상부 위장관 장애 환자에서의 스트레스반응 지각척도의 반응특성 (Response Characteristics of Perceived Stress Response Inventory in Patients with Upper Gastrointestinal Disorder)

  • 연구목적 : 상부 위장관의 소화 장애를 가진 환자들이 스트레스를 지각하였을 때 나타내는 반응 및 소화장애 증상의 여러 변인들과의 관계를 알아봄으로써 특정 정신신체질환군에서의 스트레스에 대한 반응특성과 해당 질환과의 연관성을 알아보고자 한다. 방법 : 소화장애 증상으로 인천기독병원 소화기내과를 방문하여 위내시경 소견 상 병변이 확인된 84명의 환자와 94명의 정상 대조군을 대상으로 국내에서 개발된 스트레스반응 지각척도를 사용하여 스트레스를 인지하였을 때 나타내는 반응을 비교분석하였다. 또한 환자군에서 현재 호소하는 소화장애 증상의 기간, 소화장애의 과거력, 증상의 심각도, 심리적 고통의 존재 여부에 따라 스트레스에 대한 지각반응이 어떻게 나타나는지 평가하였다. 결과 : 스트레스반응 지각척도는 8개의 하위척도로 구성되어 있으며 연구 대상자 178명에서 8개 하위척도와 척도전체의 내적 일치도는 .65~.95로써 통계적으로 유의하게 높았다. 환자군이 대조군에 비해서 척도의 전체총점과 일반적 신체증상척도, 특정신체증상척도의 접수가 통계적으로 유의하게 더 높았다. 소화장애 증상과 스트레스반응 지각척도와의 관련성은 소화장애의 기간, 소화장애의 과거력, 증상의 심각도는 특정신체증상을, 심리적 고통의 존재는 인지기능의 저하와 일반적 부정적 사고를 잘 예측해 주는 것으로 나타났다. 결론 : 실제 상부위장관에 병변을 가진 소화장애 환자들은 정상인에 비해서 강한 스트레스 반응 지각을 나타내었으며 특히 스트레스를 경험하고 인지하였을 때 감정, 인지, 행동 영역의 반응보다는 자율신경계 및 특정신체부위와 연관된 신체증상을 주로 경험하고 있음을 알 수 있다. 또한 이들은 소화장애의 기간이 길고, 과거력이 있을수록, 그리고 증상이 심할수록 스트레스에 대해 소화기 증상을 주로 한 특정신체증상으로 반응하며 심리적 고통을 주는 스트레스의 존재는 인지기능을 저하시키고 부정적인 사고반응을 일으킴을 알 수 있다.

  • 본 연구는 신체 증상을 호소하는 정상인들을 대상으로 3주간 홍삼 또는 위약을 투여하였고, 홍삼 복용군에서 위약 복용군에 비해 신체 증상 감소 정도가 더 큼을 보여주었다.

  • This study aims to report the effectiveness of complex Korean medicine (KM) therapy on a somatic symptom disorder (SSD) patient. A 58-year-old woman had severe somatic pain for more than six months and showed accompanying anxiety and depression. KM doctors administered complex KM therapy to control her pain and psychological conditions. Her pain was rated using the numerical rating scale and her anxiety and depression symptoms with beck anxiety inventory and beck depression inventory. The somatic symptoms as well as the anxiety and depression significantly improved after KM treatment. This study suggests that complex KM therapy for SSD may be effective to manage both somatic and psychological symptoms.

범불안장애 환자의 스트레스 대처방식과 신체증상 지각에 대한 인지적 특성 (Stress Coping Strategies and Cognitive Characteristics of Somatic Symptom Perception in Patients with Generalized Anxiety Disorder)

  • Objective : The purpose of this study was to investigate strategies used to cope with stress and the cognitive characteristics of somatic symptom perception in patients with generalized anxiety disorder(GAD). Methods : A total of 55 patients meeting DSM-5 criteria for GAD and 55 normal controls were recruited for participation in this study. We evaluated subjects using The Way of Stress Coping Questionnaire (SCQ), Somato-Sensory Amplification Scale (SSAS), Symptom Interpretation Questionnaire (SIQ), and the Generalized Anxiety Disorder for 7 Item (GAD-7). We analyzed data using an independent t-test and Pearson's correlation analysis. Results : In terms of SCQ, GAD patients presented significantly lower scores on seeking social support and higher scores on wishful thinking than normal controls. GAD patients had significantly greater amplification of physical sensation in SSAS and higher scores in physical, psychological interpretation subsets of SIQ than normal controls. GAD-7 scores were positively correlated with physical interpretation scores on SIQ. Conclusion : Results reveal that patients with GAD have insufficient coping strategies for stress, greater amplification of body sensations, and tendency towards a physical, psychological interpretation of somatic symptoms.

