Purpose : The purpose of this study was to investigate the variations in gait parameters according to arm swing use in post stroke hemiparesis. Methods : Sixteen patients participated in this study and walked at self-selected speeds on a Rs-scan systems. The were randomly assigned conditions: self-selected arm swing, constraint arm swing, emphasis arm swing. Results : In the comparison of parameters in each trial, both affected step length, non affected step length, affected stride length, non affected stride length, affected single support time, and non affected single support time were significantly increased and double support time was significantly decrease in emphasis arm swing when compared with both self-selected arm swing and constraint arm swing(p<.05). However, Asymmetrical ratio was significantly increased in both emphasis arm swing and self-selected arm swing when compared constraint arm swing(p<.05). Conclusion : Therefore, In this study, gait rehabilitation of patients with hemiplegia depending on what you need to apply the arm swing is considered.

Objective : Posttraumatic cerebral infarction (CI) is a well-known complication of traumatic brain injury (TBI). However, the causation and apportionment of trauma in patients with CI after TBI is not easy. There is a scoring method, so-called trauma apportionment score (TAS) for CI, consisted with the age, the interval, and the severity of the TBI. We evaluated the reliability of this score. Methods : We selected two typical cases of traumatic CI. We also selected consecutive 50 patients due to spontaneous CI. We calculated TAS in both patients with traumatic and spontaneous CI. To enhance the reliability, we revised TAS (rTAS) adding three more items, such as systemic illness, bad health habits, and doctor's opinion. We also calculated rTAS in the same patients. Results : Even in 50 patients with spontaneous CI, the TAS was 4 in 44 patients, and 5 in 6 patients. TAS could not assess the apportionment of trauma efficiently. We recalculated the rTAS in the same patients. The rTAS was not more than 11 in more than 70% of the spontaneous CI. Compared to TAS, rTAS definitely enhanced the discriminating ability. However, there were still significant overlapping areas. Conclusion : TAS alone is insufficient to differentiate the cause or apportionment of trauma in some obscure cases of CI. Although the rTAS may enhance the reliability, it also should be used with cautions.
Background and Purpose : Relationship between 17-KS.17-OHCS in 24hrs urine and Deficiency Syndrome of the Kidneys had been examined, but the study about 17-KS.17-OHCS in stroke patients was rare6'. In this study, we aimed to investigate the usefulness of 24hrs urine 17-KS.17-OHCS in stroke patients as an index for the Differentiation of Deficiency Syndrome of the Kidneys. Subjects : 66 stroke patients(male : female =2 9 : 37) were selected, they were admitted in the hospital of oriental medicine, Kyunghee university(from November 1 st, 1998 to May 30th, 2000). Their age was over 65 years. The patients who had renal malfunction, hyperthyroidism, hypothyroidism were excluded and who took chlorpromazine, spironolactone, digoxin, reserpine, hormonal agent were also excluded. Methods : After we selected the patients, we investigated the Differentiation of Syndrome by use of Diagnostic Paper and examined the level of 17-KS.17-OHCS in 24hrs urine. We compared Deficiency Syndrome with non-Deficiency Syndrome of the Kidneys using of 17-KS.17-OHCS in 24hrs urine. Results : 1. Stroke did not affect 17-KS.17-OHCS excretion in 24hrs urine. 2. In 24hrs urine, 17-KS of male stroke patients and 17-OHCS of female stroke patients were lower in patients diagnosed as a Deficiency Syndrome than non-Deficiency Syndrome of the Kidneys(p<0.05). 3. Among Deficiency Syndrome of Yin, Yang, Yang and Yin of the Kidneys group, there was no differentiation of 17-KS.17-OHCS in 24hrs urine(p>0.05).
