측방보행 훈련이 뇌졸중 환자의 균형 및 보행에 미치는 영향 (Effects of Side Walking Training on Balance and Gait in Stroke Patients)

  • 본 연구의 목적은 뇌졸중 환자를 대상으로 측방보행 훈련이 균형(기능적 보행 검사, 일어나 걸어가기 검사) 및 보행(10 미터 보행검사)에 미치는 영향을 알아보고자 하였다. 28명의 뇌졸중 환자를 대상으로 실험군(측방 보행군)과 대조군(전방 보행군)에 각각 14명씩 무작위로 배정하였다. 두 그룹 모두 재활 물리치료를 4주 동안 실시하였다. 실험군에는 4주 동안 1회당 20분씩 주 3회 측방보행 훈련을 실시하였고, 대조군에도 4주 동안 1회당 20분씩 주 3회 전방보행 훈련을 실시하였다. 측방보행 후 기능적 보행평가를 측정한 결과 실험군에서 16.86점에서 18.64점으로 유의하게 향상되었고(p<.05). 일어나 걸어가기 검사에서도 26.03초에서 22.43초로 감소하여 유의하게 향상되었다(p<.05). 10미터 보행검사에 있어서도 실험군에서 21.90초에서 19.10초 감소하여 유의하게 향상되었다(p<.05). 따라서 측방보행 훈련이 뇌졸중 환자의 균형 및 보행을 증진시키는데 유용한 훈련으로 제안할 수 있을 것이다.

성인 여성의 맨발 보행과 운동화 착용 보행 시 주기 비교 (Comparison of Barefoot and Shod Gait Cycle for Adult Women)

  • 본 연구는 맨발 보행과 운동화 착용 보행에서 보행주기의 차이를 비교하여 보행역학에 따른 신발 개발에 기초자료를 제공하고자 한다. 발의 변형과 이상이 없는 정상 성인 여성 30명을 대상으로 보행 주기를 측정하였다. 먼저 운동화를 착용하고 보행하여 주기를 측정한 후, 맨발로 보행하여 주기를 측정하여 데이터를 얻었다. 이후 두 데이터를 대응표본 T-test를 이용하여 비교하였다. 실험 결과 맨발 보행에서 입각기 좌측(p<.001), 우측(p<.005), 체중부하기 좌측(p<.009), 우측(p<.002), 전유각기 좌측(p<.002), 우측(p<.011), 양하지 지지기(p<.004)가 증가하였고, 중간 입각기 좌측(p<.016), 우측(p<.001), 유각기 좌측(p<.001)이 감소하였다. 이는 맨발 보행이 다양한 발의 감각의 입력을 증가시켜 보행안정성이 높은 보행이 가능해 졌다고 보여 지며, 향후 보행 주기에 의거하여 맨발보행과 가까운 신발 개량이 필요하다고 사료된다. 향후 신발의 개량을 위해 신발 종류에 따른 보행주기 연구가 필요할 것이다.

  • Purpose: This study investigated the effects of side walking training combined with squats on the balance and gait ability of stroke patients. The purpose of this study was to provide fundamental data regarding the use of side walking training combined with squats among stroke patients. Methods: Thirty patients with stroke were randomly divided into an experimental group (n=15) that underwent side walking training combined with squats and a control group (n=15) that performed general rehabilitation exercises. Both groups performed their respective exercises for 30 minutes, five times a week for six weeks. Balance was assessed using the functional reach test and timed up and go test, while gait ability was evaluated using the 10-meter walk test. A paired t-test was performed to compare within-group changes before and after the intervention. Differences between the experimental and control groups were analyzed using an independent t-test. For all tests, the level of statistical significance was set at α=0.05. Results: After the exercise, significant within-group improvements in balance and gait ability were observed in both the experimental and control groups (p<0.05). There was also a significant between-group difference in balance and gait ability following the intervention (p<0.05). Conclusion: While general rehabilitation is commonly employed in treating stroke patients and is relatively effective, the application of side walking training combined with squats may offer additional benefits in terms of improving balance and gait ability in these patients.

