대학생의 생활 스트레스가 수면장애에 미치는 영향 (Effect of Life Stress on the Sleeping Disorder of University Student)

  • 본 연구는 대학생의 스트레스가 수면장애에 미치는 영향을 구명하고자 광주 전남에 소재한 대학생 342명을 최종분석 대상으로 편의표본추출에 의해 시행되었다. 수면장애는 가족과 떨어져 자취하는 대학생이 많았고(p=0.042), 주거 환경에서 발생하는 소음 역시 수면장애와 연관성이 높았다(p=0.002). 또한, 대학생들이 학교생활에서 만족을 갖지 못하는 경우(p=0.007), 건강이 보통 이하 정도로 불량할 경우 수면장애가 많았다(p=0.010). 생활 스트레스 역시 수면장애와 관련이 있었고(p=0.004), 장래문제와 관련된 스트레스(p=0.002), 가치관과 관련된 스트레스가 유의한 관련이 있었다(p=0.001). 자취생활, 주거 소음, 학교생활의 불만족, 불량한 건강, 생활 스트레스, 특히 장래 문제 및 가치관과 관련된 스트레스가 수면장애와 관련이 있었다. 따라서, 대학생의 수면장애를 유발하는 스트레스에 대해 다양한 요인과의 분석을 통한 연구가 추가로 필요할 것으로 생각되었다.

불면장애 환자와 정상대조군간의 공간인지기능 비교 : 예비연구 (Comparison of Spatial Neurocognitive Function between Insomnia Disorder Patients and Normal Sleeping Control : Pilot Study)

  • 연구목적 : 만성불면증은 흔하고 가장 환자들을 괴롭히는 수면장애들 중 하나이다. 본 예비연구는 불면장애 환자들과 수면을 잘 취하는 건강인 간에 공간인지기능을 비교하기 위해 수행되었다. 연구대상 및 방법 : DSM-5상 불면장애의 진단기준을 만족하는 22명의 만성 불면장애 환자와 27명의 수면을 잘 취하는 정상대조군이 연구에 참여하였다. 전산화신경인지기능검사인 Cambridge Neuropsychological Test Automated Battery (CANTAB)를 시행하여 불면장애 환자와 정상대조군 간에 공간인지기능(spatial planning function)을 비교하였다. 결 과 : CANTAB 결과 상 Stockings of Cambridge test에서 더 저조한 problems solved in minimum moves 결과를 보였다(t = -2.499, p = 0.017) 이 항목은 나이, 성별, beck depression index 비수면점수를 통제한 ANCOVA 분석에서도 유의한 결과를 보였다(F = 5.631, p = 0.017)의 유의한 차이가 관찰되었다. 결 론 : 본 연구의 결과는 불면장애 환자들의 저조한 공간계획기능을 시사한다.

비만 초등학생들의 자아존중감 및 불안에 대한 고찰 (Self-esteem and anxiety of obese children)

  • Objectives This study was performed to figure out relations of self-esteem, anxiety and the other factors related with obesity in obese children. Methods Forty-two children(21 boys and 21 girls) those who were diagnosed obesity have been recruited. To assess psychological factors, Self-esteem inventory(SEI), Revised Children's Manifest Anxiety Scale(RCMAS), International Physical Activity Questionnaires(IPAQ), Korean Eating Attitudes Test(KEAT-26) and Children's Depression Inventory(CDI) were administered. Behavioral characteristics were also analyzed. Results Self-esteem(SEI) had significant positive relationship with physical activity(IPAQ) and sleeping time. Self-esteem had significant negative relationship with anxiety(RCMAS), eating disorder(KEAT-26) and watching TV time. Anxiety had significant positive relationship with eating disorder(KEAT-26) and watching TV time, and negative relationship with physical activity(IPAQ) and sleeping time. All of participants seemed to be having depressive tendency. Conclusion Self-esteem and anxiety were significantly related. And eating disorder, physical activity, sleeping time and watching tv also affected to the self-esteem and anxiety of obese children.

지속적 양압술과 수면중 주기적 사지운동 장애의 관계에 대한 예비적 연구 : 앙와위가 주기적 사지운동 장애와 관련되는가? (Preliminary Study of The Periodic Limb Movement Disorder Following Nasal CPAP : Is It Associated With Supine-Sleeping Position?)

