Usefulness of FAST for Evaluation of Blunt Abdominal Trauma Patients (복부둔상 환자의 혈복강 진단에 있어 FAST의 유용성에 대한 고찰)

  • Purpose: We planned to determine the diagnostic capability of focused assessment with sonography for trauma (FAST) in cases of blunt abdominal injury (BAI). Methods: A retrospective analysis of FAST sheets was performed from April 2002 to December 2004. During the study period, 135 BAI patients were evaluated with FAST at the Emergency Department of Bundang CHA Hospital. Of this group, twenty-eight patients were excluded, leaving 107 patients for analysis. Abdomen CT (computerized tomography) or exploratory laparotomy confirmed the presence of hemoperitoneum. At the secondary survey, patients underwent a three-view FAST examination (LogicQ; General Electric, Waukesha, USA) by an emergency physician, followed within 2 hours by an abdomen CT or exploratory laparotomy. The FAST examination was considered positive if it demonstrated evidence of free intra-abdominal fluid. Results: There were 45 true-positive FAST examination, 57 true-negatives, 1 false-positive, and 4 false negatives (sensitivity 91.8%, specificity 98.3%, positive predictive value 97.8%, negative predictive value 93.4%). The area under the ROC curve was 0.951 for the FAST examination. Conclusion: FAST is a highly reliable method for screening patients suspected of having BAI for the presence or absence of hemoperitoneum.

A Study on the Risk of Drug Use by Male Adolescents (남자 청소년의 약물사용 위험성에 관한 연구)

  • Purpose: The purpose was to identify the risk of drug use by adolescents. Method: The participants were 933 male students in the first grade of a high school in D city. The data were collected from Aug. 5th to Oct. 30th, 2004. The instrument was the High Risk Group Adolescent Drug User Screening Test(HIRIGADUST) developed by the Korea Adolescent Society(1996). The data were analyzed using SPSS. Results: For substance use, 64.5% of the students answered that they had drunk, 40.3% that they had smoked, and 2.0% that they had tried drug use. For scores on HIRIGADUST regarding socio-demographic characteristics, there were significant differences depending on school type, personality, academic performance, economic status, and ability to talk with parents. For scores on HIRIGADUST regarding drug using-related characteristics, there were significant differences depending on drinking experience, frequency of drinking, amount of alcohol intake, smoking experience and number of cigarettes smoked. Of the students 27.2% students were in the high risk group. Conclusion: In schools, systematic and intensive assessment of drug use should be done, and if needed, a service system connected to clinics specializing in drug addiction should be established. Prevention education should be carried out continuously.

Mediating Effect of Self-efficacy in the Relationship between Anger and Functional Health of Homeless Men (남자 노숙인의 분노와 건강기능과의 관계에서 자기효능감의 매개효과)

  • Purpose: The purpose of this study was to examine the mediation of self-efficacy in the relationship between anger and the functional health of homeless men in order to provide a basis for planning nursing interventions to improve the functional health of homeless persons. Methods: The participants were 137 homeless men who lived in homeless shelters or visited one center serving free meals for homeless persons in Seoul. Data were collected using self-report questionnaires and analyzed with the SPSS-WIN 20.0 program. The instruments were the Functional Health Pattern Screening Assessment Tool (FHPAST), Self-efficacy Scale (SES), and State-trait Anger Expression Inventory-Korean version (STAXI-K). Results: The mean score for functional health was 2.41. Overall self-efficacy was 70.82. state anger was 16.53, trait anger was 19.54, and anger expression was 25.31. There were signigicant correlations among the 3 variables, functional health, self-efficacy, and anger. Also, self-efficacy had a complete mediating effect in the relationship between anger and functional health. Conclusion: Based on the findings of this study, health management programs focusing on anger management and self-efficacy improvement are highly recommended to promote functional health in homeless persons.

