Correction of a post-traumatic scar fold in the lateral canthus using modified Z-plasty

  • Post-traumatic scar deformations are common in many skin injury cases. These are usually aesthetic problems, but if these deformations impair mobility and the patient feels uncomfortable, we must resolve them. In a patient with a lateral post-traumatic scar fold in the lateral canthus that shortened the horizontal orbital fissure and created traction on eye opening, we performed modified classic Z-plasty, resulting in aesthetic and functional improvements without recurrence.

Effects of Skin Rehabilitation Massage Therapy on Pruritus, Skin Status, and Depression in Burn Survivors

  • Purpose. Hypertrophic scarring and depression are the principal problems of bum rehabilitation. This study was done to verify the effects of skin rehabilitation massage therapy (SRMT) on pruritus, skin status, and depression for Korean bum survivors. Methods. A pretest - posttest design using a nonequivalent control group was applied to examine the effects of SRMT for 3 months in a group of 18 bum survivors. The major dependent variables - including pruritus, objective and subjective scar status, and depression - were measured at the beginning and at the end of the therapy to examine the effects of SRMT. Results. Bum survivors receiving SRMT showed reduced pruritus, improved skin status, and depression. The remaining scar also showed improvement in skin pigmentation, pliability, vascularity, and height (compared to the surrounding skin) as measured on the Vancouver Scar Scale (VSS). Conclusions. The findings demonstrate that SRMT for bum survivors may improve their scars both objectively and subjectively, and also reduce pruritus and depression.

  • ASPS against TGF-${\beta}$ is developing as scar formation inhibitor. The scar was caused by undesired collagen deposition due to overexpression of TGF-${\beta}$ in wounded tissue. The in vitro percutaneous absorption of ASPS(25mer)was investigated by using Furanz Diffusion Cell. The flux of ASPS cannot be found through normal skin due to high molecular weight (MW 10,000) and polyanionic charge. However, the skin permeation of ASPS through tape-stripped damaged skin was markedly increased. The skin fluxs of ASPS were decreased in the following order; hairless mouse> rat >human cadaver skin.

증기발생기 전열관 마모 파열 거동 (Burst Behavior of Wear Scar of Steam Generators Tubes)

  • Nuclear steam generator tubes have experienced wear degradation at tube support structure. Morphology of wear scar was analyzed by using eddy current signal. A burst test facility for steam generator tubes was established and tubes with 3 types of defects were tested. The burst test results show that the depth of wear scar is the main factor influencing the burst pressure of tubes, meanwhile, both the longitudinal length and the angle also have effect on the burst pressure. Based on test results, the burst pressure equation for wear degradation was proposed.

Coastline Extraction Using RTK GPS/GLONASS Combination Method

  • On this study, realtime GPS technique and combination of GPS/GLONASS technique are used to extracting coastline. Th£ object of coastline is Gwanganri beach located in Busan. The coastline is observed along the traces of coastline when high wave of scar by using digital map of 1:1,000 and at random time zone, coastline is surveyed along the boundary line that is contacting with sea water level. When the coastline of random time zone is converted by height of tide table, the coastline when high wave of scar and converted coastline are shown as coincident approximately.

Malignant melanoma on a thermal burn scar

  • Chronic burn scars often cause various skin malignancies at rates of up to 2%. These lesions are usually squamous cell carcinomas, but rarely, malignant melanoma is reported. We report a 67-year-old male with a malignant melanoma on a burn scar with regional metastasis. This patient presented an ulcerative lesion only in 2 weeks. After histopathological diagnosis, we performed only palliative surgery on patient's demand, and followed up the subsequent deterioration course. Our case reemphasizes the need for rapid diagnosis and treatment when suspect lesions are present on chronic burn scar. Also, physician should be in mind and inform the patient about malignant melanoma and its aggressive course.

Thorax masculinization in a transsexual patient: Inferior pedicle mastectomy without an inverted T scar

  • Transsexual individuals with gender dysphoria or gender identity disorder are rare, with a prevalence reported to range from 0.002% to 0.014%. Studies have shown that mastectomy yields significant improvements in body image and self-esteem in female-to-male transsexual patients. In patients with grade III breast ptosis, mastectomy with a nipple-areolar complex (NAC) graft is the most commonly used technique, although it has several disadvantages. In the case described herein, a bilateral mastectomy preserving the NAC in an inferior pedicle was performed. Additionally, a thin superior thoracic dermal-fat flap was preserved and eventually sutured at the previous inframammary fold, preventing an inverted T scar. This case shows the advantage of this technique for preserving the blood supply and innervation of the NAC, with a low hypopigmentation risk. Furthermore, in this technique, the patch effect does not impair the results of the NAC graft, and there is no need to use an inverted T scar that may result in thoracic feminization.