  • Introduction : Periodic limb movement disorder (PLMD) is shown to common in patients with OSA and may become evident or worsened when treated with nasal continuous positive airway pressure (CPAP). Whether this is due to im proved sleep continuity. adverse nocturnal body positioning, uncovered by CPAP, or due to the CPAP stimulus is still debat-ed. We hypothesized that the increase in PLM activity following CPAP is associated with more supine-sleeping tendencies when being treated with CPAP. In the present work, we compared differences in the PLMD index (PLMI) and sleeping position of patients with sleep disordered breathing before and after CPAP treatment. Method : We studied 16 patients (mean age 46 yr, 9M, 7F) with OSA (11 patients) or UARS (5 patients) who either had PLMD on initial polysomnogram (baseline PSG) or on nasal CPAP trial (CPAP PSG). All periodic leg movements were scored on anterior tibialis EMG during sleep according to standard criteria (net duration; 0.5-5.0 seconds, intervals; 4-90 seconds. 4 consecutive movements). Paired t-tests compared PLMD index (PLMI), PLMD-related arousal index (PLMD-ArI), respiratory disturbance index (RDI), and supine sleeping position spent with baseline PSG and CPAP PSG. Results : Ten patients (63%) on baseline PSG and fifteen patients (94%) on CPAP PSG had documented PLMD ($PLMI{\ge}5$) respectively with significant increase on CPAP PSG(p<0.05). Ten patients showed the emergence (6/10 patients) or substantial worsening (4/10 patients) of PLMD during CPAP trial. Mean CPAP pressure was $7.6{\pm}1.8\;cmH_2O$. PLMI tended to increase from baseline PSG to CPAP PSG, and significantly increase when excluding 2 outlier (baseline PSG, $19.0{\pm}25.8/hr$ vs CPAP PSG, $29.9{\pm}12.5/hr$, p<0.1). PLMD-ArI showed no significant change, but a significant decrease was detected when excluding 2 outlier (p<0.1). There was no significant sleeping positional difference (supine vs non-supine) on baseline PSG, but significantly more supine position (supine vs non-supine, p<0.05) on CPAP PSG. There was no significant difference in PLMI during supine-sleeping and nonsupine-sleeping position on both of baseline PSG and CPAP PSG. There was also no significant difference in PLMI during supine-sleeping position between baseline PSG and CPAP PSG. With nasal CPAP, there was a highly significant reduction in the RDI (baseline PSG, $14.1{\pm}21.3/hr$ vs CPAP PSG, $2.7{\pm}3.9/hr$, p<0.05). Conclusion : This preliminary data confirms previous findings that CPAP is a very effective treatment for OSA, and that PLMD is developed or worsened with treatment by CPAP. This data also indicates that supine-sleeping position is more common when being treated with CPAP. However, there was no clear evidence that supine position is the causal factor of increased PLMD with CPAP. It is, however, suggested that the relative movement limitation induced by CPAP treatment could be a contributory factor of PLMD.

  • Objects : The purpose of this study is find out significant (clinical) effect of applying acupuncture on $zh{\grave{a}}h{\grave{a}}i$(K6, 照海) of ${\bar{U}m-kyo-maek$(陰?脈) and $sh{\bar{e}}nm{\grave{a}}i$(B62, 申?) of Yang-kyo-maek(陽?脈) to a patient of circadian sleep disorder. Methods : We apply acupuncture on $zh{\grave{a}}h{\grave{a}}i$(K6, 照海) and $sh{\bar{e}}nm{\grave{a}}i$(B62, 申?) to a patient of circadian sleep disorder during 2 weeks. We check sleeping time day and night during 14 days. Acupuncture on $zh{\grave{a}}h{\grave{a}}i$(K6, 照海) and $sh{\bar{e}}nm{\grave{a}}i$(B62, 申?) has significant effect to circadian sleep disorder patient. Results : Acupuncture on $zh{\grave{a}}h{\grave{a}}i$(K6, 照海) and $sh{\bar{e}}nm{\grave{a}}i$(B62, 申?) has significant effect to circadian sleep disorder patient. Conclusions : 1. The physio-pathologic phenomenon of sleeping is relative to ${\bar{U}m-kyo-maek$(陰?脈) and Yang-kyo-maek(陽?脈) 2. Acupuncture therapy on $zh{\grave{a}}h{\grave{a}}i$(K6, 照海) and $sh{\bar{e}}nm{\grave{a}}i$(B62, 申?) has significant effect to circadian sleep disorder.

불면 장애에 대한 족욕 치료법의 체계적 문헌고찰과 메타분석 연구 (A Systematic Review and Meta-Analysis of Foot Bath Therapy for Insomnia Disorder)

  • Objectives: The purpose of this study was to provide evidence for the clinical effects of foot bath therapy for insomnia disorder through a systematic review and meta-analysis. Methods: Studies were selected from seven domestic and international literature databases. Data on diagnostic tools, pattern identification, sample size, intervention methods, outcome measurements, results, and adverse events were extracted. The quality of the selected literature was assessed using Cochrane's risk of bias (RoB) assessment tool. Results: A total of 11 studies were included in this study. The primary diagnostic tool for insomnia dis- order was the criteria for the diagnosis and therapeutic effect of diseases and syndromes in traditional Chinese medicine (CDTDSTCM), along with the Chinese classification of mental disorders-3 (CCMD-3). Treatment effects were mainly evaluated by the effective rate and the Pittsburgh Sleep Quality Index (PSQI). The meta-analysis results showed statistically higher effectiveness rates for treating insomnia disorder in the experimental groups receiving herbal medicine foot bath therapy with sleeping or herbal medication compared to the control group. The PSQI was lower in the treatment group receiving herbal medicine foot bath therapy with sleeping medication. The herbal materials used in herbal medicine foot bath therapy mainly included Radix Angelicae Sinensis, Polygonum Multiflorum, Ziziphi Spinosae Semen, Rhizoma Coptidis, Radix Salviae Miltiorrhizae, and Cortex Albizziae. The quality of the included studies was generally poor. Conclusions: This study found that foot bath therapy had clinical efficacy for treating insomnia disorder. The research findings could provide foundational evidence for establishing the clinical basis of foot bath therapy in the treatment of insomnia.

