Knowledge Structure of Chronic Obstructive Pulmonary Disease Health Information on Health-Related Websites and Patients' Needs in the Literature Using Text Network Analysis (웹사이트에 제공된 만성폐쇄성폐질환 건강정보와 연구문헌에 나타난 환자의 건강정보 요구의 지식구조: 텍스트 네트워크 분석 활용)

  • Purpose: The purpose of this study was to identify the knowledge structure of health information (HI) for chronic obstructive pulmonary disease (COPD). Methods: Keywords or meaningful morphemes from HI presented on five health-related websites (HRWs) of one national HI institute and four hospitals, as well as HI needs among patients presented in nine literature, were reviewed, refined, and analyzed using text network analysis and their co-occurrence matrix was generated. Two networks of 61 and 35 keywords, respectively, were analyzed for degree, closeness, and betweenness centrality, as well as betweenness community analysis. Results: The most common keywords pertaining to HI on HRWs were lung, inhaler, smoking, dyspnea, and infection, focusing COPD treatment. In contrast, HI needs among patients were lung, medication, support, symptom, and smoking cessation, expanding to disease management. Two common sub-topic groups in HI on HRWs were COPD overview and medication administration, whereas three common sub-topic groups in HI needs among patients in the literature were COPD overview, self-management, and emotional management. Conclusion: The knowledge structure of HI on HRWs is medically oriented, while patients need supportive information. Thus, the support system for self-management and emotional management on HRWs must be informed according to the structure of patients' needs for HI. Healthcare providers should consider presenting COPD patient-centered information on HRWs.

Patient Safety Program and Safety Culture (투약간호 중심의 환자안전 프로그램 개발 및 평가)

  • Purpose: To determine the impact of the Patient Safety Program on the safety culture of nursing department. Methods: Patient Safety Program focused on medication safety was launched by QI team and patient safety committee. Patient Safety Program was composed of the establishment of improved reporting system as s way to learn from error, 'Patient Safety Guard' movement, and continuous education for medication safety. With one group pretest-posttest design, nurses' perception of the safety culture were measured with self-administered questionnaire. Subjects were all nurses and managers in nursing department of a tertiary teaching hospital in Seoul. Collected data from survey was statistically analyzed using t-test. Results: Patient Safety Program had been continued for 20 months in participation of all nurses and managers. Safety culture was improved (pretest=2.84, posttest=2.90, p<.001; 4 point scale). Conclusions: This study indicates that there has been a statistically significant increase in the nurses' perception of safety culture. These findings suggest that Patient Safety Program had made great contribution toward system wide safety culture in the hospital. To improve safety culture, leadership supports and flexibility to apply tailored interventions to the hospital were required necessarily.

How Does Body-Shape Perception Affect the Weight Control Practices?: 2012 Korea National Health and Nutrition Examination Survey (주관적 체형인식이 체중조절방법 선택에 미치는 영향: 2012년 국민건강영양조사 자료를 이용하여)

  • Objectives: This study was to investigate how body-shape perception could influence to weight control practice both in normal and obese group. Methods: We used 2012 Korea National Health and Nutrition Examination Survey to analysis 1) weight control practices of population; 2) consistency between body-shape perception and body mass index; 3) comparison weight control practices between normal group and body mass index (BMI) obese group in perceptional obese group; 4) odds ratio of BMI obese group using herbal drugs for weight control practice in perceptional obese group. Results: We found that study population tends to choose exercise, dietary restriction, meal skip, health functional food, one-food, drug, herbal drug, fasting and self-medication in order of frequency to control weight. The agreement between body-shape perception and BMI within obese group was approximately 64% with 0.40 of Cohen's Kappa coefficient, ranging from 0.384 to 0.423. Within perceptional obese group, choosing each weight control practice methods ratios between normal BMI group and obese BMI group were not significantly different. Within perceptional obese group, obese BMI group showed significant odds ratio (2.58, 95% confidence intervals, 1.38~4.85) than normal BMI group in choosing herbal medication for weight loss when adjusting other variables. Conclusions: We concluded that body-shape perception might be an important factor for choosing weight control program, and roles of Korean medical doctors thought to be enhanced for using herbal medication for weight loss.

