Quality of Life in Patients with Skin Disease (한국어판 Skindex-29를 통해 본 피부과 환자의 삶의 질)

  • Backgrounds : Many people think that ski disease has littel influence on the quality of life in patient, but various studios show that skin disease may produce a range of impacts on QOL. Nevertheless, there is much to be learned about the association between the duality of life and skin disease in the area of Oriental dermatology. Objectives : This study was conducted fe investigate the impact of the various skin diseases on QOL and explore prognostic factors that influence outcomes. Methods : 103 patients were recruited who filled out more than $90\%$ of the skindex-29 questionnaire. The function of sum scores and three domain scores with each skin disease were investigated, as well as the correlation of each domain scores. Results : The disease with the lowest qualify of life was atopic dermatitis. followed by eczematous dermatitis, seborrheic dermatitis and acne vulgaris in order. The sum scales showed significant correlation with three domain scores in atopic dermatitis, urticaria, allergodermia, acne vulgaris, psoriasis and seborrheic dermatitis, The correlation of all three domain scores was significant in atopic dermatitis, urticaria, allergodermia, and psoriasis. Conclusions : Tn this study, skin disease affects the quality of life in physical, social, and psychological factors- Thus, we must consider the quality of life as well as clinical severity inpatients with skin disease.

The Study about Relief Effect of Essential Oil on Seborrheic Dermatitis with Co-culture System (각질형세포와 피지선세포 공배양을 통한 지루성 피부염 억제 소재 연구)

  • Seborrheic dermatitis (SD) is the skin disease occurred because of Malassezia yeast which grows on the skin and scalp, and this yeast lives on sebum lipid, and their metabolite, free lipid acids are thought to be the main irritant on skin. To find out effective cosmeceutical ingredients to treat SD symptoms, we established novel cell-based in vitro model mimicking SD symptoms. This in vitro model adopted the co-culture system with primary sebocyte & HaCaT keratinocyte. We used M. globosa yeast extract, arachidonic acid, linoleic acid and dihydrotestosterone as SD inducers. In the co-culture system with optimized concentrations for SD-inducing cocktail, the production of IL-8 and sebum lipids increased up to 2-fold, and then we screened with commercial essential oils by monitoring IL-8 as a key inflammatory biomarker. Then we found that Cinnamomum zeylanicum oil, Mentha arvensis oil effectively down-regulated IL-$1{\alpha}$, IL-6, IL-8 cytokines which over-produced by SD-inducing cocktail. Additionally, two essential oils also showed inhibitory effect on sebum lipid synthesis from primary sebocyte and growth inhibitory effect to M. globosa yeast (MICs were lower than 0.0625 %). Our recent results suggest that Cinnamomum zeylanicum oil and Mentha arvensis oil could be effective natural herbal remedies to relieve or protect scalp seborrheic dermatitis.

Cannabidiol Inhibits Lipogenesis by Regulating Akt/AMPK-SREBP-1 Pathway in Sebocytes (피지세포에서 Akt/AMPK-SREBP-1 경로를 통한 CBD의 피지 합성 억제 효능)

  • Acne is one of the most common skin diseases, mainly occurring in adolescence. The pathophysiology of acne involves not only hormonal, genetic and environmental factors, but also other factors including hyperseborrhea, inflammation, over-keratinization of follicular keratinocytes and overgrowth of Cutibacterium acnes (C. acnes). Cannabidiol (CBD) is known to relieve pain, stress and inflammation. Moreover, cannabis extracts containing CBD have been reported to be effective in treating acne. However, the therapeutic effect of CBD on acne remains unclear. Therefore, this study aimed to investigate the effect and mechanism of CBD on lipogenesis in SEB-1 sebocytes. We treated sebocytes with CBD and found that it not only inhibited lipid synthesis, but also inhibited cell proliferation by inducing apoptosis. We then demonstrated that sterol response element-binding protein-1 (SREBP-1) mediates the inhibitory effect of CBD on lipogenesis. Furthermore, Akt and adenosine monophosphate-activated protein kinase (AMPK), upstream regulators of SREBP-1, were regulated by CBD treatment. Taken together, our studies demonstrate that CBD inhibits adipogenesis by regulating the Akt/AMPK-SREBP-1 signaling pathway, providing potential for use as a therapeutic agent for acne. Further research is needed to confirm the effect of CBD on inflammation caused by hyperkeratosis, which will increase the possibility of using CBD for acne treatment.

