The Effect of Light Stimulation on the Reduction of Unilateral Neglect of Stroke Patients: Single Subject Research Design (뇌졸중 환자에게 불빛 자극이 편측무시 감소에 미치는 영향: 단일 사례 연구)

  • Objective : The purpose of this study was to investigate the effect of left visual stimuli for reducing the severity of unilateral neglect. Method : This research used a reversal design(ABAB) of single case study. The subject was a 60-year-old male patient who had left unilateral neglect. Remedial approach is more valuable method than compensatory approach in term of reducing the unilateral neglect. One of the method of remedial approach is controlled sensory stimulation. Result : I performed the line bisection test before and after the stimulation, and while doing that, put blinking lights to the patient from the left side. The average errors in the line bisection test decreased in B B', the intervention period, compared with A A'. The MVPT left response behavior score increased from 13 to 18. Conclusion : This shows that the light stimulation is effective in the reduction of unilateral neglect.

The Long-Term Effects of High-Frequency Transcutaneous Electrical Nerve Stimulation(TENS) on the Lower Limb Spasticity and the Balance in the Chronic Stroke Patients (장기간 고빈도 경피신경전기자극이 뇌졸중 환자의 하지 경직 및 균형에 미치는 영향)

  • The purpose of this study was to investigate effects of the long-term high-frequency transcutaneous electrical nerve stimulation(TENS) on the spasticity and the balance in chronic stroke patients. Twenty-six subjects with spasticity over lower limbs were allocated randomly into two groups under standard rehabilitation: (1) TENS group, (2) placebo-TENS group. TENS stimulation was applied on the both the gastrocnemius for 30 minutes, 5 days a week for 4 weeks(100 Hz, 0.25 ms, 2 times sensory threshold). The Modified Ashworth Scale(MAS) and Hand-held manual muscle tester were used to assess the ankle plantarflexor spasticity. Balance function under three conditions was measured by using force-plate and the amount of postural sway was assessed; in (1) the condition of standing with eyes opened, (2) with eyes closed and (3) the condition of standing on unstable surface with eyes opened. Both groups showed significant improvement in spasticity and balance function after treatment for 4 weeks(p<.05). Especially, TENS group showed a significant reduction of spasticity compared to placebo-TENS group(p<.05). These results suggested that additional stimulation of a long-term high-frequency TENS to standard rehabilitation induced an improved balance function and a spasticity reduction. The long-term application of high-frequency TENS will be an effective intervention for reducing spasticity and increasing balance ability in the chronic stroke patients.

The Difference of Grip Strength, Upper Limb Dexterity, and Hand Function according to Light Touch and Thermal Sensory Stimulus (촉각과 온각 자극에 따른 악력, 상지 기민성, 그리고 손 기능의 차이)

  • The purpose of this study was to investigate difference between grip strength, upper limb dexterity, and hand function according to touch and heat sensory stimulus. Forty healthy men were recruited. After obtaining subjects characteristics, examined grip strength, upper limb dexterity, and hand function, then we made touch sensory input using soft brush and tried the test again. Next day, we made thermal input using hot pack and did retest, too. The result showed significant differences appeared on every variable in grip strength and upper limb dexterity test (p<.001), and significant differences appeared on every variable except lifting small objects in hand function (p<.01). In conclusion, sensory input is to increase grip strength, upper limb dexterity, and hand function. The thermal sensory input has a tendency to increase grip strength, upper limb dexterity, and hand function than that of touch sensory input in effect of stimulus classification. The application of proper stimulus helps to facilitate upper limb and hand function in rehabilitation and in industrial field. So it is necessary to try more tests of frequency and intensity of stimulus.

