Actigraphy-Based Assessment of Sleep Parameters in Intensive Care Unit Patients Receiving Respiratory Support Therapy (호흡지지요법을 적용 중인 중환자실 입원환자의 액티그래피 측정 수면특성)

  • Purpose : This study aimed to investigate sleep parameters and to identify differences according to respiratory support therapy, sedation, and pain medication in intensive care unit (ICU) patients. Methods : In this observational study, sleep parameters were measured using actigraphy. We observed 45 sleep events in 30 ICU patients receiving respiratory support therapy. We measured the sleep parameters, time, efficiency, and wakefulness after sleep onset (WASO). The differences in sleep parameters according to the respiratory support therapy were analyzed using the Mann-Whitney U test. Results : The average daily sleep time of the participants was 776.66±276.71 minutes, of which more than 60% accounted for daytime sleep. During night sleep, the duration of WASO was 156.93±107.91 minutes, and the frequency of WASO was 26.02±25.82 times. The high flow nasal cannula (HFNC) group had a significantly shorter night sleep time (𝑥2=7.86, p =.049), a greater number of WASO (𝑥2=5.69, p =.128), and a longer WASO duration (𝑥2=8.75, p =.033) than groups of other respiratory therapies. Conclusion : ICU patients on respiratory support therapy experienced sleep disturbances such as disrupted circadian rhythm and sleep fragmentation. Among respiratory support regimens, HFNC was associated with poor sleep parameters, which appears to be associated with the insufficient use of analgesics. The results of this study warrant the development of interventions that can improve sleep in ICU patients receiving respiratory support, including HFNC.

The Effect of Hand Acupuncture Therapy and Moxibustion Heat Therapy on Dysmenorrhea Women (수지침과 뜸요법이 월경곤란증에 미치는 효과)

  • In order to find out whether Hand Acupuncture Therapy and Moxibustion Heat Therapy is effective to relieve dysmenorrhea syndrome, we performed a Quasi-experiment on a group of fourty females. The experiment was carried out during the period from April 20 to August 20, 1999. The group was divided into two sub-groups called "a Hand Acupuncture Therapy sub-group" and "a Moxibustion Heat Therapy sub-group" consisting of 20 females respectively. Hand Acupuncture Therapy and Moxibustion Heat Therapy were performed four times a week. Especially, in case of Moxibustion Heat Therapy, subjects were treated twice a times. The data analyzed by an SAS program. The results are as follows : 1) Hand Acupuncture Therapy and Moxibustion Heat Therapy are very effective to relieve dysmenorrhea syndrome. This study shows that in case of Hand Acupuncture Therapy sub-group, supposing that mean score of Menorrhalgia before treatment was 7.85, it became low to 4.50 when. subjects suffered the first menstruation and it was 2.50 at the second menstruation, and 1.60 at the third menstruation. In the mean score of Moxibustion Heat Therapy sub-group, Menorrhalgia before treatment was 7.85, it was 5.90 at the first menstruation(p<.05), and 3.00 at the second, and 1.85 at the third menstruation. 2) Among Hand Acupuncture Therapy subgroup, 9 subjects could hardly be relieved from the pain of dysmenorrhea at the first menstruation. So, they were treated additionally with the method of tonification and sedation of abdominal diagnosis of three constitution and became completely relieved at second menstruation. Meanwhile, 7 subjects among Moxibustion Heat Therapy also faced the same situation. So they were treated with Moxibustion on dorsum of hand and got effectiveness at the third menstruation after taking therapy. 3) Odinary dysmenorrhea syndrome are constipation, dizziness, anorexia, abdominal pain, lumbago, breast engorgement, abdominal distention, dysconcentration, nervousness, diarrhea, nausea & vomitting, apathy, restlessness, fatigue, aggression, leg pain, edema. After taking therapy, all of subjects were relieved from these dysmenorrhea syndrome at third menstruation. 4) All subjects were classified into five types of physical constitution with abdominal diagnosis of three constitution as follows: 18 cases of left kidney right yang excess, 8 cases of left yin right yang excess, 7 cases of left right kidney excess, 5 cases of left right yang excess, 2 cases of left yang right kidney excess.

