The Effects of Preoperative Nursing Instruction on tile Physical and Emotional Recovery of Pediatric Surgical Patients - An Experimental Study- (소아의 수술전 환자교육이 신체 및 정서에 미치는 영향)

  • Very few studies regarding the effects of preoperative nursing intervention on children's welfare after surgery have been conducted. The purpose of this study was 1) to evaluate the effects of preoperative nursing instructions on children's physical recovery and emotional state after surgery and 2) to analyse the relationship between the effects of preoperative nursing instructions and the age and general anxiety of child. The study was conducted flow: August 15th to October 30th at Severance Hospital, Seoul, Korea. A total of 41 Pediatric surgical Patients, between the age of 4 to 14, participated in the study. Twenty patients, randomly selected, were visited by the experimenter in the evening before, surgery and received specific preoperative nursing instructions. These instructions were designed to alleviate emotional stress and were adopted to the age of child. The postoperative recovery of these patients were then compared with a randomly selected control group of 21 patients who received only the routine preoperative care by the staff in the hospital Dependent variable were vomiting, pain medication, ability to void, elevation of temperature, infection, the between surgery and hospital discharge and postoperative emotional stress which was measured by the vital signs. Because of the limitation of the sample size it was difficult to obtain valid statistical results. However, the analysis of the raw data indicates that: 1) the preoperative nursing instructions appears to promote physical recovery and it seems especially effective in preventing elevation of temperature and shortening the length of the hospital stay, 2) the preoperative nursing instruction also seems to be effective in relieving the child's emotional stress(situational anxiety) after surgery, 3) the patient's general anxiety level preparatively seems to be a predictor of postoperative problems, 4) the preoperative nursing instructions were an effective means of promoting physical recovery in every age group as long as the child understood the instructions. The above data would seem to indicate that all preoperative pediatric patients four years of age or older, if they can understand, should have preoperative nursing instruction in order to alleviate postoperative stress and enhance physical recovery. The level of general anxiety preparatively deserves special attention since the amount of nursing instruction needed seems to vary with tile level of anxiety.

Study on Overcoming Interference Factor by Automatic Synthesizer in Endotoxin Test (내독소 검사에서 자동합성장치에 따른 간섭요인 극복에 대한 연구)

  • Purpose : Samsung medical ceter shall find a cause of the interference factor and suggest a solution for it. Materials and Methods : A sample of $^{18}F$-FDG, radioactive pharmaceuticals produced by TRACERlab MX and FASTlab synthesizer. Gel-clot method uses Positive control tube and single test tube. Kinetic chromogenic method uses ENDOSAFE-PTS produced by Charles River. Results : According to Gel clot method of Endotoxin Tests at FASTlab, both turbidity and viscosity increased at 40-fold dilution and Gel clot was detected. In case of TRACERlab MX, Gel clot was detected in most of samples but intermittently not in a few of them. When using ENDOSAFE-PTS, sample CV (Coefficient of Variation) of FASTlab is 0% at all dilution rates whereas spike CV is 0% at 1-fold dilution, 0~35% at 10-fold, 3.6~12.9% at 20-fold, 5.2~7.1% at 30-fold, 1.1~17.4% at 40-fold, spike recovery; 0% at one-fold, 25 ~ 58% at 10-fold, 50 ~ 86% at 20-fold, 70~92% at 30-fold, and 75~120% at 40-fold. Sample CV of TRACERlab MX, is 0% at all dilution rates whereas spike CV is 1.4~4.8% at one-fold dilution, 0.6~19.9% at 10-fold, spike recovery; 35~72% at one-fold dilution and 77~107% at 10-fold. Conclusion : Gel clot does not seem to occur probably to H3PO4 which engages in bonding with Mg2+ion contributing gelation inside PCT. Dilution which is identical to reducing the amount of H3PO4, could remove interfering effects accordingly. Spike recovery was obtained within 70~150% - recommended values of supplier - at 40-fold dilution even in kinetic chromogenic method.

A Study on Sample Preparation for the Analysis of Trace Elements in Foods of Animal Origin by Ultra High Pressure Microwave Digestion (초고압초음파분해법을 이용한 축산물내 미량금속 잔류분석을 위한 시료전처리 방법)

  • Simple and rapid sample preparation method for trace elements in foods of animal origin using ultra high pressure microwave digestion system (UHP/MDS) and inductively coupled plasma atomic emission spectrometer (ICP/AES) were developed. 1. For the digestion of sample using UHP-MDS, 20% nitric acid (v/v) was the most suitable solvent for the determination of trace elements in foods of animal origin. 2. The optimal digestion conditions for UHP-MDS were as follows: final temperature $180^{\circ}C$, final pressure 400 PSI, and magnetic power 900 W in the solid sample. For the liquid sample final temperature $170^{\circ}C$, final pressure 300 PSI and magnetic power 700 W were optimal conditions. 3. As result of interlaboratory test, the average recovery rate of the for solid sample were 88.3~99.1% for As, 82.4~93.3% for Cd, 89.2~101.2% for Hg and 86.5~93.8% for Pb, respectively. In liquid sample, it were 87.0~96.8% for As, 80.9~96.6% for Cd, 87.5~91.2% for Hg and 91.4~95.5% for Pb, respectively. 4. The average coefficient variation rate were 3.3~15.9% for solid sample and 2.9~10.8% for liquid sample.