  • Cervical pain is a rapid increase that is owing to a flexion-extension whiplash injury, unappropriated posture, chronical repetition injury from abdominal position of head and neck, excessive repeating work, chronical deficiency of excercise. Because of that is bring about muscle unbalance, tightness of cervical extensor muscle, weakness of cervical deep flexor muscles, instability of cervical region and reduction of proprioceptive sensor. Recent the role of muscle is more emphasized for preservation of sine stabilization. And cognition of integrated muscular system, importance for the operation and relation is increased to maintain stability of the motor system and pertinent function. Therefore we are going to introduce the sling exercise and stabilization exercise method for advanced efficient of cervical and upper limb and for the muscle strengthening to importance cervical stabilization through neurological program as control the reaction of cervical stabilization. Sling exercise therapy(SET) concept consists of a system of diagnosis and treatment. The system of diagnosis involves testing the muscle's tolerance through progressive loading in open and close kinetic chains. The SET system contains elements such as relaxation, increasing the range of movement, traction, training the stabilizing musculature, sensory-motor exercises, training in open and close kinetic chains, dynamic training of the mobilizing musculature, cardiovascular exercise, group exercise, personal exercise at home Sensory-motor training is an essential element of the SET concept. The emphasis is on closed kinetic chain exercise on an unstable surface, there by achieving optimum stimulation of the sensory-motor apparatus.

Deep Learning-based Approach for Classification of Tribological Time Series Data for Hand Creams (딥러닝을 이용한 핸드크림의 마찰 시계열 데이터 분류)

  • The sensory stimulation of a cosmetic product has been deemed to be an ancillary aspect until a decade ago. That point of view has drastically changed on different levels in just a decade. Nowadays cosmetic formulators should unavoidably meet the needs of consumers who want sensory satisfaction, although they do not have much time for new product development. The selection of new products from candidate products largely depend on the panel of human sensory experts. As new product development cycle time decreases, the formulators wanted to find systematic tools that are required to filter candidate products into a short list. Traditional statistical analysis on most physical property tests for the products including tribology tests and rheology tests, do not give any sound foundation for filtering candidate products. In this paper, we suggest a deep learning-based analysis method to identify hand cream products by raw electric signals from tribological sliding test. We compare the result of the deep learning-based method using raw data as input with the results of several machine learning-based analysis methods using manually extracted features as input. Among them, ResNet that is a deep learning model proved to be the best method to identify hand cream used in the test. According to our search in the scientific reported papers, this is the first attempt for predicting test cosmetic product with only raw time-series friction data without any manual feature extraction. Automatic product identification capability without manually extracted features can be used to narrow down the list of the newly developed candidate products.

A Study of Current Perception Threshold of Trigeminal Nerve after Tooth Implantation (치아임플란트 시술 후 삼차신경에서의 전류인지역치에 대한 연구)

  • This study attempted to contribute to the clinical application of implant operation by making a quantitative nerve examination using a neurometer for the evaluation of sensory disturbances that could be incurred after the implantation in the dental clinics, and it intended to establish an objective guideline in the evaluation of sensory nerve after the operation of implant. An inspection was performed with the frequencies of 2000Hz, 250 Hz and 5 Hz before and after the operations of tooth implant using $Neurometer^{(R)}$ CPT/C (Neurotron, Inc. Baltimore, Maryland, USA) for 44 patients who had performed an implant operation among the patients coming to Daejeon Sun Dental Hospital in 2006 and 30 people for control group. The measuring sites were maxillary nerve ending and mandibular nerve ending of trigeminal nerve according to the implant operating regions. The current perception threshold (CPT) by each nerve fiber was specifically responded under the electric stimulation of 2000 Hz in case of $A{\beta}$ fiber and of 250 Hz in case of $A{\delta}$ fiber and of 5Hz in case of C fiber. The CPT test could be performed to assess the damages of peripheral nerve in the trigeminal nerve area and it stimulated selective nerve fibers by generating the electricity of specific frequency in the peripheral nerve area. The nerve fibers with varied thickness were responsive selectively to the electric stimulation with different frequencies; accordingly, they applied the electric stimulation with different frequencies and the reaction threshold of $A{\beta},\;A{\delta}$ and C fibers selectively responsive to each electric current could be individually evaluated. In the assessment through the CPT, the increase and decrease of the CPT could be measured so that sensory disturbances such as hyperaesthesia or hypoaesthesia could be diagnosed. This study could obtain the following results after the assessment of the CPT before and after the implant operation. 1. In the assessment before and after the implant operation, the CPT in the frequencies of 2000 Hz, 250 Hz, 5 Hz for maxillary branch increased on the whole after the operation and the CPT for mandibular branch in the $A{\beta}$-fiber(2000 Hz) and C-fiber(5 Hz )after the operation increased statistically significantly. 2. For the groups of patients with medically compromised or its subsequent medicinal prescription, there were no significant differences before and after the implant operation and for the control groups, significantly high CPT was shown after the implant operation in the left $A{\beta}$-fiber(2000 Hz) and C-fiber(5 Hz). 3. In the comparison of the measured value of the CPT before the operation between the control group and the implant operation group, the latter group had a significantly high measured value of the CPT in the right $A{\beta}$-fiber(2000 Hz) and C-fiber(5 Hz) and there were significant differences in $A{\beta}$-fiber(2000 Hz) in the CPT assessment after the implant operation for the control group. 4. Male participants had higher CPT than female counterparts; however, there were no statistic significances. In the CPT evaluation before and after implant operation, there were no statistical differences in the male group while the right C-fiber(5 Hz) and left $A{\beta}$-fiber(2000Hz) were significantly high in the female group. 5. In the comparison between the group who complain sensory disturbance and the other group, the CPT increased on the whole in the former group, but there were no statistical significances. In the groups, whom there was an increase in VAS, the CPT after the implant operation in the right C-fiber(5 Hz) increased significantly; meanwhile, in case that the VAS mark was '0' before and after the operation, the CPT after the operation in the left $A{\beta}$-fiber(2000 Hz) increased significantly. This study suggested that the CPT measurements using $Neurometer^{(R)}$ CPT/C, provide useful information of objective and quantitative sensory disturbances for tooth implantation.

