일부 성인남녀의 무기질 (Ca, P, Fe, Na, K, Zn) 섭취와 혈압 및 혈중 지질과의 관련성 (Relation among Mineral (Ca, P, Fe, Na, K,Zn) Intakes, Blood Pressure, and Blood Lipids in Korean Adults)

  • The purpose of this study was to investigate the relation among mineral intakes, blood pressure, and blood lipids for health management of Korean adults over 20 years old. Subjects were recruited from the district where they lived, which included rural (Yoju, n = 137) , coastal (Taean, n = 100) , and urban district (Suwon, n : 117) . hnthropornekical measurement, blood pressure measurement, dietary intake assessment using 24-hour recall method, and blood collection and analysis of blood lipids were conducted. The average age, height, weight, and BMI were 54.1 years, 168.5 cm, 67.3 kg, and 24.5 kg/$m^{2}$ for men and 53.9 years,153.8 cm,59.2 kg, and 25.0 kg/$m^{2}$ for women, respectively. The mean daily intakes of total food and energy were 1219.1 g and 1740.9 kcal for men and 1071.3 g and 1432.6 kcal for women. The mineral intakes of men and women were 448.5 mg and 400.4 mg for calcium,955.3 mg and 803.7 mg for phosphorus, 13.3 mg and 11.1 mg for iron, 4302.8 mg and 3393.3 mg for sodium,2588.0 mg and 2264.9 mg for potassium, and 8.9 mg and 7.3 mg for zinc. The daily intakes of energy, calcium, zinc, vitamin $B_{2}$, and vitamin I were $76.1\%$, $60.1\%$, $73.1\%$, $68.6\%$, and $80.4\%$ of RDAs, respectively. The intakes of energy, calcium, zinc, vitamin $B_{2}$ and I were not meet Korean RDAs. Especially, calcium, zinc, and vitamin $B_{2}$ were less than $75\%$ of RDAs. Blood pressure of the subjects was 128.3/75.5 mmHg for men and 124.1/73.7 mmHg for women. Serum lipids of men and women were 180.2 mg/dL and 184.9 mg/dL for total cholesterol, 160.8 mg/dL and 137.6 mg/dL for triglyceride,41.5 mg/dL and 44.7 mg/dL for HDL-cholesterol,106.6 mg/dL and 112.7 mg/dL for LDL-cholesterol, and 3.5 and 3.3 for atherogenic index. While serum triglyceride and AI of men was significantly higher, HDL-cholesterol was lower than those of women. In the relation among mineral intakes, blood pressure, and serum lipids, we found a significantly negative correlation of intakes of calcium, calcium/phosphorus, calcium density with systolic blood pressure. There were significantly negative correlation between intake of calcium/phosphorus and serum cholesterol, calcium density and serum cholesterol, sodium intake and serum HDL-cholesterol. Based on these results, we concluded that high intakes of calcium, calcium/phosphorus and low sodium intake were associated with favorable blood pressure and serum lipid profiles in adult men and women. Relationship between vitamin D levels and intravenous immunoglobulin resistance in Kawasaki disease

  • Purpose: Vitamin D is associated with various pathological conditions such as cardiovascular diseases and cancer. We investigated the relationship between vitamin D and Kawasaki disease (KD). Methods: We performed a retrospective review of the medical records of patients with KD between February 2013 and March 2016 in Daegu Fatima Hospital. Study participants were grouped according to vitamin D serum concentration. Group 1 included patients with 25(OH)-vitamin D ${\geq}20ng/mL$. Group 2 included patients with 25(OH)-vitamin D <20 ng/mL. We analyzed the clinical characteristics and laboratory data of the 2 groups. Results: Of the 91 patients, 52 were included in group 1, and 39 in group 2. Group 1 patients had significantly higher levels of calcium, phosphate, albumin and sodium than group 2 patients did. There were no differences in clinical characteristics, but the proportion of patients with polymorphic rash was significantly higher in group 2. Resistance to intravenous immunoglobulin was more frequent in group 2 (P=0.023). No significant difference in the incidence of coronary artery complications was observed. Conclusion: Low vitamin D levels are associated with resistance to intravenous immunoglobulin therapy in KD. Vitamin D deficiency might be a risk factor for immunoglobulin resistance in KD.

