Worksite Marketing 상품과 언더라이팅 기법 (Underwriting Method of Worksite Marketing Product)

  • Internally Korean insurance market is that whole life products' growth are becoming slowdown that's why new insurance products have appeared on the market in consideration of consumer's needs recently. Externally domestic insurance market competitions has drifted from insurance industry to whole financial industry since bankasurance started. Life insurance companies should open up a new market to survive from severe competitions. Worksite marketing can be an alternative. An insurer make arrangements with an employer about an insurance terms which an insurer offers in Worksite marketing. Then eligible individuals enroll in the plans at their own discretion and pay 100 percent of the premium for coverage through payroll deductions. An employer doesn't need to pay extra money for additional benefit but can raise employee's loyalty and satisfaction of company through worksite marketing. An employee can be covered at discounted premium rate and less strict underwriting guidelines to an insurer compared to individual insurance. In developed countries specially U.S insurance market, Worksite marketing is getting very popular and growing rapidly due to the advantages. Worksite marketing has both individual insurance characters and group insurance characters. Individual insurance characters are that employees enroll in the plans at their own discretion and pay 100 percent of the premium for coverage. Group insurance characters are that actively at work and participation etc. An insurer have to reflect these two characters on Worksite marketing when an insurance company work out a plan for developing products and underwriting guidelines. When an insurer devise worksite products, one should consider participation level which means percentage of eligible employees participating. Participation is related to anti-selection. As we know underwriting is essential for every kind of insurance, especially underwriting plays major role in worksite marketing. We can see that in the below. Firstly, it has a function in calculation of premium rate. When calculate premium rate for worksite products underwriters have to estimate expected participation level and risk factors. So underwriters and acturies keep in close contact with each other. Secondly, underwriting methods are important. When an insurer underwrite worksite products, there are three kinds of underwriting methods. These are Simplified issue underwriting, Full underwriting and Guaranteed issue underwriting. Simplified issue underwriting typically requires no medical examination, but usually requires supplying satisfactory answers to one or several health and/or lifestyle questions. Full underwriting requires a complete medical history questionnaire that may further require an exam. Guaranteed issue underwriting means that coverage is issued without the employee having to provide evidence of insurability. When insurer set the GI limit are usually based on the type of industry, number of eligible employees, the average amount of coverage and participation level. In addition to insurer should have a clear definition of eligible employee on the insurance provision and application form. It will minimize possibility of trouble claims and anti-selection. An insurer also establish preexisting condition exclusion and special guidelines for late entrants. When an insurer introduce Worksite marketing to Korean insurance market, an insurer has to examine market research to analyze potential market and strategy of sales most of all. Also an insurer should review real situation of the U.S, England and Japanese market etc. There are a lot of new technologies about worksite marketing process that an insurer should learn. When an insurer consider many things which we explained it can be a real alternative.

해저 동력케이블 보호를 위한 대상 선박 선정에 관한 연구 (A Study on the Selection of Target Ship for the Protection of Submarine Power Cable)

  • 최근 제주도를 비롯한 도서지역의 전력 사용량 증가 및 해상풍력 발전단지 개발 등으로 인해 해저 동력케이블의 신규 설치가 검토되고 있다. 해저에 설치되는 동력케이블의 보호를 위해서는 선박의 투묘, 주묘, 어로 작업 등에 대한 특성을 기반으로 매설 깊이를 산출해야 한다. 그러나 우리나라는 아직까지 해저 동력케이블 보호를 위한 대상 선박의 규모와 관련된 설계기준이 전무한 실정이다. 본 연구에서는 해저 동력케이블과 유사한 국내 해저배관의 보호를 위한 설계기준을 분석하고, 동력케이블의 설치 환경을 고려한 긴급 투묘의 형태별 분류를 토대로 위험도 매트릭스 모델을 개발하여 보호 대상 선박의 규모를 해당 해역을 통항하는 선박의 크기별 누적함수 규모에 따라 설계하였다. 해저 동력케이블 보호 기준에는 설치 해역의 수심과 조류 등의 환경 조건, 투묘와 주묘 등과 같은 해양사고 조건 등을 연계하였고, 선박의 운항 환경을 항계, 연안해역과 근해해역으로 구분하여 동력케이블의 구체적인 보호 대상 선박의 규모를 검토하였다. 대상 선박의 규모 결정에 대한 적정성 및 유용성 차원에서 완도에서 제주구간에 신설 예정인 제3호 해저 동력케이블에 적용하여 검증하였다. 이러한 해저 동력케이블과 해저배관 등의 보호를 위한 대상 선박의 선정 기준은 향후 매설깊이 설정에 따른 앵커 중량 선정은 물론 해저케이블 물리적 보호시스템 개발에도 활용될 것으로 기대된다.

