Spatio-Temporal Incidence Modeling and Prediction of the Vector-Borne Disease Using an Ecological Model and Deep Neural Network for Climate Change Adaption (기후 변화 적응을 위한 벡터매개질병의 생태 모델 및 심층 인공 신경망 기반 공간-시간적 발병 모델링 및 예측)

  • This study was carried out to analyze spatial and temporal incidence characteristics of scrub typhus and predict the future incidence of scrub typhus since the incidences of scrub typhus have been rapidly increased among vector-borne diseases. A maximum entropy (MaxEnt) ecological model was implemented to predict spatial distribution and incidence rate of scrub typhus using spatial data sets on environmental and social variables. Additionally, relationships between the incidence of scrub typhus and critical spatial data were analyzed. Elevation and temperature were analyzed as dominant spatial factors which influenced the growth environment of Leptotrombidium scutellare (L. scutellare) which is the primary vector of scrub typhus. A temporal number of diseases by scrub typhus was predicted by a deep neural network (DNN). The model considered the time-lagged effect of scrub typhus. The DNN-based prediction model showed that temperature, precipitation, and humidity in summer had significant influence factors on the activity of L. scutellare and the number of diseases at fall. Moreover, the DNN-based prediction model had superior performance compared to a conventional statistical prediction model. Finally, the spatial and temporal models were used under climate change scenario. The future characteristics of scrub typhus showed that the maximum incidence rate would increase by 8%, areas of the high potential of incidence rate would increase by 9%, and disease occurrence duration would expand by 2 months. The results would contribute to the disease management and prediction for the health of residents in terms of public health.

  • Scrub typhus is a fever-related disease that is commonly diagnosed during the autumn season. The number of patients affected by this disease continues to increase every year. However, the cause of the disease and the regional differences of its incidence are poorly understood. This study examined the epidemiological features and regional differences in the incidence of scrub typhus. The monthly incidence of cases from all reports rose to more than 90% during the fall season (October~December). Based on the analysis of the 18,851 cases in the epidemiological investigation, 14,777 (78.4%) and 4,074 (21.6%) cases occurred in high and low occurrence areas, respectively. Moreover, there were more female cases than male cases (males: 7,233 [38.4%]; females: 11,618 [61.6%]; P< 0.001). The clinical symptoms, including fever, chills, myalgia, and eschar of the body, were noted in 16,688 (89.1%), 11,533 (61.2%),11,439 (60.7%), and 6,005 (45.4%) cases, respectively. The incidence rates were higher for Gyeongnam, Jeonam, Jeonbuk, and Chungnam provinces. The areas with a high occurrence of scrub typhus were consistent with the areas experiencing a high distribution of Leptotrombidium scutellare. The incidence of scrub typhus has been rising owing to the increasing population of ticks due to global warming and increased agricultural and outdoor activities. Thus, continuous management of patients and preventive environmental measures are needed to control this disease.

Geographical Distribution and Epidemiologic Factors of Chigger Mites on Apodemus agrarius during Autumn in Korea

  • A nationwide survey of chigger mites causing scrub typhus and an investigation of epidemiologic factors for chigger mites was conducted at 16 localities in 8 provinces in Korea during autumn 2009, 2012, and 2013. A total of 233 Apodemus agrarius were captured, and all were infested with chigger mites. The chigger index was highest in Chungcheongbuk-do in 2009 (358.3) and 2012 (290.1) and Chungcheongnam-do in 2013 (294.4). The predominant chigger mite species was Leptotrombidium pallidum in the northern and central parts and L. scutellare in the southern and western parts, Korea. L. pallidum was not found in Jellanam-do and Gyeongsangnam-do and the distribution of L. scutellare had been expanded in the northern parts of Korea. The chigger index of L. pallidum was positively correlated with temperature and negatively correlated with humidity. The incidence of scrub typhus is dependent on L. scutellare index. These findings could be helpful to monitor the distribution of chigger mites and to develop a preventive measures for scrub typhus in Korea.

Prevalence of chigger mites and Orientia tsutsugamushi strains in northern regions of Gangwon-do, Korea

  • The present study aimed to survey the prevalence of chigger mites and Orientia tsutsugamushi (O. tsutsugamushi) infection in the northern regions of Gangwon-do, Korea. From early February to early June 2015, a total of 17,050 chiggers were collected from striped field mice, Apodemus agrarius, in Cheorwon-gun, Hwacheon-gun, Yanggu-gun, and Goseong-gun, which are well-known endemic areas of scrub typhus in Korea. The chiggers were analyzed using molecular genomic methods, as previously described. Among the 7,964 identified chiggers, the predominant species was Leptotrombidium pallidum (76.9%), followed by L. zetum (16.4%), L. orientale (4.3%), L. palpale (0.3%), L. tectum (0.2%), and Neotrombicula tamiyai (1.8%). The chigger index (CI) was highest in Hwacheon (115.58), followed by Cheorwon (97.02), Yanggu (76.88), and Goseong (54.68). Out of the 79 O. tsutsugamushi-positive chigger pools, 67 (84.8%) were identified as the Boryong strain, 10 (12.7%) as the Youngworl strain, and only 2 were the Jecheon strain. Based on the high infestation of chiggers in striped field rodents and the high rate of O. tsutsugamushi infection in chigger mites, Hwacheon-gun and Cheorwon-gun are presumed to be high-risk areas for scrub typhus. Furthermore, L. pallidum, a major vector of scrub typhus, and the dominant O. tsutsugamushi serotype, the Boryong strain, were found in the northern regions of Gangwon-do, Korea.

