상악동저 거상술 후 Osstem Implant (US II / SS II)의 다기관 후향적 임상연구 (A RETROSPECTIVE MULTICENTER CLINICAL STUDY OF INSTALLED US II / SS II IMPLANTS AFTER MAXILLARY SINUS FLOOR ELEVATION)

  • Purpose: The purpose of this study was to evaluate the $Osstem^{(R)}$ implants (US II/SS II implants) through the retrospective study for the clinical success rate during the installation of the $Osstem^{(R)}$ implants (US II/SS II implants) by using of the procedures of maxillary sinus floor elevation. Materials and methods: The current study was researched in the 6 medical institutions: Chonnam National University, Chosun University, Pusan DaeDong Hospital, Bundang Seoul National University Hospital, Ap-Seon Clinic, and All Clinic. Based on the total number of 116 patients whose treatment was the installation of the US II/SS II implants with the procedures of the maxillary sinus floor elevation, they were conferred on the dental records of the patients under the joint consultation of the 6 medical institutions. On the dental recording charts, there were included in as the following; the name of the institutions, gender, age, with or without smoking or drinking, with or without the generalized diseases, the height of the alveolar bone on the operational sites, elapsed edentulous state period, the state of the opposed or adjacent teeth, the methods of the maxillary sinus floor elevation, secondary time period for surgery, the lengths, types, and diameters of implants, with or without bone transplantation or the types of bone, postoperative current bone height, current adjacent soft tissue state of the implants, with or without the success of the installations of the implants. We have done our survey with the clinical and radiolographical examinations and dental questionaries. The success and survival rate of the implants was evaluated. Results 1. Total number of the patients with the installation of the US II implants were 62. The 252 numbers of US II implants were installed on the 89 maxillary sinuses. The patient's mean age was 54.1 years old and there were 36 men and 27 women. 2. Total number of patients with the installation of SS II implant were 57. The 165 numbers of SS II implants were planted on the 80 maxillary sinuses. Their mean age was 48.7 years old and there were 37 men and 20 women. 3. The follow-up period was 30.7 months(21-49 mon) on average. The vertical bone loss of installed implants after the procedures of the maxillary sinus elevation was 1.1 mm on average in SS II and 1.3 mm on average in US II. There existed no statistical significance on each group. The mean enlarged bone height after the maxillary sinus floor elevation was 8.2 mm. 4. For the procedures of the maxillary sinus elevation, the Lateral approach technique occupied 87.1%, which was the most used one. In addition, the most frequently used transplanted bone was autogenous bone only which was 72.7% during the maxillary sinus floor elevation. 5. The complication of maxillary sinus floor elevation were perforation of sinus membrane, disesthesia on doner site, exposure of cover screw and exposure of maxillar bone. 6. The survival rate of US II and SS II after maxillary sinus floor elevation was 99.2% and 95.8%, respectively. And the success rate of US II and SS II after maxillary sinus floor elevation was 97.6% and 89.7%, respectively. Conclusion : On the evaluation of the analysis of our study, both US II and SS II implants showed the excellent clinical results by use of the procedures of maxillary sinus floor elevation.

Br${\aa}$nemark 임플란트의 15년 임상적 후향 연구 (A 15-year clinical retrospective study of Br${\aa}$nemark implants)

  • 연구 목적: 이 연구는 Br${\aa}$nemark 임플란트의 후향적인 연구를 통해 machined 임플란트 의 15년, TiUnite$^{TM}$ 임플란트의 5년 누적 생존율을 구하여 비교하고, 위험 요소와의 상관관계를 밝히고자 하였다. 연구 재료 및 방법: 본 연구는 1993년부터 2008년까지 고려대 구로병원 임플란트클리닉에서 임플란트 치료를 받은 환자를 대상으로 하여, 환자의 임상기록을 토대로 조사하였고, 임플란트 일차수술 후 즉시 부하를 가한 경우와 임플란트 보철물이 다른 임플란트 시스템과 결합한 경우는 제외하였다. 15년 간 총 155명의 환자에게 541개의 Br${\aa}$nemark 임플란트 중, 264개의 machined 임플란트와 277개의 TiUnite$^{TM}$ 임플란트를 식립하였고, 임플란트 수술 및 보철수복 술식은 Adell 등이 제안한 방법에 따랐다. 6개월에서 1년 간격으로 Follow-up을 위한 재내원 기간 동안 임플란트에 대한 임상검사를 실시하였다. 본 연구의 관찰 기간은 1차 수술일로부터 2008년 12월 이내의 최종 내원일까지였고, 임상검사시 동요도, 타진, screw loosening, 그리고 환자의 불편감 등을 관찰하고, Zarb와 Albreksson에 의한 임플란트 생존 범주를 근거로 누적생존률을 평가하였다. 임플란트 누적생존율(CSR)이 Kaplan Meier estimate를 이용하여 얻었으며, 각각의 위험요소가 누적생존율에 미치는 영향을 Cox proportional hazards regression을 이용하여 분석하였다(${\alpha}$=.05). 결과: 전체 Br${\aa}$nemark 임플란트의 15년 누적생존율은 86.07%이었고, machined 임플란트의 경우 15년 누적생존율은 82.89%, 5년 누적생존율은 89.21%였으나 Tiunite$^{TM}$ 임플란트의 5년 누적생존율은 98.74%로 machined surface가 Tiunite$^{TM}$ 임플란트에 비해 실패율이 4.6배 정도 더 높았다. 전신질환을 가진 환자의 경우 실패율이 32% 더 증가하였고, 상악동 거상술 또는 골이식을 동반한 수술 등 부가적인 수술을 받은 경우 임플란트 실패율이 40% 더 증가하여 위험 요소와 임플란트의 생존율은 상관 관계가 있었다. Kennedy 분류에 따른 부분 무치악부의 위치에 따라 임플란트 실패율이 통계적으로 유의하게 달랐으며, 소구치와 구치부가구치부가 전치부에 비해 임플란트 실패율이 높게 나타났다. 결론: Br${\aa}$nemark machined 임플란트와 TiUnite$^{TM}$ 임플란트는 모두 우수한 임상적 결과를 보였으나, TiUnite$^{TM}$ 임플란트가 더 성공적이었으며, 임플란트의 누적 생존율은 위험요소와 연관성이 있었다.

