Anatomical analysis of the resected roots of mandibular first molars after failed non-surgical retreatment

  • Objectives: Understanding the reason for an unsuccessful non-surgical endodontic treatment outcome, as well as the complex anatomy of the root canal system, is very important. This study examined the cross-sectional root canal structure of mandibular first molars confirmed to have failed non-surgical root canal treatment using digital images obtained during intentional replantation surgery, as well as the causative factors of the failed conventional endodontic treatments. Materials and Methods: This study evaluated 115 mandibular first molars. Digital photographic images of the resected surface were taken at the apical 3 mm level and examined. The discolored dentin area around the root canal was investigated by measuring the total surface area, the treated areas as determined by the endodontic filling material, and the discolored dentin area. Results: Forty 2-rooted teeth showed discolored root dentin in both the mesial and distal roots. Compared to the original filled area, significant expansion of root dentin discoloration was observed. Moreover, the mesial roots were significantly more discolored than the distal roots. Of the 115 molars, 92 had 2 roots. Among the mesial roots of the 2-rooted teeth, 95.7% of the roots had 2 canals and 79.4% had partial/complete isthmuses and/or accessory canals. Conclusions: Dentin discoloration that was not visible on periapical radiographs and cone-beam computed tomography was frequently found in mandibular first molars that failed endodontic treatment. The complex anatomy of the mesial roots of the mandibular first molars is another reason for the failure of conventional endodontic treatment.

  • ;A new root canal instrument and instrumentation technique: a preliminary report. Cleaning and shaping the root canal system has been and continues to be a challenge for even the most experienced endodontist. Curved. narrow canals. in particular. cause difficulties for the beginner as well as the specialist. A new instrument designed to incorporate new concepts was developed to ameliorate the problems in cleaning and shaping root canal systems. This new instrument has been given the name of SW (Senia and Wildey). The new SW instrument uses controlled right and left rotational forces. This motion was used to clean and shape simulated root canals in plastic blocks and root canals in extracted teeth. Instrumentation appeared to be easier. faster. and more precise than with conventional instruments. especially in curved canals where there was remarkable reduction of canal transportation. A mechanical version of the SW instrument was also developed. It was used to flare the coronal portion of the root canal system. Wildey WL. Various factors must be considered in an analysis of instrumentation of the root canal system: the dentin that is cut: the technique used to cut it: the design of the instruments: the material and manufacturing process used to make the instruments: the irrigant used during the procedure: and the anatomic configuration of the root canal system. An analysis of these factors clearly indicates that existing root canal instruments and techniques are less than ideal and. in fact. do not accomplish what is expected of them. Root canals must be properly. but. at the same time. destructive and unnecessary removal of dentin should be kept to a minimum. The Flex-R and Canal Master instruments were developed to address some of the shortcomings of existing instruments and techniques.   • 하악 대구치 근심치근의 danger zone에서 수종의 전동 니켈-티타늄 파일의 근관성형력을 근관성형 전후 치질 두께의 변화를 측정하여 평가하고자 하였다. 기구에 따라 총 40개의 하악 대구치를 10개씩 $Profile^{\circledR}$, GT Rotary file, Quantec 및 ProTaper 4개 군으로 나누고 각 치아당 2개의 근관을 straight up-and-down과 anticurvature 군으로 나누어 근단부 근관을 모두 30번 크기로 일정하게 확대하였다. 수정된 Bramante법을 사용하였으며 술전 및 술후의 근관 상아질 두께를 치수저 하방 1, 3 및 5 mm지점에서 측정, 이원변량분석법으로 통계분석하였다. 모든 군의 danger zone과 safe zone에서의 straight up-and-down 동작과 anticurvature 동작 사이에는 치근상아질 두께변화에 현저한 차이를 나타내지 않았다 (p > 0.05). ProTaper는 danger zone과 safe zone 모두에서 다른 기구에 비해 많은 량의 근관상아질 삭제를 보였으며 특히 분지부 3 mm 수준에서 현저하였다(p < 0.05).

  • The author observed the periodontal tissue reactions to the root canal sealers after root perforations were made intentionally in dogs. The perforations were made on 74 teeth from 7 dogs. The experiments were performed in two different modes of procedure: In Group I, the perforations were made through the root canal to the alveolar bone. In Group II, the perforations were made from site of alveolar bone to the root canals. The perforated canals in Group I were filled with gutta-percha and root canal cements; Calxyl (Calcium Hydroxide in Ringer's solution), Zinc Oxide -Eugenol cement (Z.O.E.), Kerr sealer (Rickert's paste) and AH 26 (Epoxy Resin preparations). The perforated canals in Group II were sealed with Calxyl, Z.O.E, Kerr sealer and AH26. Histologic examinations of periodontal tissue reactions were observed at various time intervals. The results were as follows; l. Cementum deposition on the perforated root surface in Group II cases showed slightly earlier than that of Group I. Healing tendency of injured alveolar bone in Group II was greater than that of Group I. 2. According to the time increase after experiment, the cementum deposition on the site of perforated dentin in Group II with intact pulp was notably thickened. Secondary dentin deposition on the root canal surface where the dentinal tubles were cut was also found in similar pattern. 3. In the cases of perforated canals sealed with Calxyl both in Group I and Group II, It revealed the earliest cementum-deposition among 4 different root canal cements. In the cases of perforated canals sealed with Kerr sealer and AH26, the cementum-deposition on the root surface was not found. 4. Proliferation of epithelium around the perforated area was first seen at 5-week cases in Group I, and at 3-week cases in Group II. 5. In all cases, dentin resorption on the site of perforated root surface was always occured.