Evaluation of Physical Growth and Developmental Status of Infants and Children of Married Immigrant Women in Rural Areas (일 농촌지역 여성결혼이주자 자녀의 신체성장과 발달 평가)

  • Purpose: The purpose of this study was to evaluate the physical growth and developmental status of infants and children of married immigrant women. Methods: The participants were 92 infants and children aged from birth to 6 yr whose mothers were married immigrant women living in one rural area. Physical growth and developmental status were evaluated by using anthropometric assessment, and Korean Denver II developmental screening test. Results: Of the participants, 10.8% were below the 3rd percentile for weight, 13.0% for length, 5.4% for head circumference, and from 69.6% to 79.3% were distributed below the 50th percentile on growth charts. Further, 18.5% were classified as having questionable development. Factors related to low physical growth in infants and children were mother's weight and mother's level in Korean language classes. Conclusion: The physical growth and developmental status of children is vulnerable and serious. The study results suggest a need for regular growth and developmental evaluations. Political support is important for these mothers. In addition, there also is a need to develop early intervention strategies to promote growth and development of the infants and children of these married immigrant women.

The First Korean Cancer Genetic Counseling Program for Nurses (국내 종양유전상담 간호사를 위한 단기 교육프로그램 개발)

  • Genetic knowledge for oncology nurses is important in Korea because oncologists are incorporating genetic counseling and genetic testing into their practice. The purpose of this paper is to describe our method of developing the first academic cancer genetic risk assessment and counseling course for Korean nurses. A one-week (non-credit) cancer genetics counseling program was constructed for master's level Korean oncology nurses. The course emphasized basic genetic concepts and principles the genetics of cancer; hereditary cancer syndromes; family history assessments; pedigree construction; risk calculation; surveillance recommendations and treatment options ethical, legal, social, and psychological issues inherent in genetic testing. The goals of this program are to: 1) provide a comprehensive knowledge base for nurses who are currently expanding their scope of practice into the genetic counseling role 2) introduce this knowledge to nurses who want to use it in their practice; and 3) provide cancer genetic knowledge and resources to Korean nursing faculty who plan to incorporate this knowledge into existing master's courses. This academically-based course is recognized as valuable by nurses, nursing faculty, and physicians. With this new knowledge nurses can begin toexpand their role in delivering comprehensive cancer care services.

Establishment of CALUX Bioassay for Dioxin Determination

  • Dioxin-like compounds are ubiquitous environmental polltants that could be accumulated in biological system and toxic to human and wildlife. Given this issue, it is important to develop a reliable dioxin detection methods for a rational risk assesment of dioxin-like compounds. In this study, we tried to set up and validate a sensitive, reliable risk assessment of dioxin-like compounds. In this study, we tried to set up and validate a sensitive, reliable and rapid bioassay model, CALUX bioassay as a screening tool for routine measurement of dioxin-like conpounds in environmental matrices. For the valisation of CALUX bioassay, firstly, we performed dose-response assay for 2,3,7,8-TCDD, most potent dioxin-like compound, using two different methods CALUX and EROD assay. Induction of luciferase activity and CYPIA catalyzed EROD activity were dose-dependently induced by 2,3,7,8-TCDD, with initial induction at 0.1 pM and maximal induction at 1 nM. In order t determine whether the CALUX bioassay could predict the effects of dioxin-like compounds, 2,3,7,8-TCDD dose-response from CALUX was compared with that from EROD assay. The correlation coefficient ($r^2$) was found to be 0.89, indicating a good correlation between two different methods and the possibility of CALUX bioassay as a useful dioxin detecting method.

Treatment of Hominis placenta pharmacopuncture for a patient with mild neurocognitive disorder: Case report

  • Hominis placenta pharmacopuncture, a treatment that injects Hominis placenta extract into acupoints, has been suggested in the literature and researches that it could be used for cognitive decline. We experienced a case of mild neurocognitive disorder treated with Hominis placenta pharmacopuncture. Hominis placenta pharmacopuncture could be a possible treatment modality producing substantial clinical result in cognitive function which is assessed with Mini-Mental State Examination-Dementia Screening (MMSE-DS), Korean Version of Montreal Cognitive Assessment (MoCA-K), and Korean-Dementia Rating Scale (K-DRS). A 84-year-old man with mild neurocognitive disorder received Hominis placenta pharmacopuncture on GV20, CV12, and bilateral ST36 for a month. The results of neuropsychological examination showed increase in scores after treatment of Hominis placenta pharmacopuncture. Before treatment, they were 15 points for MoCA-K, and 120 points for K-DRS (7.6%), but after treatment, they elevated by 21 points for MoCA-K and 137 points for K-DRS (100%). MMSE-DS score was 28 points, unchanged before and after treatment. It did not cause any side-effect. Hominis placenta pharmacopuncture could be a safe option for treating mild neurocognitive disorder.