The purpose of this study was to assess the frequently consumed dish consumption frequencies of continuous ambulatory peritoneal dialysis(CAPD) patients with osteopenia and develop recipes to improve bone mineral density of CAPD. The subjects were 96 CAPD patients with osteopenia(male 39, female 57) (osteopenia group) and 45 CAPD patients with normal BMD(male 24, female 21), matched with key variables(normal group). Fifty dishes(foods) that most frequently consumed were determined and food consumption frequency for each dish(food) for two groups were compared. Osteopenia group showed lower consumption frequency for ice-cream but higher frequency in apple. Of the 50 most frequently consumed dishes(foods), 20 dishes assessed as safe and recommendable for CAPD patients with osteopenia based on the contents of protein and mineral were selected : white boiled rice, white gruel, beef soup, steamed cabbage, roasted dried laver, fried egg, roasted bean-curd, cooked and seasoned bean sprouts, corn-starch jelly, cheese, ice-cream, orange juice, apple, grape, peach, peanut, raw lettuce, raw cucumber, and injulmi rice cake. wenty eight new dishes with modified recipes were developed for CAPD patients. Protein and mineral contents were analyzed for frequently consumed 17 dishes, assessed as modification of recipes are needed. The recipes were modified to decrease P, Na and K contents and to increase protein and Ca contents. Twenty dishes(foods) selected as having reasonable protein and mineral contents ratio or 28 newly developed dishes modified with protein and mineral contents or ratio would be helpful for nutrition education or counseling for CAPD patients with osteopenia. Dishes(foods) suggested in this study would also be useful for all CAPD patients for preventing osteoporosis.
This study was undertaken to document relations between facial skeletal pattern and dental occlusion. The data in .this study were collected from pretreatment cephalometric radiographs and study models of patients' records present in the files of Orthodontic Department, Seoul National University Hospital. Patients were selected on the basis of a mandibular plane-sella nasion angle equal to or greater than $38^{\circ}$ (high SN-MP angle) or equal to or less than $26^{\circ}$ (low SN-MP angle). Patients in the mixed dentition and with missing permanent teeth were excluded for ease of assessing tooth size / arch circumference relationships and then 30 high SN-MP and 11 low SN-MP patients were selected among them. The mean age of these two groups of patients was high SN-MP, $12.8{\pm}1.23$ years and low SN-MP, $13.0{\pm}1.48$ years. The following conclusions were obtained. 1. In the maxilla and mandible the mean tooth size of high SN-MP patients was nearlly identical to the low SN-MP patients. 2. The mean maxillary arch circumference was increased in low SN-MP group compared with high SN-MP group and a smilar, but smaller, mean increase was present in mandible. 3. The difference between the mean maxillary circumference required and the mean maillary circumference present ranged from -4.8mm in the high SN-MP group to -1.3mm in the low SN-MP group. A small range of means occurred in the mandible (high SN-MP: -4.0mm to low SN-MP: -1.8mm). 4. In the maxilla and mandible the mean arch length was nearly identical in the high and low SN-MP groups. 5. The mean incisor inclination was increased as the SN-MP angle decreased in the maxilla and mandible. 6. The men distance of the maxillary first molar from anterior border of the pterygomaxillary fissure was nearly similar between high and low groups. 7. The mean mandibular intermolar width was increased from high SN-MP to low SN-MP patients.

In acute leukemia patients, several successful methods of expression profiling have been used for various purposes, i.e., to identify new disease class, to select a therapeutic target, or to predict chemo-sensitivity and clinical outcome. In the present study, we tested the peripheral blood of 47 acute leukemia patients in an attempt to identify differentially expressed genes in AML and ALL using a Korean-made 10K oligo-nucleotide microarray. Methods: Total RNA was prepared from peripheral blood and amplified for microarray experimentation. SAM (significant analysis of microarray) and PAM (prediction analysis of microarray) were used to select significant genes. The selected genes were tested for in a test group, independently of the training group. Results: We identified 345 differentially expressed genes that differentiated AML and ALL patients (FWER<0.05). Genes were selected using the training group (n=35) and tested for in the test group (n=12). Both training group and test group discriminated AML and ALL patients accurately. Genes that showed relatively high expression in AML patients were deoxynucleotidyl transferase, pre-B lymphocyte gene 3, B-cell linker, CD9 antigen, lymphoid enhancer-binding factor 1, CD79B antigen, and early B-cell factor. Genes highly expressed in ALL patients were annexin A 1, amyloid beta (A4) precursor protein, amyloid beta (A4) precursor-like protein 2, cathepsin C, lysozyme (renal amyloidosis), myeloperoxidase, and hematopoietic prostaglandin D2 synthase. Conclusion: This study provided genome wide molecular signatures of Korean acute leukemia patients, which clearly identify AML and ALL. Given with other reported signatures, these molecular signatures provide a means of achieving a molecular diagnosis in Korean acute leukemia patents.
