Effects of Thoracic Flexibility Exercise Program on Pain, Heart Rate Variability, and Depression in Patients with Chronic Low Back Pain (흉추 가동성 운동프로그램이 만성 요통 환자의 통증과 심박 변이도 및 우울증에 미치는 영향)

  • Purpose : This study aimed to investigate the effects of a thoracic flexibility exercise program on pain, heart rate variability (HRV), and depression in patients with chronic low back pain. Methods : Fifty patients with chronic low back pain participated in this study. Each participant performed a pre-test and post-test. The outcome measures evaluated were a visual analog scale, HRV test results (automatic activity, standard deviation normal to normal [SDNN], low frequency [LF], high frequency [HF], and LF/HF ratio), and the Beck Depression Inventory. Patients underwent conventional physiotherapy for 35 minutes per session. In addition, a thoracic flexibility exercise program (trunk rotation exercise, McKenzie exercise, and supine thoracic extension exercise) were performed 3 times per week for 4 weeks. Results : After the training period, the visual analog scale results showed a significant decrease in the post-test compared to the pre-test. Automatic activity, SDNN, LF, HF, and LF/HF ratio significantly increased in the post-test compared to the pre-test. The Beck depression inventory results significantly decreased in the post-test compared to the pre-test. Conclusion : This study demonstrated that a thoracic flexibility exercise program improves a visual analog scale, HRV (automatic activity, SDNN, LF, HF, and LF/HF ratio), and the Beck Depression Inventory in patients with chronic low back pain.

Reconstruction of Pretibial Defect Using Pedicled Perforator Flaps

  • Background Coverage of defects of the pretibial area remains a challenge for surgeons. The difficulty comes from the limited mobility and availability of the overlying skin and soft tissue. We applied variable pedicled perforator flaps to overcome the disadvantages of local flaps and free flaps on the pretibial area. Methods Eight patients who had the defects in the anterior tibial area were enrolled. Retrospective data were obtained on patient demographics, cause, defect location, defect size, flap dimension, originating artery, pedicle length, pedicle rotation, complication, and postoperative result. The raw surface created following the flap elevation was covered with a split thickness skin graft. Results Posterior tibial artery-based perforator flaps were used in five cases and peroneal artery-based perforator flaps in three cases. The mean age was 54.3 and the mean period of follow-up was 6 months. The average size of the flaps was 63.8 $cm^2$, with a range of 18 to 135 $cm^2$. There were no major complications. No patients had any newly developed functional deficit of the lower leg. Conclusions We suggest that pedicled perforator flaps can be an alternative treatment modality for covering pretibial defects as a simple, safe and versatile procedure.

Treatment and retention of relapsed anterior open-bite with low tongue posture and tongue-tie: A 10-year follow-up

  • The purpose of the current report is to present 6-year long-term stability and 10-year follow-up data for an adult patient who was treated with a tongue elevator for relapsed anterior open-bite. The 19-year-old male patient presented with the chief complaint of difficulty in chewing his food. Collectively, clinical and radiographic examinations revealed an anterior open-bite, low tongue posture, and tongue-tie. The patient opted for orthodontic treatment alone, without any surgical procedure. A lingual frenectomy was recommended to avoid the risk of relapse, but the patient declined because he was not experiencing tongue discomfort. Initial treatment of the anterior open-bite with molar intrusion and tongue exercises was successful, but relapse occurred during the retention period. A tongue elevator was used for retreatment, because the approach was minimally invasive and suited the patient's requirements regarding discomfort, cost, and time. The appliance changed the tongue posture and generated an altered tongue force, which ultimately resulted in intrusive dentoalveolar effects, and a subsequent counterclockwise rotation of the mandible. The results showed long-term stability and were maintained for six years through continual use of the tongue elevator. The results of this case indicated that a tongue elevator could be used not only as an alternative treatment for open-bite, but also as an active retainer.

A Systematic Review on Rehabilitation Research for Improving the Range of Motion of Shoulder Joint Motion in Patients with Breast Cancer Resection (유방암 절제술 환자의 어깨 관절가동범위 증진을 위한 재활 연구에 관한 체계적 고찰)

  • The purpose of this study is to investigate the types and effects of rehabilitation by systematically examining the rehabilitation research for improving the shoulder ROM in breast cancer resection patients. For a systematic review, a total of 8 studies were finally selected through three databases including RISS. As a result of this study, the level of qualitative was 4 studies(50.0%) in level III, and the rehabilitation period varied from 4 weeks to 12 weeks. As for the type of rehabilitation, 3 studies(37.5%) were occupational therapy and physical therapy. The ROM of the shoulder joint was 7 studies(87.5%) of flexion, extension, and external rotation. A goniometer was used in 8 studies(100.0%) to evaluate the ROM of the joint as a measuring tool. This study is expected to provide basic data in the field of convergence when developing and applying interventional protocols for patients with limited ROM due to breast cancer resection.

