A Clinical Case Report on Treatments of Cervical Dystonia (경련성(痙攣性) 사경증(斜頸症) 환자(患者) 치험(治驗) 1예(例) 보고)

  • Objectives : A case of cervical dystonia improved after Korean medical treatments by using measurements of Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS), and Cervical Dystonia Impact Profile (CDIP-58) to evaluate the changes of symptoms. Methods : Korean medical treatments for relieving symptoms of cervical dystonia including acupuncture, herb medication, cupping therapy, and physical therapy were performed. To evaluate the effect of the treatments, TWSTRS and CDIP-58 were used. Results : TWSTRS decreased from 55 to 24.5 and CDIP-58 also decreased from 79.3 to 36.2. Conclusions : This report suggests that Korean medical treatments can be effectively used for patients with cervical dystonia. Further clinical studies are needed to verify the findings.

  • Extrapyramidal movement disorders are divided descriptively into hypokinesias(such as parkinsonism), characterized by poverty and slowness of movement : hyperkinesias(such as chorea, athetosis, dystonia, ballism, etc.), manifested by abnormal involuntary movement. Chorea refers to widespread arrythythmic movements of a forcible, rapid, jerky, restless type Choreic movements are noted for their irregularity and variability. They are generally continuous, may be simple or quite elaborate, and affect any part of the body. Dystonla refers to abnormally increased muscular tone that causes fixed abnormal postures. Some patients with dystonia also have shifting postures, resulting from irregular, forceful twisting movement that affect trunk and produce bizarre, grotesque movements and positions of the body. The most frequent and familiar type of focal dystonia is spasmodic torticollis. It consists of an involuntary turning of the head to one side - intermittent at first, then gradually worsening to the point of being more or loss continuous. The combination of blepharospasm and oromandibular dystonia is sometimes refered to as Meige's syndrome. We report two patients with dystonia and chorea in cerebral infarction at basal ganglia. We have experienced good improvement by the oriental medicine and acupuncture treatment.'rho acupuncture points of LI 4, ST 36, TE 3, GB 34, GB 41, LR 3, GB 39 were used. The therapies of herb-medicine were treated by Zibu-Ganshen(滋補肝腎), Huoxue-Xifeng-Tongluo(活血息風通絡).

Botulinum Toxin for other Head and Neck Lesions (기타 두경부 병변에서의 보툴리눔 독소의 이용)

  • The usages of botulinum toxin were most commonly for the treatment of spasmodic dysphonia in the otolaryngology field. It has been not only widely used in otolaryngology-Head Neck surgery but also plastic surgery, ophthalmology, rehabilitation medicine, and orthopedics. Now botulinum toxin is used such as blepharospasm (excessive blinking), strabismus, cosmetic, muscle spasms, upper motor neuron syndrome, severe primary axillary hyperhidrosis (excessive sweating), cervical dystonia (spasmodic torticollis), chronic migraine, bruxism, and achalasia. The indication of this drug still gradually expanding with the times. In this articles, the author will demontrate how to use the botulinum toxin for treating cricopharyngeal spasm, arytenoid dislocation, sialocele, Frey syndrome, contact granuloma, bilateral vocal fold paralysis, and mutaional falsetto instead of conventional surgical treatment.

  • ■ Objectives This clinical report is to report the effect of Korean medicine on a patient with torticollis. ■ Methods We used herbal medicine, acupuncture, moxibustion, pharmaco-acupuncture and bee venom acupuncture to treat a torticollis patient during hospitalization and out patient department (OPD) treatment for 3months. We observed the changes of symptoms using Toronto Western Spasmodic torticollis Scale (TWSTRS). ■ Results After treatment, the patient's symptoms including torticollis, pain and the general condition were improved. The TWSTRS scores were also improved after treatment. ■ Conclusion This clinical case study suggests that Korean medicine treatment could be effective in the treatment of torticollis.

  • Objectives : This report intended to estimate effects of Jung-an acupuncture on the acquired torticollis caused by muscle contraction. Methods : The patients were treated by Jung-an acupuncture, herb medication and Physical treatment daily. The improvement of the patients was judged by Toronto Western Spasmodic Torticollis Rating Scale(TWSTRS), Tsui's score and Visual Analogue Scale(VAS). Results : After 7 days of treatment, the 2 patients' neck pain and cervical movement were improved. Also, TWSTRS score, Tsui's score and VAS were all reduced. Conclusions : As the Jung-an acupuncture's theological base is organized with meridian theory and myofacial release therapy, We considered the Jung-an acupuncture is effective not only for the skin disease but also for the muscle problem such as torticollis.

