  • The objective of this study was to investigate the relation of serum calcium level, body mass index(BMI) with bone status expressed as broadband ultrasound attenuation(BUA) measured by quantitative ultrasound (QUS) and the occurrence of osteopenia among adult men and women. Two hundred eleven(63 male and 148 female) workers who worked in 4 different battery factories were recruited from March 2005 to October 2005. BUA was used as a surrogate of bone mineral density and measured at left calcaneous bone area.   • It is now apparent that many cases of amenorrhea. oligomenorrhea. corpus luteum deficiency, galactorrhea, and infertility are due to hyperprolactinemia. We investigated the relationships between serum prolactin values and factors such as menstrual pattern, frequency of galactorrhea etc, in 135 hyperproIactinemic patients at the Seoul National University Hospital during a period of 6 years, from January, 1979 to December, 1984. The results was as follows: 1. Menstrual pattern changed according to the serum prolactin level. The frequency of amenorrhea is 1.7 percent in patients with serum prolactin levels ranged from $25{\sim}40ng/ml$, whereas 72.4 percent in patients with serum prolactin levels above 100ng/ml. 2. The incidence of galactorrhea in hyperprolactinemic patients was 3.1 percent and the frequency of galactorrhea had direct relationship with the serum prolactin level and/or the frequency of abnormal menstrual pattern. 3. The incidence of pituitary tumor in hyperprolactinemic patients was 104 percent and sixty percent of patients with serum prolactin levels above 100ng/ml had a pituitary tumor . 4. There was an inverse correlation between serum prolactin and progesletone value. 5. The frequency of anovulatory menstrual cycle evidenced by basal body temperature is 23.9 percent in patients with serum prolactin levels ranged from $20{\sim}40ng/ml$, whereas 76.9 percent in patients with serum prolactin levels above 100ng/ml.

Influence of Postconceptional Age on the Renal Biomarkers in Very-Low-Birth-Weight Infants

  • Purpose: We investigated whether consecutive levels of new emerging renal biomarkers, including serum cystatin C (CysC) and urinary neutrophil gelatinase-associated lipocalin (NGAL)/creatinine (Cr) ratio, were affected by postconceptional age in very-low-birth-weight (VLBW) infants. Methods: Repeatedly measured samples for each infant were divided into four groups according to postnatal age: at birth (stage I), 3 to 7 days postnatally (stage II), 8 to 28 days postnatally (stage III), and >28 days postnatally (stage IV). The association between renal biomarkers and postconceptional age was assessed using Pearson's correlation coefficient, and the mean values of renal biomarkers in the four stages were compared using repeated-measures analysis of variance. Results: For samples measured at birth, serum CysC (r=-0.358, P=0.032) and urinary NGAL/Cr ratio (r=-0.522, P=0.001) were negatively correlated with gestational age, whereas serum Cr (r=0.148, P=0.390) was not. In addition, for all samples measured, serum CysC (r=-0.209, P=0.012), urinary NGAL/Cr ratio (r=-0.536, P<0.001), and serum Cr (r=-0.311, P<0.001) were negatively correlated with postconceptional age. Compared with the mean values of the postnatal age-specific stages, serum CysC showed no significant differences in any of the four stages. However, the urinary NGAL/Cr ratio in stage IV was significantly different from those in stages I to III. Conclusion: Although urinary NGAL/Cr ratio and serum CysC were negatively correlated with postconceptional age considering renal development, serum CysC showed no significant differences in any of the four postnatal age-specific stages. Urinary NGAL/Cr ratio at >28 days postnatally seems to be more affected by postconceptional age than serum CysC in VLBW infants.

  • This study was designed to find the effect of Minocycline loaded microcapsule applied locally to tissue by measuring drug concentration in tissue and serum by HPLC and to achieve optimal drug delivery system and duration to a specific target site. Control group were administrated minocycline intramuscularly twice a day with $0.2{\mu}g/100g$ for 1 to 10 days. In experimental group, surgical wound was created on Rt. cheek and then minocycline loaded microcapsule was applied into the space between superficial and deep layer of masseter muscle. Animals were sacrificed at 1, 3, 5, 7, 10 days after initial administration, blood was obtained from heart and right masseter muscle was excised. Blood sample was centrifuged at 3000rpm for 15min. Tissue sample was homogenized, left at room temperature for 48hr and centrifuged at 4000g for 5min. Supernatant was completely dried and dissolved in distilled water. Analysis was conducted using a ${\mu}Bondapack$ C18 column. The mobile phase was 0.2M Ammonium Oxalate/0.1M EDTA/DMF=11/4/5 solution, which was injected into the column and detected with photodiode detector at 344nm wavelength. The results were as follows : 1. This method was reliable, could be replicated and suitable for minocycline analysis in tissue as well as serum. 2. In tissue, concentration of minocycline of experimental group was higher than that of control group for 5days. 3. Except 1 day, concentration of minocycline in serum of experimental group was lower than that of control group. 4. Concentration of minocycline in tissue was much higher than that in serum. From these results, minocycline loaded microcapsule might be effective tool for local drug delivery system might be useful for treatment of infections of oral and maxillofacial region and management of infected surgical wound, minimizing systemic effects.

