Application of Rasch Analysis to the Activities-Specific Balance Confidence (ABC) Scale (Activities-Specific Balance Confidence(ABC)척도에 대한 라쉬분석의 적용)

  • This study was designed to examine, applying Rasch analysis based on item response theory, the questionnaires of the Activities-Specific Balance Confidence (ABC) scale for the elderly. The subjects were 99 institutional older adults and clients of social welfare facilities. The subjects (17 men, 72 women) ranged in age from 65 to 94 years (mean age 76.5 yrs). The Winsteps software was used to assess whether the ABC scale fits the Rasch model, to estimate the score and to refine the rating scale. The results are as follows. Twenty-two subjects were excluded as misfit persons. Four items were found to be misfits and the order of difficulty of the remaining 12 items was rearranged. Their balance confidence is indicated by -.64~1.12 logit, and the transformation formula is score=[(logit score+2.76)/(2.76+3.48)]${\times}$100. The most difficult item was "Walk outside in icy sidewalks" and the easiest item was "Walk around house." In conclusion, the ABC scale for the elderly has been proven reliable and valid. Therefore, it is expected to be used as an effective examination tool for treatment planning and screening for older adults.

A Survey on the Recognition of Rehabilitation Specialists on Therapeutic Horse Riding (재활승마에 대한 재활전문인력의 인식도 조사)

  • As a means of treatment for people with disabilities, there is a growing interest in horse riding. Horse riding improves physical conditions such as muscles, joints, and cardiopulmonary function, alleviates tension, and encourages confidence through large animals such as horses. The various therapeutic effects of therapeutic horse riding have been recognized by many medical professionals, including the American Occupational Therapy and Physical Therapy Association. On the other hand, in Korea, there is not yet a scientific basis for the therapeutic effects of specific diseases or clinical treatments. The purpose of this study was to investigate the possibility of rehabilitation related professional worker's therapeutic horse riding. Recognition and use experience of therapeutic horse riding were generally low, but doctors who wanted to use it when the expectation of effectiveness and therapeutic horse riding education were assumed were relatively high. Based on these results, it can be confirmed that the therapeutic horse riding will be recognized as a clinical treatment method in the future.

Development and Usability Test of the Prototype of the "Smart Stacking Cone" Based on Dual-task Using ICT (ICT를 이용한 이중과제 기반의 스마트 스태킹 콘의 시제품 개발 및 사용성 평가)

  • Objective : This study aimed to develop prototypes of "smart stacking cones" by combining ICT to evaluate and train the upper extremity function and dual task performance in patients with central nervous system impairment, and to identify the complementary point to the completion of the device through a usability test. Methods : This prototype comprised of a hardware and software system that enabled the evaluation and training of patients and the management of data obtained from patients' performance. Specific measurement variables were established so that patient performance could be measured correctly. Based on the measurement variables, a the prototype included a 'single task evaluation modes', 'dual task evaluation mode', 'single task training mode', and 'dual task training modes'. Additionally, a usability test was conducted to assess clinical applicability and overall satisfaction for the prototype. Results : The results of the usability test were generally found to be appropriate. The 'content adequacy' in the usability test was the area with the highest level of adequacy and the lowest level of inadequacy. Additionally, overall 'satisfaction' in the usability test was the area with the highest appropriate and inappropriate levels. Hence, the overall satisfaction results were unstable. Conclusion : Future studies should be conducted to identify the clinical effectiveness of the device by applying an upgraded smart stacking cone to an actual patient group.

Application of Picture Book Reading Training Protocol using Electronic Media and Its Effects on Reading Ability for to Borderline Intellectual Children (경계선 지능 아동을 대상으로 전자매체를 활용한 그림책 읽기 훈련 프로토콜의 적용 및 읽기능력에 미치는 영향)

  • Objective : The purpose of this study was to identify changes in reading ability among children with Borderline Intelligence by applying an electronic media reading training protocol. Methods : A picture book reading training protocol was applied to 10 childrens with borderline intelligence using electronic media to improve reading skills. This protocol was performed for 10 session once a week. After the analysis of the content validity index about the protocol presented in this study, this prococol was applied to the subjects. To analyze the changes of the reading ability for the subjects, KNISE-BAAT type A and B reading test were used. Results : According to the tests taken before and after implementing, the Application of Picture Booking Training Protocol using Electronic Media there was a significant improvement in Reading ability (Understanding words, Completion sentence, Vocabulary selection, Vocabulary arrangement, Understanding short text). However, there was no significant difference in Oral Reading. Conclusion : Application of Picture Booking Training Protocol using Electronic Media may be used as a beneficial measure to improve the reading abilities of children with Borderline Intellectual.