Effectiveness of a Fractional Picosecond 1,064-nm Laser in Improving Traumatic Scars with Depression

  • The picosecond laser has been widely used to remove tattoos, and improve wrinkles, acne scarring, and pigmented scars. This study was performed to investigate the efficacy and safety of a 1,064-nm fractional picosecond laser treatment for depressed traumatic scars. A total of twenty Korean patients with depressed scars were treated with a 1,064-nm fractional picosecond laser at a two-week interval with a spot size of 3 mm, fluence of 3.5 to 5.5 J/cm2, and frequency of 2 Hz with a combination of microlens arrays. At 4 weeks after the final treatment, there was a statistically significant improvement in the modified Vancouver Scar Scale (mVSS) and the patient satisfaction score without any significant complication. A 1,064-nm fractional picosecond laser treatment is a safe and effective method to improve traumatic wound scars with depression.

Combination Treatment of Nd:YAG Picosecond-domain Laser and Fractional CO2 Laser for Contracted Neck Scar with Hyperpigmentation

  • There is growing interest in minimizing postoperative scarring after a thyroidectomy. Among the many treatment types, laser therapy, especially picosecond-domain laser therapy, is accepted as a standard method. In the present case, a patient with a pigmented, contracted scar was treated using the combination of a picosecond laser and ablative fractional (AF) CO2 laser. After 15 sessions of 1,064-nm picosecond with micro lens array (MLA) and AF CO2 laser application, the patient showed significant improvement in their pigmented lesions and scar contracture with no noticeable side effects for 16 months. These results suggest that a combination of picosecond laser with MLA and AF CO2 laser can treat pigmented, contracted scars safely and effectively.

응급실 얼굴 열상 환자에서 Modified Hollander 점수에 의한 조직 접착제 사용과 봉합사를 이용한 봉합 후 결과 비교 (Comparison between Tissue Adhesive and Suture by Using Modified Hollander Score for Facial Wounds Treated in the Emergency Department)

  • Purpose: The object of this study is to compare the patient satisfaction in the view of scarring, cost, and hospital stay between the conventional suture method and a method using Histoacryl$^{(R)}$ (2-N-butylcyanoacrylate) adhesive for treating facial lacerations in the emergency department. Method: This study is a randomized prospective trial, which was conducted from December 2009 to January 2010. The participants include 109 patients who visited the emergency room in Deajon Eulji Medical Center. The ages ranged from 1 to 59 (mean age of 18.7), and all had facial lacerations of less than 3 cm. In order to treat the facial lacerations, an emergency medicine doctor used tissue adhesive (2-N-butylcyanoacrylate, Histoacryl$^{(R)}$) for 41 patients in the experimental group, and a plastic surgeon performed conventional suturing for 68 patients in the control group. The ER-stay and the primary treatment fee were compared in the two groups, and the scarring was evaluated 10 to 11 months from suturing by using the Modified Hollander Method and the 10-cm scaled VAS (visual analogue scale: score 0=no scar, score 10=very severe scar) Result: The ER stay was 76 minutes for the experimental group and 107 minutes for the control group, showing that statistically significantly less time of 31 minutes was taken in the experimental group. The cost of the experimental group was 40000 won (50.1%) more expensive than the control group, with an average cost of 121900 won for the experimental group and 81200 won for the control group. As for scar evaluation, 10.56 months after suturing, the experimental group showed a better result with a score of 2.6 compared to a score of 3.4 in the control group; however, this difference was not statistically significant ($p$ <0.05). As to a detailed evaluation of scar characteristics, the experimental group had a statistically significantly better result in scar elevation, with a score of 0.6 compared to a score of 1.65 for the control group. Conclusion: If appropriate patients are selected, the method using tissue adhesive directly applied by an EM doctor not only decreases ER stay but also creates similar patients satisfaction, with statistically better result in scar elevation, compared to the conventional suture method; thus, ultimately general patient satisfaction is increased.