Effects of Soaansintang(SOAT) on the hemodynamics and electrocardiogram of isolated rat hearts induced by electrical stimulation (소아안신탕(小兒安神湯)이 STRESS를 유발한 흰쥐의 적출심장(摘出心臟)에 미치는 영향)

  • It has long been known that SOAT is effective for sudden palpitation occurring unexpectedly in Oriental Medicine. However, effect of SOAT on the isolated heart has not been studied yet. The purpose of this study is to investigate the effect of SOAT on hemodynamics and ECG of isolated rat hearts induced by electrical stimulation using Langendorff perfusion apparatus for nonworking heart. SOAT extract was manufactured by water-alcohol precipitated method. Sprague-Dawley rats weighting $120{\sim}150g$ were used for the experiments, Subject animals were divided into four groups, which are consisted of 1) control(Group orally administered by normal saline 1ml for 14days), 2) sample A(Group orally administered by SOAT extract 1ml for 14days), 3) sample C(Group injected by SOAT extract 0.5ml after stimulation, 4) sample C(Group injected by SOAT extract 1ml after stimulation. To evluate the effects of SOAT on hemodynamics and ECG of isolated rat heart induced by stimulation, heart rate, left ventricular pressure, systolic power, diastolic power, coronary artery perfusion volume and ECG were measured using Langendorff apparatus in both stimulation mode(5 volts, 450 beats/min) and arrythmic mode(5 volts, 420 beats/min including 60 beats/min) The results obtained are as follows : 1. After receiving stressful electrical stimuli, isolated heart showed the heart rate, left ventricular pressure, systolic power, diastolic power, coronary artery perfusion volume were all decreased temporarily, but perfusion continued longer recovery to the control state appeared. However, the coronary artery perfusion volume diminished continuously. 2. The heart rates did not change significantly with both stimulation mode and arrhythmic mode, among experimental groups. 3. The left ventricular pressure showed with both stimulation mode and arrhythmic mode, the significant changes(p<0.05) especially in the injection sample group. In case of stimulation mode, low concentration injection group(0.5ml) was more significantly increased rather than high concentration group(1ml) and in case of arrhythmic mode, high density group(1ml) was so increased than the other(0.5ml). 4. For the systolic power and diastolic power, no significant changes were noticed in the stimulation mode, but in the arrhythmic mode of injection sample groups, significant change(p<0.05) was noticed in both systolic power and diastolic power. Specially the high concentration group(1ml) showed more significant increase than the low concentration group. 5. For the coronary artery perfusion volume, no significant change difference among sample groups was observed in both the stimulation mode and the arrhythmic mode. 6. For the ECG recordings, arrhythmia was induced by electrical stimulus of arrythmia mode and after the stimulus was removed, irregular wave appeared temporarily, but as perpusion continued, recovery to the control state was abtained like the stimulation mode. According to the above results, SOAT significantly changed the hemodynamic data from the electrically stressed, isolated hearts of connected Langendorff perfusion apparatus and we propose SOAT has the direct effects on the muscular function of heart.

Comparison between Head Space Gas Sampling and Purge & Trap Sampling in Water Analysis

  • The two main methods to prepare water samples for analyzing volatile organic compounds(VOC's) were investigated. One is the purge and trap(PT) method and another is the head space(HS) sampling method. Both methods were effective to transfer the low boiling point components from the water sample onto the capillary column. The cryo-focusing at the top of the main capillary column was an effective way to obtain the sharpness of the chromatographic peaks but could be avoided when a semi-wide bore column was used. The recovery from the same amount of the sample was better in PT than in HS but a larger sample volume in HS method could compensate the lower efficiency. Therefore PT is suitable to the analysis of drinking water where the very low concentration must be determined. HS is suitable to waste water analysis because of the easiness of the operation. The repeatability was good and similar in both methods. For the contamination of the former sample, both methods were tough and could be used without any problems. The matrix effect which could change the equilibrium parameters in HS method was find negligible in many components. The actual samples such as tap water and river water were analyzed with both methods concerning 16 components regulated in Korea.

Determination of copper(II) in various samples by flame atomic absorption spectrophotometry after column separation by adsorption of its N-benzoylphenylhydroxylamine complex on benzophenone

  • A sensitive technique for the determination of trace Cu(II) in various samples after the column preconcentration by adsorbing its N-benzoylphenylhydroxylamine (BPHA) onto benzophenone was developed. Several experimental conditions such as the pH of the sample solution, the amount of chelating agent, the amount of benzophenone, and the flowrate of sample solution and so forth were optimized. The interfering effects of diverse concomitant ions were investigated. Fe(III) and $CN^-$ interfered with more seriously than any other ions. However, the interference by these ions could be overcome sufficiently by adjusting the added volume of 0.01M BPHA to 10 mL. The dynamic range, the correlation coefficient ($r^2$) and the detection limit obtained by this proposed technique were 5.0~120 ng/mL, 0.9974, and 2.1 ng/mL, respectively. For validating this proposed technique, the aqueous samples (stream water, reservoir water, and wastewater), the plastic sample and the diluted brass sample were used. Recovery yields of 93~102% were obtained. These measured data were not different from ICP-MS data at 95% confidence level. This method was also validated by the rice flour CRM (normal, fortified) samples. Based on the results from the experiment, it was found that this proposed technique could be applied to the determination of Cu(II) in various real samples.