Validation Study of Clinical Utility and Usability on Korean Version of the Life-Space Assessment to Assess Community Mobility (지역사회 이동성을 측정하는 한국어판 생활공간 평가(Korean Version of the Life-Space Assessment; K-LSA)의 임상적 유용성 및 사용성 검증 연구)

  • Objective : The purpose of study is to validate the clinical utility and usability of the Korean version of the Life Space Assessment(K-LSA) which is an assessment tool of community mobility of older adults. Methods : Surveys on the clinical utility and usability of the K-LSA are carried out with a total of aoaa60 occupational and physical therapists. The surveys included the multiple choice questions on the clinical utility and open questions on the usability. Responses to multiple questions are post processed by frequency analysis and technical statistics, and responses to the open questions are categorized by common factors in each questions. Results : Average value of clinical utility ranges from 3.6 to 4.0 with positive responses of 'fair (3 point)', 'agree (4 point)' and 'strongly agree (5 point)' being 95~100%. Average value for clinical usability ranges from 3.6 to 4 with positive answers of 'fair (3 point)', 'easy (4 point)' and 'very easy (5 point)' being 88.3~100%. Additionally out of open-type questions of clinical usability, it was pointed out that the concept of 'neighborhood' for the life space level 3 and 4 is unclear. Conclusion : The current study and research outcomes showed that the K-LSA is a validated tool in Korean health care system for the clinical utility and usability in measuring community mobility, and that it is straightforward in practical use. It will help clinicians and therapists promote the social participation of older adults, and set an intervention goal for enhancing community mobility. It will further help clinicians and researchers in education and research for medical intervention and goal-setting.

A Systematic Review of Randomized Controlled Trials With ICT-based Convergence Study for the Elderly (노인 대상 ICT 기반 융합연구: 무작위 대조군 실험연구의 체계적 고찰)

  • This study is to examine the randomized controlled trials abroad studies on ICT-based convergence studies in the elderly, and to investigate the types and effects of ICT-based interventions. Eight studies that meet the selection criteria were selected from 326 studies identified by three databases and hand-searching. As a result of this study, the most commonly used ICT were the three internet, followed by two tablets and mobile phones, one twoway videoconferencing, one Kinect, one robot, and one Blood Glucose Monitoring System. These results could be used for ICT-based convergence studies in Korea, and further studies should be conducted with higher quality studies such as randomized controlled trials.

An Analytical Study on Research Trends in Journal of Korean Academy of Sensory Integration : From 2003 to 2014 (대한감각통합치료학회지의 연구동향: 2003년부터 2014년까지의 연구를 중심으로)

  • Purpose : This study aimed to critically analyze published research papers in Journal of Korean Academy of Sensory Integration and to investigate the research field and trends. Methods : Types of studies, research trends, levels of evidence, participants, funding sources and research areas were analyzed and described by using 97 selected studies published between 2003 and 2014. Results : Every 5 years, the most frequent type of study was experimental research. The numbers of experimental studies and review studies have been increasing. The most frequent level of evidence was one group non-randomized study, followed by case reports. The highest percentage of participants were children with disability and the most common diagnoses were pervasive developmental disorder, developmental delay and intellectual disabilities. Most of the participants was elementary school students and the most frequent sample size was below 10. The percentage of research grant from Korean Academy of Sensory Integration has been over 50% since 2008. The research areas focusing on the standardized assessments were mostly sensory modulation, interaction and development and the non-standardized assessments were behavior observation and clinical observation. Conclusion : Higher level of studies and comprehensive research methods will be needed. The standardized assessment tools with the high sensibility on sensory integration which developed in the domestic need to be developed. Thus, it will be able to contribute to the evidence based practice in occupational therapy.