연관성 규칙 기반 영양소를 이용한 골다공증 예측 모델 (Prediction model of osteoporosis using nutritional components based on association)

  • 골다공증은 주로 노인에서 나타나는 질병으로써 뼈 질량 및 조직의 구조적 악화에 따라 골절의 위험을 증가시킨다. 본 연구의 목적은 영양소 성분과 골다공증과의 연관성을 파악하고, 영양소 성분을 기반으로 골다공증을 예측하는 모델을 생성 및 평가하는 것이다. 실험방법으로 binary logistic regression을 이용하여 연관성분석을 수행하였고, naive Bayes 알고리즘과 variable subset selection 메소드를 이용하여 예측 모델을 생성하였다. 단일 변수들에 대한 분석결과는 남성에서 식품섭취량과 비타민 B2가 골다공증을 예측하는데 가장 높은 the area under the receiver operating characteristic curve (AUC)값을 나타내었다. 여성에서는 단일불포화지방산이 가장 높은 AUC값을 나타내었다. 여성 골다공증 예측모델에서는 Correlation based feature subset 및 wrapper 기반 feature subset 메소드를 이용하여 생성된 모델이 0.662의 AUC 값을 얻었다. 남성에서 전체변수를 이용한 모델은 0.626의 AUC를 얻었고, 그외 남성 모델들에서는 민감도와 1-특이도에서 예측 성능이 매우 낮았다. 이러한 연구결과는 향후 골다공증 치료 및 예방을 위한 기반정보로 활용할수 있을 것으로 기대된다.

핵연료주기 외부비용 평가 (External Cost Assessment for Nuclear Fuel Cycle)

  • 국내 원자력발전은 현재 두 번째로 큰 전력 공급 방법이며 원전의 수 역시 증가되는 것으로 계획되어 있다. 그러나, 원자력발전에 의해 발생되는 사용후핵연료에 대해서는 아직 명확한 관리 정책이 확립되어 있지 않다. 원자로 이 후 핵물질 흐름과 관련된 후행 핵연료주기는 사용후핵연료 관리를 위한 기술들의 집합이다. 따라서, 사용후핵연료 관리 정책은 핵연료주기 선택과 함께한다. 핵연료주기 선택의 중요 항목은 경제성으로 이는 사적비용과 함께 외부비용을 더해 결정되어야 한다. 직접비용 인 사적비용과 달리 간접비용인 외부비용에 대한 연구는 원전에 집중되어 있으며 핵연료주기에 대한 연구는 없는 상황이다. 본 연구에서는 핵연료주기에 적용할 수 있는 외부비용 항목들을 도출하고 정량화를 시도하였다. 핵연료주기 외부비용 평가를 위해 고려될 수 있는 핵연료주기로 OT(직접처분), DUPIC(PWR-CANDU 연결), PWR-MOX(PWR 습식재처리), Pyro-SFR (파이로 처리와 고속로 연계)의 네 가지를 선정하였다. 원자력발전의 외부비용 평가에 고려되었던 항목들을 분석하여 핵연료주기에서 에너지 공급 안보비용, 사고위험비용과 수용성 비용을 외부비용 항목으로 도출하고 추산하였다.

비만치료에 대한 이침요법의 효과에 대한 고찰: 체계적 문헌고찰 및 메타분석 (The Effect of Auricular Acupuncture for Obesity: A Systematic Review and Meta-analysis)

  • Objectives: The purpose of this study is to investigate the weight loss effects of auricular acupuncture in obese patients. Methods: Six databases (Research Information Sharing Service [RISS], Korean studies Information Service System [KISS], Oriental Medicine Advanced Searching Integrated System [OASIS], PubMed, The Cochrane Library, China National Knowledge Infrastructure [CNKI]) were searched up to May 20, 2020. Eight eligible randomized controlled trials were included the present study. The quality of included studies was assessed by the Cochrane risk of bias tool and a meta-analysis was performed by Review Manager software. A meta-analysis was conducted using a random-effects model and a subgroup analysis was performed to detect the sources of heterogeneity, identify the selection of acupuncture methods and explore its contributions to the weight loss effects. Results: Among 8 trials, 5 trials used auricular acupuncture and 2 trials used auricular acupressure, 1 trial used both types of intervention. Most commonly selected acupoints were Shenmen (TF4) and stomach (CO4). Treatment duration was six to twelve weeks, and total treatment session was six to twelve. Compared to the control groups, auricular acupuncture significantly decreased body weight, body mass index (BMI), high density lipoprotein-cholesterol (HDL) and ghrelin. For the selection of acupuncture methods, both methods performed similarly in most outcome except waist circumference (WC), body fat percentage (BFP), and triglycerides (TG). Conclusions: We found that auricular acupuncture can be effective for weight loss and controlling appetite. However, the findings should be interpreted with caution due to heterogeneity. So further vigorous and well-designed studies should be conducted to strengthen the evidence of the use of auricular acupuncture for obesity.

