PURPOSE: The purpose of this study was to investigate the effect of balance training on different support surface (affected and non-affected sides) on the balance and gait function of chronic stroke patients. METHODS: The patients were randomly assigned to 1 of 4 groups. Group 1 received balance training on the stable surface, group 2 received balance training on the unstable surface, group 3 received balance training on different support surface (affected side: stable surface, non-affected side: unstable surface), and group 4 received balance training on different support surface (affected side: unstable, non-affected side: stable). Twelve sessions (30 min/d, 3 times/wk for 4 wk) were applied. There were assessed before and after the intervention with Balancia, functional reach test (FRT), lateral reach test (LRT), timed up-and-go (TUG), and 10-meter walking test (10MWT). RESULTS: After the training, all of the groups improved significantly than before training in Balancia, FRT, LRT, TUG, and 10MWT. There were significantly variable in sway distance, FRT, LRT, TUG, and 10MWT among the 4 groups. Post hoc analysis revealed that the group 3 had significantly higher results than other 3 groups in sway distance, and FRT, LRT, TUG, and 10MWT. CONCLUSION: Balance training on different support surface (affected side: stable surface, non-affected side: unstable surface) could facilitate a stronger beneficial effect on balance and walking ability than other balance trainings on different support surface in patients with stroke.
PURPOSE: The purpose of this study was to investigate the effects of sensory motor training using ball exercise on shoulder range of motion, pain, dysfunction, and quality of life in Breast Cancer Women after Mastectomy. METHODS: A total of 26 patients from D University Hospital in Daejeon were included and randomly allocated to two groups (n=13 per group). The two groups performed the exercise five times a week for 4 weeks. A sensory motor training group (SMTG) received general physical treatment for 10 minutes and sensorimotor training for 20 minutes. A control group (CG) received general physical therapy for 30 minutes. Shoulder pain (the Visual Analog Scale), range of motion, disability (the shoulder pain and disability index) and quality of life (the Functional Assessment Cancer Therapy-Breast instrument) were measured in both groups before and after 4-weeks intervention. RESULTS: A significant difference was found in joint range of motion, shoulder pain, functional disability level, and quality of life within the two groups (p <.05). In addition, changes in joint range of motion, pain, functional disability level, and quality of life after then intervention were significantly different between the two groups (p <.05). CONCLUSION: These results suggest that a sensory motor training program is feasible, safe, and suitable exercise for shoulder functions and quality of life in Breast Cancer Women after Mastectomy.
Purpose : The purpose of this study was to compare the effects of performing feedback vs repeative tasks on lower extremity motor function and falls efficacy in chronic hemiplegic patients. Methods : 40 hemiplegic patients took part in this study. The average age of the feedback-task group was 68.45 years and 68.20 in the repeative-task group. All subjects participated in the study for 8 weeks, doing exercises 3 per day per week. All participants were assessed by using the Berg balance scale (BBS), the lower-extremity subscale of the Fugel-Meyer assessment of sensorimotor impairment (FMLE), and the falls efficacy scale (FES). The data were analyzed using a paired t-test. Results : After 8 weeks of exercise training, the results of this study were: the BBS and FMLE of hemiplegic patients showed a feedback-task and repeative-task groups (p<0.05). The FES of hemiplegic patients also showed a significant difference between the quantitative-task and qualitative-task groups (p<0.05). Conclusion : We present findings suggesting that chronic hemiplegic patients could improve their standing balance ability better through a feedback-task exercise program, as opposed to a repeative-task exercise program.
Environmental Enrichment (EE) alone is not capable of enhancing the fine digit and the forelimb functions. Therefore, we applied modified constraint-induced movement therapy (mCIMT) under the influence of EE to assess its effect on promoting improved forelimb sensorimotor functions. Focal ischemic brain injury was produced in Sprague-Dawley rats (60 rats, $250{\pm}50$ g) through middle cerebral artery occlusion (MCAO). Before MCAO induction, all rats were trained in modified limb placing tests and reaching tasks for 1 week. Then they were randomly divided into three groups: Group I: application of standard environment (SE) after MCAO induction (n=20), Group II: application of EE after MCAO induction (n=20), Group III: MCAO+EE, mCIMT and task-oriented training that was initiated at 10th day after MCAO induction (n=20). We also applied mCIMT (between 9 AM and 5 PM/daily) which included restraining the forelimb ipsilateral to the lesion using the 'Jones & Schallert' method. We assessed the change of modified limb placing, single pellet reaching test and the immunoreactivity of BDNF by immunohistochemistry (pre, 1st, 5th, 10th and 20th day). Group I showed no improved outcome, whereas group II and III significantly improved on the use of the forelimb and the immunoreactivity. The qualitative analysis of the skilled reaching test, of group III showed the greatest improvement in the fine digit and the forelimb function. These results suggest that EE combined with mCIMT is more functional in promoting enhanced fine digit and forelimb functional movements.