보건실 이용 학령후기 아동이 지각한 애착안정성, 사회적 지지가 신체화 증상에 미치는 영향 (Influence of Perceived Attachment Security and Social Support on Somatic Symptoms in Late School-Aged Children Using a School Health Clinic)

  • Purpose: The purpose of this study was to examine current status of somatic symptoms of late school-aged children using the school health clinic and to investigate the influence of perceived attachment security and social support on their somatic symptoms. Methods: For this descriptive study, self-report questionnaires were completed by fifth and sixth graders attending 'A' elementary school in Gyeonggi-do. Data from 216 students were included. Data analysis included descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and multiple regression analysis. Results: Most frequent somatic symptoms were headache, fainting, backache, numbness in a body part, and muscle ache in that order. More frequent somatic symptoms were reported by girls, students who recognized their family SES as low, students who used school health clinic often and students who were dissatisfied with school life. Somatic symptom showed negative correlations with attachment stability and perceived social support from family and teachers. In the regression analysis, the variables; low attachment stability, female gender, and low satisfaction with school affected more frequent somatic symptoms. Conclusion: Careful monitoring of late school-aged children expressing frequent somatic symptoms is required. Intervention programs to improve attachment security and satisfaction with school should be developed for school children, especially girls, presenting with somatic symptoms.

흡연, 음주, 운동과 건강생활양식 (The health lifestyle of adults related to smoking, drinking and exercise)

  • This study is a descriptive survey to explore the health lifestyle of adults. The study subjects are teachers of elementary. middle and high school. and staffs of research institutes located in Chungchung Province and Daejon city. The data was collected from Jan. to march 1997 through self reporte for structured questionnaire. Fantastic check list of Wilson and Ciliska for Health Lifestyle Assessment and DSM-III-R for somatic symptom were used as tools. Data was analysed by frequency, $X^2_test$, t-test and Anova using SAS program. The results are as follows: 1. There were statistically significant differences In drinking(t=7.75, P=.000), exercise(t=-2.99, P=.003)and interpersonal relationship(t=2.22, P=.027) among 10 health lifestyle between smoking group and non-smoking group, in drinking(t=17.98, P=.000), exercise(-4.71. P=.000), and job satisfaction(t=2.22, P=.027) between drinking group and non-drinking group, and in eating habit(t=-2.00, P=.045), drinking (t=4.47, P=000), exercise (t= -16.49, P=000), keeping traffic law(t= -2.68, P=.007), personality (t= -2.05, P=.040) and anxiety/depression(t=-3.47, P=.000) between exercise group and non-exercise group. 2. There was statistically significant difference in cardiovascular symptom(F=4.22, P=.0l) among somatic symptoms of subjects according to exercise level. 3. There was statistically significance difference in lifestyle according to smoking level(F=, 3.33, P=.011), drinking level(F=9.17, P=.0001) and exercise level(F=11.93, P=.000l), and in somatic symptom according to sex(t=-3.93, P=.0001), weight(F=3.83, P=.022), exercise level (F=3.29, P=.03) among general characteristics. 4. There was statistically significant difference between sex in general (t= -3.64, P=.0001), gastrointestinal(t=-2.21, P=.02), musculoskeletal(t=-3.92, P=.001), and total symptom (t= -3.92, P=.0001). 5. There was statistically very highly signigicant difference In weight according to smoking($x^2=25.18,\; P=.001)$ and exercise$(x^2=16.46,\; P=001)$. 6. There was statistically significant difference in frequency between smoking group, drinking group and exercise group$(x^2=24.52,\;P=.001)$. Among a number of habit, smoking, drinking and exercise are important factors of human health to prevent related disease morbidity and death. It is essential for industrial health nurse to committ in this subject considering the influence of those factors and lifestyle on health. There is also a relationship of weight with smoking and exercise, the frequency of overweight/obesiy in smoking/ no-exercise group were high. It is quite necessary for the people having cardiovascular symptom to exercise to lower morbidity and mortality. The industrial health nurse has to keep In mind on this point and consider of time and facilities of fitness of employee. It needs to explore the cause by further research on somatic symptom of women. This research shows that concerning the relationship between smoking, drinking, and exercise, health care provider must take not only management of disease, but health behaviors and lifestyle into consideration.

인성검사를 이용한 MPDS환자의 심리적 분석 (A Psychological Study on the MPDS Patients)

  • A reserch scale of 48 items selected from MMPI by McCall et al. was used to compare psychologic aspects of 80 patients with MPDS to control I subjects and control II subjects. For this, study subjects were selected 80 patients of MPDS who visited the Dept. of Dentistry. K.N.U.H., from December in 1982 to September in 1983. Control I subjects were selected 80 outpatients without MPDS who required dental treatment from December in 1982 to September in 1983. And control II subjects were selected 80 persons without MPDS who required no dental treatment and lived in Taegu. The results were as follow : In study subjects,somatic gruop and behavioral group are markedly high than control (I,II)subjects. In somatic group and behavioral group, there was no significant differencd between control I subjects and control II subjects. In somatics group and begavioral group, there was no significant difference between male and female in each subjects: In study subject, general nature somatic category was higher than head, neck, throat and GIT somatic category.(P<0.05) In study subjects, somatic group and behavioral group gradually increased according to increase in number of MPDS symptom.