Study of Developmet of External Medicine Based on the Result of Prior Study and Folk Remedy Using Scolopendra subspinipes mutilans as a Single Medicine in Korea and China (한국과 중국에서의 오공을 단방으로 사용한 민간요법과 선행연구결과를 기반으로 한 외치 한약제제 개발에 대한 고찰)

  • Scolpendra subspinipes (centipede) is pungent in flavor, activates the channels to stop pain, and, removes toxins and resolves masses. So it has been used as a folk remedy. We researched folk remedies which used centipede as a single medicine and results of its prior clinical and experimental study, and figured out that centipedes are effective on ulcerative disease, herpes zoster, clavus, insect bite, inflammatory disease, furunculus, pain disease by external therapy, and oral administration is effective on anti-cancer such as hepatoma and uterine cancer, cerebrovascular disease, inhibition of atherosclerosis, sedation of central nervous system, antipyretic action, and anti-inflammation, and centipede Herb-Acupuncture is effective on inflammatory disease and pain disease. Centipede preparation materials for external therapy were sesame oil, perilla oil, vinegar, salt, and, burnt alum. Fumigation of centipede treated paronychia, pertussis, and hemorrhoids. For the external therapy, the way of centipede powder mixing with other materials has been used. It is needed that we use dusted powder to make skin absorb its ingredients more easily, and we study intensely minor ingredients to make centipede components absorbed more effectively and do pharmacological action. Also, fumigation, a kind of another external therapy, is considered to be effective treatment of upper respiratory infection like pertussis, so it needs an in-depth study on centipede fumigation.

Acoustic Variations in Epileptic Patients with Topiramate (간질 치료제 복용으로 인한 음성학적인 변화에 대한 연구)

  • Topiramate (TPM) is a new antiepileptic drug characterized by a clinical effective reduction in seizure frequency and it represents a useful drug effective in a wide range of epileptic patients. Known side effects are represented by weight loss, hypohidrosis, anorexia, sedation, nephrolithiasis, cognitive complaints and language disorders. This study is to examine acoustic characteristics of patients with TPM. 15 patients were assessed through a Computerized Speech Lab (CSL) applied before the beginning of therapy with TPM and 3 months after medication had been stabilized. Tests had been chosen to assess voice onset time (VOT), total duration (TD), vowel formants, loudness, pitch, speaking rate, and articulation patterns. We compared the data from patients and healthy volunteers. The statistical analysis of the results did not show changes in acoustic tests, except for TD which was increased. The increase of the TD is evaluated as a deterioration of fluency. Our results suggest that patients with TPM did not experience acoustic speech changes except that fluency was declined. Unlike previous studies, the medication of TPM has nothing to do with speech problems in patients with epilepsy.

Role of colonoscopy in the diagnosis and treatment of pediatric lower gastrointestinal disorders

  • The safety and effectiveness of colonoscopy in the investigation of lower gastrointestinal tract pathology in children has been established for more than 2 decades in Korea. The skill and experience have since advanced to the point that both diagnostic and therapeutic colonoscopy are now routinely performed by most pediatric gastroenterologists. Pediatric colonoscopy differs significantly from its adult parallels in nearly every aspect including patient and parent management and preparation, selection criteria for sedation and general anesthetic, bowel preparation, expected diagnoses, instrument selection, imperative for terminal ileal intubation, and requirement for biopsies from macroscopically normal mucosa. Investigation of inflammatory bowel disease, whether for diagnosis or follow-up evaluation, and suspected colonic polyps are the most common indication for pediatric colonoscopy. The child who presents with signs and symptoms of lower gastrointestinal disorder should undergo colonoscopy with biopsy to make the diagnosis, as well as to help determine the appropriate therapy. This review introduces practical information on pediatric colonoscopy, the author's experiences, and the role of colonoscopic examination in the diagnosis and treatment of pediatric lower gastrointestinal disorders.

Maxillary Sinusitis by Staphylococcus aureus Infection in a Thoroughbred Gelding: Case Report

  • A 4-year-old gelding Thoroughbred racehorse, which had been undergoing antibiotic therapy at a local veterinary clinic, was referred to the KRA veterinary center with a 20-day history of continuous right nasal discharge. Patient's history, endoscopic examination, and radiographic examination revealed primary maxillary sinusitis. Under sedation, surgical intervention was performed to collect samples and remove the accumulated mucopurulent exudate in the sinus. Swab samples were collected from the sinus during surgery for cytology and antimicrobial susceptibility testing. Only one type of bacteria was cultured, and molecular analyses of 16S ribosomal RNA gene sequences identified it as Staphylococcus aureus (S. aureus). The isolate was resistant to multiple antibiotics, which are frequently used in equine practice. Trimethoprim-sulfamethoxazole was chosen based on antibiotic susceptibility test, trephination, and sinus lavage using saline were applied to treat bacterial sinusitis. The clinical signs improved after 1 month and the patient resumed training. This report describes S. aureus isolated from bacterial maxillary sinusitis in a horse and its antibiotic susceptibility.