  • In the hand, few vessels can be classified as 'perforators'. Even today, the debate continues on whether or not branches of the digital artery should be considered as perforators. However, 'perforator' and 'perforator flaps' have become extremely popular within the microsurgery literature, and as such, the terminology has found widespread adoption across the hand surgery field. The finger is the most important sensory organ for tactile stimulation. The glabrous skin can be distinguished between the fingertip and the proximal tissue, with the fingertip having a higher functional and, therefore, reconstructive priority. Thus, sacrifice of the proximal glabrous skin can be justified in the reconstruction of fingertip defects. In this paper, the author reviews two flaps, 'lateral digital artery perforator flap' and 'volar digital artery perforator flap', both of which uses short branches of the digital artery as a vascular pedicle and are useful in fingertip construction.

Changes of Sensibility by Multi Sensory Stimulation with Suiting Fabrics (복합자극에 의한 슈트용 직물의 감성 변화)

  • 본 연구에서는 슈트용 직물의 촉각 자극 시 나타나는 감성 특성을 파악하고, 이를 바탕으로 직물의 스치는 소리가 공감각적 감성변화에 어떠한 영향을 미치는지를 분석하기 위해 물리적 평가 및 주관적 평가를 실시하였다. 피험자 20명을 대상으로 직물의 촉감을 평가하게 한 후, 그 직물의 스치는 소리를 제시하여 감각과 감성을 평가하도록 하였다. 복합자극으로는 직물별 tm치는 소리와 촉감을 동시에 제시하여 때 감성을 평가하였다. 실험결과 촉감에 대한 감성차원으로는 '심미성'과 '개성'의 두 가지 요인이 나타났고, 소리에 대한 감성차원으로는 '심미성'과 '고급감', '개성'의 세 가지 요인이 도출되었다. 심미성을 유발하는 촉각성 요인으로는 'pliable'과 'cool', 개성을 유발하는 촉감성 요인으로 'smooth'와 'cool'로 나타났다. 청각자극에 의한 감성은 'loudness'가 가장 큰 영향을 미쳤다. 촉감자극에 의한 직물 별 소리 자극을 부가하였을 경우 직물 소리의 물리량에 따라 심미성 요인에서 감성의 상승 및 하강 효과를 발견할 수 있었다.

  • Trigeminal neuralgia is a condition characterized by excruciating paroxysms of pain in lips, gums, cheek, or chin, and very rarely, in the distribution of the ophthalmic division of the trigeminal nerve. There are many treatments of trigeminal neuralgia, such as, medical treatment, electrical stimulation, radiation therapy and ablative procedures. Infraorbital nerve block with pure ethyl alcohol is an ablation procedure for trigeminal neuralgia. We injected pure ethyl alcohol into the infraorbital foramen for pain control. The results were as follows; 1) The infraorbital nerve block with pure alcohol was an a simple and an effective method. 2) Complication, included a mild sensory deficit and mild edema over the infraorbital area.
