말운동프로그램 향상을 위한 한국어 비단어 중재접근법의 확립 및 임상 적용 (Development and clinical application of Korean-version nonword intervention to improve speech motor programming)

  • 본 연구는 국외에서 개발된 비단어 중재접근법을 수정 및 보완하여 한국어 비단어 중재접근법을 확립하고, 아동기 말실행증 아동에게 직접 적용하여 그 효과를 확인하기 위해 실시되었다. 궁극적인 목적은 비단어를 이용한 중재가 아동기 말실행증의 말운동프로그래밍 능력을 개선시켜 비단어 산출 및 단어로의 일반화에 효과적인지 알아보기 위함에 있다. 중재는 아동기 말실행증의 진단 특성을 보이는 5세 6개월의 남아를 대상으로, ABA설계를 사용한 단일대상연구를 실시하였다. 중재에 사용된 비단어는 아동 맞춤형으로 제작하였으며, 한 회기당 60분씩 주 2회로, 총 12회기를 실시하였다. 그 결과 중재한 3음절 비단어의 모든 지표가 향상되었으며, 중재하지 않은 3음절, 4음절 비단어 및 단어로의 일반화를 확인하였다. 단, 단어로의 일반화 효과는 비단어로의 일반화 효과에 비해 미비하였다. 비단어 중재는 대상 아동의 말운동프로그래밍 능력을 개선시키는데 효과적이었다. 그 결과 운동프로그래밍 손상에 기인한 전환 오류가 크게 감소하였고, 중재하지 않은 비단어의 산출 능력이 큰 폭으로 증가하였다. 그러나 강력하게 습관화된 단어 오류를 완전히 개선시키는 데에는 한계가 있었으며, 이는 보다 집중적이고 반복적인 중재 일정을 제공했을 때 기대할 수 있는 결과일 것이다.

  • Aberrations in VOT measures in apraxia of speech are indicative of speech motor programming impairment. In English, overlaps of VOT between voiceless and voiced plosives have been frequently observed in patients with apraxia of speech. Unlike English, Korean plosives constitute trichotomy in terms of manner of production, that is, voiceless aspirated /p', t', k'/ ; voiceless or voiced, weakly aspirated /p-b, t-k, k-g/ ; or voiceless, heavily aspirated /p, t, k/. In this spectrographic study, VOT measures during Korean plosives produced by a patient with apraxia of speech were compared to those by age- and gender- matching normal subjects. The results indicated that there were partial overlaps between VOT of /b, d, g/ and those of /p, t, k/, implying that the errors were phonetic in nature. In addition, larger VOT variabilities in apraxia of speech compared to the normals were noted.

한국어 구어 실행증 환자에 대한 점진적 8단계 치료 기법의 임상적 효과: 사례연구 (Eight-step Continuum Treatment for Korean Apraxia of Speech Patient: A Case Study)

  • This study aimed at clarifing clinical effects of eight-step continuum treatment in a patient who showed apraxia of speech after stroke. The eight-step continuum treatment consisted of 8 steps and its clinical efficacy has been proven with American apraxic patients. However, it has not been clinically proven to be effective in Korean patients with apraxia of speech as of yet. Therefore, this study was conducted in an effort to provide preliminary clinical evidence regarding its effectiveness regardless of the linguistic differences between Korean and English. The therapy took place twice a week for 6 months, a total of 48 times. The results showed that the patient's receptive language was improved from 83% to 89% and 37% in accuracy, and expressive language from 15% to 37%. It seemed that spontaneous recovery did not playa role in his improvement since the study was conducted 2 years after the stroke. In addition, the improvement of expressive language was much greater(22%) than that of receptive language(6%), which implied that the therapy was effective in apraxia of speech because apraxia of speech is relatively confined to expressive ability, more specifically motor programming and sequencing.

청각적, 시각적 자극제시 방법과 음절위치에 따른 일반아동의 음운인식 능력 (Phonological awareness skills in terms of visual and auditory stimulus and syllable position in typically developing children)

  • This study aims to compare the performance of syllable identification task according to auditory and visual stimuli presentation methods and syllable position. Twenty-two typically developing children (age 4-6) participated in the study. Three-syllable words were used to identify the first syllable and the final syllable in each word with auditory and visual stimuli. For the auditory stimuli presentation, the researcher presented the test word only with oral speech. For the visual stimuli presentation, the test words were presented as a picture, and asked each child to choose appropriate pictures for the task. The results showed that when tasks were presented visually, the performances of phonological awareness were significantly higher than in presenting with auditory stimuli. Also, the performances of the first syllable identification were significantly higher than those of the last syllable identification. When phonological awareness task are presented by auditory stimuli, it is necessary to go through all the steps of the speech production process. Therefore, the phonological awareness performance by auditory stimuli may be low due to the weakness of the other stages in the speech production process. When phonological awareness tasks are presented using visual picture stimuli, it can be performed directly at the phonological representation stage without going through the peripheral auditory processing, phonological recognition, and motor programming. This study suggests that phonological awareness skills can be different depending on the methods of stimulus presentation and syllable position of the tasks. The comparison of performances between visual and auditory stimulus tasks will help identify where children may show weakness and vulnerability in speech production process.
