Study of surgical anatomy of portal vein of liver segments by cast method and its clinical implications

  • Portal vein provides about three-fourths of liver's blood supply. Portal vein is formed behind the neck of pancreas, at the level of the second lumbar vertebra and formed from the convergence of superior mesenteric and splenic veins. The purpose of this study is to review the normal distribution and variation, morphometry of portal vein and its branches for their implication in liver surgery and preoperative portal vein embolization. It is also helpful for radiologists while performing radiological procedures. A total of fresh 40 livers with intact splenic and superior mesenteric vein were collected from the mortuary of Forensic Department, JSS Medical College and Mysuru Medical College. The silicone gel was injected into the portal vein and different segments were identified and portal vein variants were noted. The morphometry of portal vein was measured by using digital sliding calipers. The different types of portal vein segmental variants were observed. The present study showed predominant type I in 90% cases, type II 7.5% cases, and type III 2.5% cases. Mean and standard deviation (SD) of length of right portal vein among males and females were $2.096{\pm}0.602cm$ and $1.706{\pm}0.297cm$, respectively. Mean and SD of length of left portal vein among males and females were $3.450{\pm}0.661cm$ and $3.075{\pm}0.632cm$, respectively. The difference in the Mean among the males and females with respect to length of right portal vein and left portal vein was found to be statistically significant (P=0.010). Prior knowledge of variations regarding the formation, termination and tributaries of portal vein are very helpful and important for surgeons to perform liver surgeries like liver transplantation, segmentectomy and for Interventional Radiologists.

Ergonomic Evaluation of Young Agricultural Operators Using Handle Equipment Through Electromyography and Vibrations Analysis Between the Fingers

  • Background: Agricultural handle equipment is present on all production areas' farms. They are handy and portable; however, excessive use can lead to acute traumas or accidental injuries. Repetitive movements, awkward postures, and hand-arm vibrations predispose them to pain and work-related musculoskeletal disorders. The purpose of this study was to observe the interaction of handle equipment in terms of electromyographic activity and analyze the postural work-related alterations. Materials and methods: Twenty male agricultural operators, mean age 24±1.54 years, underwent the electromyographic analysis testing their muscular activities with a brushcutter, electric saw, and hedge trimmer in four different test conditions. Results: The brushcutter proved to be the agricultural handle equipment with the higher mean frequency (3.37±0.38 Hz) and root mean square (5.25±1.24 ms-2). Furthermore, the digital postural analysis showed a general asymmetry of the main arm and the respective side of the trunk. The head resulted right inclined in the anterior frontal plane by 5.7°±1.2°; the right scapula lower than the left in the posterior frontal plane (8.5°±1.8°), and a working trunk inclination of 34.15°±5.7°. Conclusions: Vibrations of handle equipment and awkward working postures represent a risk for agricultural operators. Preventive measures are required to avoid young operators from experiencing musculoskeletal disorders all lifelong.

Increacing profit of dairy farm through improvement of raw milk quality : According to SCC and mastitis (유질개선을 통한 낙농가 소득증대 - SCC와 유방염을 중심으로-)

