청소년과 부모의 인간관계를 통해 본 신뢰의식: 토착심리학적 접근 (The basis of trust in relationships: Indigenous psychological analysis of adolescents and their parents)

  • 이 연구에서는 청소년과 부모의 인간관계를 통해 본 신뢰의식을 탐구하기 위해 토착심리학적 접근을 시도하였다. 이를 위해 청소년이 부모, 교사, 친구에 대해 신뢰하는 이유와, 청소년의 부모집단이 자녀, 배우자, 교사에 대해 신뢰하는 이유를 규명하고자 하였다. 또한 청소년과 부모집단이 다양한 인간관계에서 각 사람을 신뢰하는 정도를 비교하였다. 분석대상은 총 1,737명이었으며, 청소년(중-274명, 고-305명)과 그들의 부(579명) 모(579명)로 구성되었다. 분석 결과, 청소년들이 부모를 신뢰하는 가장 대표적인 이유는 부모의 '희생'이었으며, 그 다음으로 '혈연관계'가 부각되었으며, 이외에 '존경함' '나를 믿어줌' '의지가 됨' '지도와 조언'어 포함되었다. 교사를 신뢰하는 이유로는 '학업 지도'와 '스승에 대한 무조건 신뢰'가 대표적인 이유였으며, '아껴주고 관심있음' '존경함' '상담과 인성지도' '부모와 같은 존재' '학생들을 위한 고생과 희생'이 포함되었다. 친구 신뢰의 이유로서 '의지가 됨' '친밀함' '친구에 대한 무조건 신뢰' '나를 이해함'이 지적되었는데, 정서적 지원의 중요성이 시사되었다. 또한 부모가 자녀를 신뢰하는 이유로 '성실함'과 '정직함'과 같은 도덕성의 측면이 강조되었고, '혈연관계' '자녀에 대한 기대와 대화' '순종함' '학업에 충실'이 신뢰의 이유로 나타났다. 배우자를 신뢰하는 이유로는 '성실함' '가족을 위한 헌신' '정직함' '배우자에 대한 무조건 신뢰' '대화와 상호배려'의 순서로 지적되었다. 학부모 입장에서 교사를 신뢰하는 이유로는 '교육자에 대한 무조건 신뢰' '학생 이해와 사랑' '성실함'이 중요하였다. 한편 학교수준별 성별 학업성적별 청소년집단의 부모, 친구, 교사에 대한 신뢰의 정도를 비교한 결과, 중 고등학생별로 부모에 대한 신뢰는 차이가 없었으나, 중학생이 교사를, 고등학생이 친구를 더욱 신뢰하였다. 또한 성별로는 남자가 여자보다, 학업성적별로는 상집단이 하집단보다 부모나 교사 및 친구를 더욱 신뢰하였다. 연령별 성별 학력별 부모집단의 자녀, 배우자, 교사에 대한 신뢰 정도를 비교한 결과, 의미있는 차이가 없었다. 부모와 자녀의 인간관계 신뢰 정도를 상호 비교한 결과, 아버지와 어머니의 배우자신뢰와 자녀신뢰 정도는 자녀의 부모신뢰 정도와 정적 상관이 있었으며, 어머니의 교사신뢰와 자녀의 교사신뢰 수준도 정적 관계를 보였다.

고혈압·당뇨병 진단자의 영양표시 활용과 질환관리교육의 연관성: 2018년 지역사회건강조사 자료를 활용한 횡단연구 (The association between nutrition label utilization and disease management education among hypertension or diabetes diagnosed in Korea using 2018 Community Health Survey: a cross-sectional study)

  • Objectives: This study examined the association between the experience of disease management education and the use of nutrition labels according to the sociodemographic characteristics and health behaviors of people diagnosed with hypertension and diabetes living in the community. Methods: Among the participants from the Community Health Survey (2018), 74,283 individuals diagnosed with hypertension or diabetes were included in the study population. According to gender, this study evaluated nutrition label use by the experience of disease management education, individual sociodemographic characteristics, and health behavior. Finally, using multiple logistic regression analysis, the association between disease management education and nutrition labels was calculated using the odds ratio (OR) and 95% confidence interval (CI). Results: Males (24.5%) experienced more disease management education than females (22.6%). In addition, younger age, higher education level, and higher equalized personal income experienced more disease management education (P < 0.001). The educational experience rate was higher in the male subjects who did not smoke or were involved in high-risk alcohol consumption (P < 0.001). In addition, the rate of disease management education experience was significantly higher for both men and women who exercised by walking (P < 0.001). The use of nutrition labels was higher in females (9.9%) than males (5.8%), and both males and females were significantly higher in young age, high education, high income, and professional and office positions (P < 0.001). The utilization rate of nutrition labels was high in non-smoking male subjects and high-risk-drinking female subjects. In addition, the utilization rate of nutrition labels was significantly higher in males and females who exercised by walking and those who experienced disease management education (P < 0.001). After adjusting for individual sociodemographic characteristics, health behavior, and disease management education, the use of nutrition labels was high among females (OR 3.19, 95% CI 2.85-3.58), high income (Q4; OR 1.62, 95% CI 1.41-1.87, Q5; OR 1.58, 95% CI 1.37-1.84) and highly educated (high school; OR 2.87, 95% CI 2.62-3.14, above college; OR 5.60, 95% CI 5.02-6.23) while it was low in the elderly (OR 0.43, 95% CI 0.40-0.47), and economically inactive (OR 0.86, 95% CI 0.76-0.96). The use of nutrition labels was high in non-smokers (OR 1.29, 95% CI 1.13-1.48), nonhigh-risk drinkers (OR 1.22, 95% CI 1.08-1.38), and subjects who exercised walking (OR 1.44, 95% CI 1.34-1.54). There was no difference in the utilization rate of nutrition labels according to obesity, and the utilization rate of nutrition labels was significantly higher in subjects who had experienced disease education (OR 1.34, 95% CI 1.24-1.44). Conclusions: Education on the use of nutrition labels, which contributes to food selection for healthy eating, might be a tool for dietary management. Moreover, the utilization rate can be a good indicator for predicting the proportion of the population practicing the guide for disease management. Improving the utilization rate of nutrition labels through disease management education can be a useful intervention for people with chronic diseases who need healthy eating habits for disease management and preventing complications, particularly those diagnosed with hypertension and diabetes.