Comparison of Sampling Filters for Airborne Hexavalent Chromium in Plating Operation (도금공정 공기중 6가 크롬 시료채취여과지 비교에 관한 연구)

  • Hexavalent chromium may reduce on polyvinyl chloride (PVC) filter during sampling and storage of sample. Recently, new or modified filters for preventing Cr(VI) from the reduction has been introduced. Thus, this study was performed to compare the reduction behaviors of Cr(VI) on several sampling filters and to find the most appropriate filter for airborne Cr(VI) sampling in plating operation. The results were as follows. 1. There were statistically significant differences among PVC, polytetrafluoroethylene (PTFE). glass fiber (GF) and polyvinylidene fluoride (PVDF) filters in recovery rates of spiked Cr(VI) samples by storage time(p<0.05). There was no significant difference between PVC and PTFE filters(p>0.05). The PVC and PTFE filters showed higher recoveries than GF and PVDF filters(p<0.05). 2. The quartz fiber(QF) filter treated with an alkali solution(2% NaOH/3% Na$_2$CO$_3$, 1% NaOH) showed a significantly higher recovery of Cr(VI) by storage time than other filters(GF and QF filter)(p<0.05). There was no difference in recovery of Cr(VI) between alkali-treated and untreated GF it filters(p>0.05). But the QF filters treated with two alkali solution showed a significantly higher recovery than the untreated QF filter(p<0.05). There was no significant difference in recovery of Cr(VI) between QF filters treated with 1% NaOH and 2% NaOH/3% Na$_2$CO$_3$(p>0.05). In conclusion, treatment of QF fillers with alkali solution was most effective in protecting from the reduction of Cr(VI).

Separation and Recovery of Iodide in Radioactive Waste for $^129I$ (방사성폐기물 중의 $^129I$ 정량을 위한 요오드의 분리 및 회수)

  • For the disposal of low-level radwaste from nuclear power plant need the determination of levels of radio nuclides in radwaste. These nuclides include the difficult-to-measure nuclides, so indirect methodology for the determination of the difficult-to-measure nuclides have to be developed. In this work, for the determination of $^129I(t_{1/2}=1.57{\times}10^7 years)$ in low-level radwaste from nuclear power plant is investigated. Recovery of Iodide in simulated waste($UO_2$ pellet) as a soluble and radwaste(resin, woolen fabric)as a insoluble samples are measured. After pretreatment of sample, $I_2$ are extracted from aqueous solution with $CCl_4$. Then I are extracted from $CCl_4$ with 0.1M $NaHSO_3$ aqueous solution. iodide in aqueous solution are determined by ion chromatography. The overall recovery yield is 76.7 (RSD 1.7%) for mixed-acid digestion method. Incase of woolen fabrics, overall recovery yield is 74.3 (RSD 2.2%) and recovery of iodide in resin 56.5(RSD 5.6%) for alkaline fusion method.

The Characteristics and Financial Status of the Users of the Debt Management Program of the Credit Counseling and Recovery Service (신용회복지원제도 이용자의 특성과 재무상태 분석 : 신용회복위원회 채무조정신청자를 대상으로)

  • The purpose of this study was to analyze the characteristics and financial status of credit delinquents utilizing the debt management program of the Credit Counseling and Recovery Service between January-June in 2007. Total sample of 41,355 cases was analyzed using the statistical program SPSS(Version 12.0). For analysis, descriptive statistics, F-test, Scheffe test, t-test, logit analysis and regression analysis were employed. People in the age range of 30-40s, males, high-school graduates, married couples, part-time employees, costfree residents and residents in other regions were relatively high users of the debt management program. Reasons of credit delinquency were diverse and was combined to credit default. However, increases in expenses and income reductions were found to be the most frequent reasons. Financial conditions of delinquents were worse than those of average persons shown on the national statistics. It was also found that age, sex, educational level, occupation, region of residence, home-ownership, reason of delinquency, income and total outstandings of debt were significant determinants of short-term debt burden which was measured by the ratio of monthly payment to income and long-term debt burden which was measured by repayment period.

  • This study explored and described the depression of Koreans for the true nature of the experience and recovery process. The sample included 13 men and women between 18 and 69 who experienced and recovered from depression. Participants attended interviews producing data, which was analyzed using Consensual Qualitative Research(CQR) method. The result yielded 4 domains, the life before depression, the experience of depression, the recovering process of depression, after the recovery of depression.