Efficacy of Epidural Metoclopramide in Reducing Nausea and Vomiting Associated with Postoperative Epidural Morphine (술후 경막외 Morphine으로 인한 오심 및 구토에 대한 경막외 Metoclopramide의 효과)

  • Background: To date, there are no controlled studies assessing the effect of metoclopramide administered epidurally to prevent nausea and vomiting associated with epidural morphine for postoperative analgesia. This study was undertaken to determine the effectiveness of continuous epidural infusion of metoclopramide, combined with epidural morphine, in reducing nausea or vomiting associated with epidural morphine and minimizing the side effects of metoclopramide. Methods: Sixty patients undergoing elective gynecologic surgery were randomly assigned to one of two study groups. Patients received continuous epidural morphine infusion (6.0 mg/day) following a bolus loading dose of 3.0 mg (Group A), or epidural mixture of morphine (6.0 mg/day) plus metoclopramide (20 mg/day) following a bolus loading dose (morphine 3.0 mg, metoclopramide 10 mg)(Group B). For the first 24 postoperative hours, incidence of nausea or vomiting, need for antiemetic therapy, level of sedation, degree of pain and pruritus, and adverse effects associated with metoclopramide were evaluated. Result: Incidence of nausea or vomiting and number of patients who required antiemetic therapy were significantly less in Group B, than in Group A (P<0.05). There were no significant differences between groups with regard to adverse effects associated with metoclopramide such as sedation, extrapyramidal reaction and other side effects (P=NS). Conclusion: We conclude simultaneous titration of morphine and metoclopramide via epidural continuous infusion following epidural bolus injection of the mixture reduces nausea or vomiting associated with epidural morphine while preventing side effects of metoclopramide.

The Effects of Propofol and Thiopental Continuous Infusion on Serum Potassium Disturbances in Neurosurgical Patients

  • Objective : The potassium disturbance associated with thiopental continuous infusion in neurosurgical patients is well known. However, the effect of propofol continuous infusion on serum potassium levels has not been investigated extensively. Methods : We reviewed the medical records of 60 consecutive patients who received coma therapy or deep sedation for intracranial pressure control using either thiopental or propofol between January 2010 and January 2012. Results : The overall incidence of hypokalemia (K<3.5 mmol/L) was comparable between thiopental and propofol groups (89.2% vs. 82.6%). But, the incidence of moderate to severe hypokalemia (K<3.0 mmol/L) was significantly higher in thiopental group (51.4% vs. 13.0%, p=0.003). The lowest potassium level (2.9 mmol/L vs. 3.2 mmol/L, p=0.020) was lower in thiopental group. The patients in the thiopental group required greater potassium replacement than the propofol group patients (0.08 mmol/kg/h vs. 0.02 mmol/kg/h, p<0.001). On multivariate analysis, thiopental [odds ratio, 95% confidence interval, 7.31 (1.78-27.81); p=0.005] was associated with moderate to severe hypokalemia during continuous infusion. The incidence of rebound hyperkalemia (K>5.0 mmol/L, 32.4% vs. 4.3%, p=0.010) and the peak potassium concentration (4.8 mmol/L vs. 4.2 mmol/L, p=0.037) after the cessation of therapy were higher in thiopental group. On multivariate analysis, thiopental [8.82 (1.00-77.81); p=0.049] and duration of continuous infusion [1.02 (1.00-1.04); p=0.016] were associated with rebound hyperkalemia once therapy was discontinued. Conclusion : Propofol was less frequently associated with moderate to severe hypokalemia after induction and rebound hyperkalemia following the cessation of continuous infusion than thiopental.

A Study on the Blood-Letting Therapy in Elementary Questions (("황제내경소문(黃帝內經素問)" 중(中) 사혈(瀉血)에 관한 연구(硏究))

