Systematic Review on the Sasang Type-specific Pathophysiological Symptoms of Sleep (사상체질별 수면 소증(素證)에 대한 체계적 고찰연구)

  • Objectives: The purpose of this study was to review previous clinical studies on underlying mechanisms of sleep-related type-specific pathophysiological symptoms among the Sasang types. Methods and Procedure: We reviewed seven research databases from December 2003 to August 2015 with the keywords Sasang typology, constitution and sleep. The Sasang type-specific sleep-related symptoms were analyzed based on seven categories, including subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleep medications and daytime dysfunction. Results: Total of twelve studies were included in the analysis. The Tae-Yang type showed low subjective sleep quality, long sleep latency and frequent awakening, and the So-Yang type showed long sleep latency. The Tae-Eum type presented high subjective sleep quality, short sleep duration, frequent snore, toss and turn during the sleep, and insomnia and low frequency of dream. The So-Eum type showed long sleep latency in male and high frequency of dream. The Eum type combining Tae-Eum and So-Eum types had higher subjective sleep quality, longer sleep duration and higher frequency of dream than the Yang type combining Tae-Yang and So-Yang types.Conclusions This study reviewed type-specific sleep-related characteristics and discussed possible pathophysiological mechanisms involved. Differences in sleep characteristics among the Sasang types might stem from type-specific temperaments and require further study.

Biopsychological Validation of the Sasang Personality Questionnaire in Middle School Students (중학생에서 사상성격검사(SPQ)의 생리심리적 타당화 연구)

  • Objectives The Sasang Personality Questionnaire (SPQ) measures the Yin-Yang temperament of Sasang typology with proven structural and clinical validity in adults, however it was not examined with teenagers. The purpose of this study was to validate the biopsychological structure of SPQ in middle school students.Methods 681 Korean high school students (356 boys and 325 girls) completed SPQ, Junior version of Temperament and Character Inventory (JTCI), and height and weight measures. The correlation between SPQ and JTCI subscales were examined, and the differences of SPQ and JTCI subscales, Body Mass Index (BMI) and Pondera Index (PI) among high (30%), middle (40%), and low (30%) SPQ total score groups were investigated with Analysis of Variance (ANOVA). The profile analysis was also performed to compare JTCI subscale profiles of three SPQ total score groups.Results The SPQ score was significantly (p<0.001) correlated positively with JTCI Novelty-Seeking and negatively with JTCI Harm-Avoidance. The JTCI Novelty-Seeking score of high SPQ group was significantly (p<0.001) higher than that of low SPQ group, and the JTCI Harm-Avoidance score of low SPQ group was significantly higher than that of high SPQ group. The JTCI subscale profiles for three SPQ groups were significantly different for boys and girls. Significant correlations or differences for the PI and BMI among SPQ groups were not found.Conclusions This study presented that the biopsychological structure of SPQ is robust as shown in adults. The SPQ would be a useful clinical measures of Sasang typology in pediatric patients.

Research on the Usage Frequency of Sasang Constitutional Herbal Formula with Case Record Form (증례기록지를 이용한 체질처방 사용빈도 조사)

  • Objectives : The aim of this study was to survey the usage frequency of Sasang constitutional herbal formula in the clinlcal field. Methods : We searched the case report forms from 2031 subjects in the 11 oriental hospitals and 10 oriental clinic. Subjects were diagnosed by the Sasang Constitution specialist in oriental hospital, who had remarkable improvement in their chief complain after Sasang Constitution medication. We counted the usage frequency of each constitutional herbal formula. Results and Conclusions : In TE type, Cheongsimnyeonja-tang, Yeoldahanso-tang, Jowiseungcheong-tang, Taeeumjowi-tang were used in high frequency. In SE type, various herbal formula were used in a similar frequency. In SY type, Yanggyeoksanhwa-tang, Hyeongbangjihwang-tang, Dokhwaljihwang-tang were used in high frequency. Additionally, the use of specific herbal formula was concentrated in SY type.