동물모델에서 시호가용골모려탕(柴胡加龍骨牡蠣湯)의 수면유도 및 항불안 효과 (Sleep-promoting and Anti-anxiety Effects of Shihogayonggolmoryo-tang in Mice)

  • Shihogayonggolmoryo-tang (ST) is a Korean medical herb cocktail which has been used to treat anxiety induced insomnia. In this study, we will examine sleep-promoting and anti-anxiety effects of ST, and investigate its mechanism. ICR mice were divided into three groups for the first examination : control group (n=11), ST50 group (50 mg/kg, po, n=11), ST200 group (200 mg/kg, po, n=11). Sleep-promoting effect was confirmed by measuring the sleeping duration time and sleeping onset time after thiopental sodium treatment (50 mg/kg, ip). ICR mice were divided into five groups for the second examination : control group (n=11), ST200 group (200 mg/kg, po, n=11), ST200+Flumazenil group (ST 200 mg/kg, po, flumazenil 0.3 mg/kg, ip, n=11), diazepam group (1 mg/kg, ip, n=11), diazepam+flumazenil group (diazepam 1 mg/kg, ip, Flumazenil 0.3 mg/kg, ip, n=11). Anxiety behavior and sleep-promoting effect was confirmed by open field test and measuring the sleeping duration time and sleeping onset time. Expression levels of c-fos in tuberomammillary nucleus (TMN) and ventrolateral preoptic nucleus (VLPO) were analyzed by immunohistochemistry. ST treated group showed significantly decreased anxiety behavior and enhanced sleeping duration time and sleeping onset time concentration dependently. The expression of c-fos was significantly upregulated in VLPO as sleep-inducing center and TMN as downregulated in arousal center by ST treatment. In addition, all effects of ST were reversed by flumazenil. Our results suggest that ST has sleep-promoting and anti-anxiety effects through regulating arousal center (TMN) and sleep-inducing center (VLPO).

황연해독탕가감방 전탕액 도포로 치료한 하복부피진 1례 (External Use of Hwangyeonhaedog-tang Gagambang in the Treatment of Lower Abdominal Dermatitis - A Case Report)

  • A 69-year old female stroke patient was admitted on August 7, 1998. She had seborrheic dermatitis on her lower abdomen for 25 years. The patient received many treatments but was unable to find a cure. During her hospitalization for stroke, the patient had trouble sleeping. The doctors hypothesized the dermatitis to be the cause of the sleeping disorder and attempted to treat it. The doctors chose Hwangyeonhaedog-tang Gagambang for treatment and smeared it on the patient's abdomen. After ten days of treatment, the dermatitis disappeared and did not reoccur. The patient's abdomen continued to show no symptoms of dermatitis after being released for 2 years.

A Study on LED Lighting Control according to Sleep Stage using PPG Sensor of Wearable Device

  • Recently, as the sleep disorder problem of modern people deepens, the interest towards quality of sleep is increasing. To increase the quality of modern people's sleep. This paper has suggested an LED lighting control system according to the sleep stage using PPG sensors of wearable devices. The pulse of the wrist radial artery was measured using a wearable device mounted with PPG sensor, which enables heart rate-measuring, and by using the point that heart rate lowers during stable sleep than non-sleeping, the LED lighting of indoors was controlled, which is the disturbing element when sleeping. For the performance evaluation, a 10-Fold cross analysis was conducted for performance evaluation, and a result of an average accuracy 87.02% was obtained as a result. Therefore, the LED lighting control system according to the sleep stage using a wearable device of this paper is expected to contribute to raise the quality of the user's life.

수면과다증을 동반한 심인성 요실금 환자 호전 1례 (Case Report of a Psychogenic Urinary Incontinence Patient with Hypersomnia Disorder)

  • Objective: This study evaluates the effects of traditional Korean medicine (including Guibi-tang) on a psychogenic urinary incontinence patient with hypersomnia disorder.Method: Herbal medicine, acupuncture, and moxibustion were carried out. The effect of the treatments was then evaluated via the Beck depression inventory (BDI), the Beck anxiety inventory (BAI), the symptom checklist-90-revised (SCL 90-R), the Epworth sleepiness scale (ESS), and the revised urinary incontinence scale (RUIS), as well as the number of urinations, incontinence levels, and sleeping times during the day.Results: After the treatments, both the urinary incontinence and hypersomnia symptoms showed improvement.Conclusion: This study suggests that traditional Korean medicine treatments are effective in caring for patients who suffer from psychogenic urinary incontinence with hypersomnia disorder.
