A Case Report of Tinnitus with Sudden Sensorineural Hearing Loss Treated with Melonis Calyx Vomiting Therapy (과체 토법을 병용한 돌발성 난청으로 인한 이명 환자 치험 1례)

  • Objectives : The purpose of this study is to report the efficacy of using oriental medical treatment withMelonis Calyxvomiting therapy for sudden sensorineural hearing loss(SSNHL).Methods : We experienced one case of SSNHL treated with oriental medical treatment(acupuncture, herbal medicine) andMelonis Calyxvomiting therapy(MCVT). MCVT is a traditional treatment, which induces vomiting by eatingMelonis Calyxpowder with water. To evaluate the results of this treatment, we measured the hearing recovery, decrease of tinnitus and vertigo. The degree of hearing improvement was evaluated by Siegel's Criteria. Decrease of tinnitus and vertigo was assessed by Visual Analogue Scale(VAS).Results : The pure tone 5 frequency average improved by 39㏈(marked recovery, from 64㏈ to 25㏈). VAS of tinnitus decreased from 10 to 1, and vertigo decreased from 4 to 0.Conclusions : This study suggests that using MCVT with oriental medical treatment is effective on SSNHL.

  • Objective : The purpose of this study is to know the effect of Korean medical treatments on sudden sensorineural hearing loss(SSNHL) without systemic or intratympanic steroid treatments.Method : We conducted retrospective review who was diagnosed as SSNHL and treated with Korean medicine in Dept. of Otolaryngology Kyung Hee University Hospital at Gangdong. There were two patients with SSNHL who refused to be treated with western medicine and were treated with Korean medicine including acupuncture, moxibustion, and herbal medicine.Result : One patient recovered completely after eight days of admission, and another patient partial recovered during 30 days through Korean medical treatments.Conclusion : Through these studies, SSNHL is expected to treated more effectively by conventional Korean medical treatments than corticosteroid treatments alone. But the additional study is needed to reveal the exact effects of Korean medical treatments.

  • Recently the efficacy of all ear drops in perforated tympanic membranes has been questioned and the safety of using the drugs intratympanically has been challanged. Many drugs not normally ototoxic when given systemically become ototoxic when administered via the middle ear. According to Stupp and others (1973) among neomycin, polymyxin-G, gentamycin, erythr omycin, tetracycline and penicillin, only penicillin was found to be free of toxic effects. The ototoxic antibiotics probably reach the inner ear by permeating the round window membrane, into the perilymph and then through Reissner's membrane to the endolymph. The main pathological changes are degeneration of stria vascularis, degeneration of sensory epithelia and degeneration of ganglion cells. We report with reference, a case of profound sensorineural deafness following the application of Terramycin powder intratympanically, in a 63 year old woman.

  • In this paper, we proposed a method of a hearing aid suitable for the sensorineural hearing impaired. Generally as the sensorineural hearing impaired have narrow audible ranges between threshold and discomfortable level, the speech spectrum may easily go beyond their audible range. Therefore speech spectrum must be optimally amplified and compressed into the impaired's audible range. The level and frequency of input speech signal are varied continuously. So we have to make compensation input signal for frequency-gain loss of the impaired, specially in the frequency band which includes much information. The input sigaal is divided into short time block and spectrum within the block is calculated. The frequency-gain characteristic is determined using the calculated spectrum. The number of frequency band and the target gain which will be added input signal are estimated. The input signal within the block is processed by a single digital filter with the calculated frequency-gain characteristics. From the results of monosyllabic speech tests to evaluate the performance of the proposed algorithm, the scores of test were improved.

Auditory Recognition of Digit-in-Noise under Unaided and Aided Conditions in Moderate and Severe Sensorineural Hearing Loss

  • Background and Objectives: The speech-in-noise test is typically performed using an audiometer. The results of the digit-in-noise recognition (DIN) test may be influenced by the flat frequency response of free-field audiometry and frequency of the hearing aid fit based on fitting rationale. This study aims to investigate the DIN test in unaided and aided conditions. Subjects and Methods: Thirty four adults with moderate and severe sensorineural hearing loss (SNHL) participated in the study. The signal-to-noise ratio (SNR) for 50% of the DIN test was obtained in the following two conditions: 1) the unaided condition, performed using an audiometer in a free field; and 2) aided condition, performed using a hearing aid with an unvented individual earmold that was fitted based on NAL-NL2. Results: There was a statistically significant elevation in the mean SNR for the severe SNHL group in both test conditions when compared with that of the moderate SNHL group. In both groups, the SNR for the aided condition was significantly lower than that of the unaided condition. Conclusions: Speech recognition in hearing-impaired patients can be realized by fitting hearing aids based on evidence-based fitting rationale rather than by measuring it using free-field audiometry measurement that is utilized in a routine clinic setup.

