Lifestyle Factors Including Diet and Leukemia Development: a Case-Control Study from Mumbai, India

  • In India, among males, leukemia rates vary across the country. The present unmatched hospital-based case-control study conducted at Tata Memorial Hospital included subjects registered between the years 1997-99. There were 246 leukemia cases and 1,383 normal controls. Data on demographics, lifestyle, diet and occupation history were recorded. Cigarette (OR=2.1) and bidi smoking (OR=3.4) showed excess risk for leukemia. Odds ratios were 3.9 for fish-eaters, 0.40 for chilli eaters, 1.5 for milk drinkers and 0.60 for coffee drinkers, compared to non-drinkers/eaters. However, neither exposure to use of pesticides nor cotton dust showed any excess risk for leukemia.

한국 여성 유방암의 위험요인 분석 (A Study on the Risk Factors for Breast Cancer in Korean Women)

  • This study was done to provide basic data on prevention of breast cancer by identifying and analyzing risk factors for breast cancer in Korean women. The data were collected from three hospitals. The data from Jan. 1995 to Dec. 1997 was based on the patient recordings and personal contacts if possible. Data from 1998 to 1999 was collected from 280 inpatients and outpatients diagnosed as breast cancer. From this data, information about the risk factors of breast cancer could be acquired. The factors are scaled according to the degree of risk. Menstrual history, obstetrical history, family history. lactation period, life style were given 5 point, and body mass index was given 4 point. Using one-way ANOVA. the most important risk factors were identified and analyzed by multiple regression. The conclusion of this study are as follows : General factors which would show an effect on the results of the multiple regression included risk factors such as menstrual history, obstetrical history, lactation period, family history, and life style factors including mortal state, monthly income, educational level, job and age. Family history(R=0.481) was found to be the highest risk factor(23.1%) affecting breast cancer risk. The next factors were dying hair(R=0.603), drinking(R=0.846), body mass index(R=0.885), smoking(R=0.916), age of menopause(R=0.937), pregnancy age of first full-term(R=0.957), eating vegetable(R=0.980) and the number of full term pregnancies in that order. The sum total(R=0.986) of all the above factors accounted for 97.3% of the risk of breast cancer for Korean women. In conclusion, it was found that family history and dyeing hair were the highest risk factors for breast cancer. It is recommended that those with a family history of breast cancer should have regular breast examinations, and those who often dye their hair should reconsider this practice.

건강믿음모형을 이용한 금연성공자의 재흡연에 영향을 미치는 요인 분석: 금연클리닉 등록자를 중심으로 (Factors Associated with Relapse to Smoking Behavior Using Health Belief Model)