A Study on the Effect of the Craniocervical Posture on Hyoid Bone Movement with Sitting Posture in Wheelchair (휠체어에 앉은 자세에서 두경부 자세가 설골 움직임에 미치는 효과에 관한 연구)

  • The purpose of this study was to identify the effect of the craniocervical posture on swallowing and hyoid bone movement with sitting posture in wheelchair, and to provide basic data for developing a treatment program for dysphagia. A total of sixty healthy adults without a history of dysphagia who were guardians or care assistants of inpatients at Hospital K underwent video fluoroscopy performed by physiatrists from April to July 2009. The results of the fluoroscopy were analyzed based on the measurement of hyoid bone movements. The hyoid bone movement(horizontal, vertical, and diagonal) was significantly high in the neutral craniocervical, chin-tuck, and sixty degrees extension postures, in descending order(p<.05). It was found that the neutral craniocervical posture is the most effective posture for hyoid bone movement with sitting posture in wheelchair.

Development of Korean Version of the Dementia Eating Evaluation Tool based on Behavioral Observation (행동관찰 기반 치매 식이 평가 도구의 한국판 개발)

  • Objective : This study introduces domestic and overseas systematic assessment tools that can identify eating problems of dementia patients based on abnormal behavior observations and turns them into Korean through the verification of content placement by expert groups. Methods : Three types of assessment tools were selected for final development in Korean version through several meetings based on a wide range of relevant literature searches. The 3 selected assessment tools were first translated by the researchers, and a 9-person expert team was used to verify the Content Validity Index. Results : The EBS content equivalence calculation shows that all 6 questions and 1 response item had a CVI value 0.9, and all items were included in Korean EBS without modification. The EdFED content equivalence calculation showed that all 11 questions had CVI value 0.9, which was included in the Korean edition of EdFED without modification. The content equivalence calculation of the FDI showed that all 19 questions had a CVI of 0.8 or higher, and all items were included in the Korean version of the FDI without modification of the item. Conclusion : Korean versions of the EBS, EdFED and FDI, which are based on behavioral observation and diet tools for people with dementia, have been developed. Early determination of problems related to diet in dementia patients and providing proper intervention through observational Korean version assessment tools is vital in terms of strengthening patient nutrition and reducing caregivers' burden.

The Effects of Trunk Stabilization Training on a Stroke Patient's Postural Control and Activities of Daily Living: A Sing Subject Design (뇌졸중 환자의 체간안정화 훈련이 자세조절과 일상생활동작에 미치는 영향: 단일사례연구)

  • Objective : This study intends to suggest an effective clinical intervention method for stroke patients by applying a trunk stabilization training program to a single stroke patient and investigating and analyzing the effects of the program on postural control and activities of daily living. Methods : The subject of this study was a 75 year-old female stroke patient hospitalized in C Hospital located in Gyeongju, Gyeongsangbuk-do. As for the research design, A-B-A reversal design was used with a single subject design; and the research period was a total of five weeks from April 21, 2014 to May 23, 2014. As for the research process, a total of 15 sessions were carried out including three sessions of baseline(A), nine sessions of an intervention period(B), and three sessions of maintenance(A'); and the trunk stabilization program was applied during the intervention period. To evaluate the participant's postural control and activities of living life, the Postural Assessment Scale for Stroke(PASS) and the modified Barthel Index(MBI) were used. Visual analysis was used in which collected data were plotted on a graph, using Microsoft Office Excel 2013. Results : In the postural control test, the mean scores improved from 11.7 in the baseline phase to 14.8 in the intervention phase and 15.3 in the maintenance phase. In the evaluation of activities of daily living, the score improved from 66(medium dependence) during the baseline period to 80(low dependence) after intervention and at the maintenance phase. Conclusion : It was found that the trunk stabilization training program showed effects on the stroke patient's postural control or alignment, and furthermore a beneficial improvement in the patient's performing the activities of daily living, as well.
