The congenital cystic adenomatoid malformation of the lung consists of an enlarged, meaty, multicystic lobe with smooth-walled cysts of varying sizes, which can communicate with major bronchi through malformed air passages that usually lacks cartilage. This abnormality is usually symptomatic in infancy with signs of respiratory distress such as tachypnea, substernal retraction and cyanosis. Prompt surgical resection is choice of treatment in life-threatening respiratory distress patients. We recently experienced a case of congenital cystic adenomatoid malformation of the lung in a patient with situs inversus totalis. The patient was 40 days old female who showed severe respiratory difficulty. Emergency left middle lobectomy was undergone successfully. Her postoperative course was uneventful. She was discharged from hospital on the postoperative eighth day in good condition.
Congenital diaphragmatic hernia is an important cause of respiratory distress in the newborn. Eventration, with which these conditions are easily confused, may also Produce severe respiratory distress in infancy or be an asymptomatic radiographic finding. Harrington`s classification of diaphragmatic hernias into two categories, traumatic and nontraumatic, is most widely accepted. Nontraumatic hernias are [1] the congenital types, composed of the posterolateral [Bochdalek], those through the esophageal hiatus, the parasternal [/Morgagni], and those through a defect left by partial absense posteriorly, and [2] the acquired types, composed of those through the esophagea/hiatus [sliding and paraesophageal] and those the sites mentioned above under the congenital hernias. During the period from 1970 up to October 1978, 21 cases of diaphragmatic hernia were treated in department of cardiovascular and thoracic surgery. 11 cases of Bochdalek hernias, 1 case of Morgagni hernia, 5 cases of diaphragmatic eventration and 3 cases of hiatal hernia [2 cases of paraesophageal and 1 cases of sliding type], were experienced. 3 cases of 20 died of respiratory insufficiency, 2 cases of mortality were combined with left lung hypoplasia with Bochdalek hernia.
Paradoxical vocal fold movement (PVFM) is characterized by aberrant vocal fold adduction. Although the exact pathogenesis is unknown, botulinum toxin injection, behavioral techniques, including speech therapy, bio-feedback, and cognitive-behavioral psychotherapy are considered for treatment of PVFM. The effectiveness of these treatments is not fully evaluated because of the rarity of disease. We present a case of 16-year-old female with sudden onset of respiratory distress associated with PVFM refractory to several treatments and spontaneously resolved later.
Coronavirus disease 2019 (COVID-19) is a viral infection produced by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus epidemic, which was declared a global pandemic in March 2020. The World Health Organization has recorded around 43.3 billion cases and 59.4 million casualties to date, posing a severe threat to global health. Severe COVID-19 indicates viral pneumonia caused by the SARS-CoV-2 infections, which can induce fatal consequences, including acute respiratory distress syndrome (ARDS). The purpose of this research is to better understand the COVID-19 and ARDS pathways, as well as to find targeted single nucleotide polymorphism. To accomplish this, we retrieved over 100 patients' samples from the Sequence Read Archive, National Center for Biotechnology Information. These sequences were processed through the Galaxy server next generation sequencing pipeline for variant analysis and then visualized in the Integrative Genomics Viewer, and performed statistical analysis using t-tests and Bonferroni correction, where six major genes were identified as DNAH7, CLUAP1, PPA2, PAPSS1, TLR4, and IFITM3. Furthermore, a complete understanding of the genomes of COVID-19-related ARDS will aid in the early identification and treatment of target proteins. Finally, the discovery of novel therapeutics based on discovered proteins can assist to slow the progression of ARDS and lower fatality rates.
Purpose: The purpose of this study was to compare symptom severity and the impact of presence of depressive and/or anxiety symptoms among patients with head and neck cancer (HNC). Methods: One hundred and fifteen patients diagnosed with HNC completed two questionnaires: Hospital Anxiety and Depression Scale (HADS) and the M. D. Anderson Symptom Inventory-Head and Neck Cancer (MDASI-HN). Results: Of the total sample, 55.6% reported depressive symptoms and 33% reported anxiety symptoms. Patients who reported either depressive or anxiety symptoms also had significantly more severe symptoms. The most severe symptom was dry mouth. Participants who were depressed reported that interference in the enjoyment of life due to symptoms was the most distressing whereas participants with anxiety reported symptom interference in work was of most concern. Of patients reporting moderate to severe symptoms, more than 60% reported depressive symptoms. Of patients reporting moderate to severe feelings of being distress and feeling sad, more than 70% reported anxiety symptoms. Conclusion: Patients with HNC reporting moderate to severe symptoms also report depressive and/or anxiety symptoms. The results suggest the need to develop an integrated nursing intervention of both physical and emotional symptoms for patients with HNC.
