Clinical and Radiological Results after Arthroscopic Superior Capsular Reconstruction in Patients with Massive Irreparable Rotator Cuff Tears

  • Background: Massive, irreparable rotator cuff tears (RCTs) are a challenging clinical problem in young patients. In recent years, arthroscopic superior capsular reconstruction (ASCR) is a popular treatment in the massive, irreparable RCTs. However, studies reporting clinical results of ASCR are rare in the literature. Methods: Between 2013 and 2015, six patients underwent ASCR. One patient treated with dermal allograft, while five patients with autogenous fascia lata graft. Demographic data, as well as preoperative and last follow-up clinical data including pain, range of motion (ROM), strength, American Shoulder and Elbow Surgeons system, the Constant system, the University of California at Los Angeles system, the Simple Shoulder Test, and the Shoulder Pain and Disability Index system were obtained. Acromiohumeral distances and Hamada classification were measured on standard anteroposterior x-ray. Results: All patients were men, and the average age was $59.5{\pm}4.18years$ (range, 53-65 years).The minimum follow-up was 18 months with a mean follow-up was $27.33{\pm}7.58months$ (range, 18-36). All patients had postoperative improvement in pain scores and functional scores. The ROM and strength did not improve after surgery. The Hamada score progressed of radiographic stage in 2 patients. In the case of dermal allograft, there was graft failure 6 weeks after ASCR. Conclusions: Our results support the ASCR as a viable treatment for surgical salvage in massive, irreparable RCTs. This treatment option may provide patients with decreased pain and increased function. And studying our case of dermal allograft failure provides opportunities to decrease graft failure in ASCR using dermal allograft.

The Effects of Several Halophytes on Insulin Resistance in Otsuka Long-evans Tokushima Fatty Rats (OLETF 쥐에서 칠면초와 세발나물의 인슐린 저항성 개선 효과)

  • We evaluated preventive effects of Suaeda japonica (SJ) and Spergularia marina Griseb (SMG) on the insulin resistance in Otsuka Long-Evans Tokushima Fatty (OLETF) rats. The 10-week old OLETF rats were fed diets containing 3% (w/w) SJ and SMG for 18 weeks. Fasting blood glucose levels in SJ and SMG groups, measured using the oral glucose tolerance test, were lower than that of the control rats. The SMG group showed significantly lower levels of insulin, glycated hemoglobin, triglyceride, and total cholesterol than the control group. In addition, these levels were relatively lower in the SJ group than those in the control rats. The SJ and SMG groups had relatively lower protein levels of nuclear factor-kappa B (NF-${\kappa}B$) p65 in adipose tissue and serine phosphorylated insulin receptor substrate 1 (IRS-1) in skeletal muscle than the control group. These results suggest that SJ and SMG prevent insulin resistance and SMG in particular reduces blood triglyceride and total cholesterol levels.

${\gamma}$-NGF Produced in CHO Cells Does Not Cleave Mouse Ren-2 Prorenin

  • We have recently demonstrated, by protein and cDNA sequence analysis, that prorenin converting enzyme (PRECE) in the mouse submandibular gland is identical to the epidermal growth factor-binding protein (EGF-BP)type B. However, type A and C did not show prorenin converting activity. To demonstrate whether r-NGF is involved in prorenin processing, we have cloned cDNA of r-NGF and examined prorenin converting activity using the CHO cell expression system, Trypsin converted the 33 kDa r-NGF precursor produced in CHO cells to a two-chain form, 9.4 and 16.4 kDa polypeptide chains, which has been known as an active form of r-NGF in mouse SMG (Server and Shooter, 1976). However, the two chain forms of r-NGF did not reveal prorenin-processing activity. Thus, only PRECE is involved in prorenin processing in mouse SMG. This result shows that their substrate specificities appear to be very strict, although some kallikreins share a high degree of amino acid sequence identity.

  • This paper describes the full load test results of IGBT VVVF inverter for the railway propulsion system. The 1,650kVA IGBT VVVF inverter has been developed. Therefore, the field test is performed in SMG 6 Line to confirm its the reliability and performance. The train consists of 4M4T(4 Motor car 4 Trailer Car) and the electrical equipment for field test are as follows VVVF inverter 4 sets, 16 traction motors and 2 SIVs. The propulsion system is composed with IC4M(1-Controller 4-Motors). The results of propulsion system which have the excellent acceleration/deceleration and the jerk characteristics as well as starting ability on slope are taken through the field test.