A Study on the Priorities in the Roles of Community Pharmacists in Aged Society Using AHP (AHP를 이용한 고령사회 약국약사의 역할에 대한 우선순위 연구)

  • This study aimed to identify priorities in community pharmacist's roles in aged society using Analytic Hierarchy Process. We developed a survey questionnaire for AHP, which consisted of 19 pharmacist's roles selected from the FIP/WHO guidelines. The AHP survey questionnaire was self-administered by 127 pharmacists. The results showed that pharmacists put medication therapy management at the highest priority, preparation, dispense, distribution of medication at the second priority, and efficient healthcare system at the lowest priority. Among the 19 roles, new roles such as monitoring of medication therapy results, team-based health care, and transitions of care obtained relatively high priority.

Case study of Oriental Medicine Treatment with Mae-sun therapy of the spinal cord injury due to lumbar burst fracture (방출성 요추 골절로 인한 척수손상 환자에 대해 매선요법을 가미한 한방치료를 시행한 치험례)

  • Objective : Traumatic lumbar burst fracture causes significant spinal cord injury. This report is intended to estimate the efficacy using oriental treatment on a patient with lumbar burst fracture and spinal cord injury. Methods : From 21th December, 2009 to 5th February, 2010, 1 female inpatient diagnosed with lumbar burst fracture and spinal cord injury was treated with general oriental medicine therapy : mae-sun therapy ; acupuncture ; moxibustion ; pharmacopuncture ; physical therapy and herbal medication. TUG, SCIMII and VAS were used for evaluation of gait disturbance and pain in both feet. Measurement of self voiding amount and remaining amount through CIC was used for evaluation of neurogenic vesical dysfunction. Results : The patient showed a certain degree of improvement in gait disturbance, pain in both feet and neurogenic vesical dysfunction through above evaluation methods. Conclusion : Oriental treatments such as mae-sun therapy, acupuncture and moxibustion therapy, pharmacopuncture therapy and herbal medication can be effective for spinal cord injury due to traumatic lumbar burst fracture.

Case Report on Churg-Strauss Syndrome (Churg Strauss 증후군 중풍 환자의 치험 1례)

  • There are few case reports on the treatment of churg strauss syndrome by oriental medicine. We experienced one case of a female patient, 47 ages, who had symptoms; Rt. hemiplegia, aphasia, M/S dull, Rt. facial palsy. We used acupuncture, physical therapy, herbal medication and taping therapy. After 6 months of Acupuncture therapy, Herbal medication, physical therapy and taping therapy, a remarkable improvement was made for cerebral infarction patient who accompany churg strauss syndrome. The patient could take self-ambulation after treatment and many symptoms have improved. Cerebral infarction patient who accompany churg strauss syndrome is not common ,but in case the treatment by korean traditional medicine, there was improvement in patient's state. We think that it need the further study and clinical practice for Churg Strauss Syndrome.

A study on the Medical records in the Gupyubang ("급유방(及幼方)" 에 기재된 의안(醫案)에 대한 연구(硏究))

  • Objectives : The Medical Records(醫案) are important materials in studying the developmental process of Korean Traditional Medicine. The purpose of this study was to investigate the medical records which were described in Gupyubang(及幼方), the first Korean book that specialized in pediatrics. Methods : 85 Medical records about medical traits of diagnosis and treatments in Gupyubang were analyzed and those were translated in Korean. Results : Medical records were analyzed as follows; The number of Male was 76 and the number of Female was 9. Prepubertal period was 16 and preschool period 14. Acute febrile convulsion was 6. Measles and blood symptom was 5. Ring worm pain, vomiting & diarrhea, colic pain was 4. Most of them were self treatment except one. 77 cases were improved and 6 cases were dead or worse. The medical records used oral medication, external medication, acupuncture, moxibustion and surgical manners. Conclusion : This study showed that Gupyubang is a experiential book and alse the medical records in Gupyubang was usefulness and had practical value.

