Sino-Bronchiectasis 에 대한 임상적 고찰 (Clinical and Pathological Study of Sino-bronchiectasis)

  • Author studied the pathogenesis of Sino-bronchiectasis with review of clincal and pathological findings of resected lungs in 24 eases of bronchiectasis,and the results were as follows: 1. Of the 24 eases of bronchiectasis, Sino-bronchiectasis were found in 11 cases and these cases most commonly, developed in first and second decade of life. In a majority of these cases,the main symptoms were the signs of wet bronchiectasis. 2. The typical roentgenograrn changes of plain chest x rays were increase in size and loss of definition of the markings and bronchographically revealed the cylindrical or tubular form in majority of these cases. 3. Pathologically Sino-bronchiectasis involved specific segmental areas of the lungs such as left basal and S4+5 segments and revealed commonly tubular dilatation. The most common findings of bronchial wall of dilated bronchus were the inflammatory cellular infiltration and cylindrical hyperplasia of mucus epithelial layers. 4. It was suspected that there were some causal relations in pathogenesis between chronic sinusitis and bronchiectasis.

원발성 뇌하수체 농양 - 2례 보고 - (Primary Pituitary Abscess - Two Cases Report -)

  • Pituitary abscess is a very rare disorder, since antibiotics therapy has become widely available. Clinically and radiologically, the preoperative diagnosis of pituitary abscess is difficult because of its features which may be identical to those of any space-occupying lesions of the sella turcica. A combination of clinical features, such as meningitis, paranasal sinusitis and panhypopituitarism with intrasellar cystic lesion with homogeneous ring enhancement on computed tomography or magnetic resonance image should raise the suspicion of a pituitary abscess. Drainage of the abscess through trans-sphenoidal approach, appropriate antibiotics therapy, endocrine assessment and hormonal replacement should bring complete recovery to the patient. The authors review the literature and report two cases of primary pituitary abscess.

왕초피나무 Zanthoxylum coreanum Nakai의 성분연구(成分硏究)(I) (Studies on the Constituents of Zanthoxylum coreanum Nakai)

  • Pericarp of Zanthoxylum coreanum Nakai have been used as a crude medicine for the treatment of ozena, rheumatoid, nasal sinusitis, meno-xenia, dyspepsia, toothache, sore throat, pains in the limbs, etc. in Korea. A sterol fraction and two compounds, compound I and II were isolated from the roots, stems, leaves of Zanthoxylum coreanum Nakai. The sterol fraction was identified as a mixture of ${\beta}-sitosterol$ and campesterol. Compound I, colorless prismatic crystals, $C_{13]H_{13}NO_4$, mp. $175{\sim}177^{\circ}$, was proved to be skimmianine which is one of the alkaloids and compound II, $C_{14}H_{28}O_2$, mp. $53{\sim}55^{\circ}$, white powdered crystals was identified as myristic acid.

  • Realizing the scope of utilizing by-products of silk cocoons by applying appropriate methods is the immediate crave to optimize returns. The nutritive value of pupae suits for human diet, feed for poultry, carps, fish, rabbits, piggery and dogs. The pupal skin, fat, oil, cocoon palade have applications in oleo chemical, soap, glycerin, cosmetic, artificial fibres, membranes and n-triacontanol isolation. The pupal proteins Chitin, Shinki fibroin, Serrapeptidase, glucosamine are latent precursors of post surgical, anti-carcinogenic, anti-inflammative, anti-bacterial, anti-histaminic, gastric, hepatitis, pancreatitis, leukocytopenia, neurological, ophthalmic, blood pressure, cardiac and diabetic medicines and for preparation of vitamins A, E and K. The silk and its proteins sericin and fibroin are potentially used for wound healing, diabetes, impotence, sinusitis, arthritis, edema, cystitis, epididymitis, tissue regeneration, cancer, post-surgical trauma and used as anti-oxidatives, bio-adhesives, ultra violet screens and bio-active textiles. The waste cocoons can be used in making art crafts like garlands, carpets, overcoats, decoratives and greeting cards. The in-depth research towards utility optimization and make aware this reality to sericulturists, reelers, weavers, traders, entrepreneurs, policy makers etc., is the upright want of the today's Sericulture industry.