  • 이 연구는 보건소 금연클리닉에 등록하여 6개월 금연에 성공한 사람들의 장기적인 금연 지속 여부와 재흡연에 영향을 미치는 요인을 파악하기 위하여 실시되었다. 2008년 1월부터 2009년 2월까지 C시 보건소 금연클리닉에 등록하여 6개월 금연에 성공한 920명 중 조사에 응한 남성 159명을 최종 분석 대상으로 하였다. 연구 자료는 금연클리닉 등록카드와 2010년 9월 20일부터 10월 20일까지 실시된 우편설문을 통해 수집되었다. 연구에 포함된 변수는 응답자들의 흡연상태, 인구사회학적 특성, 금연클리닉 등록 당시의 흡연관련 특성, 금연클리닉을 통하여 제공받은 서비스, 건강행태, 건강믿음모형의 구성 변수 등이었다. 연구의 결과는 다음과 같다. 6개월 금연성공자 중 지속적 금연성공자는 57.2%, 중간 흡연자는 17.0%, 재흡연자는 25.8%였다. 재흡연자들의 재흡연 이유는 스트레스가 쌓여서 76.2%, 본인의 의지가 약해서 16.7%, 주위의 유혹에 의해서 7.1%로 나타났다. 응답자의 인구사회학적 특성, 과거의 흡연관련 특성, 제공받은 금연상담서비스, 건강행태 요인 등은 6개월 금연 성공자의 1년 후 재흡연 여부에 유의한 영향을 미치지 못하였으며, 건강믿음모형의 구성요소 중 (1) 흡연으로 인한 질병의 발생 가능성, (2) 흡연으로 야기되는 질환의 심각성, (3) 금연의 건강증진에 대한 도움, (4) 금연으로 인한 스트레스와 사회생활에 대한 장애정도, (5) 자기효능감이 단변량분석에서 재흡연 여부에 유의한 영향을 미치는 것으로 나타났다. 서열 로짓 분석을 시행한 결과 이 중 (4) 금연으로 인한 스트레스와 사회생활에 대한 장애정도, (5) 자기효능감만이 재흡연 여부에 유의한 영향을 미쳤으며, 특히 자기효능감이 매우 중요한 영향을 미치는 것으로 밝혀졌다. 이 연구를 통해 보건소 금연클리닉을 통하여 6개월 금연에 성공한 남성들 중 상당수가 재흡연을 하게 된다는 것을 알 수 있었다. 재흡연자들은 다시 담배를 피우게 된 이유를 스트레스의 증가나 자신의 의지부족 때문이라고 하였으며, 재흡연에 영향을 미치는 요인이 금연자의 인구사회학적 변수나, 과거의 금연습관보다는 심리학적 요인, 특히 자기효능감과 금연과 관련된 인지된 장애임도 밝혀졌다. 이러한 연구결과는 금연클리닉 등록자 중 재흡연 위험이 높은 사람을 선별하여 맞춤식 관리를 하는데 도움을 줄 것으로 기대되며, 금연 클리닉 등록자를 위한 스트레스 해소나 자기효능감 강화 프로그램 개발이 시급함을 시사한다. 이 연구는 일개 중소도시의 금연클리닉 등록자 중 남성만을 대상으로 하였고, 우편조사로 인한 낮은 응답률, 조사 시기에 따른 회상 오차, 금단 증상, 행동계기(cue to action), 금연에 대한 사회적 지지와 같은 변수가 누락되는 등 연구방법론 상에 제한이 있어 연구결과의 일반화에 주의를 요하며, 향후 연구에서는 이러한 문제점을 보완할 필요가 있다.

흡연이 운동에 미치는 단기 효과 (Immediate Effect of Cigarette Smoking on Exercise)

  • 연구배경 : 장기적 흡연이 폐암이나 만성폐쇄성폐질환등의 폐질환 및 여러 심장질환의 위험인자인 것은 잘 알려져 있으나 흡연이 단기적으로 운동시 호흡기 및 심혈관계 기능에 어떠한 영향을 미치는지에 대한 연구는 많지 않다. 흡연시에는 일산화탄소를 흡입하으로, 일산화탄소 흡입후 운동시에 나타나는 호흡수, 심박수, 호흡량 및 심장출량의 증가와 최대산소섭취량의 감소가 흡연후 운동시에도 관찰될 것으로 예상된다. 목적 : 흡연의 운동부하시 심폐기능에 대한 단기적 효과를 알아보고자 하였다. 방법 : 흡연자 13명을 대상으로 시간당 3개피를 5시간 흡연한 흡연일 및 흡연하지 않고 100% 산소를 15분간 흡입한 금연일을 무작위 순서로 정하여 두 번 단계적 운동부하 검사를 시행하였다. 결과 : 대상자 13명의 평균 연령은 $25{\pm}4.9$세, 평균 흡연력은 $6{\pm}5$ pack-years였고, 금연일과 흡연일간 일반 폐기능검사의 결과에는 차이가 없었다. 혈중 일산화 탄소혈색소량의 평균은 금연일 $1.45{\pm}0.83%$, 흡연일에 $5.97{\pm}1.34%$로서 흡연일에 유의하게 높았다(p<0.01). 무산소역치는 금연일 $1.53{\pm}0.20$ L/min, 흡연일에 $1.33{\pm}0.24$ L/min로서 흡연일에 유의하게 낮았으며(p<0.05) 최대산소섭취량은 금연일 $2.39{\pm}0.32$ L/min, 흡연일에 $2.09{\pm}0.32$ L/min로서 흡연일에 유의하게 낮았다(p<0.05). 안정시 심박수는 금연일에 분당 $75.46{\pm}5.83$회, 흡연일에 분당 $84.38{\pm}11.06$회로서 흡연일에 유의하게 높았으나(p<0.05) 최대심박수는 금연일에 $160.38{\pm}9.09$회, 흡연일에 $161.23{\pm}8.09$회로서 차이가 없었다. 결론 : 일반 폐기능검사는 양일간에 차이가 없었으나 금연일에 비해 흡연일에 무산소역치와 최대산소섭취량의 감소가 관찰되어 흡연은 단기적으로 운동부하시 심혈관계장애를 초래한다고 사료되었다.