The Correlates of Influenza Vaccination among Korean Elderly Men and Women (남.녀 노인의 인플루엔자 백신 접종 실태와 영향요인)

  • Purpose: This study aimed to investigate the rate of and factors associated with influenza vaccination in Korean elderly people. Methods: For this retrospective cross-sectional study, we used data from the Korean National Health and Nutrition Examination Survey (KNHANES) 2008. A total of 1,516 men and women aged 65 or above were included. The data were analyzed using descriptive statistics and logistic regression. Results: The overall rate of influenza vaccination in 2008 was 73.6%. The vaccination rate was lower in women (73.2%) than in men (74.2%; p<.001). It was lower in the elderly aged 80 and above, those living in metropolitan areas, smokers, those who did not receive regular health screening, those who had no history of hospitalization within one year, and those who had liver diseases than in the other groups. The rate of vaccination was higher in those with cancer. Logistic regression analysis revealed that for both men and women, the significant correlates associated with vaccination were demographic factors, chronic disease and health behavior. Conclusion: The influenza vaccination coverage needs to be improved. Efforts should be made to reach the elderly population with relatively low influenza vaccination rates and to increase awareness about the importance of vaccination at a personal and community level.

Associations of Dietary Intake Levels with Ante-natal Depression in Pregnant Women (임부의 식이섭취 수준과 산전우울의 관련성)

  • Purpose: This study was conducted to identify the association of dietary intake levels with ante-natal depression among pregnant women. Methods: Secondary data analysis was done using Can Pro software (version 3.0) to evaluate the diet intake levels with 24 hour recall diary method with 130 pregnant women visiting a general hospital. Statistical analysis included descriptive statistics, t-test, Mann-Whitney U test and adjusted logistic regression using the SPSS/WIN program. Results: Whole dietary intake levels of pregnant women were insufficient according to Recommended Nutrient Intake (RI) with the except of phosphorus intake. There were significant differences in RI by gestation period and high risk pregnancies. Non-depressed women showed more intake in niacin (z=1.33, p=.018) and zinc (t=3.99, p=.048) than depressed women did. Niacin was a significant determinant of ante-natal depression (Exp (B)=5.88, p=.47, 95% CI [1.02~23.83]). Conclusion: Dietary intake assessment would be necessary during pregnancies for the screening of ante-natal depression. For ante-natal depression care, a tailored dietary intervention should be applied for women with insufficient nutrient intake.

BmKn-2 Scorpion Venom Peptide for Killing Oral Cancer Cells by Apoptosis

  • Scorpion venom peptides recently have attracted attention as alternative chemotherapeutic agents that may overcome the limitations of current drugs, providing specific cytotoxicity for cancer cells with an ability to bypass multidrug-resistance mechanisms, additive effects in combination therapy and safety. In the present study, BmKn-2 scorpion venom peptide and its derivatives were chosen for assessment of anticancer activities. BmKn-2 was identified as the most effective against human oral squamous cells carcinoma cell line (HSC-4) by screening assays with an $IC_{50}$ value of $29{\mu}g/ml$. The BmKn-2 peptide killed HSC-4 cells through induction of apoptosis, as confirmed by phase contrast microscopy and RT-PCR techniques. Typical morphological features of apoptosis including cell shrinkage and rounding characteristics were observed in treated HSC-4 cells. The results were further confirmed by increased expression of pro-apoptotic genes such as caspase-3, -7, and -9 but decrease mRNA level of anti-apoptotic BCL-2 in BmKn-2 treated cells, as determined by RT-PCR assay. In summary, the BmKn-2 scorpion venom peptide demonstrates specific membrane binding, growth inhibition and apoptogenic activity against human oral cancer cells.
