Incidence of factors of risks in the self-regulation as prevention of the tobacco in adolescents

  • Background: the self-regulating in adolescent's smokers as prevention is one of the lines of the work team in the consultation of Ceasing Tobacco. Objective: to identify the factors of risks that impact in the self-regulation to prevent the tobacco in adolescents. The investigation embraced one period from March 2017 to September 2018. Method: a descriptive study of traverse court was used. Registered to 31 students for sampling intentional non probabilistic, of an universe of 50 adolescent students. It was used empiric methods; clinical histories, interviews structured and the questionnaire. Conclusion: the female sex prevailed, where 54,8 % between the 12 to 19 years of age. Results: the incidence the factors of risks that impact in the self-regulation to prevent the tobacco in adolescents are the group contagion with 54,7 %, family problems for a 29,0 % and situational depression with 16,1 %, where it is necessary the self-regulation that should have the adolescents in the life.

담배 가격인상이 흡연률과 흡연귀속사망에 미칠 영향에 대한 연구 -대도시 일부 사무직 근로자를 대상으로- (A Study on The Effect of The Tobacco Price Raise on The Smoking Rate and Smoking Attributable Death)

  • 본 연구에서는 대도시 일부 사무직 근로자를 대상으로 흡연에 관한 설문조사를 통해 흡연률, 과거흡연률, 가격변동시 예상흡연률과 그에 따른 흡연의 귀속 사망수 감소 기대수를 조사, 분석하였다. 그 결과는 다음과 같았다. 1. 대도시 지역 남성 사무직 근로자의 흡연률은 59.5%였고 과거흡연률은 28.8%였으며, 과거흡연자를 제외하고 흡연률을 산출할 경우 흡연률은 67.4%였다. 2. 남성흡연자중 담배의 가격과 무관하게 금연할 의사가 있다고 응답한 사람의 분율은 55.3%였고, 담배의 가격이 4배 이상 인상될 경우 금연하겠다고 응답한 사람의 분율은 61.5%로서 반수 이상의 흡연자가 금연하겠다는 의사를 가지고 있는 것으로 나타났다. 3. 남성 흡연자의 흡연시작연령은 반수 이상(62.3%) 만 20세 미만으로, 미성년자 시기에 흡연을 시작하는 경우가 많았다. 4. 담배의 가격인상폭을 크게 할수록 금연 의사가 있는 사람의 비율이 늘어서, 4배이상 가격을 인상할 경우 예상 흡연률이 26.7%였다. 매년 20%씩 담배의 가격이 오를 경우 예상 흡연률은 46.2%였다. 5. 남성을 대상으로 흡연과 관련이 깊은 주요 8개 질환군(폐암, 위암, 췌장암, 후두암, 식도암, 뇌혈관질환, 허헐성심질환, 만성폐쇄성폐질환)의 귀속위험을 추산한 결과 매년 25,863명이 흡연에 기인해서 사망하는 것으로 추산되었다. 이는 20세 이상 총 남성 사인 분류가능 사망수 128,875명의 20.1%이다. 6. 담배의 가격인상시 기대되는 흡연률을 적용하여 계산한 결과 담뱃값을 4배 이상 올렸을 때 금연 실천률을 100%로 가정했을 때 귀속위험은 13,527명으로서 매년 홉연으로 인한 사망의 47.7%인 12,336명의 사망을 줄일 수 있는 것으로 추산되었다. 7. 담배의 가격인상시 금연하도록 동기부여가 되는 사람중에서 실제로 금연하는 사람의 비율을 10%, 25%, 50%로 가정했을 때, 기대 사망수 감소는 가격을 4배 이상 인상했을 때 각각 1,112명, 3,483명, 5,796명 이었다.