Comparison of Steroid Treatment with and without Hyperbaric Oxygen Therapy for Idiopathic Sudden Sensorineural Hearing Loss

  • Background and Objectives: In this study, we compared the outcomes of patients with idiopathic sudden sensorineural hearing loss who underwent steroid treatment with or without hyperbaric oxygen (HBO) therapy and were followed-up in our clinic. Subjects and Methods: Patients were divided into two groups according to their treatment regimen. Steroid group received intravenous 1 mg/kg methylprednisolone which was due to be completed in 2-3 weeks with decreasing doses, and five doses of 0.5 mL intratympanic dexamethasone. Steroid+HBO group received the same steroid treatment with the addition of HBO therapy. The audiologic results of both treatment groups were compared after considering the patients' risk factors. Results: There was no significant difference between the steroid and Steroid+HBO groups in terms of hearing gain and degree of recovery, both at all degrees of hearing loss, and in severe and profound hearing loss. Hearing gain was similar when evaluated by audiogram type and admission time in both treatment groups. Conclusions: We found that the addition of HBO therapy to systemic plus intratympanic steroid treatment did not affect hearing gain at all degrees of hearing loss in this study. Furthermore, audiogram type and admission time did not affect hearing gain between the two groups.

Beneficial Effect of Ginseng Extract on Sudden Sensorineural Hearing Loss (돌발성난청에서 홍삼추출물의 추가적인 치료효과)

  • Sudden idiopathic sensorineural hearing loss is a disease that develops within several hours to several days. Its etiology has not yet been verified, but the disturbance of the circulation of blood in the inner ear, inner-ear hydrops, and viral infection are considered possible causes of the disease. This study was conducted to evaluate the effect of Panax ginseng extract, which is known to have a vasodilatory effect, on sudden sensorineural hearing loss. Sixty-nine patients suffering from sudden sensorineural hearing loss were admitted to Korea University Anam Hospital from March to December 2008. They were divided into the experimental (30 ears) and control (39 ears) groups. Ginseng extract (2700 mg/day, 4 weeks) was added to the therapeutic regimen in the experimental group. The effect of ginseng extract therapy was analyzed according to the factors relating to the prognosis. A considerable hearing improvement was documented in both groups (32.2 dB in the experimental group and 25.8 dB in the control group). However, there was little beneficial effect of ginseng extract on additional hearing improvement compared with control. The total recovery rate of the experimental group (80.0%) was better than that of the control group (58.9%), and the experimental group's high-tone hearing gain at 3 kHz (29.7 dB) was better than that of the control group (21.7 dB). The results of the study suggest that the effects of ginseng therapy tend to be superior to those of the conventional therapy, but the difference between the two is not statistically significant. The hearing gains tend to be in the higher frequencies and may be due to the promotion of cellular differentiation from the supporting cells.

Panax ginseng (Korea Red Ginseng) repairs diabetic sensorineural damage through promotion of the nerve growth factor pathway in diabetic zebrafish

  • Background: Diabetic sensorineural damage is a complication of the sensory neural system, resulting from long-term hyperglycemia. Red ginseng (RG) has shown efficacy for treatment of various diseases, including diabetes mellitus; however, there is little research about its benefit for treating sensorineural damage. Therefore, we aim to evaluate RG efficacy in alloxan-induced diabetic neuromast (AIDN) zebrafish. Methods: In this study, we developed and validated an AIDN zebrafish model. To assess RG effectiveness, we observed morphological changes in live neuromast zebrafish. Also, zebrafish has been observed to have an ultrastructure of hair-cell cilia under scanning electron microscopy. Thus, we recorded these physiological traits to assess hair cell function. Finally, we confirmed that RG promoted neuromast recovery via nerve growth factor signaling pathway markers. Results: First, we established an AIDN zebrafish model. Using this model, we showed via live neuromast imaging that RG fostered recovery of sensorineural damage. Damaged hair cell cilia were recovered in AIDN zebrafish. Furthermore, RG rescued damaged hair cell function through cell membrane ion balance. Conclusion: Our data suggest that RG potentially facilitates recovery in AIDN zebrafish, and its mechanism seems to be promotion of the nerve growth factor pathway through increased expression of topomyosin receptor kinase A, transient receptor potential channel vanilloid subfamily type 1, and mitogen-activated protein kinase phosphorylation.

  • 본 논문에서는 청각 장애인의 보다 향상된 보청 환경을 조성하고자 청각손실을 심리음향 모델을 적용하여 감음 신경성 난청을 보정하는 알고리즘을 제안한다. 제안한 알고리즘에서는 난청의 유형은 내이에서부터 중추 뇌에 걸친 감음계와 신경계의 장애에서 비롯되는 감음신경성 난청(sensorineural hearing loss)으로 주파수 영역상에서 MTH(minimum hearing threshold)가 균일하지 않게 상승하게되어 가청영역이 좁아지는 문제점을 해결하기 위한 방법으로 각각의 주파수 밴드마다 멀티밴드 압축 알고리즘을 적용하였다. 그러나 이 경우 각각의 주파수 밴드에 따른 서로 다른 가청 영역의 영향에 의한 변형된 스펙트럼 모양으로 인해 spectral contrast reduction과 변형된 마스킹 특성으로 인해 음성 변별력에 제한을 가하게 된다. 이것은 주변 주파수 성분들에 의한 마스킹 효과에 의한 것으로, 신호에 대한 난청인이 느끼는 지각 영역(perceptual domain)에서의 해석과 심리음향 모델 파라미터를 통한 보청기의 개발이 이루어져야 하며, 본 논문에서 그 알고리즘을 적용하였다.