  • Objectives : Therapeutic effect of Balance Appliance of functional crebrospinal technique (FCST for meridian and neurologic yinyang balance was observed in two refractory torticollis cases. Methods : A unidentified severe torticollis two cases with several months of duration was managed by the Balance Appliance on temporomandibular joint (TMJ), combined with acupuncture and manual medicine. Results : Assessment was made by self assessment of subjective symptoms and clinical observation. The patient reported over-90% remission and returned to ordinary daily life after $3{\sim}5$ months of therapy, which effect was reported to maintain for 6 months. Conclusions : An impressive effect was observed and further clinical and biological research on FCST is expected.

Development of the Last Mass Diameter Prediction Model for Congenital Muscular Torticollis Infants Provided Physical Therapy (물리치료를 받은 선천성 근성 사경 환아의 최종 종괴 지름 예측 모형 개발)

  • Purpose: The pathophysiology of congenital muscular torticollis (CMT) is that the sternoclavicularmastoid (SCM) is shortened on the involved side by fibrosis, leading to an ipsilateral tilt and contralateral rotation of the face and chin. The aim of this study was to examine the effect of physical therapy and develop a mass diameter prediction model for infants with CMT. Methods: Fifty six patients were diagnosed with CMT between April 2003 and December 2008. Infants with neurological complications, and spasmodic and ocular torticollis were excluded. Physical therapy was applied to those masses in the SCM muscles of those infants after checking their physical findings and the diameter of the mass with ultrasonography. Their physical findings and mass diameter was reevaluated when their neck tilt was under $5^{\circ}$. Results: The mean age when physical therapy was started was 35 days. After a mean 90 days of treatment, the subjects showed improvement in the neck tilt. Subjects whose neck tilted above $15^{\circ}$ showed significant improvement in neck tilt decreased their mass diameter (p<0.01). Facial symmetric infants showed a shorter recovery duration than the facial asymmetric infants (p<0.05). A mass decreasing model based on the diameter of the mass, facial symmetry or not and the physical therapy start day after birth was developed by linear regression. Conclusion: Physical therapy is an effective treatment for CMT. The change in the diameter of the mass on the SCM muscles after treatment can be predicted.

Meridian Yinyang Balance Treatment of Temporomandibular Joint Improves Cervical Spine Alignment in Spasmodic Torticollis Cases, a Medical Imaging Study (경추부 근육긴장이상에 대한 턱관절 경락음양 균형치료시 영상의학적으로 관찰된 경추정렬 개선 효과)

  • Postural balance of temporomandibular joint (TMJ) reflects the neuromuscular balance. TMJ posture treatment for meridian yinyang balance has been asserted to have an impact upon cervical spine alignment with an episodic good clinical result for refractory torticollis cases. A retrospective study with one-group pretest-posttest design was conducted to review cervical spine imaging data and clinical symptom changes in cervical dystonia cases to explore the effect of TMJ balance treatment. Review of clinical symptoms and cervical spine imaging data including computed tomography and simple x-ray of 9 cervical dystonia cases revealed significant improvement of cervical spine alignment and clinical symptoms. Improvement of cervical spine alignment may be an underlying mechanism of TMJ balance treatment, as a way of balancing the whole-body meridian system.

  • The purpose of this study is to report a case of cervical dystonia and the effects of a series of traditional Korean medicine treatments applied to the patient. The patient was treated with conventional treatment and traditional Korean medicine consisting of acupuncture, moxa, chuna therapy, and herbal medicine (Hoesu-san-gami) for 48 days. Progress was measured by Tsui's score, the Toronto Western Spasmodic Torticollis Rating Scale, and the Neck Disability Index. We observed some improvements in the patient's symptoms: Tsui's score of 15 to 11, TWSTRS of 63.3 to 47.8, and NDI of 24 to 17. These findings indicate that the application of traditional Korean medicine to cervical dystonia can yield decent clinical outcomes.

A Case of Listeriosis Occurred in the Korean Native Goat (한국재래산양에서 발생한 리스테리아병)

  • A survey was performed on the outbreak of listeriosis in a Korean native goat in Animal Breeding Station of Gyeongsang National University, Chinju, Korea. Clinical signs in the affected goat were fever, dullness, inappetence, nasal discharge, slobbering, dysphagia, circling, incoordination, recumbency, unilateral facial paralysis, torticollis, dyspnea, spasmodic paddling movement of the limbs and death at 6 days after onset. No significant gross lesions were seen. The histopathological lesions were consisted of perivascular edema and cuffs by round cells and microabscess with infiltration of neutrophils, lymphocytes and macrophages in the cerebrum. Gram-positive organism was observed in the microabscess lesions. Listeria monocytogenes was isolated exclusively from the brain tissue by cultural examination and the lesions induced by experimental infection with the isolate were characterized by severe conjunctivitis in rabbit and hepatic necrotic foci in mouse. This seems to be the first report of listeriosis in Korean native goat.
