Extensive calcific myonecrosis of the lower leg treated with free tissue transfer

  • Calcific myonecrosis is a rare condition in which hypoperfusion due to compartment syndrome causes soft tissue and muscle to become calcified. As calcific myonecrosis gradually deteriorates, secretions steadily accumulate inside the affected area, forming a cavity that is vulnerable to infection. Most such cases progress to chronic wounds that are unlikely to heal spontaneously. After removing the calcified tissue, the wound can be treated by primary closure, flap coverage, or a skin graft. In this case, a 72-year-old man had extensive calcific myonecrosis on his left lower leg, and experienced swelling and increasing tenderness. After removing the muscle calcification, we combined two anterolateral thigh free flaps, which were harvested from the patient's right and left thigh, respectively, to reconstruct the wound with a dead-space filler and skin-defect cover at the same time. The patient recovered without revision surgery or major complications.

안면부의 유구낭충증의 1증례 (A Case of Cysticercosis on the Face)

  • 포충증은 주로 유구낭충에 감염된 환자에 larval cysticercoid stage때 soft tissue에 전이되어 이 cysticercus가 죽음으로써 변성되고 석회화를 일으켜 국소적 소견으로는 nontender mass, 그리고 X-ray 상 typical calcification을 일으키며 조직학적 소견으로는 fibrotic cyst속에 scolex의 hooks와 sucker를 볼 수 있다. 만약 cysticercosis가 두뇌속으로 전이된다면 두뇌의 침범된 부위의 국소증상이 나타날 수 있으며 예후는 매우 나쁘다. 조직학적 소견상 포충증으로 확진되면 환자를 입원시켜 전신을 검사하여 nontender mass가 만져지면 적출하여 주어야 한다. 저자들은 우측 안면부에 nontender mass를 주소로 하여 내원한 24세 남자에서 우측 안면부에 전이된 유구낭충증 1 례를 경험하였기에 문헌적 고찰과 함께 보고하는 바이다.

이마에 발생한 석회화된 지방종 (Calcified Lipoma of the Forehead)

  • Purpose: Although lipoma is known as one of the most common soft tissue tumors, calcification in lipoma is very rare in its occurrence. This calcified lipoma has been reported by some as a result of regression of lipoma, but its genesis is not clearly known yet with various opinions being discussed regarding its possible metabolic relation to hypercalcemia or hyperphosphatemia to be considered as a regression phase of lipoma. The authors would like to present this unusual case of calcified lipoma. Methods: A 50-year-old male patient visited our hospital with complaint of an enlarging mass on his right forehead which has been acknowledged for 5 years' period. On physical examination, a mass was observed on his forehead as palpable, non-tender, mobile and firm in its consistency. Ultra sonogram examination revealed a well-demarcated mass (1.92 cm) with central echoic zone at deep layer of forehead. Mass excision and biopsy were performed subsequently. Results: According to the pathological report, the diagnosis confirmed the lipoma consisting of grown-up adipocyte and calcification. Neither growing lipoma nor relapse was observed for postoperative three years' follow-up of the patient. Conclusion: Now that the calcified lipoma was successfully removed and cured by a simple mass excision, authors hereby report the case of calcified lipoma on forehead.

  • The authors experienced one case of peripheral giant cell granuloma occurred at the gingiva of right maxillary molar in a 12-year-old male patient. The lesion showed amorphous calcification within soft tissue mass which made difficult to differentiate this lesion from peripheral ossifying fibroma and peripheral odontogenic fibroma clinically and radiographically. The final diagnosis was made histologically.

종격동에 발생한 골외성 골육종;1례 보고 (Extraskeletal Osteogenic Sarcoma of the Mediastinum; 1 Case Report)

  • Extraskeletal osteogenic sarcoma is a rare malignant tumor of soft tissue, and its predilection sites are the extremity, retroperitoneum, trunk, and the head and neck area. To our knowledge 5 cases of primary involvement of the mediastinum have been reported. Because of its rarity and difficulty in exact diagnosis preoperatively, we report an extraskeletal osteogenic sarcoma in the anterior mediastinum. The patient was a thirty eight old male. He complained of cough and sputum over 2 months. The chest roentgenogram and the chest MRI[magnetic resonance image] were done and showed anterior mediastinal mass with calcification. Excision of the mass was done under the preoperative impression of thymoma, and the pathologic report was extraskeletal osteogenic sarcoma of the mediastinum.

  • This benign canine histiocytoma was the most common single tumor type, followed lipoma, adenoma, soft tissue sarcoma, mast cell tumor and lymphosarcoma [2]. Canine cutaneous histiocytoma (CCH) most commonly occur as solitary lesions, head and pinnae are prefermential sites, that undergo spontaneous regression. The histologic appearance varies greatly, depending on the age of the lesion and the degree of necrosis and secondary inflammation. The age-specific incidence rate for CCH drops precipitously after 3 years, although histiocytomas occur in dogs of all ages [5]. Their etiology and pathogenesis are unknown. This case reports the gross and histopathological findings of canine cutaneous histiocytoma of observed in a young dog with localized calcification of lingual muscle. (omitted)

  • Radiological analysis of chest lesions detected on chest radiographs or CT scans begins with their classification into parenchymal, pleural, or extrapleural lesions according to their presumed origin. The mediastinum is divided anatomically into the anterior, middle, and posterior mediastinal compartments, and localizing a mediastinal mass to one of these divisions can facilitate their differential diagnosis. A differential diagnosis of a mediastinal mass is usually based on a number of findings, including its location; the structure from which it is arising; whether it is single, multifocal (involving several different areas or lymph node groups), or diffuse; its size and shape; its attenuation (fatty, fluid, soft-tissue, or a combination of these); the presence of calcification along with its characteristics and amount; and its opacification following the administration of contrast agents.