Subclinical Vitamin D Insufficiency in Korean School-aged Children

  • Purpose: Recently, vitamin D insufficiency has increased and has been correlated to growth and puberty in children. This study was conducted to find the prevalence of subclinical vitamin D insufficiency and its influence on school-aged children in Korea. Methods: The subjects of this study were 397 children aged 7 to 15 years who had been tested for 25-OH vitamin D3 among the outpatients of the Department of Pediatrics in Eulji General Hospital from March 2007 to February 2011. Data for age, sex, comorbidities, serum 25-OH vitamin D3, height, weight, body mass index (BMI), and sunlight exposure time were collected before and after 3 months of vitamin D administration, retrospectively. Results: Vitamin D insufficiency was present in 343 (86%) of the subjects. In the vitamin D insufficient group, chronological age was $8.96{\pm}1.72$ years, mean height (z-score [z]) was $0.51{\pm}1.26$, mean BMI (z) was $0.81{\pm}2.20$, and bone age was $10.26{\pm}1.75$ years. In the vitamin D sufficient group, chronological age was $9.61{\pm}1.77$ years, mean height (z) was -$0.66{\pm}0.98$, mean BMI (z) was -$0.01{\pm}1.16$, and bone age was $9.44{\pm}2.12$ years. A paired t-test showed that three months after vitamin D administration, the mean 25-OH vitamin D3 level in the insufficient group increased to $24.38{\pm}10.03$ ng/mL and mean BMI (z) decreased to $0.67{\pm}1.06$. Conclusion: In Korean school-aged children, vitamin D insufficiency were relatively higher and may be closely related with higher BMI. Insufficient rise of the level of vitamin D after supplementation suggest the new supplementation guidelines, especially for Korean children.

Studies on Vitamin Mineral Interactions in Relation to Passive Transfer of Immunoglobulins in Buffalo Calves

  • Status of blood minerals and their absorption by neonate calves as influenced by fat soluble vitamins supplementation in their respective mothers, mineral supplementation in calves themselves has been evaluated. The objective was to know the impact of antioxidant vitamin supplementation to advance pregnant buffaloes, on enhanced acquired immunity during first few hours after birth, in relation to weight gain in buffalo calves. Advance pregnant buffaloes (n = 30) consisting of average body weight of $550{\pm}15$ kg and of 4-6 parity were fed on 25 kg green (green Jawar-Sorghum bicolor), 2-3 kg wheat straw and 3-4 kg concentrate mixture individually per day. Intramuscular injections of vitamin triplex A $D_3$ E consisting of -2,500,000 IU of vit A -Palmitate; 2,500,000 IU of vitamin $D_3$ and 1,000 IU of vit E (dl-alpha tocopherol acetate) were given per dose, a month prior to parturition, twice at 15 days interval to 15 dams. Rest of the 15 pregnant buffaloes served as negative controls. Secretion of immune proteins, immunoglobulin (Ig) enhanced by 80% in colostrum. The blood serum levels of Zn, Cu, Ca, Mg were measured from birth to 90 days in calves. A significant (p<0.05) difference between the blood serum Zn levels of calves born to vitamin supplemented and non-supplemented dams was measured and a positive correlation between blood serum Zn levels and injections of vitamins was identified. Association of Zn and Cu with passive immunity status has been identified in these calves. A significant positive correlation between Zn and Cu was also identified which showed a change under the impact of vitamin supplementation in buffaloes. The study signifies the role of micronutrients supplementation in dams prior to parturition, in calf immunity development. The study indicates significant mineral - vitamins interactions during this process.