남매에서 발생한 Kartagener씨 증후군 - 2례 보고 - (Kartagener's Syndrome: A Report of 2 Cases (Review of the literature and report of two cases in same family))

  • Kartagener`s syndrome is a clinical entity composed of situs inversus, bronchiectasis and chronic sinusitis. This syndrome is rare and is usually detected in childhood. Whether the bronchiectasis in Kartagener`s syndrome is congenital or acquired is still controversial. Some familial cases of this syndrome have been reported, and in these cases the genetic pattern is compatible with an autosomal recessive inheritance with some degree of pleotropism. The authors experienced having 2 cases of Kartagener`s syndrome among 6 siblings in a family. The case No. 1 have had bilateral Caldwell Luc operation, posterior ethmoidectomy, left upper Iobec-tomy with excellent result. The case No. 2 denied to have surgical management.

Osteoma of the Frontal Sinus with Secondary Subdural Empyema Formation

  • Osteomas of the paranasal sinuses rarely lead to intracranial manifestations. The authors report an unusual case of a frontal sinus osteoma leading to subdural emyema formation. A 19-year-old man presented with headache and fever one month after minor facial trauma. Neuroradiological studies revealed subdural empyema in left frontal lobe with moderate cerebral edema and a osteoma in the left frontal sinus with sinusitis of maxillary sinus. The patient was surgically treated in one stage operation of decompressive craniectomy, removal of subdural empyema with frontal sinus osteoma, and endoscopic sinus surgery via cranial and nasal route. The patient recovered very well after surgery and postoperative antibiotic therapy. The etiology of intracranial infection and the treatment strategy are to be discussed.

  • We wish to show our experiance with threating a rare congenital brain malformation-encephalocele. It is a protusion of brain matter with greater incidence in the Far East. Our case is even more curious because of the site of occurrence-frontobasal. Most of encephalocele occur in the occipital region. In this article we report a case of a 57-year-old woman, without deformations on the face, which had epileptic seizures and in spite of receiving antiepileptic drug. She was also frequently treated for sinusitis. She never had rhinoliquorrhea, nor was she diagnosed to have meningitis. In the last few years she had difficulty breathing on her right nostril. After she was diagnosed with encephalocele and treated surgically her recovery was complete and she is without the seizures.

  • For the earlier diagnosis and treatment of delayed wound healing in extraction socket, we investigated the clinical, radiographic and histopathologic features of 106 patients with delayed wound healing diagnosed by biopsy. The patients were enrolled at the Department of Oral Pathology, Chosun University Dental Hospital. Among 106 delayed wound healing diagnosed by biopsy, cysts showed most significant 25 cases (23%), osteomyelitis showed 13 cases (12%), chronic maxillary sinusitis and squamous cell carcinoma showed 12 (11%) and 11 (10%) cases, respectively. We have analyzed these lesions through relevant literatures.

Approach to Frontal Sinus Outflow Tract Injury

  • Frontal sinus outflow tract (FSOT) injury may occur in cases of frontal sinus fractures and nasoethmoid orbital fractures. Since the FSOT is lined with mucosa that is responsible for the path from the frontal sinus to the nasal cavity, an untreated injury may lead to complications such as mucocele formation or chronic frontal sinusitis. Therefore, evaluation of FSOT is of clinical significance, with FSOT being diagnosed mostly by computed tomography or intraoperative dye. Several options are available to surgeons when treating FSOT injury, and they need to be familiar with these options to take the proper treatment measures in order to follow the treatment principle for FSOT, which is a safe sinus, and to reduce complications. This paper aimed to examine the surrounding anatomy, diagnosis, and treatment of FSOT.

  • Gastroesophageal reflux (GER) defined as passage of gastric contents into the esophagus without symptoms is a common physiologic gastrointestinal problem in infants, children and even in adults. But gastroesophageal reflux disease (GERD) defined as symptoms or complications of GER is a disease entity to find out the reason and treat. After the era of 1970 we have been studying about GERD with the development of diagnostic and therapeutic methods. I already introduced the nature and the fundamental knowledge of GERD in the opening symposium of KSPGHAN in 1998. Now, I will introduce the guidelines for evaluation and treatment of GERD which was recommended by the North American Society for Pediatric Gastroenterology and Nutrition and American Pediatric Association which was published in 2001. And I will introduce progressing subjects and the forecoming issues to be solved in near future. Those are as followings. Does GERD cause otolaryngologic symptoms such as chronic sinusitis and chronic otitis? Is GERD inherited? Can we find out the child who will become GERD in adult life and the way to reduce the incidence of Barrett's esophagus and esophageal cancer? Is long term PPI therapy safe in children?
