마비말장애 연구문헌에서 살펴본 말평가의 청지각적 요소 (Auditory-Perceptual Variables of Speech Evaluation in Dysarthria Literature)

  • Perceptual judgement method is frequently used in evaluating dysarthric speech. Although most of speech pathologists and researchers focus on the 38 perceptual features provided by Darley, Aronson & Brown(1969) during evaluation, there are additional characteristics that may be useful to describe dysarthria in literature. We reviewed previous dysarthria literature and selected 46 perceptual characteristics that could be examined at various subsystems of speech production. We also provided explanations and rationale for the rating method for each of the perceptual characteristics. This attempt might aid to offer a basic ground for developing a diagnostic tool of dysarthria.

Investigating the Effects of Hearing Loss and Hearing Aid Digital Delay on Sound-Induced Flash Illusion

  • Background and Objectives: The integration of auditory-visual speech information improves speech perception; however, if the auditory system input is disrupted due to hearing loss, auditory and visual inputs cannot be fully integrated. Additionally, temporal coincidence of auditory and visual input is a significantly important factor in integrating the input of these two senses. Time delayed acoustic pathway caused by the signal passing through digital signal processing. Therefore, this study aimed to investigate the effects of hearing loss and hearing aid digital delay circuit on sound-induced flash illusion. Subjects and Methods: A total of 13 adults with normal hearing, 13 with mild to moderate hearing loss, and 13 with moderate to severe hearing loss were enrolled in this study. Subsequently, the sound-induced flash illusion test was conducted, and the results were analyzed. Results: The results showed that hearing aid digital delay and hearing loss had no detrimental effect on sound-induced flash illusion. Conclusions: Transmission velocity and neural transduction rate of the auditory inputs decreased in patients with hearing loss. Hence, the integrating auditory and visual sensory cannot be combined completely. Although the transmission rate of the auditory sense input was approximately normal when the hearing aid was prescribed. Thus, it can be concluded that the processing delay in the hearing aid circuit is insufficient to disrupt the integration of auditory and visual information.

The Effect of the Speech Enhancement Algorithm for Sensorineural Hearing Impaired Listeners

  • Background noise is one of the major complaints of not only hearing impaired persons but also normal listeners. This paper describes the results of two experiments in which speech recognition performance was determined for listeners with normal hearing and sensorineural hearing loss in noise environment. First, we compared speech enhancement algorithms by evaluation speech recognition ability in various speech-to-noise ratios and types of noise. Next, speech enhancement algorithms by reducing background noise were presented and evaluated to improve speech intelligibility for sensorineural hearing impairment listeners. We tested three noise reduction methods using single-microphone, such as spectrum subtraction and companding, Wiener filter method, and maximum likelihood envelop estimation. Their responses in background noise were investigated and compared with those by the speech enhancement algorithm that presented in this paper. The methods improved speech recognition test score for the sensorineural hearing impaired listeners, but not for normal listeners. The results suggest the speech enhancement algorithm with the loudness compression can improve speech intelligibility for listeners with sensorineural hearing loss.

디지털 보청기 사용자를 위한 압신 알고리즘의 성능 연구 (A Study on the Performance of Companding Algorithms for Digital Hearing Aid Users)

  • Companding algorithms have been used to enhance speech recognition in noise for cochlea implant users. The efficiency of using companding for digital hearing aid users is not yet validated. The purpose of this study is to evaluate the performance of the companding for digital hearing aid users in the various hearing loss cases. Using HeLPS, a hearing loss simulator, two different sensorinerual hearing loss conditions were simulated; mild gently sloping hearing loss(HL1) and moderate to steeply sloping hearing loss(HL2). In addition, a non-linear compression was simulated to compensate for hearing loss using national acoustic laboratories-non-linear version 1(NAL-NL1) in HeLPS. In companding, the following four different companding strategies were used changing Q values(q1, q2) of pre-filter(F filter) and post filter(G filter). Firstly, five IEEE sentences which were presented with speech-shaped noise at different SNRs(0, 5, 10, 15 dB) were processed by the companding. Secondly, the processed signals were applied to HeLPS. For comparison, signals which were not processed by companding were also applied to HeLPS. For the processed signals, log-likelihood ratio(LLR) and cepstral distance(CEP) were measured for evaluation of speech quality. Also, fourteen normal hearing listeners performed speech reception threshold(SRT) test for evaluation of speech intelligibility. As a result of this study, the processed signals with the companding and NAL-NL1 have performed better than that with only NAL-NL1 in the sensorineural hearing loss conditions. Moreover, the higher ratio of Q values showed better scores in LLR and CEP. In the SRT test, the processed signals with companding(SRT = -13.33 dB SPL) showed significantly better speech perception in noise than those processed using only NAL-NL1(SRT = -11.56 dB SPL).

HINT 검사를 이용한 양이의 어음이해에 있어서 노화와 고주파수 청력의 효과 (Effects of Age and High Frequency Hearing Loss on Binaural Speech Understanding Using HINT Study)

  • It has long been known that high-frequency sensorineural hearing loss (HFHL) can contribute to difficulty in speech understanding by elderly listeners. This study evaluated the relative contribution of HFHL and age to speech understanding. Subjects included adult middle-aged and old groups with normal hearing or with limited HFHL. The Hearing-in-Noise-Test (HINT) was used to measure speech perception performance in quiet and in noise. The middle-aged groups showed significant effects of HFHL for speech intelligibility in quiet and in noise, but the old groups showed the difference in quiet only due to high frequency hearing. The results suggest that HFHL may affect speech intelligibility differently with age and therefore hearing aid selection needs to take into account the influence of age.