신체증상장애와 관련된 분노정서특질과 아동기 역경 경험 (Anger-Related Characteristics and Childhood Adversity in Somatic Symptom Disorder)

  • Objective : The present study aims to investigate differences in anger-related features in patients with somatic symptom disorder (SSD) compared to healthy controls, and to examine whether anger trait and anger regulation strategy are associated with clinical characteristics in patients with SSD. In addition, we examined the relationship between childhood adversity and SSD. Methods : 26 patients with SSD and 28 healthy controls were included. Anger-related features were assessed with State-Trait Anger Expression Inventory (STAXI). Clinical somatic symptoms were assessed using the somatization subscale of the Symptom Checklist-90-Revised and the Somatosensory Amplification Scale. Childhood adversity was assessed by the Childhood Traumatic Events Scale. Multivariate analysis of covariance was performed. Results : Disease status of SSD had a significant overall effect on anger-related features (Wilks λ=0.725, F(5, 44)=3.332, p=0.012). Patients with SSD showed a significantly high Trait-Anger (p=0.017) and they had a high score in both Anger-Out (p=0.013) and Anger-In (p=0.001) of anger expression styles. In particular, a directed inward style of anger expression was significantly associated with somatization symptom severity (p=0.003). Regarding childhood adversity, more childhood extreme illness was experienced by the SSD group than the control group (p=0.012). Within the SSD group, childhood extreme illness was associated with higher Trait-Anger (p=0.027) and Anger-Out (p=0.001). Conclusion : The present findings suggest that trait anger, anger expression styles, and childhood adversity of extreme illness may be involved in SSD. Further studies are needed to explore the role of anger-related features and its relationship with childhood adversity in the pathophysiology of SSD.

소방공무원의 지각한 현장출동 충격경험과 외상 후 스트레스가 신체증상 및 대처방식에 미치는 영향 (The Influence of Field-Shock Experience and Post Traumatic Stress Perceived by Fire Officials upon Somatic Symptoms and Coping Methods)

  • 본 연구는 소방공무원들의 지각한 현장출동 충격 경험과 외상 후 스트레스가 신체증상 및 대처방식에 미치는 영향을 규명하기 위하여 강원도 소재 소방안전센터에 근무하고 있는 소방공무원 남녀 362명을 대상으로 조사하였다. 수집된 자료는 Windows용 SPSS 19.0과 AMOS 19.0을 사용하여 빈도분석, 타당도와 신뢰도 분석을 위한 요인분석과 Cronbach's ${\alpha}$ 분석, 기술통계와 상관관계를 실시하였고, 가설 검정을 위해 구조방정식 모형을 이용한 분석을 실시하였다. 모든 통계분석은 유의수준은 p<.05로 설정하였다. 결과는 다음과 같다. 첫째, 지각한 현장출동 충격 경험은 외상 후 스트레스에 영향을 미치는 것으로 나타났다. 둘째, 지각한 현장출동 충격 경험이 신체증상에 영향을 미치는 것으로 나타났다. 셋째, 지각한 현장출동 충격 경험은 대처방식에 영향을 미치는 것으로 나타났다. 넷째, 외상 후 스트레스는 신체증상에 영향을 미치는 것으로 나타났다. 다섯째, 외상 후 스트레스는 대처방식에 영향을 미치는 것으로 나타났다. 여섯째, 신체증상은 대처방식에 영향을 미치는 것으로 나타났다. 이러한 결론으로 볼 때 충격적 스트레스를 해소하기 위한 제도적 방안으로 전 세계적으로 검증이 되어 실제 운영되고 있는 위기상항 스트레스 해소법(Critical Incident Stress Debriefing : CISD)을 소방서의 실정에 맞게 도입하여 운영하는 것이 필요할 것이다.

다기관 모집 외상 후 스트레스장애 환자에서 외상의 특성과 신체적 증상과의 관계 (The Relationship between Somatic Symptoms and the Trauma-Related Characteristics of Patients with PTSD : A Korean Multicenter Study)

  • Objective : The aim of this study is to examine the characteristics of somatic symptoms in patients with PTSD according to trauma type. Methods : The subjects of this study were 84 patients diagnosed with PTSD according to the DSM-IVTR and CAPS criteria. The subjects ranged in age from 18 to 76 years, and they were recruited from 18 hospitals across the nation. All participants were asked about their trauma history, and they all completed the Davidson Trauma Scale (DTS), Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), Impact of Event Scale-Revised (IES-R) and Korean-Physical Somatic Symptom Inventory (K-PSI). The items of the K-PSI were categorized into 5 groups according to organ system, and the participants were classified into 6 groups according to trauma type. We compared the results of the social demographical scale, DTS, BDI, STAI, IES-R and each recategorized subscale of the K-PSI among the six groups. Results : There were significant differences between the 6 groups in terms of the Korean-Physical Somatic Symptom Inventory scores for each organ system, except for the cardiopulmonary system, as well as the BDI scores. Post hoc analysis revealed differences between the combat-related trauma group and all other groups but not among any of the other groups. Conclusion : Our result showed that there were significant differences in the somatic symptom scores among the 6 trauma groups. However, patient age and the time elapsed since the traumatic event may have hada crucial influence on the result of this study.