  • The study for a effect of monitoring on bovine mastitis was conduced for improvement of raw milk from Jan. to Dec. in 1996. Sampling the milk of 367 cows(1, 406 quarters) from 5 herds in Inchon and were carried out California mastitis test(CMT), somatic cell count(SCC), isolation of pathogens and antibiotic sensitivity tests. The results were summarized as follows, 1. The number of bovine mastitis was 177 cows(48.2%) and 371 quarters(26.4%) : clinical mastitis : 25 cows(6.8%), 32 quarters(2.3%) and subclinicsl mastitis : 152 cows(41.4% ), 339 quarters(24.1%). Incidence rate of mastitis by season were Summer 52.0%, Fall and Winter 48% and Spring 41%. Incidence rate of mastitis by quarters were Summer 30%, Fall 28%, Winter 25% and Spring 21%, respectively. 2. In the distribution of CMT degree by quarter, CMT positive(CMT$\pm$) of 1, 406 quarters milk were 50.1% (704 quarters). The ratio of CMT positivity by quarter were left front quarter 55.8%, right front quarter 48.9%, right hind quarter 48.6% and left hind quarter 47% The ratio of CMT positivity by season were Summer 54.1%, Fall 49.7%, Spring 48.5% and Winter 48% 3. The highest mean SCC by season among 5 herds was "A" herd. Mean SCC (cell/ml) of A herd were Summer 2, 032, 000cells/ml, Fall 1, 109, 000cells/ml, Winter 782, 000cells/ml and Spring 577, 000cells/ml. The lowest mean SCC by season among 5 herds was "E" herds. Mean SCC of E herd were Summer 1, 064, 000cells/ml, Spring 795, 000cells/m1, Fall 429, 000cells/ml and Winter 400, 000cells/ml. Mean SCC of the other herds by season were little difference. 4. The milk samples of "A" herd were collected from 10 cows. In 3 seasons, mean SCC of No. 2 and 3 cows were than 1, 000, 000cells/ml. In 1 season, mean SCC of No. 6, 7 and 8 cows were than 1, 000, 000cells/ml. The more than mean SCC 1, 000, 000cells/ml of cows by season were distributed Summer 4 cows, Winter 3 cows, Spring and Fall 1 cow respectively. The milk samples of "B" herd were collected from 14 cows. In 3 seasons, mean SCC of No. 1 cow was more than 1, 000, 000cells/ml. In 2 seasons, mean SCC of No. 5, 9 and 14 cows were more than 1, 000, 000cells/ml. In 1 season, No. 3, 6 and 7 cows were more than 1, 000, 000cells/ml. The more than mean SCC 1, 000, 000cells/ml of cows by season were distributed Fall and Winter 4 cows respectively, Summer 3 cows and Spring 1 cow. The milk samples of "C" herd were collected from 18 cows. In 2 seasons, mean SCC of No. 16 cow was more than 1, 000, 000cells/ml. In 1 season, mean SCC of No. 1, 2, 6, 7, 13, 15 and 18 cows were more than 1, 000, 000cells/ml respectively. The more than mean SCC 1, 000, 000cells/ml of cows by season were distributed Summer 5 cows, Fall 3 cows, Spring 2 cows and Winter 1 COW. The milk sampes of "D" herd were collected 24 cows. In 3 season, mean SCC of No. 14 cow was more than 1, 000, 000cells/ml. In 2 seasons, mean SCC of No. 14 and 18 cows were more than 1, 000, 000cells/ml. In 1 season, mean SCC of No. 1, 2, 3, 8, 12, 17, 19, 20 and 21 cows were more than 1, 000, 000cells/ml. The more than mean SCC 1, 000, 000cells/ml of cows were distributed Fall 15 cows, Spring and Winter 4 cows respectively and Summer 3 cows. The milk samples of "E" herd were collected from 27 cows. In 2 seasons, mean SCC of No. 6, 7 and 21 cows were more than 1, 000, 000cells/ml. In 1 season, mean SCC of No. 2, 4, 7, 11, 14, 16 and 23 cows were more than 1, 000, 000cells/ml. The more than mean SCC 1, 000, 000cells/ml of cows were distributed Spring and Fall 5 cows respectively, Summer and Winter 2 cows, respectively. 5. The rate of isolated pathogenic microorganisms from bovine mastitis were summarized as follows : Staphylococcus sp 168 strains(45.8%), Streptococcus sp 82 strains(22.3%), Gram(-) sp 45 strains(12.3%), Gram(+) sp 51 strains and the other sp 21 strains(5.7%). 6. The highest of antibiotic sensitivity test of each microorganism was summarized as follows : Staphyolcoccus sp - cephalosporin 76%, gentamicin 55%, Streptococcus sp - ampicillin 61%, cephalosporin 63%, Gram(-) sp - gentamicin 58%, Gram(+) sp - cephalosporin 63%, The other sp - cephalosporin 90%. Microorganisms showed the highest sensitivity(68%) to cephalospsorin. Microorganisms showed the highest sensitivity(68%) to cephalospsorin.

Anatomical Distribution of Colorectal Carcinoma in Iran: A Retrospective 15-yr Study to Evaluate Rightward Shift

  • Background: Although more than two third of colorectal cancers are localized on the left side, recent studies suggest a right ward shift in anatomical distribution with increase in proximal colon cancers. The aim of the present study was to determine the anatomical distribution of colorectal cancer in a referral center over a 15 year period. Method: Records of patients who underwent colectomy in the Cancer Institute of Iran from 1994 to 2009 were retrieved. Data including anatomical localization, year of diagnosis, patient age and gender, tumor histology and differentiation, and disease stage were extracted. Tumors located from the cecum to the distal transverse colon were classified as right side and those occurring from the splenic flexure to the descending colon as left-sided. Cancer of rectum and recto-sigmoid junction were considered as rectal cancers. Results: A total of 442 patients including 220 (49/8%) men and 222 (50/2%) women with mean age 53 were included. Most patients were in stage II &III (47.1% and 33% respectively). There were 157 (35.5 %) colon cancers and 285 (64.5%) rectal cancers. 43.3% of the colon cancers were right sided and 56.7% were left sided. There was no statistically significant increase in right sided cancer during the period of the study. There were no significant differences in age at diagnosis, gender, grade and stage of tumor between the right and the left sided cancers. Conclusion: No proximal shift over time was identified in our study.