PURPOSES : The purpose of this study is to suggest a specific investigation guideline to decide priority of repairing old concrete pavements that pile up substantially. METHODS : In this study, a principle of division of homogeneous sections was proposed to reflect the characteristics of the pavement reasonably in the specific investigation results. In addition, a checklist and guideline of field investigation were suggested for the old concrete pavement sections, which require inspection toward their durability and structural performance. Furthermore, the items of laboratory test necessary to the old concrete pavement were suggested based on the existing laboratory test considering characteristics of the old concrete pavement. The present condition of the old concrete pavement could be analyzed by the test results. RESULTS : A method of division of homogeneous sections suitable for the specific investigation of the old concrete pavement was suggested. The proportions of distress severity of pavement sections were compared by distress type to figure out the present state of the old concrete pavement. Scaling, durability cracking (or alkali-silica reaction), and longitudinal spalling were selected as the most severe distress types. The detailed positions of the sections were also suggested. The checklist of the specific investigation was categorized by field survey and laboratory test, and its evaluation criteria were proposed. The three types of the sections of durability cracking (or alkali-silica reaction), bridge connection, and asphalt overlay were selected as the sections of the field survey. The compressive strength, void structure, and chloride penetration depth were suggested as the items of the laboratory test. CONCLUSIONS : A fundamental level of the guideline was suggested in this study to resolve the problem of old concrete pavement. Appropriate guidelines related to the repair of the old concrete pavement should be provided by performing additional research efforts.
Purpose: Women who undergo gynecological surgery have moderate and severe sensation and distress of pain despite the advent of patient controlled analgesia (PCA). The purposes of this study were to describe perception of non-pharmacological therapy for postoperative pain control and examine changes of pain sensation and distress in women who had gynecological surgery. Method: The sample consisted of 52 women who were having gynecological surgery. Subjects who agreed to participate in the study were asked for their opinion about non-pharmacological approaches for postoperative pain control using a structured study questionnaire. Pain sensation and distress were assessed by VAS in the morning and afternoon for 2 days following the surgery. Result: About 50% of the subjects thought that non-pharmacological methods such as relaxation, music, massage, or meditation would be helpful for their postoperative pain control. If both pharmacological and non-pharmacological therapy were given for pain control, 96% of subjects reported it would be effective. Nurses can apply techniques of relaxation, deep breathing, meditation, and music therapy to surgical patients along with PCA. Expected sensation and distress of pain was high, but pain levels gradually decreased over time. However, subjects experienced moderate levels of pain postoperatively although they used PCA. Conclusion: The effect of a combined method of pharmacological and non-pharmacological approach needs to be tested if postoperative pain is to be decreased more.

Background: Extracorporeal membrane oxygenation (ECMO) has recently attracted interest as a treatment for severe acute respiratory distress syndrome (ARDS). However, the outcomes of this procedure in post-surgical settings have not yet been characterized. In this study, we evaluated the outcomes of ECMO in patients with severe postoperative ARDS. Methods: From January 2007 to December 2012, a total of 69 patients (aged $58.3{\pm}11.5$ years, 23 females) who underwent venovenous ECMO to treat severe postoperative ARDS were reviewed. Of these patients, 22 (31.9%) had undergone cardiothoracic surgery, 32 (46.4%) had undergone liver transplantation, and 15 (21.7%) had undergone other procedures. Results: Thirty-four patients (49.3%) were successfully weaned from ECMO, while the other 35 patients (50.7%) died on ECMO support. Among the 34 patients who were successfully weaned from ECMO, 21 patients (30.4%) eventually died before discharge from the hospital, resulting in 13 hospital survivors (18.8%). Multivariable analysis showed that the duration of pre-ECMO ventilation was a significant independent predictor of death (odds ratio [OR], 2.25; 95% confidence interval [CI], 1.29 to 3.90; p=0.004), whereas the concomitant use of continuous venovenous hemodialysis (CVVHD) was associated with improved survival (OR, 0.55; 95% CI, 0.31 to 0.97; p=0.038). Conclusion: Although the overall survival rate of patients treated with ECMO for postoperative ARDS was unfavorable, ECMO offered an invaluable opportunity for survival to patients who would not have been expected to survive using conventional therapy. CVVHD may be beneficial in improving the outcomes of such patients, whereas a prolonged duration of pre-ECMO ventilator support was associated with poor survival.
Pneumomediastium in a newborn baby is a rare condition and is usually manifested by respiratory and circulatory distress syndrome. We recently have experienced a newborn baby of severe tension pneumomediastinum associated with bilateral pneumothorax. The patient in this report was a day old female and the mother of the baby a lot of difficulties during her delivery and the aid of vaccuum was necessary. This patient was received closed thoracotomy and followed by explothoracotomy and excision of tension multiple air bubbles. The post-op. course is not uneventful.
Cystic hygroma is a cystic tumor developed by lymphatic stasis due to congenital blockage of regional lymphatic drainage. It ususally occurs at neck and axilla. However, in some cases of cervical cystic hygroma, cervical portions of the cysts may extend into the mediastinum. We experienced a case of huge cervico-mediastinal cystic tumor in 11 month old girl presenting a large fluctuating neck mass and severe respiratory distress. Surgical resection was done through combined approach of cervical incision and median sternotomy. Pathological diagnosis was confirmed to cervico-mediastinal cystic hygroma.