토양유실을 고려한 양서파충류의 서식지 관리지역 선정방법 (A Methodology for Selection of Habitat Management Areas for Amphibians and Reptiles Considering Soil Loss)

  • As disaster risk and climate change volatility increase, there are more efforts to adapt to disasters such as forest fires, floods, and landslides. Most of the research, however, is about influence of human activities on disaster and there is few research on disaster adaptation for species. Previous studies focusing on biodiversity in selecting conservation areas have not addressed threats of disaster in the habitats for species. The natural disasters sometimes play role of drivers of ecological successions in the long run, but they might cause serious problems for the conservation of vulnerable species which are endangered. The purpose of this study is to determine whether soil loss (SL) is effective in selecting habitat management areas for amphibians and reptiles. RUSLE model was used to calculate soil loss (SL) and the distribution of each species (SD) was computed with MaxEnt model to find out the biodiversity index. In order to select the habitat management area, we estimated the different results depending if value of soil loss was applied or not by using MARXAN, a conservation priority selection tool. With using MARXAN, conservation goals can be achieved according to the scenario objectives, and the study has been made to meet the minimum habitat area. Finally, the results are expressed in two; 1) the result of soil loss and biodiversity with MATRIX method and 2) the result of regional difference calculated with MARXAN conservation prioritization considering soil loss. The first result indicates that the area with high soil loss and low species diversity have lower conservation values and thus can be managed as natural disturbances. In the area where soil loss is high and species diversity is also high, it becomes where a disaster mitigation action should be taken for the species. According to the conservation priorities of the second result, higher effectiveness of conservation was obtained with fewer area when it considered SL in addition to SD, compared to when considered only biodiversity. When the SL was not taken into consideration, forest area with high distribution of species were important, but when SL considered, the agricultural area or downstream of the river were represented to be a major part of habitats. If more species data or disaster parameters other than soil loss are added as variables later, it could contribute as a reference material for decision-making to achieve various purposes.

혈액종양 입원 환자 대상 임상약사의 처방중재활동 및 회피비용 분석 (Cost Avoidance and Clinical Pharmacist Interventions on Hospitalized Patients in Hematologic malignancies)

  • Background: Patients with hematologic cancers have a risk of drug-related problems (DRPs) from medications associated with chemotherapy and supportive care. Although the role of oncology pharmacists has been widely documented in the literature, few studies have reported its impact on cost reduction. This study aimed to describe the activities of oncology pharmacists with respect to hematologic diseases and evaluate the associated cost avoidance. Methods: From January to July 2021, patients admitted to the department of hemato-oncology at Seoul National University, Bundang Hospital were studied. The activities of oncology pharmacists were reported by DRP type following the Pharmaceutical Care Network version 9.1 guidelines, and the acceptance rate was calculated. The avoided cost was estimated based on the cost of the pharmacy intervention, pharmacist manpower, and prescriptions associated with the intervention. Results: Pharmacists intervened in 584 prescriptions from 208 patients during the study period. The most prevalent DRP was "adverse drug event (possibly) occurring" (32.4%), followed by "effect of drug treatment not optimal" (28.6%). "Drug selection" (42.5%) and "dose selection" (30.3%) were the most common causes of DRPs. The acceptance rate of the interventions was 97.1%. The total avoidance cost was KRW 149,468,321; the net profit of the avoidance cost, excluding labor costs, was KRW 121,051,690; and the estimated cost saving was KRW 37,223,748. Conclusion: Oncology pharmacists identified and resolved various types of DRPs from prescriptions for patients with hematologic disease, by reviewing the prescriptions. Their clinical service contributed to enhanced patient safety and the avoidance of associated costs.