근관형성 방법에 따른 근관변위와 잔존치질의 두께에 관한 연구 (EVALUATION OF ROOT CANAL TRANSPORTATION AND REMAINING DENTIN/CEMENTUM THICKNESS FOLLOWING HAND AND ULTRASONIC INSTRUMENTATION)

  • The purpose of this study was to evaluate the root canal transportation and remaining dentin / cementum thickness after using hand and ultrasonic instrumentation in the curved mesial root canals of extracted human mandibular molars. Fourty - six clear polyether blocks were made and randomly divided into two groups: hand instrumentation group with K - Flex files and ultrasonic instrumentation group with Suprasson SP unit. All root canals were instrumented to a size corresponding to a # 30 K - Flex file 1mm short from the radiographic apex. The roots were then sectioned perpendicular to the long axis so the apical and middle third could be evaluated with the Zoom stereomicroscope. The results were as follows : 1. In the total amount of removed dentin at middle third level, there was not significant difference between the hand instrumentation and ultrasonic instrumentation (P>0.05). 2. In the total amount of removed dentin at apical third level, there was more removed by the ultrasonic instrumentation than hand instrumentation(P<0.005). 3. In the transportation width, there was not significant difference between the two groups at both sectioned levels(P>0.05). 4. It was suggested that the canal was transported distally at middle third level and mesially at apical third level by booth techniques.

New trends of root canal disinfection and treatment strategies for infected root canal based upon evidence-based dentistry

  • The main objectives of root canal therapy are cleaning and shaping and then obturating the root canal system in 3 dimensions to prevent reinfection. Many instrumentation techniques and devices, supported by an irrigation system capable of removing pulp tissue remnants and dentin debris, have been proposed to shape root canals. But current regimens in chemomechanical debridement using instrumentation and irrigation with NaOCl are not predictably effective in root canal disinfection. These findings are not surprising because the root canal system is complex and contains numerous ramifications and anatomical irregularities. The microorganisms in root canals not only invade the anatomic irregularities of the root canal system but also are present in the dentinal tubules. Therefore further disinfection with an effective antimicrobial agent may be necessary and it well1mown that use of intracanal medication will lower bacterial count in infected root canals. Calcium hydroxide has a long history of use in endodontics, and more attention has been given to the use of calcium hydroxide as intracanal dressing for the treatment of infected pulp. However, when treatment is completed in one visit, no intracanal medications other than intracanal irrigants are used. Recently, a mixture of a tetracycline isomer, an acid, and a detergent(MTAD), has been introduced as a final rinse for disinfuction of the root canal system. It has been shown that MTAD is able to remove the smear layer with minimal erosive changes on the surface of dentin, and is effective against Enterococcus faecalis, a microorganism resistant to the action of other antimicrobial medications. In another study, the ability of MTAD was investigated to disinfect contaminated root canals with whole saliva and compared its efficacy to that of NaOCl Based on the results, it seems that MTAD is significantly more effective than 5.25% NaOCl in eradicating bacteria from infected root canals. In the cytotoxicity evaluation, MTAD is less cytotoxic than engenol, 3% $H20_2,\;Ca(OH)_2$ paste, 5.25% NaGCl, Peridex, and EDTA and more cytotoxic than 2.63%,1.31% and 0.66% NaOCl. Is it promising or transient?

근관치료 시 초음파 세정 기술을 이용한 잔사 제거의 효율성 (The efficacy of ultrasonic irrigation technique on debris removal during root canal treatment)

  • 목적: Passive irrigation (PI)과 Passive ultrasonic irrigation (PUI)의 상아질 잔사 제거 효율성을 비교하는 것이다. 연구 재료 및 방법: 16 mm 길이로 표준화된 하악 소구치 치근을 근관 확대 후 종단으로 이등분 하였다. 분리된 한쪽 상아질벽에는 길이 4 mm, 너비 0.2 mm, 깊이 0.5 mm의 표준화된 구를 형성하였고, 반대쪽에는 직경 0.3 mm, 깊이 0.5 mm인 3개의 반구 형태의 함요를 형성하였다. 각 구와 함요에는 NaOCl과 혼합된 상아질 잔사로 채우고, 퍼티 인상재를 사용하여 재조합하였다. Group 1은 PI를 이용하고 Group 2는 PUI를 이용하여 각 근관내를 세정하였다. 현미경과 디지털카메라로 절편의 근관벽을 촬영하고, 구와 함요에 잔존하는 상아질 잔사의 양을 scoring system을 이용하여 평가하였다. 결과: 근관의 중앙 1/3에서 group 1이 group 2에 비해 잔사를 더 많이 제거하였지만, 치근단부에서는 유의한 차이가 없었다. 결론: 하악 소구치에서 근관에 기구가 닿지 못하는 부위를 세정시, 근관의 중앙 1/3에서 PUI를 사용하는 것이 PI에 비해 상아질 잔사의 제거 효율이 우수했지만, 치근단부에서는 PUI와 PI의 유의한 차이가 없었다.