미국 담배소송의 변천과 보건법정책 효과 (The Development of Tobacco Litigation in USA and it's Impact of Law and Politics in Public Health)

  • Since mid-1960s the reports from the Surgeon General, the World Health Organization, and other health experts state that there is no risk-free level exposure to smoking and secondhand smoke. Tobacco smoke is made up of more than 7,000 chemicals. Hundreds are toxic, and at least 70 are carcinogens. The chemicals in tobacco smoke reach smoker's lungs quickly every time smoker inhale causing damages immediately. Inhaling even the smallest amount of tobacco smoke can also damage smoker's DNA, which can lead to cancers. Smoking is responsible for more than 87% of lung cancers, but there are a host of other chronic diseases directly related to exposure to tobacco smoke. It's also a major cause of heart disease, stroke, aortic aneurysm, peripheral arterial disease and most of the other diseases. In the United States, each year with more than from 440,000 to 520,000 deaths caused by smoking and exposure to involuntary smoke. They conclude that smoking is the single most important source of preventable morbidity and mortality. The United States of America have about 60-year history of tobacco litigation. Tobacco litigation has been an important tool in tobacco control strategies aimed at limiting the activities of tobacco companies and providing redress to people who have become ill as a result of their use of tobacco products. Tobacco litigation is a kind of tort litigation. Quite often, as in the asbestos and other mass tort litigation episodes, tobacco litigation can play an educational role, warning the public about the magnitude of health risks that might otherwise be less clearly perceived. Tobacco litigation allows smokers, their families or other victims of smoking to sue tobacco companies in order to be compensated for the harm they have suffered. Potential benefits of tobacco litigation include compensation for smoking-related damages, strengthening regulatory activity, publicity, documents disclosure and changing tobacco industry behavior. And also tobacco litigation can limit the political activities of tobacco industry, protect human rights of smokers and non-smokers, increase burden to tobacco price-up and enhance the effects of law and politics in public health.

신종담배의 출현과 정책방향: 전자담배 및 가열담배에 대한 전략 (The Government Policies of New Tobacco Products: Strategies for Managing Electronic Cigarettes and Heated Tobacco Products)

  • The term new tobacco products (NTPs) refers to the new alternatives to conventional cigarettes. There are several kinds of NTPs in South Korea. The present study discusses the most widely used NTPs namely electronic cigarette (ECs) and heated tobacco products (HTPs). The aims of this study are to evaluate the risk related to the use of ECs and HTPs, introduce policy examples across different countries of management of this issue, and finally, present some policy implications of the problem and our response strategies. Since the advent of ECs, there has been a lot of debate about its risk. Some studies have reported that ECs are less harmful than conventional cigarettes and that they are effective in aiding smoking cessation. Nevertheless, the efficacy of ECs in smoking cessation and its potential health risks are still unclear. However, the obvious fact is that it is not harmless. Regulations on ECs differ from country to country. In many countries, they are strictly regulated as tobacco or toxic substances; however, in the United Kingdom, the use of ECs are included as part of their smoking cessation policy, and in Japan, they are treated as a form of medication. On the other hand, HTPs are the most recently introduced NTPs and they have attained sensational popularity because of the wrongly held belief that they are less harmful to health. So, what about our policy response to these two tobacco products? The research on ECs requires more systematic statistical monitoring, such as monitoring the ratio of dual-users. Further, the new EC smokers should be identified taking into account that the arguments for the use of ECs often emphasize smoking cessation or less risks to health, the government should further strengthen its policy to prevent those claims. The HTPs market experienced a very sharp growth and continues to grow because the government policy is too passive. Taking this as a lesson, it is necessary to approach NTPs, such as HTPs, proactively and increase their contribution to the National Health Promotion Fund by imposing greater taxes on them. Finally, considering the likelihood of NTPs being promoted as a less harmful tobacco product, it is essential to strictly regulate tobacco companies' publicity from the very beginning to ensure that potential consumer s are not mislead.

일부 미적용 다중이용시설의 실내 공기 중 알데히드류 및 휘발성유기화합물 노출로 인한 건강위해성 평가 (Health Risk Assessment of Occupants in the Small-Scale Public Facilites for Aldehydes and VOCs)