The Research Trends in Articles Published in the Journal of Sasang Constitutional Medicine in 1989-2018 (1989-2018년 사상체질의학회지 게재논문들의 연구동향 분석)

  • Objectives The purpose of this study was to identify the research trends and major fields of Sasang Constitutional Medicine by analyzing the topics of papers contained in the Journal of Sasang Constitutional Medicine from the first issue to 2018. Methods To classify the research themes of the paper, theory, Characteristics in Sasang Constitution(SC), Diagnosis in SC, Treatment in SC, etc. categories and subcategories were created. Two researchers reviewed the title, abstract, and text of the paper and classified them according to the category classification. In case of disagreement, the category was determined through discussion with one of the remaining researchers. Results & Conclusions A total of 1,169 papers published in the Journal of Sasang Constitutional Medicine from the first issue to 2018. The number of published papers has increased steadily since the first issue of the Journal of Sasang Constitutional Medicine, and in 2008, the largest number of papers were published, but since then, the number of papers has gradually decreased. When classifying the categories of published papers according to research themes, the Theory field occupied a large portion in the early stages, but since then, the proportion has decreased steadily, and the number of publications has plummeted since 2012. On the other hand, in Treatment in SC field, its proportion has soared to account for about half of the papers since 2001. In Characteristics in SC and Diagnosis in SC fields, since the early 1990s, the number of publications has been consistently maintained.

  • In order to find out the relationship between the Eight Principle Pattern Identification of Yin-Yang, Exterior-Interior and the Sasang constitution, we analyzed the clinical data from 500 women with menstrual pain and women without menstrual pain. In the previous study, the subject's information of Typology Complexion Pulse and Symptom was collected, and Eight Principle Pattern Identification was executed based on this. Later, The relationship between the Sasang constitution and the Eight Principle Pattern Identification was statistically analyzed. The obvious difference between the experimental group and the control group in the patterns of Yin-Yang and Exterior-Interior is that patients who complain of menstrual pain do not maintain harmony with the yin-yang ratio, it can be said that the patterns of Yin-Yang and Exterior-Interior can be a identification standard that significantly obscures the condition of the disease. There was a significant difference between the Sasang constitution and the Eight Principle Pattern Identification of Yin-Yang. There was no significant difference between the Sasang constitution and the Eight Principle Pattern Identification of Exterior-Interior. It is assumed that the relationship between the Eight Principle Pattern Identification and the Sasang constitution has changed depending on the difference a view of emotional Qi and pathogenic Qi.

  • The Sasang constitutional medicine, a traditional Korean medical typology, was initially proposed to the oriental medical field in 1894 by the Korean Confucian scholar Jae-Ma Lee. The key principle of his theory is that all humans can be categorized into four groups (Tae-Yang, Tae-Eum, So-Yang, and So-Eum) based on the function of major organs such as the heart, liver, kidney and lungs as well as other characteristics of the body. Now there are many scientists that try to confirm the method to classify the four group using genetics. In this study, we hypothesized that some genes are may associated with Sasang constitution on the basis of the theory of Jae-Ma Lee who insist everybody can be divided from the birth and can not be changed during the life. Before the genetic research, we first suggest the genetic research model. It may be effective to carry out genetic research concerned with Sasang constitution.

Characteristics of each Sasang Constitutional Type as assessed by the Temperament and Character Inventory (TCI) (기질 및 성격 검사(TCI)를 통한 사상체질 특성 연구)

  • 1. Objective: This study was designed to analyze the characteristics of each Sasang constitutional type using the Temperament and Character Inventory (TCI). 2. Methods: A total of 135 healthy subjects (age 19-30) participated in this study, consisting of 35 Soyang-type, 66 Taeeum-type, 30 Soeum-type, and 4 Taeyang-type subjects. A Sasang Constitutional Medicine specialist determined each subject's Sasang constitutional type based on the Two Step Questionnaire for Sasang Constitution Diagnosis (TS-QSCD) and comprehensive constitutional assessment. The subjects were also asked to complete the TCI. 3. Results: Among the four temperament dimensions, the Soyang- and Taeeum-type subjects showed significantly higher scores than the Soeum-type subjects in Novelty Seeking (NS). The Soeum-type subjects showed significantly higher scores than the Taeum- and Soyang-type subjects in Harm Avoidance (HA) (Soeum type > Taeeum type > Soyang type). The Soyang-type subjects showed significantly higher scores than the Soeum-type subjects in Reward Dependence (RD). The Soyang- and Taeeum-type subjects showed significantly higher scores than the Soeum-type subjects in Persistence (P). Among the three character dimensions, the Soyang-type subjects showed significantly higher score than the Taeeumand Soeum-type subjects in Self-Directedness (SD) (Soyang type > Taeeum type > Soeum type). The Soyang-type showed significantly higher scores than the Taeeum-type in Cooperativeness (C). 4. Conclusions: Differences in temperament and character were discovered across the Sasang constitutional types. The Sasang constitutional characteristics should be investigated in larger studies in the future. Similar studies could add to the growing knowledge of Sasang constitutional typology and contribute to a more accurate Sasang constitutional diagnosis.

