the temporomandibular joint arthrosis. At the patients who had long-lasting symptom and sign, late click or degenerative change of the temporomandibular joint, the anterior repositioning splint had less effect on than we had expected. So we are now to report that we must pay attention to use of anterior repositioning spint.
After making the surgical treatment plan, the surgical movements are duplicated in the model surgery. During this procedure, reference points and lines are drawn on the base of the models over the dental arch, and sawcuts are made according to these marked osteotomy lines. This method requires more accuracy for better postsurgical results in that the surgical splint which enables the surgeon to position the jaws intraoperatively is made from the casts as repositioned by the model surgery, and finally it will define the postsurgical results. This technique, however, has been found to be inexact, especially when the jaws are moved in several dimensions simultaneously. To overcome this, different methods have been developed for an accurate repositioning of the jaws as planned. A new appliance, Surgical Jaw Relator, was devised by the author for the simple 3-dimensional relocation of the upper and lower models, resulting in the easy construction of the splints such as centric relation splint, intermediate and final splint. This article describes an introduction and a clinical application of this appliance.
This study was performed to diagnose the existence and the degree of TMJ dysfunction using Pantronic-PRI and to investgate the degreeof dysfunction under treatment and to evaluate the period until dysfunction were disappeikared. For this study, 12 patients who had visited at the TMJ Clinic of Dankook University were investgated with Pan-PRI. 8 exper imental group patients were treated with occlusal splint therapy and 4 control group patients were not altered of occlusion during this study. The results were as follows. 1. Occlusal splint therapy was effective on treatment of TMJ dysfunction. 2. in the slight dysfunction group, dysfunction was disappeared after 4-5 weeks since occlusal splint therapy was started. 3. Pan-PRI is useful to evaluate the existence of TMJ dysfunction, the degree of dysfunction results of treatment with occlusal splint therapy and to decide other occlusal treatment modalies.

Adequate positioning of the hand is a critical step in hand fracture operative repair that can impact both the clinical outcome and the efficiency of the operation. In this paper, we introduce the use of a thermoplastic splint with an added thumb stabilizing component as a means to increase the surgeon's autonomy and to streamline the patient care pathway. The thermoplastic splint is custom fabricated preoperatively by the specialist hand therapist. The splint is used prior, during, and post operation with minimal modification. The thumb component assists maintaining the forearm in a stable pronated position whilst drilling and affixing metal work. This is demonstrated in the video of removal of metal work and open reduction and internal fixation of a metacarpal fracture.