1차 영유아 구강검진을 경험한 부모의 심층면담 (In-Depth Interview of Parents Experienced First Infant Oral Examination)

  • 본 연구는 1차 영유아 구강검진을 수검한 부모의 경험을 심층적으로 분석하여 실질적인 구강검진 사업의 개선방안을 파악하는 데 목적이 있다. 1차 영유아 구강검진을 수검하였고 자녀의 월령이 18~29개월인 부모 10명을 대상으로 심층면담을 실시하였으며, 녹음된 내용의 개념정리 및 범주를 도출하여 다음과 같은 결론을 얻었다. 첫째, 본 연구의 불만족스러운 검진의 주된 이유는 형식적이기 때문인 것으로 나타났다. 빠르게 진행됨과 동시에 자녀의 구강을 제대로 살피지 않는 상황에 부모들은 실망했고 무료로 실시되기 때문에 일반 치과진료를 받기 위한 검진에 비해 더 형식적으로 이루어진다는 의견이 있었다. 무성의한 치과 의료진의 태도에 불만이 있었으며 부모와 대화하려는 태도가 아닌 일방적인 의사소통 등에 문제점을 제기했다. 둘째, 참여자들 대부분이 형식적이기만 한 영유아 구강검진을 다시 받아야 하는지 의문을 가지고 있었다. 셋째, 구강검진표가 우편 발송되어 수령이 어렵고 영유아건강검진 문진표에 비해 빈약한 정보를 포함하고 있는 문진표에 그리고 결과통보서에 나와 있는 치아번호나 치과용어 등은 부모가 이해하는 데 어려움이 따르는 것으로 나타났다. 넷째, 영유아 구강검진 개선에 대한 의견으로는 검진 후 구강건강관리정보 제공과 수검 시 직접적인 구강건강관리방법 교육 등이 있었다. 또한 부모대상의 영유아 구강건강관리 교육에 대해서는 첫째, 교육의 부재로 자녀의 구강을 관리하는 데 혼란을 겪고 있으며 올바른 정보를 얻고자 하는 부모들의 의견을 확인하였다. 둘째, 현재 보건소에서 제공되고 있는 모자수첩이나 기타 기관들에서 구강건강관리에 관한 교육 자료를 제시하고 있으나 부모들의 접근이 용이하지 않고 내용 또한 포괄적이지 않아 자녀구강건강관리를 위한 정확하고 실질적인 교육자료를 원하는 것으로 나타났다. 셋째, 부모들이 교육받기를 원하는 내용으로는 월령에 따른 구강관리방법, 구강관리용품 선택에 도움이 되는 내용, 구강건강지식, 치과 진료에 대한 지식으로 나타났다. 이상의 결과를 볼 때 영유아 구강검진이 효율적으로 운영되어 부모들을 통한 영유아기 자녀의 올바른 구강건강관리를 도모하기 위해서는 부모들의 의견을 적극 수렴해 형식적인 검진으로만 인식되고 있는 구강검진의 문제점을 개선하여 실질적인 도움을 줄 수 있는 제도로 변화되어야 하는 것으로 확인되었으며 영유아 구강검진과 관련한 반복적 연구를 통해 효율적인 사업운영 방안의 모색이 필요하다.