  • Scrub typhus is a acute febrile illness by Orientia tsutsugamushi that is endemic in Asian-Pacific areas. It is characterized by fever, rash, eschar, lymphadenopathy. Therapy with Tetracycline, Doxycycline, Chloramphenicol is currently recommended. Here we report a case of tsutsugamushi disease in a 66 year old man had fever, nausea, vommitting, dyspnea, who was successfully treated with East-West intergrative medicine therapy - Antibiotics therapy and Korean traditional medicine(Sasang medicine and acupuncture treatment) - and symptoms improved.

Recent increase of human granulocytic anaplasmosis and co-infection with scrub typhus or korean hemorrhagic fever with renal syndrome in Korea

  • We report 17 patients with human granulocytic anaplasmosis between January 2015 and September 2018 at two tertiary university hospitals in Korea. Monthly incidence peaked in May and June. Among these patients, we identified three who were co-infected with scrub typhus, and one patient with hemorrhagic fever with renal syndrome.

Research Advances of Leptotrombidium scutellare in China

  • Leptotrombidium scutellare is one of the 6 main vectors of scrub typhus in China. It has been found in more than 15 provinces of China. Especially in Yunnan, it was found to be mainly distributed in some mountainous areas with high altitude, low temperature and low precipitation. Rodents and some other small mammals were the most common hosts of L. scutellare. To date, more than 40 host species of L. scutellare have been recorded with very low host specificity, and the main hosts varied in different geographical regions. L. scutellare had a strong resistance against the cold environment, and the temperature and humidity were 2 important factors affecting its growth and development. Among different individuals of their rodent hosts, L. scutellare mites often showed an aggregated distribution pattern, which reflected the interspecific cooperation of the mites. The chromosome karyotype of L. scutellare was 2n=16 and all the 8 pairs of chromosomes were short rod-shaped with metacentric or sub-metacentric types. The isozyme spectrum supported that L. scutellare, L. deliense and L. rubellum were in the same species group. Based on the natural infection, experimental transmission and epidemiological evidence, L. scutellare has been eventually confirmed as the second major vector of scrub typhus in China, which is second only to L. deliense.

  • 쯔쯔가무시병은 추수 시기에 유행하는 대표적인 급성 열성질환으로 여전히 증가 추세에 있다. 의심되는 환자에게 Doxycycline 투여 후 급속한 해열이 질환의 독특한 특징이기도 한데 Doxycycline 투여 96시간 후에도 지속적인 고열과 근육통이 있었던 환자에게 Clarithromycin으로 변경 후 급속한 해열과 함께 증상이 호전된 증례를 보고한다.

Risk Factors for Mechanical Ventilation in Patients with Scrub Typhus Admitted to Intensive Care Unit at a University Hospital

  • Background: The purpose of this study was to evaluate the risk factors for mechanical ventilation in the patients with scrub typhus admitted to intensive care unit (ICU) at a university hospital. Methods: We retrospectively selected and analyzed clinical data from the medical records of 70 patients (32 men, 38 women) admitted to the ICU with scrub typhus between 2004 and 2014. The patients had a mean${\pm}$standard deviation age of $71.2{\pm}11.1years$ and were evaluated in two groups: those who had been treated with mechanical ventilation (the MV group, n=19) and those who had not (the non-MV group, n=51). Mean ages of the MV group and the non-MV group were $71.2{\pm}8.3years$ and $71.2{\pm}11.1years$, respectively. Results: Significant differences between the two groups were observed with respect to acute respiratory failure (p=0.008), Acute Physiology and Chronic Health Evaluation (APACHE) II score (p=0.015), Sequential Organ Failure Assessment (SOFA) score (p=0.013), death (p=0.014), and ICU duration (p<0.01). Multivariate analysis indicated that the following factors were significantly associated with mechanical ventilation: acute respiratory failure (p=0.011), SOFA score (p=0.005), APACHE II score (p=0.011), platelet count (p=0.009), and lactate dehydrogenase (LDH) (p=0.011). Conclusion: Thus, five factors-acute respiratory failure, SOFA score, APACHE II score, platelet count, and LDH-can be the meaningful indicators for mechanical ventilation for the patients with scrub typhus admitted to ICU.

  • Objectives: This study aimed to assess the status of Scrub typhus, Leptospirosis and HFRS(hemorrhagic fever with renal syndrome) among farmers with febrile illness. Methods: We involved a total of 841 farmers who had febrile illness(508 and 333 village residents were, respectively, three and four district of Gyeongju city) selected during autumn of 2014 and 2015. Data were collected by survey questionnaires and blood sample examination. Results: Serum response rate for Scrub typhus and Leptospirosis was 1.5% each and 1.9% for HFRS. Serum response rate for Scrub typhus was significantly higher for tick-bite cases(38.5%), while Leptospirosis was significantly higher for those who were in fruit-planting work(23.1%). Similarly, serum response rate was significantly higher for HFRS who were working in venyl green house work(25.0%). Conclusions: Government authority should develop effective and efficient preventive strategies to create awareness of infectious diseases among farmers. Extending information, education and communication be reached to farmers that could change their perception and help early diagnosis and treatment and reduce the disease burden and its complication.