Effect of post space preparation drills on the incidence of root dentin defects

  • Objectives: This study investigated the incidence of root dentin defects after the use of different post space preparation (PSP) drills. Materials and Methods: Seventy-two bovine incisors were selected and obtained 14-mm-long root sections. Twelve roots served as controls with no intervention (G1). The 60 root canals remaining were instrumented using the crown-down technique with the ProTaper Next system and obturated using the lateral condensation technique. Specimens were randomly distributed into 5 groups (n = 12) according to the operative steps performed: G2, root canal instrumentation and filling (I+F); G3, I+F and PSP with Gates-Glidden drills; G4, I+FI+F and PSP with Largo-Peeso reamers; G5, I+F and PSP with Exacto drill; and G6, I+F and PSP with WhitePost drill. Roots were sectioned at 3, 6, 9, and 12 mm from the apex, and digital images were captured. The presence of root dentin defects was recorded. Data were analyzed by the χ2 test, with p < 0.05 considered to indicate statistical significance. Results: Root dentin defects were observed in 39.6% of the root sections. No defects were observed in G1. G5 had significantly more cracks and craze lines than G1, G2, and G3 (p < 0.05), and more fractures than G1, G2, G3, and G4 (p < 0.05). When all root sections were analyzed together, significantly more defects were observed at the 12-mm level than at the 3-mm level (p < 0.05). Conclusions: PSP drills caused defects in the root dentin. Gates-Glidden drills caused fewer root defects than Largo-Peeso reamers and Exacto drills.

Shaping ability and apical debris extrusion after root canal preparation with rotary or reciprocating instruments: a micro-CT study

  • Objectives: The aim of this study was to evaluate the shaping ability of the TruShape and Reciproc Blue systems and the apical extrusion of debris after root canal instrumentation. The ProTaper Universal system was used as a reference for comparison. Materials and Methods: Thirty-three mandibular premolars with a single canal were scanned using micro-computed tomography and were matched into 3 groups (n = 11) according to the instrumentation system: TruShape, Reciproc Blue and ProTaper Universal. The teeth were accessed and mounted in an apparatus with agarose gel, which simulated apical resistance provided by the periapical tissue and enabled the collection of apically extruded debris. During root canal preparation, 2.5% sodium hypochlorite was used as an irrigant. The samples were scanned again after instrumentation. The percentage of unprepared area, removed dentin, and volume of apically extruded debris were analyzed. The data were analyzed using 1-way analysis of variance and the Tukey test for multiple comparisons at a 5% significance level. Results: No significant differences in the percentage of unprepared area were observed among the systems (p > 0.05). ProTaper Universal presented a higher percentage of dentin removal than the TruShape and Reciproc Blue systems (p < 0.05). The systems produced similar volumes of apically extruded debris (p > 0.05). Conclusions: All systems caused apically extruded debris, without any significant differences among them. TruShape, Reciproc Blue, and ProTaper Universal presented similar percentages of unprepared area after root canal instrumentation; however, ProTaper Universal was associated with higher dentin removal than the other systems.

근관충전용 실러의 물리적 성질에 관한 연구 (A STUDY ON THE PHYSICAL PROPERTIES OF ROOT CANAL SEALERS)

  • The purpose of this study was to compare and estimate the physical properties of five root canal sealers classified Calciobiotic root canals sealer as calcium hydroxide based sealer, Apatite root sealer type II as calcium phosphate based sealer, AH-26 as resin based sealer, Canals and Pulpdent root canals sealer as zinc oxide eugenol based sealer. The author investigated dimensional change and flow rate of canal sealers, diametral tensile strength and shear bond strength of sealers to dentin to evaluate the physical properties on affect of complete obturation of root canal and performed the total 100 specimens of each 25 sealers under the condition of root temperature according to manufacturer's instructions. All specimens were stored at $37{\pm}1^{\circ}C$ in 100 % relative humidity. A microscope for measurement of micro distance is used for the dimensional change test and evacuation methods using vaccum were used for the flow rate test. The result differed by the storage time measured on the tests of diametral tensile strength and shear bond strength to dentin. The following results were obtained ; 1 On the test of dimensional change, Canals and Pulpdent expanded slightly, AH-26 and Apatite showed the severe shrinkage after 48 hours. 2. AH-26 and Apatite were the excellent with each 24.59mm, 31.19mm after 3 minutes in the aspect of flow property. 3. On the diametral tensile strength, Calciobiotic root canals sealer showed the highest strength with 27.13kg/$cm^2$ after 48 hours, Apatite root sealer type II showed highest strength with 84.57kg/$cm^2$ after 120 hours. 4. On the shear bond strength to dentin, AH-26 was most excellent with 55.73kgf/$cm^2$ after 24 hours and with 134.71kgf/$cm^2$ after 120 hours.