일 지역 여성의 유방암 위험요인과 유방암 조기검진 행위 분포 (Risk Factors and Early Screening Behavior for Breast Cancer in Rural Women)

  • Purpose: The aim of this study was to identify risk factors for breast cancer and early screening behavior in women in the community. Method: The participants were 125 women residing in W city. Data was collected using an instrument developed by the researchers. Analysis was done using descriptive statistics, and the $x^2$ test. Result: For risk based on the Gail Model, age (above 50 years) had a distribution of 24.8%, first degree family history, 4.9%, age at first full term pregnancy, 13.8%, and benign breast cancer history, 4.9%. For risk based on other common risk factors, menopause had a distribution of 20.7%, did not breast feed, 15.4%, history of HRT, 7.3%, meat preference, 35.0%, and history of smoking or drinking, 2.4% and 43.5%, respectively. There was a significant difference in BSE and mammography screening behavior ($x^2=22.5$, p<.00), but no difference in distribution of risk factors and screening behavior. Conclusion: For effective prevention of breast cancer, it is necessary to develop an instrument for risk assessment and, through assessment, select women at high risk. It is also necessary to provide education and appropriate recommendations on screening behavior.

중풍 환자의 호전도와 연관성이 있는 인자들에 대한 연구 - 제천 세명대 부속 한방병원 입원 환자를 중심으로 - (Study on the Factors that Have Correlation with Improvement of Cerebrovascular Accidents)

  • The purpose of this study is to evaluate factors that have correlation with improvement of Cerebrovascular accidents(C.V.A) and to decide ranking of influence about improvement of C.V.A. This observation was made on 153 subjects of C.V.A. that were diagnosed through brain MRI or brain CT. They were hospitalized in the Semyung University Oriental Medicine Hospital from the January 1st 2006 to December 31th 2007. The subjects of this study are divided into two groups. The one group has slight motor disturbance, and the other group has severer motor disturbance. Based on medical treatment chart, we analyze differences of many factors like past history, family history, drinking, smoking, several symptoms with C.V.A., etc between two groups. As a result, The past history of cerebrovascular disease and past history of hypertension are the most influencing factors in improvement of C.V.A.

한국 폐경 여성의 골다공증 위험 인자에 관한 연구 (Risk Factors for Postmenopausal Osteoporosis in Korea)

  • Objective : To identify the risk factors for postmenopausal osteoporosis in Korea Materials and methods : Bone mineral density (BMD) at the lumbar spine and femoral neck was measured by dual energy X-ray absorptiometry in 808 apparently normal postmenopausal Korean women. Questions about life style, demographic parameters, medical history and social habits etc. were asked on these women; 2ll women with normal bone mineral density, and 597 women with osteopenia or osteoporosis. Results 'Age of >50 years, low body mass index (BMI; <18.5 kg/m2), long duration of menopause(>10 years), and previous history of fracture were associated with increased prevalence of osteopenia or osteoporosis. Women without the outside activity also showed a higher frequency of low bone mass, Risk for osteopenia or osteoporosis was low in women with high BMI (>23 kg/m2) and women with job. The prevalence of low bone mass appeared to be independent of the following parameters: socioeconomic status, familial history, smoking, drinking, exercise, previous use of oral contraceptive, coffee or milk consumption, and degree of sunlight exposure. Conclusion 'Age, BMI, duration of menopause, previous history of fracture and degree of outside activity are the risk factors for postmenopausal osteoporosis in Korea.