Effects of Walking Training according to Rhythmic Auditory Stimulation Speed Control Balance of Stroke Patients

  • Purpose: In this study, based on the error augmentation, we performed walking training with increased rhythmic auditory stimulation speed on the affected side (IRAS) and walking training with decreased rhythmic auditory stimulation speed on the unaffected side (DRAS). The purpose of this study was to verify whether motor learning was effective in improving balance ability. Methods: Twenty-eight subjects with chronic stroke were recruited from a rehabilitation center. The subjects were divided into three groups: an IRAS group (10 subjects), a DRAS group (9 subjects), and control group (9 subjects). They received 30minutes of neuro-developmental therapy and walking training for 30minutes, five times a week for three weeks. Static and functional balance ability were measured before and after the training period. Static balance was measured by balancia software. Functional balance was measured by the timed up and go test (TUG) and the berg balance scale (BBS). Results: After the training periods, the IRAS group showed a significant improvement in TUG, BBS, area 95% COP, and weight distribution on the affected side when compared to both the DRAS group and control group (p<0.05). Conclusion: Based on the results of this study, it is possible to consider error augmentation methods of motor learning if rhythmic auditory stimulation is applied to stroke patients in clinical practice. If the affected side is shorter than the unaffected side, the affected side should be adjusted to the increased rhythmic auditory stimulation speed, which is considered to be an effective intervention to improve balance ability.

균형추를 장착한 4족 로봇의 보행 안정성에 관한 연구 (A Study on the Walking Stability of the Quadruped Robot with WBO)

  • There are many types of walking robots in the world. For dynamic walking of the robots it is necessary to keep its dynamic stability. The dynamic stability is influenced by the position of ZMP (zero moment point). In this paper we study the control of the ZMP position of walking robot. For experiment we developed a quadruped robot and analyzed the dynamic stability of the robot. Developed robot has 2 joints at each leg and WBO (weight balancing oscillator) on the body of the robot. The WBO is designed to move linearly from side to side when the robot walks dynamically. Walking test was performed to verify the validity of the proposed methods. Especially we showed that the dynamic stability of the robot can be improved without sacrifice of the walking speed by control the WBO.

마비측 시야 유도 보행 훈련과 리듬 청각 자극 훈련이 만성 뇌졸중 환자들의 보행에 미치는 효과 (Effect of Gait Training in Chronic Hemiplegia Patients with Inducement to Vision and Rhythmic Auditory Stimulation)

  • Purpose: The purpose of the study is to see the effect of walking when training for chronic stroke patients, Affected side vision impairment induced gait training with rhythmic auditory stimulation in parallel. Methods: This study was the general walking training group, the training group was divided damage induced affected side view, affected side view damage induced by rhythm auditory stimulation training three groups. Intervention period was 4 weeks. 5 times per week, per 30 min entered arbitration. Affected Side vision impairment induced induction training was conducted by installing a red cones, rhythmic auditory stimulation was applied to a regular rhythm training using a smartphone application (Metronome). Paired t-test was performed to see the difference between each group and before the intervention. One-way ANOVA was used to find out the significance probability between each group. Statistical significance was set at 0.05 probability. Results: Regular walking group, rather than affected side vision guided walking group could see increases in the FGA and DGI (P<0.05). And vision impairment induced affected side view and rhythmic auditory stimulation induction training this group than in the combination group walking group obtained higher scores on the FGA, DGI score(P<0.05). Conclusion: If you want to enforce certain rhythmic auditory stimulation of the visual field and side damage induced during gait training for stroke patients will be able to expect a better effect.

뇌졸중 환자의 물리치료 결과에 관한 연구 (A Study on Results of Physical Therapy of Storke Patients)

  • The authors studied 74 patients with cal therapy Kosin University Medical March 1994 through February 1995. The results were, as follows: 1. Average age of the subject patients was 59.3 years old, their age ranged from 20 to 84 years old, and the incidence between male and female was 1:1.24 2. In respect to the economy level the middle class revealed the highest incidence, 40(54%) in 74cases. 3. The cause of injury revealed the highest incidence in hypertension 52 patients(72 %), more than all revealed the highest incidence 23 patients(44 %) in 60th years old. 4. The form of injury revealed the highest incidence in infarction 36 patients(49 %), more then all revealed the highest incidence 13 patients (36 %) in 60th years old. 5. As for the region cerebral artery of infarction patients, middle cerebral artery was the highest region of injury 72 %. 6. As for the physical paralysis side, the left side showed higher incidence 40 patients(54 %) compared to the right side 31 patients (42 %). 7. The period of therapy was 56 days, in average extending from 7 days to II months, nore then all there were the most numerous patients within 3 weeks 28 patients(38 %). 8. The upper side of the upper lims had improved 67 patients(91 %), the lower side 48(65 %). In the cases of the lower limb, the upper side had improved 70(95 %), the lower side 58(78 %). In short, patients had improved more the lower limb than the upper limb, the upper side(elbow and shoulder part) then the lower side(wrist and finger) of the upper limb. In the cases of the lower limb, patients had improved more the upper side (knee and hip part) then the lower side(foot and ankle) 9. In the cases of patients can be walking among subject patients 55(74 %) and patients cannot be walking 19 (26 %), In the cases of patients utilized assistant apparatus 34 (62 %) among self-walking, completely patients can be self-walking 21(28 %).