Objectives : Acupuncture manipulation, a kind of sophisticated hand movements, has been considered a fundamental skill for acupuncture practice. In this study, we aimed to develop acupuncture manipulation education system(AMES) using visual feedback of acupuncture manipulation. We also investigated whether or not acupuncture practice-$na{\ddot{i}}ve$ students could enhance their acupuncture manipulation skills after AMES training. Methods : Using AMES and motion sensor, we visualized a time-series motion template(intended motion) and participant's own motion(actual motion) manipulating an acupuncture needle. Ten students were trained with complex lifting/thrusting techniques for 8 training trials. We compared the motion pattern error of the students between the first and the last trials. Results : In our pilot experiment, half of the participants showed significantly improved manipulation skills in complex lifting/thrusting techniques after training with AMES which is developed in this study, while the other half of the participants did not show significant improvements. Conclusions : The AMES could be useful in acupuncture-manipulation training for students. Our findings suggest that novice can improve sophisticated hand movement for acupuncture manipulation with sensorimotor learning using visual feedback.
PURPOSE: This study aimed to investigate the effects of pressure sense perception training (PSPT) on various surfaces on the somatosensory system, balance, and walking ability in chronic stroke patients. METHODS: Thirty patients with stroke participated in this study and were randomly assigned to one of three groups; group 1 received the general physical therapy and the PSPT on a stable surface, group 2 received the general physical therapy and the PSPT on an unstable surface, and group 3 received the general physical therapy alone. Participants in group 1 and group 2 underwent 30 min/session, 3 days per week, for 4 weeks. Pressure error (PE) was used to evaluate changes of proprioception. The Balancia, Functional reach test (FRT), and Timed Up and Go (TUG) were used to assess the balance ability, and the 10m Walking Test (10-MWT) was used to assess walking ability. RESULTS: Experimental groups (group 1 and group 2) showed significant differences in PE, FRT, TUG, and 10-MWT compared to the control group (p <0.05). Group 2 (PSPT on an unstable surface) was significantly different in PE, FRT, and 10-MWT from group 1 (p <0.05). No significant differences were observed for other measures. CONCLUSION: Pressure sense perception training on an unstable surface might be a significantly more effective method for improving somatosensory function, balance, and walking ability, than PSPT on a stable surface.
PURPOSE: A visual feedback method was proposed to induce brain stimulation in a stroke patient, and among them, there was a treatment using a mirror. On the other hand, mirror therapy focuses only on the functional changes in body movements, and analysis of neurophysiological mechanisms of brain activity is lacking. In addition, studies on evaluating the activity and response generated in specific brain regions during visual feedback training using mirrors are insufficient. METHODS: Fifteen healthy adults (male: 10, female: 5, Years: 23.33 ± 1.23), who were right-handed were recruited. By attaching the C3, Cz, and C4 channels in the sensorimotor cortex using an electroencephalogram, training was performed under the conditions without mirror-based visual feedback (No-condition) and with visual feedback (Tasks-condition). At this time, the immediate activity of the mu-rhythm in response to training was separated and evaluated. RESULTS: The tasks-condition of C3, Cz, and C4 channels activated the relative mu-rhythm rather than the no-condition, and all showed significant differences (p < .05). In addition, in all channels at the start time, the tasks-condition was more active than the no-condition (p < .05). The activity of the cortical response was higher in the tasks-condition than in the no-condition (p < .05). CONCLUSION: The mu-rhythm activity can be evaluated objectively when visual feedback using a mirror is applied to healthy subjects, and a basic analysis protocol is proposed.