  • This study was to assess the lifetime cancer and non-cancer risk of exposure of worker and user at public facilities in Korea to volatile organic compounds (VOCs). We measured the concentrations of two aldehydes and five VOCs in indoor air at 424 public buildings that 8 kinds of public facilities (70 movie theaters, 86 offices, 86 restaurants, 70 academies, 22 auditoriums, 30 PC-rooms, 30 singing-rooms and 30 bars) all over the country. There were estimated the human exposure dose and risks with averages of the using-time and frequency for facility users and office workers, respectively. Carcinogens (formaldehyde, acetaldehyde, and benzene) were estimated the lifetime excess cancer risks (ECRs). non-carcinogens (toluene, ethylbenzene, xylene, and styrene) were estimated the hazard quotients (HQs). The average ECRs of formaldehyde and benzene for facility worker and user were $1{\times}10^{-3}{\sim}1{\times}10^{-4}\;and\;1{\times}10^{-4}{\sim}1{\times}10^{-5}$ level, respectively, in all facilities. HQs of four non-carcinogens did not exceed 1.0 for all subjects in all facilities. The estimated ECRs for restaurant and auditorium were the highest, and the PC-room and bar were the next higher facilities. Furthermore, people in a smoking facility had the highest cancer risk. Higher ECRs of formaldehyde and benzene were observed in indoor smoking facilities such as restaurant and auditorium. Higher HQs of toluene and xylene were observed at the restaurant and office building.

Estimated Carotenoids Intake in Korean Adults Using Food-frequency Questionnaire: Association with Smoking, Drinking and Other Life-style Factors

  • The aim of this study was to determine the association between, smoking, exercise, sex, and dietary carotenoids ($\alpha$-carotene, $\beta$-carotene, $\beta$-cryptoxanthin, lutein + zeaxanthin, lycopne) intake in Korean middle-aged adults. Food-frequency questionnaire were analyzed from 304 healthy adults (115 men, 189 women) aged 20 - 59. The self-administered questionnaire contained subject s habitual diet and alcohol intake were the previous 3 months. Data on frequency of 102 foods, including vegetables, fruits, beverages and legumes were analyzed. Total dietary carotenoids intake were 27.13 $\pm$ 3.09 mg/d for men and 26.71$\pm$ 2.82 mg/d for women. It was found that smoking had no significant contribution to the dietary intake of carotenoids. Among other lifestyle factors that had significant correlation was the amount of exercise time. The increases in exercise time was associated with increase in carotenoids intake (r= 0.121, p : 0.04). The major contributors of $\alpha$-carotene and $\beta$-carotene were carrots consumed as single-food item or carrot juice. Lutein and Zeaxanthin intake mainly came from spinach and most lycopene intake was derived from tomato products not fresh tomatoes. Persimmon was the major contributor of $\beta$-cryptoxanthin. These findings provide valuable information on understanding the unique pattern of dietary intake of Korean, which might help identify the risks for developing various diseases.

Modifying Effect of Suicidal Ideation on the Relationship Between Asthma and Cigarette Use Behaviors Among Korean Adolescents

  • Objectives: Although cigarette smoking is known to be related to the exacerbation of asthma symptoms, several studies have indicated that the prevalence of cigarette smoking among asthmatic adolescents is similar to or even higher than that among non-asthmatic adolescents. The aim of this study was to evaluate the relationship between asthma and cigarette use behaviors and whether or not the presence of suicidal ideation modifies this relationship among Korean adolescents. Methods: We analyzed data from the 2008 Korea Youth Risk Behavior Web-based Survey, which included a nationally representative sample of middle and high school students. Multiple logistic regression models were used to calculate odd ratios and 95% confidence intervals of cigarette use behaviors among current asthmatics, former asthmatics, and nonasthmatics, after adjusting for gender, grade, school records, socioeconomic status, current alcohol use, and suicidal ideation. Results: Of 75 238 study participants, 3.5% were current asthmatics and 4.5% were former asthmatics. Compared with non-asthmatics, asthmatics were more likely to report current cigarette use, frequent and heavy cigarette use, and cigarette use before 13 years of age. There were statistically significant interactions between asthma and suicidal ideation in cigarette use behaviors. Conclusions: This study demonstrated that asthmatic adolescents are more likely than non-asthmatic adolescents to engage in cigarette use behaviors and the presence of suicidal ideation is an effect modifier of the relationship between asthma and cigarette use behaviors. Particular attention should be paid to the awareness of health risks of cigarette smoking and mental health problems among asthmatic adolescents.