복부비만과 허혈성 뇌졸중 (Abdominal Obesity as a Risk Factor of Ischemic Stroke in Case-control study)

  • Objective: The purposes of this study are to know the relations of abdominal obesity and ischemic stroke and to know what index could represent the abdominal obesity appropriately. Methods. We have done case-control study and recruited 97 ischemic stroke patients and 117 controls. Case is matched by control by individual matching. All participants had questionnaire, interview and then were examined waist-hip ratio, waist circumference and body mass index. Results : WC, WHR and Hypertension history had differences in two groups, case and control groups. But BMI and other factors weren't significant. According to Quartile of Waist Circumference, the two groups had the differences in hypertension history, diabetes history, smoking status, WHR, BMI, and weight and in the Quartile of Waist-Hip Ratio Quartile, past history of hypertension and diabetes, WC, BMI and weight had the significances. Sex, Age Adjusted and Multivariate Odds Ratio (95% Confidence Interval) of WC Quartile are 2.083, 1.628, 4.491 and 4.418, 4.964, 12.306, and in WHR, the Ors are 2.252, 5.743, 15.776 and 2.632, 8.918, 23.596. Conclusions: We knew from these results that abdominal obesity is very important risk factor of ischemic stroke and WHR more than WC is a good indicator of abdominal obesity, so we should reduce the WHR to prevent of ischemic stroke.

뇌졸중에 영향을 미치는 생활습관 요인 -흡연, 음주, 비만, 식습관을 중심으로- (The Lifestyle Factors in Stroke Etiology: Smoking, Alcohol Consumption, Obesity, Perception of Saltness)

  • Stroke is a serious disease despite recent improvement in medical and surgical treatment. Hence, identification of modifiable risk factors for stroke is important. This case-control study was done to demonstrate that relationship between smoking, alcohol consumption, obesity, perception of saltness and the incidence of stroke and to identify that smoking, alcohol consumption, obesity and perception of saltness, after adjusting for age, hypertension. A structured interview was carried out from April 15, 1996 to May 3, 1996 in Yonsei Medical Center. The study group consisted of 59 neurologically confirmed stroke patients as the study group and 59 non-stroke patients as controls. Analysis of the data was done by means of ${\chi}^2$-test and logistic regression analysis. The results were as follows. In the study group: 1) Hypertension in males had a 10.2 odds ratio (p<0.05), cardiovascular disease in females had a 11.3 odds ratio (p<0.05) and a farnily history of stroke in males had a 3.1 odds ratio (p<0.05). 2) Females smoking one or more cigarettes had a 8.3 odds ratio (p<0.1), but males had no direct relationship with odds ratio of 1.5 (non-significant). 3) Alcohol consumption in males had a 0.4 odds ratio, and in females had a 0.8 odds ratio. The odds ratio was decreased in alcohol consumption group (non-significant). 4) Males with more than 20 cigarettes pack-years history had a 2.5 odds ratio (p<0.05), more than 25 Body Mass Index had a 3.1 odds ratio (p<0.05) and more than 220 ml ethanol weekly consumption had a 1.5 odds ratio (non-significant). 5) Female smokers had a 8.3 odds ratio (p<0.1), drinkers a 0.8 odds ratio and more than 25 Body Mass Iidex, a 43.1 odds ratio (p<0.05). 6) Females without saltness perception from a 0.5% salt solution had a 6.8 odds ratio (non-significant). 7) By logistic regression analysis independent risk factors for stroke in males were found to be hypertention, age, and obesity. The study was limited because number of subjects was too small for practical implications. However, like as other results, this study suggest that people should be advised to control hypertension, and obesity since these carry a risk of stroke.

Risk Factors for Upper and Lower Urinary Tract Cancer Death in a Japanese Population: Findings from the Japan Collaborative Cohort Study for Evaluation of Cancer Risk (JACC Study)

  • Background: The incidence of bladder cancer is lower in Asian than in Western countries. However, the crude incidence and mortality of bladder cancer have recently increased in Japan because of the increased number of senior citizens. We have already reported risk factors for urothelial cancer in a large population-based cohort study in Japan (JACC study). However, we did not evaluate the cancer risk in the upper and lower urinary tract separately in our previous study. Materials and Methods: Here we evaluated the risk of cancer death in the upper and lower urinary tracts, separately, using the database of the JACC study. The analytic cohort included 46,395 males and 64,190 females aged 40 to 79 years old. The Cox proportional hazard model was used to determine hazard ratios and their 95% confidence intervals. Results: Current smoking increased the risk of both upper and lower urinary tract cancer deaths. A history of kidney disease was associated with an increased risk of bladder cancer death, even after controlling for age, sex and smoking status. Conclusions: The present study confirmed that current smoking increases the risk of both upper and lower urinary tract cancer deaths and indicated the possibility that a history of kidney disease may be a risk factor for bladder cancer death in the Japanese population.