Cloning and Characterization of cDNA Encoding Potentially Functional Mouse Glandular Kallikrein

  • We cloned a cDNA (pPRC-1) which was comprised of 841 nucleotides from the cDNA library of a male ICR mouse submandibular gland ($SMG^+$). The nucleotide sequences of pPRC-1 were identical to those of exons 2 and 3 of the mGK-21 gene, a potentially functional glandular kallikrein identified in a Balb/c mouse, except for one nucleotide residue. Although this substitution changes Ile (ATT) in pPRC-1 to Val (GTT) in mGK-21, this difference has been explained by strain polymorphism. From the amino acid sequences predicted from its cDNA, we speculated that mGK-21 gene products/pGK21 consist of 261 amino acids including the $NH_2$-terminal signal peptide (residues 1~17), the short propeptide (residues 17~24), and the active peptide (residues 25~261). Although we did not demonstrate the enzyme activity of pGK21, it was assumed that pGK 21 was involved in the maturation of certain bioactive polypeptide(s) in mouse SMG for the following reasons : (a) mGK-21 gene was apparently expressed in a male ICR mouse SMG: (b) the proposed active site $His^{65}$, $Asp^{120}$, and $Ser^{213}$ residues were completely conserved in pGK21 just like other glandular kallikreins; (c) the cloned cDNA was translated to a predicted 27 kDa polypeptide chain in vitro: (d) the 27 kDa polypeptide chain produced by CHO cells was produced to a putative active form by trypsin.

Effects of Stretching and Meditation on Anxiety, Depression, and Quality of Life in Patients with Sequelae of COVID-19 (코로나 19 후유증 환자에게 스트레칭과 명상이 불안과 우울 및 삶의 질에 미치는 효과)

  • Purpose: This study aims to confirm the effects of stretching and meditation intervention on mental health-related anxiety, depression, and quality of life in college students in their 20s with sequelae of COVID-19. Methods: 30 men and women were randomly assigned to the stretching and meditation intervention group (10 people), the stretching intervention group (10 people), and the meditation group (10 people), and the experiment was conducted for 4 weeks. As for the analysis method, a paired t-test was performed for intra-group changes after 4 weeks, and one-way analysis of variance was performed for between-group changes. In addition, the significance level was set at 0.05. Results: Within-group changes in anxiety and depression were significantly different in all groups after 4 weeks (p < 0.05). There was a significant difference between SMG and SG and between SMG and MG in the change between groups after 4 weeks (p < 0.05). In the change in quality of life, there was a significant difference in all groups after 4 weeks (p < 0.05). There was also a significant difference between SMG and MG in the change between groups after 4 weeks. Conclusion: Based on the results of this study, the combined method of meditation and stretching intervention had a positive effect on the mental health of adults in their 20s and suggests the possibility of using it to improve mental health after recovering from COVID-19.

Deep Learning-Based Algorithm for the Detection and Characterization of MRI Safety of Cardiac Implantable Electronic Devices on Chest Radiographs

  • Objective: With the recent development of various MRI-conditional cardiac implantable electronic devices (CIEDs), the accurate identification and characterization of CIEDs have become critical when performing MRI in patients with CIEDs. We aimed to develop and evaluate a deep learning-based algorithm (DLA) that performs the detection and characterization of parameters, including MRI safety, of CIEDs on chest radiograph (CR) in a single step and compare its performance with other related algorithms that were recently developed. Materials and Methods: We developed a DLA (X-ray CIED identification [XCID]) using 9912 CRs of 958 patients with 968 CIEDs comprising 26 model groups from 4 manufacturers obtained between 2014 and 2019 from one hospital. The performance of XCID was tested with an external dataset consisting of 2122 CRs obtained from a different hospital and compared with the performance of two other related algorithms recently reported, including PacemakerID (PID) and Pacemaker identification with neural networks (PPMnn). Results: The overall accuracies of XCID for the manufacturer classification, model group identification, and MRI safety characterization using the internal test dataset were 99.7% (992/995), 97.2% (967/995), and 98.9% (984/995), respectively. These were 95.8% (2033/2122), 85.4% (1813/2122), and 92.2% (1956/2122), respectively, with the external test dataset. In the comparative study, the accuracy for the manufacturer classification was 95.0% (152/160) for XCID and 91.3% for PPMnn (146/160), which was significantly higher than that for PID (80.0%,128/160; p < 0.001 for both). XCID demonstrated a higher accuracy (88.1%; 141/160) than PPMnn (80.0%; 128/160) in identifying model groups (p < 0.001). Conclusion: The remarkable and consistent performance of XCID suggests its applicability for detection, manufacturer and model identification, as well as MRI safety characterization of CIED on CRs. Further studies are warranted to guarantee the safe use of XCID in clinical practice.