본 연구는 체계적 고찰을 통하여 뇌성마비 아동에게 적용한 작업치료의 세부적인 치료 방법과 치료 효과를 알아보고자 하였다. 연구대상논문은 2003년 1월부터 2012년 12월까지 출판된 논문이었고, 전자데이터 베이스인 PubMed를 이용하여 검색하였으며, 사용한 검색어는 'occupational therapy' OR 'exercise therapy' OR 'activities of daily living' OR 'splints' OR 'self-help devices' OR 'early intervention(education)' OR 'parents/education' OR 'parents/psychology' OR 'professional family relations' OR 'play and playthings' OR 'upper extremity' AND 'cerebral palsy'이었다. 분석대상논문은 총 18개이었다. 뇌성마비 아동에게 적용한 작업치료 방법은 보조기 26.2%, 부모교육 26.2%, 환경조정 9.6%, 감각운동기능 훈련 19.0%, 일상생활활동 19.0%로 나타났다. 뇌성마비 아동에게 적용한 작업치료의 효과는 보조기 40.0%, 부모교육 42.9%, 환경조정 40.0%에서 통계학적으로 유의하였다. 본 연구의 결과는 뇌성마비 아동의 작업치료에 있어서 근거를 마련하는데 중요한 자료가 될 것이다.
Growth-associated protein (GAP-43)은 손상된 신경 재생 시 axonal growth를 확인할 수 있는 가장 널리 알려진 단백질이다. 본 연구의 목적은 좌골신경 손상을 준 쥐에 저강도 운동 수행 후 좌골신경에서 발현되는 GAP-43 단백질의 양을 정량적으로 분석하고, 손상된 부분으로부터 신경섬유의 재생 정도를 조직학적으로 분석함으로써 운동이 말초신경 손상을 가한 쥐에 미치는 영향을 분석하는 것이다. CAP-43의 정량적인 분석은 western blotting을 통해 확인하였고, 좌골신경의 조직학적인 분석은 immunofluorescence staining과 anterograde로 DiI 주입, 척수 내의 motor neuron은 retrograde로 DiI를 주입 한 후 형광현미경에서 관찰하였다. 본 연구에서는 좌골신경이 손상된 쥐에게 운동을 수행시킴으로써 CAP피3단백질이 비운동 그룹보다 더 많은 양이 발현되고, 손상된 부위에서 재생하는 신경섬유의 길이와 양이 더 많은 것을 확인할 수 있었다. 또한 운동을 수행한 그룹에서 DiI에 염색된 motor neuron의 수가 더 많이 증가하는 것을 관찰하였다. 따라서 이러한 결과들은 운동이 좌골신경이 손상된 쥐의 재생을 촉진시킬 것으로 생각된다.
휴전국인 우리나라는 의무복무제도를 시행하고 있다. 이에 취업 및 진학을 한 20대 초반의 사회 초년생은 군입대 문제로 많은 고민과 큰 걱정을 하고 있다. 이에 군에 대한 부정적인 인식과 군 생활 적응은 사회가 같이 풀어야 할 문제이다. 따라서 방송사 등 문제 해결을 위해 다각적으로 노력하고 있으나 역부족이다. 실제 군 입대 후 스트레스 및 불안정 심리상태의 장병 비중이 높다. 따라서 본 연구는 군 생활에서 일상적인 방법을 통해 스트레스, 불안감을 감소시키고, 집중력을 향상 시키고자 실행하였다. 주의 집중력 증가 및 스트레스, 불안감의 변화를 측정하기 위해 SMR파 대역의 주파수를 직접 노출시킨 실험군과 그렇지 않은 대조군으로 실험을 진행하였다. 실험군과 대조군 대한 대응표본 t-검정 결과는 신뢰수준 95%에서 t=2.487, p=0.042로 유의차가 인정되었다. 즉, 가상사격영상에서 특정 주파수를 제시한 실험군의 피실험자들이 스트레스 및 불안정 심리의 완화효과가 발생하였다. 본 실험의 결과를 활용하면 젊은 장병들의 군 생활 적응에 효과가 높은 것으로 사료된다. 추후 연구에서는 심전도와 뇌파의 상관관계의 확인이 필요하다.