Vitamin C가 방사선과 Aflatoxin B1을 투여한 흰쥐의 혈청과 간장의 지질성분 및 지방산 조성에 미치는 영향 (The Effects of Vitamin C on Lipid Contents and Fatty Acid Compositions of Serum and Liver in Rats Treated with Radiation or Aflatoxin B1)

  • Aflatoxin은 Aspergillus속 곰팡이로부터 생성되며 사람에게 있어서 간독성 및 간암을 유발하는 잠재력을 가진 곰팡이 독소이며, 지질과산화 반응은 aflatoxin $B_1$에 의한 세포 산화적 손상 시 발생하는 주요 현상 중의 하나이다. 방사선은 수술, 항암약물요법과 더불어 임상 시 중요한 치료방법이나, 정상세포에 방사선을 조사하였을 때 반응성이 높은 활성산소와 과산화라디칼$(OH{\cdot})$을 생성하여 세포막의 불포화지방산을 지질과산화물로 변성시켜 세포 산화적 손상을 일으킨다. 따라서 본 연구의 목적은 aflatoxin $B_1$과 방사선조사 후 유도된 간과 혈청의 지질성분 및 지방산 조성에 미치는 vitamin C의 영향을 조사하기 위해 수행되었다. X-ray 조사는 실험기간 내 단 1회로 실험사육기간 첫 일에 조사하였고 X-ray 조사 후 vitamin C를 투여하였으며 vitamin C 투여 1시간 후 $AFB_1$을 투여하였다. Vitamin C와 $AFB_1$은 모두 복강투여로 실험 사육 첫 일부터 1회 시작하여 3일에 한번씩, 5회 반복 투였으며 실험동물 사육기간은 총 15일로 하였다. 혈청 총콜레스테롤 농도는 $1\sim5$군에서 유의적인 차이를 볼 수 없었는데 방사선과 $AFB_1$을 함께 처리한 6군에서는 $130.37{\pm}9.14mg/dL$로서 전군에서 가장 높은 수치를 나타낸 반면 6군에 vitamin C를 혼합 투여한 7군에서는 $90.23{\pm}6.15mg/dL$로서 가장 낮았으며 이는 중성지질과 유사한 경향이었다. 또한 간장의 총콜레스테롤과 유리 콜레스테롤도 이와 유사한 경향이었다. HDL 농도는 방사선과 $AFB_1$을 함께 처리한 것에 vitamin C를 혼합 투여한 7군이 $59.57{\pm}9.56mg/dL$로서 p<0.01 수준에서 현저히 증가하였다. 혈청의 인지질과 콜레스테롤 에스테르의 지방산 조성에서 vitamin C를 혼합 투여한 3,5,7군에서 PUFA 함량이 증가한 반면, SFA 함량은 감소하여 P/S 비율, n-3P/n-6P 비율은 증가하는 경향이었으며 이는 간장의 인지질, 콜레스테롤 에스테르, 총 지질의 지방산조성에서도 같은 경향을 볼 수 있었다.

Vitamin D and fibromyalgia: a meta-analysis

  • Vitamin D is a cofactor responsible for autoimmune disorders. There is no agreement in the studies investigating the association between vitamin D and fibromyalgia. This study aims to combine the conflicting results of the primary studies which compared these patients with control groups regarding the serum concentration of vitamin D. This meta-analysis has been designed based on PRISMA guidelines. Relevant keywords were searched in PubMed, Science direct, Scopus, Cochrane, and Google scholar and primary studies were selected. After screening the eligible studies according to inclusion/exclusion criteria, we investigated the risk of bias in the selected studies and also the heterogeneity between the primary results using Cochrane (Q) and I-squared ($I^2$) indices. The primary results were combined using inverse variance method and Cohen statistics as well as a random effects model. Publication bias was assessed using Egger test. Sensitivity analysis was applied to investigate the influence of each primary study on the final result of the meta-analysis. Suspected factors in the heterogeneity were assessed using meta-regression models. We entered 12 eligible studies in the meta-analysis including 851 cases compared with 862 controls. The standardized mean difference of Vitamin D between the two groups was -0.56 (95% confidence interval: -1.05, -0.08). Our meta-analysis showed that vitamin D serum levels of patients with fibromyalgia was significantly lower than that of control group.