A Study on Nocturnal Inversion at Urban Area in Winter (동절기 도심지 야간 기온역전에 관한 연구)

  • In urban area, inversion layer prevents air pollutants from being dispersed. To determine nocturnal inversion characteristics at SookMyung Girls' High School near high-rise residential commercial complex buildings in Seoul, air temperature and humidity data were observed at 10minute interval from December 1st, 2007 to May 2nd, 2008. Temperature-Humidity (T-H) sensors were installed at SMG (SookMyung_Ground) and SMR (SookMyung_Roof), and then these two data were compared, analyzed and examined to illustrate air temperature differences of SMG and SMR. After the analysis, the maximum nocturnal inversion was observed by $3.3^{\circ}C$ at 18:10 on December 26th 2007, at that time the weather condition was clear and weak wind. Nocturnal inversions began 2-3 hours before sunset and it reached maximum inversion just after sunset and continued through nighttime until sunrise or 1-2 hours after sunrise. In terms of seasonal variation nocturnal inversions occurred dominantly in March and April rather than winter season.

The Effect of Tactile Stimulation on Two Point Discrimination, Hand Function, and ADL in Impaired Characteristics of Stroke Patient (뇌졸중 환자의 장애 특성에 따른 접촉 감각 자극이 두점 구별, 손 기능 및 일상생활수행력에 미치는 효과)

  • PURPOSE: This study find out the effect of improved two point discrimination (TPD), hand function and activities of daily living (ADL) performance through tactile stimulus of upper limb (U/L) in impaired characteristics of stroke METHODS: We selected 26 stroke patients in BMH who has problems with neglect, sensory and motor deficits. Patients were divided into 3 group with neglect group (NG), sensorimotor deficits group (SMG) and motor deficit group (MG). To compare each group we used assessment tools such as two point discrimination on affected side (TPDas) and unaffected side (TPDus), Manual functional test on affected side (MFTas) and unaffected side (MFTus) and Korean version modified barthel index (K-MBI). RESULTS: 1) In the NG, tactile stimulus on U/L was statistically important for TPDas (forearm, index finger tip) also SMG and MDG was statistically important for TPDas. 2) In the NG, SMG, there was statistically important for MFTas, MFTus and in the MG. K-MBI also was statistically importance. Among three group, there was an statistically important difference for TPTus (forearm, thenar, hypothenar), MFTas and MFTus. We analyzed the relationship among TPD, MFT and K-MBI and There was negative relationship between TPD, MFT and There was positive relationship between TPD and K-MBI CONCLUSION: In impaired characteristics of stroke patients, tactile stimulus on U/L influenced on two point discrimination, hand function and ADL's. And we also found relationship among somatosensory, hand function, and ADL performance.

Influence of Land Use on the Pollution Load in the Saemangeum Basin (새만금 유역에서 토지 이용이 오염부하에 미치는 영향 평가)

  • The SMG project has been driven to secure food and water resources by closing of the SMG dyke for the preparation of the unification of Korean peninsular. It was investigated for pollution loads, land use distribution and water consumption for environmental assessments in two watersheds, the Mankyeong River (MK) and the Dongjin River (DJ) to assess the role of agricultural land on the alleviation of pollution loads to the SMG basin. It is needed to give the priority in managing pollution sources to conserve freshwater in the Saemangeum (SMG) basin after the completion of the SMG reclamation from tideland. The MK has $700million\;m^3$ water of which 14.1% were used for living, 73.6% for agriculture and 12.3% for industry. The DJ has $505million\;m^3$ water of which 3.0% for living, 94.5% for agriculture and 2.5% for industry. As compared to proportion of each land of total area, agricultural land was 1.4 times larger, livestock farming 7 times larger, forest 0.74 times smaller, and built-up area 0.67 times smaller in DJ watershed than in MK watershed. Pollution sources in MK and DJ watersheds were originated at a higher proportion from population including the sewage disposal and a livestock farming area rather than from the land. Water consumption and land use distribution influenced the water quality of the rivers; DJ watershed had far lower value of electric conductivity, $BOD_5$, TN and TP than MK watershed. A large proportion of paddy field also influenced to reduce pollute loadings after rainfall; DJ watershed, which has a relatively large area of paddy fields, had a far lower delivery load after rainfall than MK watershed even though DJ watershed had large livestock farming area. As paddy fields was irrigated by Iksancheon water, 37% of nitrogen, 50% of phosphates and 14.0% of $BOD_5$ was removed by the paddy field just after flowing 150 meter, and rice plants could remove TN 100.0 kg, $P_2O_5$ 24.0 kg, and $K_2O$ 119.2 kg per hectare at harvest by irrigation of Iksancheon water. Conclusively, rice paddy fields played a positive role to conserve the water quality in the Iksancheon watershed.