Modified application of SAMe-TT2R2 scoring system in Asian patients with atrial fibrillation for the selection of oral anticoagulants

  • Background/Aims: The SAMe-TT2R2 score is used for assessing anticoagulation control (AC) quality with warfarin. However, it is hard to apply SAMe-TT2R2 score in Asian patients with atrial fibrillation (AF), because it has not been proven in those populations. This study aimed to validate the SAMe-TT2R2 score in Asian patients with AF and suggest a modified SAMe-TT2R2 score for this population. Methods: We analyzed 710 Korean patients with AF who were using warfarin. The AC quality was assessed as the mean time in therapeutic range (TTR). Each component of SAMe-TT2R2 score was evaluated for the relationship with AC. Further clinical factors that predict AC were analyzed. Identified factors were re-assorted and constructed as SA2Me-TTR scoring system. Results: Of the components of the SAMe-TT2R2 score, female, age, and rhythm control were associated with AC. Heart failure and renal insufficiency were newly identified factors associated with AC. The modified SA2Me-TTR score was reconstructed with the relevant risk factors (S, female gender, 1 point; A, age < 60 yr, 2 points; Me, medical history of heart failure, 1 point; T, treatment for rhythm control, 1 point; T, history of stroke or transient ischemic attack, 1 point; R, renal insufficiency, 1 point). The modified SA2Me-TTR score demonstrated an excellent relationship with the grading of AC. The modified SA2Me-TTR score ≤ 1 identified patients with good AC (hazard ratio 2.46, 95% CI 1.75-3.47). Conclusions: The modified SA2Me-TTR score was useful for guiding oral anticoagulants selection in Asian patients with AF.

간호학에 적용한 캡스톤 디자인의 적용사례 및 결과 (Capstone Design Trail in Nursing Education and Its Outcome)

  • 본 논문은 간호교육에 캡스톤 디자인을 적용한 사례를 제시하고 적용한 결과에 대해 분석한 방법론적 연구이다. 연구대상자는 기본간호학 교과목을 이수한 간호대학생 중 연구 참여에 동의한 학생들로, 프로젝트 팀을 구성하여 팀활동으로 진행하였다. 기본간호학 수업을 통해 학습한 지식을 시작으로 지식-팀구성-주제선정/분석-디자인/개발-모의실험-평가/피드백의 과정으로 수행하였으며, 연구기간은 2015. 5. 1~2015. 10. 30일 까지 6개월간 진행하였다. 최종 결과물로 욕창 예방을 위한 욕창예방양말(BBS: Bed Sore Socks)이 개발되었고, 욕창예방양말의 효과를 검증하기 위해 요양병원에 입원 중인 와상환자에게 무작위 대조군 실험설계로 BBS를 7일간 적용하여 욕창 발병과 욕창위험 점수를 평가 하였다. 모의 무작위 대조군 실험결과, 중재군의 욕창발병률($x^2=.40$, p= .500)과 욕창위험점수(z= -.45, p= .690)에 효과를 보였으나 통계적으로 유의하지 않았다. 공학계열을 중심으로 적용되고 있는 캡스톤 디자인을 본 연구에서는 간호교육에 적용하고 그 사례를 제시하였다. 추후 연구에서는 간호학 분야에서의 캡스톤 디자인 교육의 다양한 적용가능성과 교과목에 적용 후 그 성과를 분석해 보는 연구가 필요하다.

라돈 농도 분포도 작성을 위한 단변량 크리깅 기법의 정량적 비교 (Quantitative Comparison of Univariate Kriging Algorithms for Radon Concentration Mapping)

  • 토양, 암석, 지하수로부터 실내에 유입되는 라돈은 인간에게 큰 위해를 끼치는 방사능 가스이다. 라돈 가스의 위해성을 확인하기 위해 실내 라돈 농도를 측정해 오고 있는데, 추가적인 분석 수행을 위해서는 신뢰성 높은 분포도 작성이 매우 중요하다. 본 연구에서는 비대칭 분포를 나타내는 라돈 농도의 공간 분포도 작성을 위해 단변량 크리깅 기법들의 비교를 목적으로 정규 크리깅, 비선형 자료 변환 기반의 로그 정규 크리깅, 다중 가우시안 크리깅과 지시자 크리깅의 예측 능력을 비교하였다. 예측 능력을 비교 분석하기 위해 잭나이프 방법을 이용하여 검증을 수행하였으며, 자료 구간별 오차와 샘플링 밀도의 차이에 따른 오차도 추가적으로 분석하였다. 남한 지역을 대상으로 한 사례 연구 결과에서 전반적으로 정규 크리깅에 비해 비선형 자료 변환 기반 크리깅 기법들이 좋은 예측 능력을 보였으며, 비선형 자료 변환 기반 크리깅은 로그 정규 크리깅, 다중 가우시안 크리깅 순으로 좋게 나타났다. 그러나 공간 패턴과 높은 값의 재생산을 고려할 때, 높은 값의 예측 능력은 정규 크리깅이 가장 우수하였다. 본 연구의 결과는 비대칭 분포 자료의 공간 예측을 위한 크리깅 기법의 선정에 유용하게 사용될 것으로 기대된다.