퇴행성 신경초종의 세침흡인 생검에 대한 세포학적 소견 (Cytologic Findings of Fine Needle Aspiration Biopsy of Ancient Schwannoma)

  • Ancient (degenerated) schwannomas are benign tumors that display pronounced degenerative changes including cyst formation, calcification, hemorrhage, and hyalinization. The tumors are usually infiltrated by large numbers of inflammatory cells such as neutrophils, lymphocytes, siderophages, and histiocytes. They are located in deep locations such as the retroperitoneum and must be differentiated from malignant soft tissue tumors because spindle cells with nuclear atypia may be present. In the fine needle aspiration biopsy. The cytologic findings of two cases of ancient schwannoma occurring in posterior mediastinum and retroperitoneum, respectively, are described. Computerized tomographic findings of both cases showed changes of cystic degeneration or necrosis. A case occurred in retroperitoneum revealed features of destruction of vertebral body to suggest a malignant soft tussue tumor radiologically. The cytologic findings of aspiration biopsy of both tumors revealed that the tumor cells were spindle in shape with elongated nuclei. Some of tumor cells were arranged in a palisading fashion. Cell structures that resemble the Verocay bodies were observed. Some of the tumor cells showed pleomorphic bizarre nuclei, but no mitotic activity or chromatin clumping was seen. Inflammatory cells, siderophages and histiocytes were scattered in fibrillar material. These cytologic findings are important in the diagnosis of ancient schwannoma and in the differentiation of this tumor from the malignant spindle cell tumor.

Myocardial degeneration in Russian rat snake (Elaphe schrenckii)

  • A female Russian rat snake, Elaphe schrenckii, was presented for loss of movement. Physical examination showed the swelling in the area of heart. Radiographic examination revealed cardiomegaly, pericardial effusion, and a soft opacity in the area of swelling. Although pericardiocentesis to remove fluid out from the heart as well as vigorous treatments were given to the Russian rat snake, it died during treatments. Postmortem examination confirmed pericardial effusion of pale yellow, translucent fluid with mild dilation of the right atrium and ventricle. Formalin -fixed paraffin embedded tissue sections were stained with routine H&E and the classical von Kossa's method for histopathological demonstration. Histopathological examination revealed multifocal calcification in myocardium and consists of the displacement of muscular fiber by limy deposits. Congestive heart failure was suspicious for the snake when it was alive. In wild reptiles, muscle degeneration has been reported with nutrition disorders but the present case is the first report of myocardial degeneration in a Russian rat snake and contributes to the rare reports of cardiac disease in snakes.

The Imaging Features of Desmoid Tumors: the Usefulness of Diffusion Weighted Imaging to Differentiate between Desmoid and Malignant Soft Tissue Tumors

  • Purpose: To evaluate the imaging findings of desmoid tumors using various imaging modalities and to evaluate whether diffusion-weighted imaging (DWI) can help differentiate between desmoid and malignant tumors. Materials and Methods: The study included 27 patients with pathologically confirmed desmoid tumors. Two radiologists reviewed 23 computed tomography (CT), 12 magnetic resonance imaging (MRI) and 8 positron emission tomography-computed tomography (PET-CT) scans of desmoid tumors and recorded data regarding the shape, multiplicity, size, location, degree of enhancement, and presence or absence of calcification or hemorrhage. The signal intensity of masses on T1- and T2-weighted imaging and the presence or absence of whirling or band-like low signal intensity on T2-weighted imaging were recorded. The apparent diffusion coefficient (ADC) values of the desmoid tumors in nine patients with DWIs were compared with the ADC values of 32 malignant tumors. The maximum standardized uptake value ($SUV_{max}$) on PET-CT images was measured in 8 patients who underwent a PET-CT. Results: The mean size of the 27 tumors was 6.77 cm (range, 2.5-26 cm) and four tumors exhibited multiplicity. The desmoid tumors were classified by shape as either mass forming (n = 18), infiltrative (n = 4), or combined (n = 5). The location of the tumors was either intra-abdominal (n = 15), within the abdominal wall (n = 8) or extra-abdominal (n = 4). Among the 27 tumors, 21 showed moderate to marked enhancement and 22 showed homogeneous enhancement. Two tumors showed calcifications and one displayed hemorrhage. Eleven of the 12 MR T2-weighted images showed whirling or band-like low signal intensity areas in the mass. The mean ADC value of the desmoid tumors ($1493{\times}10^{-6}mm^2/s$) was significantly higher than the mean of the malignant soft tissue tumors ($873{\times}10^{-6}mm^2/s$, P < 0.001). On the PET-CT images, all tumors exhibited an intermediate $SUV_{max}$ (mean, 3.7; range, 2.3-4.5). Conclusion: Desmoids tumors showed homogenous, moderate to marked enhancement on CT and MRI scans and a characteristic whirling or band-like pattern on T2-weighted images. DWI can be useful for the differentiation of desmoid tumors from malignant soft tissue tumors.