Are Serum Vitamin D Levels Associated With Dry Eye Disease? Results From the Study Group for Environmental Eye Disease

  • Objectives: Dry eye disease (DED) is an increasingly important public health problem in Korea. Previous studies conducted in Korea have reported inconsistent results regarding the protective effects of vitamin D on DED, and these discrepancies may be related to the relatively simple questionnaire that has been used. Thus, we evaluated the association of serum vitamin D levels with DED using the ocular surface disease index (OSDI). Methods: The present study evaluated data from participants in the Study Group for Environmental Eye Disease (2014-2015). This group included data from 752 participants, and data from 740 participants (253 men and 487 women) were analyzed in the present study. DED severity was evaluated using the OSDI. Results: Higher serum vitamin D levels were associated with a non-significantly reduced risk of DED in the crude analysis (odds ratio [OR], 0.991; 95% confidence interval [CI], 0.971 to 1.011) and in the adjusted analysis (OR, 0.988; 95% CI, 0.966 to 1.010). In the crude analysis of no/mild DED vs. moderate/severe DED, men exhibited a decreased risk with increasing serum vitamin D levels (OR, 0.999; 95% CI, 0.950 to 1.051), while women exhibited an increased risk (OR, 1.003; 95% CI, 0.979 to 1.027). In these analyses, we found no significant associations. Conclusions: The findings of the present study support previous reports that serum vitamin D levels are not associated with DED.

THE CHANGES OF C-REACTIVE PROTEIN IN THE PATIENTS ASSOCIATED WITH MANDIBULAR FRACTURE (하악골 골절 환자에서의 C-Reactive Protein의 변화)

  • It is well known that concentration of C-reactive protein(CRP) in the serum increase as nonspecific reaction of the various tissue injury. The CRP, synthesized in the hepatocyte, is one of 'acute phase proteins' in the serum. The main signal patterns of this protein are regulated by synthesis of interleukin-I secreted from macrophage in the area of tissue injury. Many studies were performed for quantitative analysis for CRP according to various surgical operation, but the study for fracture patients associated with trauma, especially in mandible, are rare. The mandible fracture have intrinsic danger for infection in oral bacteria associated with open wound in oral cavity, and, are difficult for detection of tissue reaction between surgical swelling and infection by facial swelling. In this study, quantitative analysis for CRP associated tissue injury in mandibular fracture and surgical intervention was done, the results were as follows: 1. After initial mandibular trauma, the value of serum CRP diminished sequentially, most high value was presented in post-traumatic 2 days. 2. The CRP was diminished significantly 2 days after surgical intervention, and maintained normal value in 5 days after surgery. 3. The change of CRP are higher value in surgical intervention than initial trauma, it suggested that tissue injury from surgery was severe than trauma. 4. The high value of CRP was obtained in mandibular fracture combined soft tissue injury than no associated soft tissue injury. 5. In measurement of CRP according to surgical approach, highest serum value in patients of combined intra-oral and extra-oral approach was showed, and intra-oral approach, extra-oral approach, in sequential orders. 6. The CRP value are more higher in patient of 2 fracture site than only one fracture site. From the results obtained in this study, CRP has showed different values in mandibular fracture associated with severity of tissue injury and surgical intervention, and quantitative analysis of CRP value in serum can be applied to the clinical management of mandibular fracture.

Determination of Inorganic Elements in Women Blood Serum using Instrumental Neutron Activation Analysis (중성자방사화분석법을 이용한 성인여성 혈청중의 무기 원소 분석)

  • In this study, instrumental neutron activation analysis was used to assess the concentration level of inorganic trace elements in Korean women blood serum. It was found out that high concentration of Na and Cl incurs analytical interference, but 12 elements of Br, Ca, Cl, Co, Cr, Cs, Fe, K, Na, Rb, Se, Zn can be determined under the condition of interference minimization. Serum samples collected from 63 women were analyzed and the concentration level and range of the elements were evaluated. NIST SRMs were analyzed simultaneously for analytical quality control. The average values of Na and Cl determined in serum samples are around 3000 mg/L, Ca is 100 mg/L and K is 200 mg/L. Besides, Br, Se and Zn have concentration level of 6.0, 0.1 and 1.0 mg/L, respectively. It was found that there is no significant difference between the present values and reported values.