There are many causes of oral mucosal diseases, so accordingly, there are various treatments available. The most commonly used agents include adrenocortical hormones, antifungals, antivirals, antibacterials, and immunosuppressants. However, it must also be noted that improving oral hygiene and nutrition, and reducing stress are effective in symptom relief. Furthermore, patients with existing diseases of the oral mucosa should avoid behavior that may cause an increase in pain. Unfortunately, many patients are unaware of the activities that may lead to increased pain and therefore do not avoid these activities. The aim of this study was to investigate and analyze the behavior of patients with oral mucosal disease with regard to activities that led to increase pain. This cross-sectional study was performed on a sample of patients with oral mucosal disease selected from the Oral Medicine Clinic of the Pusan National Hospital during March to August 2013. These patients were randomly selected. From a total of 479 patients, 116 patients with mucosal disease were selected and 73 fully completed questionnaires were included in the analysis. Data were collected by using self-completed questionnaires. The results were as follows: Mean score of Question 13 (Not smoking) is $2.47{\pm}1.11$. Mean score of Question 11 (Not drinking alcohol or not using mouthwash containing alcohol) is $2.22{\pm}1.15$. The other questions resulted in scores lower than 1.5. The answers to the questions were scored according to the following assigned numerical values: not keeping = score of 0; little keeping = score of 1; often keeping = score of 2; always keeping = score of 3. In conclusion, patients with oral mucosal diseases unknowingly engage in activities that result in an increase in pain. Therefore, they need to be educated about how to behave to protect oral mucosal lesion.
This article presents a concept analysis of the gratitude experience of diseased patients. The Rodgers' Evolutionary Method was used for conducting the analysis. A search of CINAHL, MEDLINE, PsycARTICLES, and Springer databases was conducted using "gratitude or appreciation or thanks" and "patient or illness" as a key word, 22 final articles were selected. Three critical attributes of gratitude in patients were identified: positive emotions, acceptance of the current status, and a driving force to plant the will of life. In addition, two antecedents of gratitude in patients were identified: interactions with people or the environment, and the perception of a favorable stimuli or help. Two consequences of gratitude in patients were identified: an increased compliance in implemented treatment, and an enhancement of trust relationship. The concept analysis describes diseased patients' gratitude. This paper will become the basis for future clinical research related to diseased patients' gratitude.
We selected 100 patients necessitating the surgical or non-surgical treatment among the patients who visited our hospital. We administered the newly developed non-steroid, PROFENID capsule (25mg per capsule), to the selected patients Via per OS and obtained the following results. 1) Cases of 56% at about 15 minutes after the administration of Profenid capsules, 35% at about 30 minutes after the administration of Profenid capsules, 35% at about 30 minutes and 6% at 60 minutes or more presented the initial effects of that drug respectively. 2) In the patients with the comparatively heavy swelling, the appearances of the effect of that drug were relatively delayed. 3) Sexual difference was not seen in that drug effect. 4) Untoward effects of that drug seemed to be not found in this case study.

Objective : The sacroiliac joint complex is often related with functionally incapacitating pain in old aged people. The purpose of this study is to delineate the investigation strategies and to determine the long-term effect of radiofrequency [RF] neurotomies for pain arising from sacroiliac Joint dysfunction[SIJD]. Methods : Sixteen patients were diagnosed as having chronic pain from SIJD by comparative controlled blocks on L5 dorsal rami, sacroiliac Joints and deep interosseous ligaments. After confirming the positive response [more than 50% of pain relief], sensory stimulation was applied to detect the 'pathological' branches. Subsequently, RF neurotomies were performed on the selected nerve branches. Surgical outcome was graded as successful, moderate improvement, and failure after a 6month follow-up period. Results : Stimulation intensity was 0.45V to elicit pain response in the L5 dorsal rami and lateral sacral branches. The number of RF-lesioned nerve branches was 6per patient. The average number of lesions for each branch was 1.3. Most commonly selected branches were L5 dorsal ramus [88%] and S2-upper division [88%]. Ten patients [63%] reported a successful outcome according to the outcome criteria after 6months of follow-up, and five patients [31%] reported complete relief [100%]. Five patients [31%] showed moderate improvements. One patient reported failure. Conclusion : RF neurotomy of lateral sacral branches is an excellent treatment modality for the pain due to SIJD, provided that comparative controlled block shows a positive response.