"삼국사기(三國史記)"에 기록된 의약내용(醫藥內容) 분석 (Study on Medical Records In ${\ulcorner}$the Historical Records of the Three Kingdoms${\lrcorner}$)

  • We tried to observe the features of ancient medical practice by analysing the records related to medicine in the book, ${\ulcorner}$the Historical Records of the Three Kingdom${\lrcorner}$ of which content includes the features of medicine in mythology, plague, delivery of twins, drugs, medical system, shamanism, constitutional medicine, psychiatry, forensic medicine, deformity, a spa, medical phrase, health and welfare work, religion, death. physiological anatomy, Taoist medicine, acupuncture, the occult af of transformation and etc. Our initial concern was about where to draw line as of medical field and we defined medicine in more broad meaning. The book ${\ulcorner}$the Historical Records of the Three Kingdoms${\lrcorner}$ describes the world of mythology by way of medicine which is not clearly a conventional one. There appears records of birth of multiple offsprings 7 times in which cases are of triplets or more. Delivering multiple offsprings were rare phenomenon though such fertility was highly admired. This shows one aspect of ancient country having more population meant more power of the nation. Of those medical records conveyed in that book includes stories of childbirth such as giving birth to a son after praying, giving birth to Kim Yoo-shin after 20 months after mother's dream of conception, and a song longing for getting a laudable child. Plagues were prevalent throughout winter to spring season and one can observe various symptoms of plagues in the record. Of these epidemic diseases, cold type might have been more common than the heat one. Appearance of epidemic diseases frequently coincided with that of natural disasters that this suggests a linkage between plague and underlying doctrine on five elements' motion and six kinds of natural factors. There exists only a few names of diseases such as epidemic disease, wind disease, and syndrome characterized by dyspnea. Otherwise there appeared only afflictions that were not specified therefore it remains cluless to keep track of certain diseases of prevalence. Since this ${\ulcorner}$Historical Records of the Three Kingdoms'${\lrcorner}$ wasn't any sort of medical book, words and terms used were not technical kind and most were the ones used generally among lay people. Therefore any mechanisms of the diseases were hardly mentioned. Some of medicinal substances such as Calculus Bovis, Radix Ginseng, Gaboderma Luciderm, magnetitum were also in use in those days. 53 kinds of dietary supplies appears in the records and some of these might have been used as medicinal purpose. Records concerning dicipline of one's body includes activities such as hunting, archery, horseback riding etc. In Shilla dynasty there were positions such as professor of medicine, Naekongbong(內供奉), Kongbong's doctor(供奉醫師), Kongbong's diviner(供奉卜師). As an educational facility, medical school was built at the first year of King Hyoso's reign and it's curricula included various subjects as ${\ulcorner}$Shin Nong's Herbal classic${\lrcorner}$, ${\ulcorner}$Kabeul classic of acupuncture and moxbustion${\lrcorner}$, ${\ulcorner}$The Plain Questions of the Yellow Emperor's Classic of Internal Medicine${\lrcorner}$, ${\ulcorner}$Classic of Acupuncturer${\lrcorner}$, ${\ulcorner}$The Pulse Classic${\lrcorner}$, ${\ulcorner}$Classic of Channels and Acupuncture Points${\lrcorner}$ and ${\ulcorner}$Difficult Classic${\lrcorner}$. There were 2 medical professors who were in charge of education. To establish pharmacopoeia, 2 Shaji(舍知), 6 Sha(史), 2 Jongshaji(從舍知) were appointed. In Baekje dynasty, Department of Herb was maintained. Doing praying for the sake of health, doing phrenology also can be extended to medical arena. Those who survived over 100 years of age appear 3 times in the record, while 98 appears once. The earliest psychiatrist Nokjin differentiated symptoms to apply either therapies using acupuncture and drug or psychotherapy. There appears a case of rape, a case of burying alive with the dead, 8 cases of suicide that can characterize a prototype of forensic medicine. Deformity-related records include phrases as follow: 'there seems protrudent bone behind the head', 'a body which has two heads, two trunks, four arms.', 'a body equipped with two heads' In those times spa can be said to be used as a place for he리ing, convalescence, and relaxation seeing the records describing a person pretended illness and went to spa to enjoy with his friends. Priest doctors and millitary surgeons were in charge of the medical sevice in the period of the Three Kingdoms by the record written by Mookhoja(墨胡子) and Hoonkyeom(訓謙). Poor diet and regimen makes people more vulnerable to diseases. So there existed charity services for those poor people who couldn't live with one's own capacity such as single parents, orphans, the aged people no one to take care and those who are ill. The cause of affliction was frequently coined with human relation. There appeared the phenomenon of releasing prisoners and allowing people to become priests at the time of king's suffering. Besides, as a healing procedure, sutra-chanting was peformed. There appears 10 cases of death related records which varies from death by drowning, or by freezing, death from animals, death from war, death from wightloss and killing oneself at the moment of spouse's death and etc. There also exist certain records which suggest the knowledge of physiology and anatomy in those times. Since the taoist books such as ${\ulcorner}$Book of the Way and Its Power(老子道德經)${\lrcorner}$ were introduced in the period of Three Kingdoms, it can be considered that medicine was also influenced by taoism. Records of higher level of acupuncture, records which links the medicine and occult art of transformation existed. Although limited, we could figure out the medical state of ancient society.