Vegetarian Diets and Estrogen Metabolism in Korean Premenopausal Women

  • It has been suggested that dietary fat increases the risk of breast cancer by elevating serum estrogen concentrations. However, studies on a relationship between fat intake and breast cancer risk have shown contradictory results, possibly because the levels of fat intake in study populations were too high to observe differences. Also, the effect of other dietary factors may present. The present study was performed to investigate the relationship between diet and estrogen metabolism in premenopausal women whose usual fat intake is relatively low compared to their western counterparts. Twenty lacto-ovo vegetarians (LOV) and twenty omnivores participated in the study. Three day food records including a Saturday or a Sunday were used to estimate nutrient intake. Serum lipids, estradiol, sex-hormone binding globulin, and urinary estradiol were measured. Study results showed $24.8\%$ and $20.9\%$ of energy intake were provided from fat in omnivorous and LOV subjects, respectively. Serum and urinary levels of estradiol were two times higher in omnivores. Fat intake was not related to either serum estradiol nor urinary estradiol when the Spearman correlation coefficient analysis was performed. Carbohydrate, total dietary fiber and soluble dietary fiber intakes were negatively related to serum estradiol concentration. Legumes, vegetables and fruit consumption showed significantly negative relationships with both serum and urinary estradiol concentrations. These results indicate lower estrogen availability may be associated with plant food­based diets in premenopausal women whose usual diets contain less than $25\%$ of energy as fat.

The Effects of Dietary Carbohydrate on Serum Triglyceride Concentrations in Korea (우리나라 성인의 당질섭취가 혈청 중성지방수준에 미치는 영향)

  • The relationship between dietary carbohydrate (CHO) and fasting triglyceride (TG) concentrations was examined in 2,689 men, 1,908 premenopausal women, and 966 postmenopausal women aged 20-69 years in the 1998-Korean Health and Nutrition Survey. The serum TG concentrations in fasting blood samples were measured and dietary CHO intake was assessed by using 24-hour recall method. As CHO intake increased, the TG concentrations were significantly affected (127.4 mg/dl, 132.4 mg/dl, 134.8 mg/dl, 142.6 mg/dl, 147.2 mg/dl) in postmenopausal women, while CHO intake was not significantly associated with TG concentrations in men and in premenopausal women. For the lowest to the highest quintiles of CHO, the mean TG concentrations were 120.0 mg/dl and 140.3 mg/dl in postmenopausal women with body mass index (BMI) <25 kg/m$^2$ and 135.6 mg/dl & 155.3 mg/dl in postmenopausal women with BMI$\geq$25 kg/m$^2$. In multivariate analysis, BMI, waist hip ratio (WHR), glucose, smoking, and drinking habit were associated with serum TG concentrations in men. In premenopausal women, BMI, WHR, and glucose levels were associated with serum TG concentrations, while age, BMI, WHR, glucose levels, and CHO intake were associated with serum TG concentrations in postmenopausal women. In conclusion, CHO intake is related to serum TG concentrations in postmenopausal women in Korea.

Dietary Zinc Analysis and Changes of Zinc Nutriture with Zinc Supplementation in Korean Adults (한국성인의 아연 섭취실태 및 아연보충에 의한 아연 영양상태 변화)

  • The purpose of this was to analyse zinc intakes and effect of Zn(30mgZnSO4/day) supplementation on plasma zinc level, serum HDL-cholesterol and serum Alkaline Phosphatase (AP) activity of Korean adults. The men consumed 8.52($\pm$2.08)mg of zinc, and the women consumed 6.4($\pm$2.62)mg of zinc. Although protein intakes of subjects were lower than normal values. The first source of zinc was cereal and grain group, the second was meat, fish, egg and soybean group. Two food groups supplied about 80% of zinc. After two weeks of zinc supplementation, the zinc concentration in plasma was significantly increased. The highest plasma zinc level was 78.80ug/dl(men), 76.04ug/dl(women) at 2 weeks after zinc supplementation(p<0.05). Serum DHL-cholesterol was significantly decreased by zinc supplementation. The lowest serum HDL-cholesterol level was 39.29mg/dl(men), 44.84mg/dl(women) at 4 weeks after zinc supplementation(p<0.01). Serum AP activity was significantly increased by zinc supplementation. The highest AP activity was 86.40units/L(man), 67.93units/L(women) at 2 weeks after zinc supplementation(p<0.05). It seems that the supplementation of 30mg ZnSO4/day can be beneficial for improving zinc nutriture. However it can be negative factor on coronary heart disease because serum HDL-cholesterol was significantly decreased(p<0.01)