A Brief Replication Study Comparing Stimulants and Non-Stimulants for Attention-Deficit/Hyperactivity Disorder Treatment with a Focus on the Compliance, Efficacy, and Satisfaction

  • Objectives: The aim of this study was to compare the compliance, efficacy, and satisfaction associated with methylphenidate and atomoxetine for treating attention-deficit/hyperactivity disorder (ADHD). Methods: The subjects were 44 patients who met the Diagnostic and Statistical Manual of Mental Disorder-5 diagnostic criteria for ADHD and were treated with methylphenidate or atomoxetine. The methylphenidate formulations included immediate release (IR), extended release (ER), and osmotic-controlled release oral delivery system (OROS). Patients and parents reported the average number of days per week the medication was taken. Efficacy was assessed using the ADHD Rating Scale. Satisfaction with medication scale (SAMS)-parent report form and SAMS-self-report form were used to evaluate parents' and patients' satisfaction, respectively. Results: Patients and parents were more satisfied with methylphenidate than with atomoxetine. There were no significant differences in the compliance with and efficacy of methylphenidate and atomoxetine. Compliance with methylphenidate IR and ER was markedly lower than that with OROS methylphenidate or atomoxetine. Conclusion: Methylphenidate OROS formulation can be considered a suitable option given its high rates of compliance, satisfaction, and efficacy.

Factors Associated with Self-Assessment of Halitosis in Adult (일부 성인에서 자가 구취 인식도와의 관련요인 분석)

  • The aim of this study was to determine the prevalence of self-assessment halitosis and to offer the basic information that establishes a strategy of effective health promotion and to enhance the quality of life. This study performed a questionnaire survey. The questionnaire was filled out by the 362 adults, who lived in Incheon Metropolitan City from May to July, 2010. The data were analyzed using a Chi-square test in the SPSS version 12.0 program. The prevalence rate of self-assessment halitosis was 24.3%. Hypertension, diabetes, medication and inadequate oral hygiene practice were significantly associated with self-assessment halitosis. Other factors of self-assessment halitosis were older age and occupation. As a result, dental professionals have to investigate a systemic disease history thoroughly. And they have to educate regular checkup of oral health and to perform a continuous feedback. The self-assessment halitosis has a negative influence on mental health and social life, so dental professionals must take into consideration about the halitosis.

The Relationship of Self Efficacy and Social Support to the Psychosocial Adjustment in People with Epilepsy (간질환자의 사회심리적 적응과 자기효능.사회적 지지와의 관계 연구)

  • The main purpose of this study was to identify the relationship of self efficacy and social support to the psychosocial adjustment in people with epilepsy. Data were collected from October 1 to October 15, 1999 from 101 people with epilepsy who were being treated regularly at one of the university hospitals located in Seoul. The research instruments were a questionnaire to gather demographic and disease-specific data, the Epilepsy Psycho- Social Effects Scale developed by Chaplin et al(1990), the Epilepsy Self Efficacy Scale developed by DiIorio et al(1992a) and translated by Park(1999), the Norbeck Social Support Questionnaire developed by Norbeck et al(1981) and translated by Oh(1985). Data were analyzed using the SPSS program. The results are as follow : 1. Of the 14 psychosocial adjustment areas, 75 of 101 subjects experienced problems in ten or more areas and 28 in all 14 areas. The severity of the psychosocial adjustment problem was moderate or more in six areas. 2. The score for self efficacy was an average of 1103.86 out of a possible 1800, for social support 117.57 for total functional out of a possible 720, and 48.21 for total network out of a possible 264. There were an average of five people on the network. The main network people were parents, brothers and sisters, spouse, friends. 3. Of the 14 psychosocial adjustment areas, six areas correlated with self efficacy and 'problems with taking medication' area had a negative correlation with social support. In conclusion, people with epilepsy have various problems in psychosocial adjustment. Nursing interventions using self efficacy should be developed to improve psychosocial adjustment in people with epilepsy. Also, instruments and interventions for regimen-specific supports which are suitable for epilepsy should be developed.