  • In this Paper, an Audio-Visual Teaching aid (AVTA) for use in a classroom and with Internet is presented. A system, which was designed and tested, consists of a wireless Microphone system, Text to Speech conversion Software, Noise filtering circuit and a Computer. An IBM compatible PC with sound card and Network Interface card and a Web browser and a voice and text messenger service were used to provide slightly delayed text and also voice over the internet for remote teaming, while providing scrolling text from a real time lecture in a classroom. The motivation for design of this system, was to aid Korean students who may have difficulty in listening comprehension while have, fairly good reading ability of text. This application of this system is twofold. On one hand it will help the students in a class to view and listen to a lecture, and on the other hand, it will serve as a vehicle for remote access (audio and text) for a classroom lecture. The project provides a simple and low cost solution to remote learning and also allows a student to have access to classroom in emergency situations when the student, can not attend a class. In addition, such system allows the student in capturing a teacher's lecture in audio and text form, without the need to be present in class or having to take many notes. This system will therefore help students in many ways.

  • In case of the hearing impairment with tinnitus and increased sound sensitivity, it is known that the patients tend to appeal the psychologically oriented social handicap rather than communication disability. The audiologist who is responsible for such patients in aural rehabilitation should pay special attention to the counseling techniques including tinnitus retain therapy (TRT), ear protector, noise generator, or specific acoustic training based on close cooperation and rapport. And then the audiologist should try to lessen their reaction to the tinnitus by using a hearing aid. This therapies tries to focus not a. total approach but a treatment to lessen the severity of tinnitus. This paper as a case report that a unilateral sharply slopped sensorineural hearing impaired person with tinnitus and increased sound sensitivity by using four channel digital signal processing (DSP) hearing aid with programming increment at low level (PILL).

Auditory Recognition of Digit-in-Noise under Unaided and Aided Conditions in Moderate and Severe Sensorineural Hearing Loss

  • Background and Objectives: The speech-in-noise test is typically performed using an audiometer. The results of the digit-in-noise recognition (DIN) test may be influenced by the flat frequency response of free-field audiometry and frequency of the hearing aid fit based on fitting rationale. This study aims to investigate the DIN test in unaided and aided conditions. Subjects and Methods: Thirty four adults with moderate and severe sensorineural hearing loss (SNHL) participated in the study. The signal-to-noise ratio (SNR) for 50% of the DIN test was obtained in the following two conditions: 1) the unaided condition, performed using an audiometer in a free field; and 2) aided condition, performed using a hearing aid with an unvented individual earmold that was fitted based on NAL-NL2. Results: There was a statistically significant elevation in the mean SNR for the severe SNHL group in both test conditions when compared with that of the moderate SNHL group. In both groups, the SNR for the aided condition was significantly lower than that of the unaided condition. Conclusions: Speech recognition in hearing-impaired patients can be realized by fitting hearing aids based on evidence-based fitting rationale rather than by measuring it using free-field audiometry measurement that is utilized in a routine clinic setup.

점막하 구개열에서 Furlow 구개성형술의 수술시기 (Furlow Palatoplasty in Submucous Cleft Palate-Timing of Operation)

  • Purpose: In order to determine the differences in speech outcome based on timing of operation in submucous cleft palate, we have reviewed our experiences in the Furlow palatoplasty over the last 11 years. Methods: From March 1996 to March 2006, 38 submucous cleft palate patients received Furlow palatoplasty. 10 developmentally delayed patients were excluded and 5 patients were lost to follow up. The rest 23 patients were reviewed. Speech was evaluated preoperatively and postoperatively, and speech therapy was performed accordingly. Perceptual speech assessment included hypernasality, nasal emission and articulation disorder. Cinefluorography was performed to aid perceptual assessment. Based on timing of operation, the patients were divided into 3 groups as following: Group A under 24 months(8 patients), Group B from 25 to 48 months(6 patients), and Group C over 49 months (9 patients). Except 1 patient under speech therapy yet, resultant speech was compared. 200707Results: The rate of abnormal speech was higher in Group C(3/9, 33.3%) than in Group A(0%) or B(0%). All 3 patients who had been discontinued of speech therapy from the parent's judgment had abnormal speech. The reason for the discontinuation was that the regular speech therapy was a burden at school age. Any patients who had continued speech therapy had normal speech. Conclusion: The results of our study shows that operative timing is associated with speech development. Maintenance of speech therapy was an important factor for normal speech development. It will be helpful to perform a palatoplasty before 48 months of age to complete speech therapy before the school age.

후두전절제술 후 음성재활 (Voice Rehabilitation after Total Laryngectomy)

  • Total laryngectomy remains as an important treatment option in selected patients with laryngopharyngeal cancers, which inevitably sacrifices naturally produced voice. Much effort has been devoted to voice restoration for these laryngectomized patients. Several ways of voice rehabilitation after total laryngectomy have been developed and utilized thus far, including tracheoesophageal shunt speech, esophageal speech, pneumatic speech aid, and electrolarynx. Of these, tracheoesophageal shunt speech appears to be the most effective voice restoration method, while other trials might also be useful in special situations. Nevertheless, each method has its own unique mechanisms of voice production, thus has its advantages and drawbacks in clinical setting. In this review, we discuss the currently available management options for the rehabilitation of laryngectomized voice.