Ca과 Vitamin D 보충이 폐경 이후 여성의 뼈대사에 미치는 영향 (The Effect of Ca and Vitamin D Supplementation on Bone Metabolism in Postmenopausal Women)

  • This study was designed to investigate the effects of Ca and/or vitamin D supplementation for 53 weeks on bone metabolism in postmenopausal women. The subjects were healthy 18 women aged from 59 to 69 years old. They were divided into three groups : placebo, Ca(1000mg/day) supplementation and Ca(1000mg/day) with vitamin D(12.5$\mu\textrm{g}$/day) supplementation. During the experimental periods except for metabolic studies, the subjects ate their usual diets and the use of drugs as well as excessive exercise was prohibited. Metabolic studies were conducted in the 1st week and in the 53rd week of the experimental periods. The subjects ate experimental diets which consisted of 1787.3kcal, 69.6g of protein, 561.5mg of Ca and 1078.6mg of P daily during both of the metabolic study periods. The results were summarized as follows; 1) Bone density of the second lumbar spine and trochanter measured after treatment decreased significantly in control group as compared with pre-experimental level(p<0.05). On the contrary, bone density of femoral neck and Ward's triangle in Ca group and the second lumbar spine in Ca.Vit D group increased significantly after treatment. 2) Serum PTH and calcitonin levels did not show any significant differences among groups before and after treatment. But serum PTH level increased significantly in all groups after treatment(P<0.05). 3) Serum Ca and P levels did not show any significant differences among groups before and after treatment. But serum Ca level increased significantly in all groups after treatment (P<0.05) and serum P level decreased significantly in Ca.Vit D group after treatment(P<0.05). 4) Mean 24-hours fecal Ca excretion of Ca group was the highest in the 1st week of treatment(P<0.01), and that of control group was the lowest in the 53rd week of treatment(P<0.01). Fecal Ca excretion increased significantly in control and Ca.Vit D group in the 53rd week of treatment(P<0.05). Urinary Ca excretion did not show any significant differences among groups in the 1st and 53rd week of treatment, but that of Ca.Vit D group was the highest the 1st week of treatment(P<0.01). In the 53rd week of treatment Ca and Ca.Vit D group showed positive Ca balance, but control group showed negative Ca balance. The above results showed that it will be difficult to prevent degenerative bone loss without Ca and/or vitamin D supplementation in postmenopausal women eating Korean usual diets.

Vitamin D Status of Breastfed Filipino Infants Aged Less Than 6 Months in an Urban Community

  • Purpose: This study aimed to determine the serum 25-hydroxy-vitamin D (25(OH)D) status of breastfed infants less than six months old and their mothers, and factors affecting the status. Methods: This cross-sectional study was done on breastfed, term, Filipino infants less than six months old who were seen at local health centers and clinics in an urban area. The serum 25(OH)D levels of these infants and their mothers were determined, and their demographic data, nutritional status, sun exposure behavior, and maternal vitamin D intake were analyzed for correlation using regression models. Results: Among the 131 infants, 101 (77%) had vitamin D deficiency (VDD), which was defined as having 25(OH)D levels <37.5 nmol/L, and 13 (10%) had vitamin D insufficiency (VDI), with levels >37.5-50 nmol/L. Conversely, maternal VDD with levels <50 nmol/L was seen in 31 (24%) mothers and maternal VDI with levels 50-75 nmol/L, in 63 (48%) mothers. Infant age and maternal 25(OH)D status were independent predictors of infant VDD. Infants less than three months old were found to have a six-time increased risk of infant VDD (p=0.004). Infants who had mothers with VDD had a six-time increased risk, whereas those with maternal VDI had a four-time increased risk of infant VDD (p=0.049 and p=0.020, respectively). Conclusion: Both infant and maternal VDD and VDI were seen to be highly prevalent in this tropical, urban community. Young infants and maternal VDD/VDI independently increased the risk of infant VDD, whereas lack of sun exposure of the mothers increased the risk for maternal VDI.