Tobacco Chewing and Adult Mortality: a Case-control Analysis of 22,000 Cases and 429,000 Controls, Never Smoking Tobacco and Never Drinking Alcohol, in South India

  • Background: Tobacco is consumed in both smoking and smokeless forms in India. About 35-40% of tobacco consumption in India is in the latter. The study objective was to describe the association between chewing tobacco and adult mortality. Materials and Methods: A case-control study was conducted in urban (Chennai city) and rural (Villupuram district) areas in Tamil Nadu state in South India. Interviewed in 1998-2000 about 80,000 families (48,000 urban and 32,000 rural) with members who had died during 1995-1998. These were the cases and their probable underlying cause of death was arrived at by verbal autopsy. Controls were 600,000 (500,000 urban, 100,000 rural) individuals from a survey conducted during 1998-2001 in the same two study areas from where cases were included. Results: Mortality analyses were restricted to non-smoking non-drinkers aged 35-69. The age, sex, education and study area adjusted mortality odds ratio was 30% higher (RR:1.3, 95%CI:1.2-1.4) in ever tobacco chewers compared to never chewers and was significant for deaths from respiratory diseases combined (RR:1.5, 95%CI:1.4-1.7), respiratory tuberculosis (RR:1.7, 95%CI:1.5-1.9), cancers all sites combined (RR:1.5, 95%CI:1.4-1.7) and stroke (RR:1.4, 95%CI:1.2-1.6). Of the cancers, the adjusted mortality odds ratio was significant for upper aero-digestive, stomach and cervical cancers. Chewing tobacco caused 7.1% of deaths from all medical causes. Conclusions: The present study is the first large study in India analysing non-smoking non-drinkers. Statistically significant excess risks were found among ever tobacco chewers for respiratory diseases combined, respiratory tuberculosis, stroke and cancer (all sites combined) compared to never tobacco chewers.

디젤엔진 배출물질과 폐암발생 위험에 관한 고찰 (A Review on Diesel Engine Exhaust and Lung Cancer Risks)

  • Objectives: Diesel engine exhaust (DE) accounts for a significant percentage of air pollutants that are associated with various health outcomes including mortality, asthma, chronic bronchitis, respiratory tract infection, etc. In June, 2012, the International Agency for Research on Cancer (IARC) released the assessment results that classified DE as "carcinogenic to humans" (Group 1). This review is therefore focused on the lung cancer risks of DE. Methods: Literatures were searched using PubMed with key words of "diesel exhaust", "lung cancer", and other related terms for the period between 1990 and 2012. A total of 295 articles were searched and sixteen epidemiologic studies were identified as potentially relevant. Results: Sixteen epidemiologic studies about the lung cancer risks of workers exposed to DE in various occupations were summarized in two tables, 1) retrospective cohort studies and 2) case-control studies. Increased lung cancer risk, although not always smoking adjusted, was observed in 6 out of 8 retrospective cohort studies and 4 of 8 case-control studies. Conclusions: Diesel fuel is widely used in Korea. Exposure to DE is confirmed to be a human carcinogen by IARC. Noncancer health risks of DE also need careful attention as DE is a major source of fine-particle pollution. Along with the efforts for reducing the DE emission through improvements of diesel engines and fuel, and the use of alternative fuels, comprehensive health risk assessment of DE should be conducted to minimize the adverse health effects.

Cigarette Smoking and other Risk Factors for Kidney Cancer Death in a Japanese Population: Japan Collaborative Cohort Study for Evaluation of Cancer Risk (JACC study)

  • Background: Cigarette smoking is the largest single recognized cause of human cancers. In Western countries, many epidemiologists have reported risk factors for kidney cancer including smoking. However, little is known about the Japanese population. Materials and Methods: We evaluated the association of smoking with the risk of kidney cancer death in the Japan Collaborative Cohort (JACC) Study. Participants included 46,395 males and 64,190 females. The Cox proportional hazards model was used to determine age-and-sex adjusted relative risks. Results: A total of 62 males and 26 females died from kidney cancer during the follow-up of 707,136 and 1,025,703 person-years, respectively. Heavy smokers (Brinkman index >1200), fondness of fatty foods, hypertension, diabetes mellitus (DM), and obesity were suggested to increase the risk of renal cell carcinoma while walking was suggested to decrease the risk. Even after controlling for age, sex, alcohol drinking and DM, heavy smoking significantly increased the risk. Conclusions: The present study suggests that six factors including smoking may increase and/or reduce the risk of kidney cancer in the Japanese population. Because of the small number of outcomes, however, we did not evaluate these factors after adjusting for all possible confounding factors. Further studies may be needed to confirm the findings in this study.