Study on the Sasang Type Diagnosis Using Objective Biopsychological Measures (객관적인 생리심리 지표를 사용한 사상체질 진단 연구)

  • Introduction The previous study showed that the psychological Sasang Personality Questionnaire (SPQ) and physical Body Mass Index (BMI) were clinically useful for predicting Sasang types diagnosed with certified clinical specialist and QSCCII. The purpose of current study was to examine its clinical usefulness using large sized clinical sample. Methods The age, sex, height, weight, circumference of waist and hip, Sasang Personality Questionnaire (SPQ), and Sasang type of 2,049 participants listed on the Korean Medicine Data Center were acquired. The anthropometrics of BMI, Ponderal Index (PI), Waist-Hip Ratio (WHR), and Basal Metabolic Rate (BMR) were calculated. The discriminant analysis using SPQ subscales, BMI, age and sex were performed to predict Sasang types. And, the ANCOVA with age and sex as covariate was also used to examine differences among diagnosed and predicted Sasang type groups in biopsychological features. Results The discriminant analysis showed 82.0% of percentage correctly predicted. And, the biopsychological characteristics of predicted Sasang type groups were significantly similar to those of diagnosed Sasang type groups. In both predicted and diagnosed Sasang type groups, the anthropometric measures were in increasing order of So-Eum, So-Yang, and Tae-Eum types, and the psychological measures were in increasing order of So-Eum, Tae-Eum, and So-Yang types as shown in previous studies. Discussion and Conclusion The well validated biopsychological variables of SPQ and BMI showed theoretical and clinical usefulness, and were found to be useful for diagnosing Sasang types in clinical setting. The current study might be useful for the East-West integrative medicine and evidence-based teaching.

A Study on the Physical and Psychological Characteristics in Sasang Constitutional Medicine (사상체질에 따른 신체적, 심리적 특성의 연구)

  • 1. Objectives: The purpose of this study was to analyze the physical and psychological characteristics of each Sasang type groups which were classified by Sasang medicine specialist. 2. Methods: The subjects were 115 (85.2% of male) university students with $26.8\pm5.4$ yrs. of age and $12.9\pm1.7$ yrs. of education. We can get 30 of Soyang type, 39 of Taeeum type, 46 of Soeum type after the Sasang type classification. Their physical characteristics were measured with Bioelectrical Impedance Analysis (Inbody 720, Biospace, Seoul), and psychological characteristics with Temperament and Character Inventory (Maumsarang, Seoul). The general linear model (SPSS 12.0, SPSS Inc, Chicago) were used for the analysis with gender and age as covariance. 3. Results and Conclusions: Taeeum type group ($75.64\pm13.01$, $26.12\pm4.16$, $15.74\pm2.42$, $11.30\pm1.80$, $3.93\pm0.62$, $18.43\pm7.70$) showed significantly bigger weight, intra-cellular water, extra-cellular water, protein, mineral, body fat mass than Soeum type group ($61.32\pm8.56$, $22.54\pm3.87$, $13.70\pm2.15$, $9.74\pm1.68$, $3.35\pm0.51$, $10.79\pm3.97$). In obesity degree such as Body Mass Index and Waist-hip ratio, and circumference measures in chest, waist and hip, we found significant differences in the order of Taeeum>Soyang>Soeum. As a psychological perspectives Soyang type group ($41.90\pm13.32$) has higher NS (Novelty-Seeking) score than Soeum type group ($35.22\pm11.22$) with marginal significance (p=0.052). In sub-scale measurements Soeum type group ($10.61\pm3.69$) has significantly lower value than other two groups in P3 (Ambition). The results of present study showed that Taeeum type by specialist has same physical characteristics as previously reported QSCC II based studies. Temperament profile of each Sasang type group will serve as an important evidence for further study showing that the Sasang typology has biological basis.

  • Childhood obesity leads to adult obesity in case of failure of obesity treatment in childhood. It has been reported recently that the various risk factors that cause childhood obesity may lead to diabetes and arteriosclerotic cardiovascular disease in adulthood, thus the treatment of childhood obesity is more important. Sasang Typology advocated by the Korean traditional medical doctor in the late Joseon Dynasty, Je-Ma Lee (李濟馬), separated into four types of human constitution. It is known that Tae-Eum type (太陰人) is likely to be obese compared with other constitutions. This study examined the effects that influence on indicators related obesity and metabolic diseases through conducting aquatic application program at Tae-Eum type children who are severely obese (BMI > 30) during 12 weeks. Moreover, this study investigated the herbal prescriptions for obesity of Tae-Eum type (太陰人).