과량의 비타민 E 및 Cholesterol 첨가식이가 흰쥐의 혈청과 간의 비타민 E 및 Cholesterol 농도에 미치는 영향 (Effects of Excess Vitamin E and Cholesterol Supplements on Levels of Vitamin E and Cholesterol in Rat Serum and Liver)

  • 본 실험을 통해 뚜렷한 경향을 보여주는 몇가지 결과를 요약하면 다음과 같다. 식이vitamin E의 과량 첨가는 쥐의 혈청에 반영되어 나타났으며, 식이중의 cholesterol은 이 증가 효과를 감소시키는 경향을 보여주었다. 식이 cholesterol첨가로 인해 쥐의 혈청 cholesterol농도는 영향을 받지 않은 반면, 간의 cholesterol농도는 증가되었다. 과량 vitamin E와 cholesterol 두가지를 식이에 첨가하였을 경우 (IV-ECh IV'-ECh) 간의 cholesterol농도는 식이에 cholesterol만을 첨가한 경우 (III-Ch, III'-Ch)보다 더욱 높게 나타났다. 그리고 vitamin E만을 첨가한 II-E와 II'-E군에서는 간의 cholesterol농도를 낮추어주는 경향을 보였다. 간의 vitamin A 농도는 쥐가 성장함에 따라 감소하였으며 각 실험군 간에는 유의적인 차이를 보이지 않았으나, B군의 경우 과량 vitamin E 식이는 간 vitamin A농도를 감소시키는 경향으로 나타났다. Vitamin E, vitamin A, cholesterol 및 다른 여러 요소들간의 복잡한 상호관계를 알아내기 위해, age가 다른 그리고 여러 species의 동물을 이용한, 장기간의 단계적이고도 체계적인 많은 연구가 요구되어진다.

계란의 영양적 특성 및 건강에 미치는 영향 (Nutritional roles and health effects of eggs)

  • Purpose: The aim of this study is to examine the effects of egg consumption and suggest proper guidelines for consumption of eggs by determining the relationship between eggs and cholesterol. Methods: Literature review was conducted on the relationship between nutritional, functional properties of eggs and serum cholesterol, as well as cardiovascular disease. Results: Eggs, which are a good protein food with complete amino acid composition, contain vitamin A, riboflavin, vitamin $B1_2$, folic acid, vitamin D, vitamin E, vitamin K, calcium, iron, choline, selenium, ${\beta}$-carotene, lutein, zeaxanthin, etc. However the egg yolk has a high cholesterol content, which is associated with chronic diseases, including heart disease and hypertension. As a result, its intake is subject to regulation. Outbreak of heart disease by yolk intake can show different results depending on the characteristics of the subjects, amount of egg intake, and the implications of other foods eaten. It is difficult to determine whether eggs are beneficial, as they are the main supplying source for other major nutritive elements as well. Several research studies insist that when cholesterol intake increases by 100 mg, the level of serum cholesterol increases by 2.2~4.5 mg/dL and when serum cholesterol increases by 1%, the risk of heart disease increases by 2%. This indicates that a large intake of eggs can increase the risk of heart disease. Although the cholesterol of egg yolk and serum cholesterol are correlated, it is insufficient to conclude that only cholesterol and not other components are related to heart disease. In fact, other components in egg such as various unsaturated fatty acids and phospholipids could be related as well. Rather than concluding egg as a 'good' or 'bad' food according to its cholesterol content, it is important to define egg as a part of dietary patterns. Conclusion: Generalizing an indiscriminate and uniform amount of egg intake for all seems inadequate. However, patients with diabetes or heart disease should pay particular attention to the amount of egg intake. As for the norm, eating egg with vegetables as a substitute for other animal products seems beneficial.