A Study on the Difference of Computational Thinking and Attitude according to Left and Right Brain Preference (초등학교 로봇 활용 교육에서 좌우뇌선호에 따른 컴퓨팅사고력, 로봇에 대한 태도의 차이)

  • This study intends to examine the effects of education using robot on computational thinking and attitudes according to left and right brain preference. To execute the study, 17 elementary students at A youth training center in Seoul were selected to participate in the education. And the mean difference was analyzed using matched pair t-test and Mann-Whitey U test. As a result, after participating in the education, there was some statistical significance found in the scores to test student's computational thinking, but not statistical significance on the attitudes. Furthermore, there was no significance found in computational thinking or attitude according to the left and right brain preference. The purpose of this study can be found in that it expands the understanding of education using robot by verifying the efficacy and analyzing differences according to left and right brain preference.

Liver dose reduction by deep inspiration breath hold technique in right-sided breast irradiation

  • Purpose: Deep inspiration breath hold (DIBH) is a well-established technique that enables efficient cardiac sparing in patients with left-sided breast cancer. The aim of the current study was to determine if DIBH is effective for reducing radiation exposure of of liver and other organs at risk in right breast radiotherapy (RT). Materials and Methods: Twenty patients with right-sided breast cancer were enrolled in this study. Three-dimensional conformal RT plans were generated for each patient, with two different computed tomography scans of free breathing (FB) and DIBH. Nodes were contoured according to the Radiation Therapy Oncology Group contouring guidelines. Dose-volume histograms for the target volume coverage and organs at risk were evaluated and analyzed. Results: DIBH plans showed significant reduction in mean liver dose (5.59 ± 2.07 Gy vs. 2.54 ± 1.40 Gy; p = 0.0003), V20Gy (148.38 ± 73.05 vs. 64.19 ± 51.07 mL; p = 0.0003) and V10Gy (195.34 ± 93.57 vs. 89.81 ± 57.28 mL; p = 0.0003) volumes compared with FB plans. Right lung doses were also significantly reduced in DIBH plans. Heart and left lung doses showed small but statistically significant improvement with application of the DIBH technique. Conclusion: We report that the use of DIBH for right-sided breast cancer significantly reduces the radiation doses to the liver, lungs, and heart.

Radionuclide Quantitation of Left-to-Right Cardiac Shunts Using Deconvolution Analysis (Deconvolution 분석(分析)을 이용(利用)한 좌우단락량(左右短絡量) 측정(測定)의 의의(意義))

  • The purpose of this study is to investigate significance of new method for quantification of left to right cardiac shunts using deconvolution analysis as an adjunct to gamma variate analysis in patients with bad bolus injection. In the present study, quantitative radionuclide angiocardiography (QRAC) was performed with and without deconvolution analysis (DA) in 37 patients with left to right shunt and 103 control patients without shunt. The results obtained were as follows; 1) The mean value of $Q_p/Q_s$ in 103 control patients was $1.10{\pm}0.12$ without DA, and was $1.01{\pm}0.03$ with DA. 2) Correlation(r) between oximetry and QRAC with DA was 0.87 and correlation(r) between oximetry and QRAC without DA was 0.61. 3) The 13 patients with left to right shunt, whose $Q_p/Q_s$ was greater than 3.0 by QRAC without DA, was studied by DA. Then the $Q_p/Q_s$ values were measurable in these 13 patients by DA and they showed significant correlation with oximetry. (r=0.68) The results indicate that deconvolution analysis technique for quantification of a left to right shunt provides more reliable and accurate shunt quantification, and reduces the influences of poor bolus injection.