  • Blood-Letting Therapy is a rational and ecological medical treatment by which we can heal most of the diseases by removing the static blood which precipitates in the blood vessel and blocks the flowing of blood. And the static blood is the generic term for the injurious, bad, dead and precipitated blood which is blocked the capillary vessel. The Yellow Emperor's Canon of Internal Medicine says that "the patient is treated with drugs internally and stone acupuncture externally. "In the old texts, the blood-letting therapy is mentioned as blood-letting, network vessel pricking, bloodletting, pricking, and arousing pulses etc and it is noted down as the method of network vessel pricking in 'On the Application of Needles' of Spiritual Pivot. Nine-pricking therapy, twelve-pricking therapy and five-pricking therapy are recorded in the methods of network vessel pricking and among them, the method of squeezing blood after pricking the affected part is explained as the network vessel pricking. There are four methods of network vessel pricking, pricking, picking, cluster needling and scatter-pricking and they are fluidly applied to the various symptoms of diseases. In 'On Discriminative Treating for Patients of Different Regions' in Elementary Questions, Ki-baek emphasizes "most of the local people, there are black in skin and loose in striate, and their diseases are mostly of carbuncle kind. It is suitable to treat the disease with stone therapy to prick with stone, so the stone therapy is transmitted from the east. "And in 'On the Corresponding Relation Between the Eum and Yang of Man and All Things' in Elementary Questions, when the Emperor asked Ki-Back, he answered "sthenia means the sthenia of evil, and deficiency means the deficiency of healthy energy. When the blood is sthenic, the evil should be discharged by pricking when out letting the blood; deficiency of vital energy is the asthenia of channels and network vessels, so the energy should drain from the channel which is not deficient, to replenish. "And in this case we can use the methods of 'Breaking out the static bloods', 'driving out the static bloods', blood-letting'. With this we can infer that the blood-letting therapy is made use of the important medical treatments from the ancient times. Especially in referring to the principles of treatment in The Yellow Emperor's Canon of Internal Medicine, it mostly alluded to acupuncture therapies and only eleven times to medicinal treatments. This is to verify that the blood-letting therapy formed the foundation of the medical art. In Dong's Therapy of Acupuncture-Moxibustion and Bloodletting, Dong Kyeong-Chang gave emphasis on the points that there must be extravasated bloods without exception in the serious illnesses which is old, unnatural, accompanying acute pains and so we can revive our body‘s sprit by circulating 'gi' and static blood piled up in the network vessel, regulating the weakness and strength, and controling the disharmony of the internal organs. The blood-letting therapy has effect on the orifice in emergency, such as fore draining, freeing network vessels, harmonizing gi and blood, relieving pain, dispersing swelling and concretion, sedation, resolving toxin as well as strengthening the heart, relieving itching. So it has distinguished effect on all kinds of medical treatment to the modern people. But by the change of social customs and the confucianism of confucius - it is widely spread on the period of North and South Dynasties, 'Wi' and 'Jin' in china and the period of the Three States in korea - The blood-letting therapy which was regarded as the most important medicinal treatment withered rapidly. And Confucius accentuated the importance of our body and all its members, loyalty and filial piety and banned any damage of our body under no circumstances. As a result of it, the therapy of blood-letting had a rapid decrease and barely kept itself in existence in both countries. What is worse, at the period of Japanese colonial rule of korea and our nation's founding of early stage, it has been withered by the high-handed policy to change Oriental Medicine into modern medical science. So the therapy of blood-letting barely kept itself in existence in some Buddhist temples. Another case, it has handed down as a old-fashioned quick fix in folk remedies. But all kinds of the contamination of heavy metals and the misuses of antibiotics are widely spread nowadays, which increased diseases of adult people and incurable diseases as modern society unavoidably made its way into a highly industrial society. To make us more miserable, the western medical science - the antibiotics and surgical operation medical science - already reveals itself into a limit. The necessity of a new medical science which can give a security to the patients who are suffering from the diseases of adult people and the incurable diseases is especially come into the force nowadays. In view of the results after bibliographically studying on the blood-letting Therapy in Elementary Questions of the Yellow Emperor's Canon of Internal Medicine, the blood-letting therapy has acted for the important Oriental medicinal science and has been clarified the prominent effects on the diseases of adult people and the incurable diseases. So it is regarded as an appropriate thing that we lay out a determined theory of the blood-letting therapy and of course prevent the unwanted side effects from inappropriate medicinal treatments, and make full use of clinic by elevating the curative value and that we win back our self-respect of medical treatment which is dominated from the western medical science and ultimately contribute to national medical welfare.

A study on the Early Postoperative Hemodynamic Changes after Correction of Congenital Heart Defects associated with Pulmonary Hypertension (폐동맥고혈압증을 동반한 선천성 심기형 환아들에서 술후 조기 혈류역학적 변화에 대한 연구)

  • Surgical correction of congenital cardiac defects in infants and children with an elevated pulmonary arterial pressure or pulmonary vascular resistance carries a significant early postoperative mortality. And accurate assessments of cardiac output is critically important in these patients. From April 1988 through September 1989, serial measurements of cardiac index, ratio of pulmonary-systemic systolic pressure, ratio of pulmonary-systemic resistance, central venous pressure, left atrial pressure, and urine output during the first 48 hours after the cardiac operation were made in 30 congenital cardiac defects associated with pulmonary hypertension. Cardiac index showed significant increase only after 24 hour postoperatively and this low cardiac performance in the early postoperative period should be considered when postoperative management is being planned in the risky patients. There were no variables which showed any significant correlation with cardiac index. In 12 cases[40%], pulmonary hypertensive crisis developed during the 48 hours postoperatively, and they were treated with full sedation, hyperventilation with 100 % 0y and pulmonary vasodilator infusion. In all patient with preoperative pulmonary hypertension, surgical placement of a pulmonary artery catheter is desirable to allow prompt diagnosis of pulmonary hypertensive crisis and to monitor subsequent therapy.