아킬레스건 파열 수술 후 걷기 및 달리기 운동역학적 분석 (Biomechanical Analysis of Walking and Running after a Surgically Repaired Achilles Tendon Rupture)

  • Objective: The purpose of this study was to investigate the difference in muscle strength, kinematics, and kinetics between injured and non-injured sides of the leg after Achilles Tendon Rupture surgery during walking and running. Method: The subjects (n=11; age = 30.63 ± 5.69 yrs; height = 172.00 ± 4.47 cm; mass = 77.00 ± 11.34 kg; time lapse from surgery = 29.81 ± 10.27 months) who experienced Achilles Tendon Rupture (ATR) surgery participated in this study. The walking and running trials were collected using infrared cameras (Oqus 300, Qualisys, Sweden, 100 Hz) on instrumented treadmill (Bertec, U.S.A., 1,000 Hz) and analyzed by using QTM (Qualisys Track Manager Ver. 2.15; Qualisys, U.S.A). The measured data were processed using Visual 3D (C-motion Inc., U.S.A.). The cutoff frequencies were set as 6 Hz and 12 Hz for walking and running kinematics respectively, while 100 Hz was used for force plate data. Results: In ATR group, muscle strength there were no difference between affected and unaffected sides (p> .05). In kinematic analysis, subjects showed greater ROM of knee joint flexion-extension in affected side compared to that of unaffected side during walking while smaller ROM of ankle dorsi-plantar and peak knee flexion were observed during running (p< .05). In kinetic analysis, subjects showed lower knee extension moment (running at 2.2 m/s) and positive ankle plantar-flexion power (running at 2.2 m/s, 3.3 m/s) in affected side compared to that of unaffected side (p< .05). This lower positive ankle joint power during a propulsive phase of running is related to slower ankle joint velocity in affected side of the subjects (p< .05). Conclusion: This study aimed to investigate the functional evaluation of the individuals after Achilles tendon rupture surgery through biomechanical analysis during walking and running trials. Based on the findings, greater reduction in dynamic joint function (i.e. lower positive ankle joint power) was found in the affected side of the leg compared to the unaffected side during running while there were no meaningful differences in ankle muscle strength and walking biomechanics. Therefore, before returning to daily life and sports activities, biomechanical analysis using more dynamic movements such as running and jumping trials followed by current clinical evaluations would be helpful in preventing Achilles tendon re-rupture or secondary injury.

PNF의 방산을 이용한 간접치료가 뇌졸중환자의 정적 자세에서의 체중지지 및 보행 속도에 미치는 영향 : 증례보고 (Influence of Body Weight Support and Walking Speed in the Static Posture of Stroke Patients using Indirect PNF Treatment: A Case Report)

  • Purpose : The purpose of this study was to examine the effects of PNF of irradiation using an indirect treatment of the stroke patient's static weight support and walking speed. Methods : Was carried out a total of nine different the PNF method of treatment for patients with left hemiplegia stroke. PNF of treatment is not in the non-paralyzed side of the trunk and upper and lower extremities by applying resistance and increased strength of the affected side. Assessment of treatment weight support both feet measured and modified using the timed up and go test were compared before and after treatment. Results : Treatment of the paralyzed side of the quadriceps and hamstring, gluteus medius muscle strength increased, and both sides of the foot body weight support improved walking speed was increased. Conclusion : Irradiation using PNF indirect treatment improves the function of the gait of stroke patients.

보행자의 선호 보행방향에 관한 조사 및 분석 (Survey and Analysis of Pedestrians' Preferences on Walking Directions)

  • Why do some countries walk on the right and others on the left? People have a dominant hand which leads to a natural tendency to favor one side of the road or another depending on the means of transportation being used. The primary objective of this study was to investigate the stereotype of Korean regarding preferred walking direction in encountering various facilities and provide the appropriate information to traffic policy makers. Six hundred Korean male and female subjects aging from 12 to 83 were selected to investigate the various statistics about their preferred walking direction and their employment characteristics on walking diverse facilities. The walking directions of eleven different facilities were asked along with other relative subjects' characteristics(e.g., age, gender, hand and foot dominance) to determine the relationship among these obtained data. The descriptive statistics showed that 73.7% and 26.3% were preferred walking right and left direction respectively. Moreover, various statistical analysis revealed that general tendency of walking direction was varied by hand and foot dominances. There were strong tendency that right-handed people prefer walking right side of the road and vise versa, hence this should be considered in setting up traffic policies. As a concluding remark, it is better to design traffic policies and regulations in the way that peoples' preference and expectation.