Optimal Height of Shielding Plate of Radiation in Posteroanterior Chest Radiography for Pregnant Women on Korea (한국인 임산부의 흉부 후-전 방향 방사선검사 시 적절한 차폐막 높이)

  • The purpose of this study is to provide the basic data for reducing unnecessary radiation dose to the abdomen and fetus of pregnant women by presenting proper height of shielding protector for efficient abdominal shielding in chest PA examination of Korean pregnant women. The subjects of this study were 288 persons who were eligible for this study among 798 pregnant women who had chest PA examination from January 1, 2015 to December 31, 2016 Retrospective study was performed. Measurements was performed from the apex of the right and left lungs to costophrenic angle of the right and left lungs and to the lowest costophrenic angle among the right and left lungs at the top of the image(this line called Joo's line in this study). The mean of the right and left lung height of pregnant women were 259.09 mm and 263.57 mm, respectively. Also, the average height of the Joo's line designed by the researcher for proper abdominal radiation protection was 322.15 mm. For proper and efficient abdominal radiation protection for pregnant women, it is necessary to adjust the shielding according to the height of the pregnant woman. It is appropriate that the height of the shielding protector should be adjusted so that the upper part of the shield is located at 342.30 mm below from upper part of the detector.

A Survey on the Changes in Industrial Noisy Environment and Rearing loss of Workers (산업장 소음환경과 근로자 청력손실에 변동에 관한 조사)

  • In order to evaluate the noisy environment and hearing loss of workers served in noisy working environment, the author investigated 212 manufacturing industries located in Ulsan Industrial District that could be observed for 3 successive years from 1986 to 1988. The obtained results were as follows: 1. There was increased tendency in the number of workers served in noisy working environment and that of examined of hearing loss for three years. 2. In the noise level of working environment, the number of industries less than 89dB(A) was increased every year, while more than 90dB(A) was in decreasing tendency. 3. Mean hearing loss by frequency was the most prominent in 4,000Hz, the level of hearing loss was in increasing tendency yearly, and that of left eat was higher than right ear in almost all type of industry. 4. In 1986, the level of hearing loss by type of industry was highest in manufacture of electric and electronic, and followed by paper and plywood, and metal products in right ear: that was in the order of manufacture of electric and electronic, metal products and textile products in left ear. In 1987, that was in the order of manufacture of metal products, machinery and others in right ear, and metal products, machinery and food stuff in left ear in 1988, manufacture of others, food stuff and machinery in both ear. 5. In hearing loss by service duration, right ear of 5-9 years group was higher than that of less than 5 years in 1987, whereas in 1988, the longer in service duration, the higher in the level of hearing loss in both ear. 6. In 1986, 1987 and 1988, the prevalence rate of noise-induced hearing loss were showed increasing tendency as 0.4% ,0.8% and 1.5% , respectively, and manufacture of textile products was highest(1.0%) in 1986, machinery(1.2%) in 1987 and others(2.8%) in 1988. 7. The proportion of grade E in early loss index were 76.1% (1986), 78.2% (1987) and 80.5% (1988) in left ear, 75.9% (1986), 76.4% (1987) and 75.9% (1988) in right ear.

Shape and anatomical relationship of the mental foramen to the mandibular premolars in an Indian sub-population: a retrospective CBCT analysis

  • Objectives: This study assessed the shape and anatomical relationship of the mental foramen (MF) to mandibular posterior teeth in an Indian sub-population. Materials and Methods: In total, 475 existing cone-beam computed tomography records exhibiting 950 MFs and including the bilateral presence of mandibular premolars and first molars were assessed. Images were evaluated 3-dimensionally to ascertain the position, shape, and anatomical proximity of MFs to mandibular teeth. The position and shape of MFs were measured and calculated. The Pythagorean theorem was used to calculate the distance between the root apex of the mandibular teeth and the MF. Results: MFs exhibited a predominantly round shape (left: 67% and right: 65%) followed by oval (left: 30% and right: 31%) in both males and females and in different age groups. The root apices of mandibular second premolars (left: 71% and right: 62%) were closest to the MF, followed by distal to the first premolars and mesial to the second premolars. The mean vertical distance between the MF and the nearest tooth apex calculated on sagittal sections was 2.20 mm on the right side and 2.32 mm on the left side; no significant difference was found according to sex or age. The distance between the apices of the teeth and the MF was ≥ 4 mm (left; 4.09 ± 1.27 mm and right; 4.01 ± 1.15 mm). Conclusions: These findings highlight the need for clinicians to be aware of the location of the MF in treatment planning and while performing non-surgical and surgical endodontic procedures.