Efficacy of Herbal Medicines for the Treatment of Seborrheic Dermatitis : Systematic Review (지루성 피부염의 한약 치료 효과에 대한 체계적 문헌고찰)

  • 목적 : 체계적 문헌고찰을 통하여 지루피부염에 한약복용의 효과와 연구의 질을 평가하기 위함. 방법 : OASIS, NDSL, EMBASE, Pubmed, Cochrane Library, CINAHL, CNKI, WANFANG, CiNii, J-STAGE를 이용하여 검색하였다. 검색은 2018년 6월 21일에 시행되었으며 P는 지루성 피부염, I는 한약 복용으로 진행하였다. 두 명의 연구자가 독립적으로 연구를 평가하고 선정하였으며, 의견이 다를 경우 제 3자가 개입하여 다수결로 결정하였다. 결과 : 중복 검색을 제외하고 총 952개의 연구가 검색되었으며 제목과 초록을 검토한 결과 897개의 연구가 제외되었다. 나머지 55개 연구의 전문을 검토한 결과 4개의 RCT연구가 최종 선정되었다. 선정된 연구는 발행연도, 대상, 중재법, 평가방법, 효과, 부작용, RoB를 통해 평가하였다. 한약 복용을 한 치료군에서 대조군에 비해 유의하게 호전을 보였으며 중대한 부작용은 없었다. 결론 : 지루성 피부염에 있어 한약 복용은 효과적이며 안전성이 있는 하나의 치료법이다. 향후 다양하고 체계적인 RCT의 시행이 요구된다.

A study on how lifestyle and shampoo habits affect middle-aged men with seborrheic Scalp - especially office workers (30~50대 중장년층 남성의 생활과 샴푸 습관이 지루성 두피에 미치는 영향에 관한 연구 : 사무직 군무 종사자들을 중심으로)

  • This study surveyed 150 men aged 30 to 50, a demographic which seborrheic dermatitis is common. The time of day one shampoos and recurrent fatigue were found to have a statistically significant impact on symptoms contributing to the main causes of seborrhea, including itchiness, excessive oil, acne, and redness on the scalp (p<0.01). Study results confirm lifestyle and shampoo habits, such as the frequent sense of fatigue, a routine work schedule, susceptibility to stress, the time of day one uses shampoo, and whether hair styling products are used, affects seborrheic scalp disease. This study found seborrheic scalp disease for office workers primarily resulted from shampoo habits, rashes were predominantly caused by a failure to rinse shampoo and dry hair completely, while secondary aggravating factors included irregular work schedules, recurrent tiredness, and chronic stress.

A Clinical Study of According to Prevalence of Dermatoses in a Oriental Medicine Hospital of Gang-Dong Area in Seoul (서울 강동지역 소재 한방병원 피부과 내원환자의 질환 빈도에 따른 임상적 관찰)

  • Objective and Methods : To study the prevalence and patterns of common dermatoses and compare these to previous reports, we reviewed 128 new outpatients who visited Allergy/Derma-cosmetic Clinic at Kyung Hee Neo Medical Center in Gang-Dong area of Seoul from January to June, 2007. Results : The results of this study are summarized as follows : 1. Among the 128 outpatients, the total number of male patients were 40(31.2%) and female patients were 88(68.8%). 2. In the age distribution, the most frequently visited age groups were 10-19 years old and 20-29 years old(each 21.1%). 3. The most common dermatoses were atopic dermatitis(27.3%), acne(14.1%), allergic contact dermatitis(11.7%), other eczemas(7.8%) and seborrheic dermatitis(7.0%). 4. Among atopic dermatitis patients, the most populated age groups were 0-9 years old and 10-19 years old(each 37.1%), these results are different from past studies revealing that 0-9 years old patients are dominant. 5. Among acne patients, the most frequent age groups were 20-29 years old(50%) and 30-39 years old(27.8%), these results are different from past studies revealing that 20-29 years old and 10-19 years old patients are dominant. 6. Among Allergic contact dermatitis patients, each age group showed no difference of population. And Allergic contact dermatitis patients visited outpatients department at early stage. Conclusion : This study shows that Oriental medicine is popular especially in atopic dermatitis, acne and allergic contact dermatitis. We should investigate long-period and multi-centered study in order to know prevalence tendency of dermatoses in Oriental Medicine. And it is nessessary to research using more efficient evaluation method and treatment for these special diseases.