Monitoring of Gamma-ray Bright AGN : The Multi-Frequency Polarization of the Flaring Blazar 3C 279

  • We present results of long-term multi-wavelength polarization observations of the powerful blazar 3C 279 after its ${\gamma}$-ray flare on 2013 December 20. We followed up this flare by means of single-dish polarization observations with two 21-m telescopes of the Korean VLBI Network, carried out weekly from 2013 December 25 to January 11, and at 22, 43, and 86 GHz, simultaneously. These observations were part of the Monitoring Of GAmma-ray Bright AGNs (MOGABA) program. We Measured 3C 279 total flux densities at 22, 43, and 86 GHz, showing a mild variability of a factor of ${\leq}50%$ over the period of our observations. The spectral index ranged from -0.13 to -0.36 at between 22 and 86 GHz. The degree of linear polarization was in the range of 6 ~ 12 %, and slightly decreased with time at all frequencies. We found Faraday rotation measures (RM) of -300 to $-1200rad\;m^{-2}$ between 22 and 43 GHz, and -800 to $-5100rad\;m^{-2}$ between 43 and 86 GHz. The RM values follow a power law ${\mid}RM{\mid}{\propto}{\nu}^{\alpha}$, with a mean ${\alpha}$ of 2.2, implying that the polarized emission at these frequencies travels through a Faraday screen in or near the jet. We conclude that the regions emitting polarized radio emission may be different from the region responsible for the 2013 December ${\gamma}$-ray flare, and that these regions are maintained by the dominant magnetic field perpendicular to the direction of the radio jet at milliarcsecond scales.

  • Coupling beams have been used in bearing wall system during last decades. Practically their sectional effects are fully considered in analysis stage to control lateral displacement because they have good contribution to the stiffness of bearing wall system. But the high resultant forces of coupling beam are not fully satisfied in design stage because coupling beams are restricted in sectional size. In this paper the performance of bearing wall system with coupling beam has been evaluated based on improved equivalent linearization procedure of FEMA 440. 15 storied building is selected for analysis. Variables for performance evaluation are natural period, degree of coupling and soil site. To evaluate performance, demand capacity spectrum is calculated based on KBC 2005. As a result, for the most of the cases the life safety limit of chord rotation of coupling beam is less than the performance point of system for soil site $S_D$. That means that the coupling beam can be severly damaged before the system reaches at performance point.

  • Fiber Bragg grating which has a blazed ang1e to the plane of incident wave generates a side-mode as well as main-mode. The side-mode has an identical characteristics with a long period grating that couples with a cladding mode, so rejects the special wavelength. We experimented on the side-mode characteristics with two fibers which Ge doped depressed index fiber and very high photosensitive H$_2$ loaded step index fiber according to the tilted angle. Also, using a phase mask equipped with rotation plate which has 0.02$^{\circ}$ resolution, we can control the bandwidth and the peak value of a total loss spectrum by aligned tilted grating in a fiber and using this, tried to compose the ASE band rejection filter of the Erbium doped fiber amplifier.

Effect of lumbar stabilizing exercise using the gym ball on psychosocial status in elderly women with chronic low back pain (짐볼을 이용한 요부안정화 운동이 만성요통 여성노인의 정신사회적 건강수준에 미치는 영향)

  • Objective : The purpose of this study was to analyze the effect on psychosocial status in elderly women with chronic low back pain after practicing lumbar stabilizing exercise using the gym ball, and to developed and activated on exercise program for healthy senile life. Methods : The subjects were 23 elderly women with chronic low back pain. Mean age of subjects was $67{\pm}13$ years. The lumbar stabilizing exercise using the gym ball was performed 3 times a week during the 8 weeks study period. The research tool were ODI(Oswestry Disability Index), VAS(Visual Analogue Scale), SES(Self-esteem Scale), GDSSF-K(Geriatric Depression Scale Short Form Korea Version), PSS (perceived stress scale), Life satisfaction, and MMSE-K. The analysis of data was processed by the statistical program SPSS 12.0 and paired t-test and frequency analysis were used to compare psychosocial status between the pretest and post tests. Result : The lumbar stabilizing exercise using the gym ball increased significantly SES and Life satisfaction of subjects, and ODI, VAS(trunk flexion & trunk extention), GDSSF-K, and PSS were decreased significantly(p<.05). While VAS(trunk rotation) were not significantly. Conclusion : The lumbar stabilizing exercise using the gym ball improved psychosocial status in elderly women with chronic low back pain.

  • Angle concepts have a multifaceted nature such as quantitative aspects as the amount of rotation, qualitative aspects as geometric shapes, and relationship aspects made with planes or lines. This study analysed angle concepts and introduction methods of angles in elementary mathematics textbooks which have been used from the Syllabus Period to the 2015 Revised Mathematics Curriculum. First, the concepts of angles in mathematics textbooks focus through the definitions, representations, and components of angles presented in mathematics textbooks are analyzed. Secondly, how various aspects of each angle are sequenced through the tasks or activties in the introduction of lesson is looked. As a result of analysis, the methods of introducing angles in the changes of mathematics textbooks have mainly focused on learning about geometric shapes and relations of components. In the mathematics classroom, students should experience various aspects of geometric shapes, rotations, relational aspects of points, lines and surfaces, and support and link them to form a wide range of concepts.

Removal of Gaseous Styrene using a Pilot-Scale Rotating Drum Biotrickling Filter (Pilot-scale 회전식 드럼 바이오필터를 이용한 Styrene 제거)

  • A new type of biofilter, a rotating drum biotrickling filter(RDBF), was developed and operated for the removal of styrene from industrial waste gas. The porous polyurethane foam sheet was used as a packing materials for the RDBF and a pure culture of Gram-positive bacterium Brevibacillus sp. SP1 was used as an inoculum. The reactor showed a short start-up period of 18 days, during which uniform biofilms were developed on the packing. During a steady operation at an incoming styrene concentration of $200ppm_v$ and a retention time of 0.5 min, a high and stable removal of styrene over 95% was observed. The maximum elimination capacity was estimated to be $125g/m^3{\cdot}hr$. The outstanding performance was attributed to an efficient gas-liquid mass transfer and the appropriate supply of nutrient solution to the biofilm microorganisms on the packing by the rotation of the drum.