  • "Chimumchang" seems to be a kind eczema. The cause and symptom of eczema is similar to that of chimumchang. so, I investigated the literatures of oriental medicin and recent medicine. The results are summarized as follows. 1. Chimumchang seems to be a kind of eczema. Allergic contact dermatitis, atopic dermatitis, seborrheic dermatits, neurodermatitis, nummular dermatitis, xerotic eczema and so on are similar to chimumchang on cause & symptom. 2. The cause of chimumchang are summer vital energy excess(夏脈太過), sehwa excess(歲火太過), herat meridian wind-heat(心經風熱), heart fire(心火), wind taken spleen dampnees(脾濕受風), and so on. The cause of eczema are contact of external agent, immunological problem, heridity, infection, abnormalities of sebaceous glands, neurologic dysfuction, and so on. 3. The symptoms of chimumchang have been described to the three pattern on literature of oriental medicine until now. 1) scratching slightly, It has exudation, and combination. 2) Vital energy on skin is irritable by psychological problem. At the acute stage the patient is complain of itching & pain, and gradually the exudation expands in whole body. 3) early peorid it's size is very small, and at first the patient is complain of itching, later complain of pain. The exudation change to ulcer and expands in the whole body. 4. In the investigation of prescription(Total: 34, Oral medicine: 4, Ointment: 30), the most frequently used oral medicine is sungmatang(4 times), and the most frequently used ointment are kohosan, korvunsan and kegwanhyuldobang.

A Case Study on Xerotic Eczema Treated by Oriental Medicine (수부(手部) 건성습진(乾性濕疹)에 대한 한방치료(韓方治療) 증례(證例) 보고(報告))

  • Eczema is a general term for skin diseases showing itching, vessiculopapular eruption, and erythema when acute, or lichenification and scale if chronic. It is classified as irritant dermatitis, atopic dermatitis, seborrheic dermatitis, neurodermatitis, nummular eczema, statis dermatitis, infective dermatitis, xerotic eczema, and pompholyx by afflicted regions and symptoms. Therapy for eczema is allopathic, and includes application of ice- pack to erythema, and application of emollients and steroids to the affected parts during subacute or chronic stages. Eczema is understood as ulceration(瘡), tinea(癬), wind syndrome(風證), and dryness syndrome(燥證) in terms of oriental medicine. Xerotic eczema is understood in dryness syndrome. Nourishing blood(養血) therapy is used. This is a case study on which xerotic eczema occurred on a patient's afflicted left palm and fingers after stroke. The patient's xerotic eczema was attributed to dryness syndrome. Therefore, Samul-tang(四物湯) and Saenghyeoryunbu-eum(生血潤膚飮) were used for nourishing blood. This therapy was effective on eczema and accompanying symptoms such as dizziness and numbness of the left hand.

  • The study was clinically performed with the 128 out patients of dermatic disease who had visited the Dept. of dermatology, Dongzhimen Hospital, Beijing traditional medical college from May 2nd to February 14th 1996. The results were as follows. 1. The dermatologic findings which showed higher incidence in this study were Acne of Melasma 27 cases($21.0\%$), Psoriasis 25($19.5\%$), Eczema including Contact dermatitis 21($16.4\%$), Seborrheic dermatitis including Alopecia of hair 12($9.3\%$) and Urticaria 11($8.5\%$). 2. The sex ratio of male to female was 4.5 : 5.5. The peak incidence of age group was 30-39($21.8\%$). 3. In the duration of disease, 37 cases($28.9\%$) were between 1 year and 5 years. 27($21.1\%$) were over 10 years. 4. About the single herb drugs, they are Angelicas gigantis Radix, Paeoniae Radix and Salviae Radix in order of frequencies. In the case of pill type, Danggui gosam hwan. Chunggan hwan. Whaeo hwan and Shinnongjiyang hwan are most frequently used. In the case of topica, Whangyeongo, Juachanggo and Soeungo are used in high frequencies. 5. For the test and surgical treatment KOH surgical treatment of Molluscum contagiosum in 8 cases and the bloody treatment of Chronic psoriasis in 3 cases.

A Clinical Study on Alopecia (脫毛에 對한 臨床的 考察)

  • The alopecia is classified to Alopecia areata, Telogen effluvium, Anagen effluvium and Androgenetic alopecia. The exact cause of alopecia is not known, but a inherent and immunologic anc psychosomatic and endocrinic factors are suspected. We analysed clinical study in 77 patients, who had visited to the Dept. of Dermatology, Hospital of Oriental Medicine, Kyung Hee University to treat alopecia from June, 1994 to March, 1995. The results were summarized as follows; 1. In the series of 77 patients, $52\%\;were\;male\;and\;48\%$ were female. The peak incidence of age group was 20 to 29 ($55\%$). 2. The peak incidence of age group at on set was 20 to 29 ($49\%$). 3. The most frequent feature was insomnia ($25\%$). Frequent appeared signs and symptoms in descending order were head itching, head scale, fatigue and dizziness. 4. The personal history of patient were seborrheic dermatitis ($17\%$), atopic dermatitis, disorder of thyroid gland and acne in descending order. 5. The patients with family history occupied $25\%$. 6. Shineung Yangjin Dan was most used in treatments as $36\%$.