확장시킨 경중격 절개방식을 통한 승모판 수술의 80례 임상 분석 (Clinical Study of 80 Cases of Mitral Valve Operations Via Extended Transseptal Approach)

  • 연구배경 : 승모판의 노출을 위한 여러 접근방식 중, 확장시킨 경중격 절개방식은 다른 방법에 비하여 쉽고 빠르게 승모판막에 접근할 수 있으며 모든 판막구조물의 노출이 잘 되어 월등한 방법이라 할 수 있다. 그러나, 이러한 장점에도 불구하고 혹자는 광범위한 절개 및 동방결절 동맥의 손상으로 치명적인 부정맥이 발생하지 않을까 우려하여 이용을 기피하는 경향이 있고 반론을 재기하는 사람들도 많이 있다. 재료 및 방법 : 따라서 본 교실에서는 1992년 9월부터 1997년 7월까지, 모두 80명의 환자에서 확장시킨 경중격 절개방식을 적용한 후, 어떠한 장·단점이 있나 알아보고 수술후 심장율동에 미치는 영향을 알아보았다. 결과 : 78명의 환자가 승모판 치환수술을 받았고 2명은 좌심방 점액종 제거수술을 받았다. 승모판 치환수술을 받은 78명중 38명의 환자가 동시에 대동맥판 치환수술(n=22), 삼첨판성형술(n=14), 관상동맥 우회로조성술(n=1), 심실중격결손증 교정수술(n=1) 등을 받았다. 수술 후 총 74명에서 평균 23.3개월간 외래추적관찰을 하였다. 병원사망률은 3.8%(3례)를 보였고, 술전 심방세동을 보이던 46명의 환자중 1명이 수술후 정상 동조율로 전환 되었으며, 술전 정상동조율을 보였던 34명은 술후에도 계속 정상 동조율을 유지하였다. 평균 대동맥 차단시간은 71분이었고, 승모판막 단일 수술인 경우는 평균 62분이었다. 수술후 가장 흔한 부정맥은 서맥으로 일시적인 심장조율로 호전되었으며 다른 수술과 연관된 특별한 합병증은 없었다. 결론 : 승모판막수술시 확장시킨 경중격 절개방식의 적용으로 특별한 합병증 없이 좋은 결과를 얻을 수 있고 특히 좌심방의 크기가 작은 경우 많은 도움이 될 것으로 사료된다

경도의 방광요관역류가 있는 소아에서 99mTc-dimercaptosuccinic acid 신 스캔의 필요성 (Necessity of 99mTc-dimercaptosuccinic acid scan in infants with low grade vesicoureteral feflux)

  • 목 적 : DMSA 신 스캔은 신장에 대한 높은 방사능의 위험성과 낮은 grade의 역류시 불필요한 것으로 인식되어 최근 연구들에서는 낮은 방광요관역류가 있을 시에는 조기검사에 포함시키지 않는 것으로 보고되고 있다. 그러나 실제 임상에서는 낮은 grade의 역류 환아에서도 신피질결손이 보이는 경우가 있어 DMSA 신 스캔의 조기검사로서의 필요성에 대해 재조명하여 보고자 하였다. 방 법 : 1995년부터 2004년까지 상계백병원 소아과에서 요로 감염으로 진단된 환아들 중 신장초음파, DMSA 신 스캔, 배뇨성 방광 요도 조영술을 모두 시행한 189명의 환아를 대상으로 하였다. 검사들은 요로감염으로 진단된 지 적어도 1개월 이내에 시행하였다. 방광요관역류의 정도는 grade I-II는 경도(low grade), grade III은 중등도(moderate grade), grade IV-V는 중증(high grade)으로 각각 분류하였다. 결 과 : DMSA 신 스캔상 신피질결손은 총 189명 중 67명, 378 신단위 중 82 신단위에서 보였다. 신피질결손의 빈도는 역류가 없을 때 28%, 경도일 때 38%, 중등도일 때 53%, 중증일때 100%로 역류가 심할수록 신피질결손의 빈도도 높아지는 것으로 나타났다(P<0.01). 중등도일 때의 신피질결손의 빈도는 경도일 때의 신피질결손의 빈도와 의미있는 차이는 없었다(P>0.05). 결 론 : 본 연구에서는 역류가 없을 때와 경도의 역류일 때의 신피질결손의 빈도가 28%, 38%로 많은 비율을 차지하고 있고, 중등도일 때의 신피질결손의 빈도는 경도일 때와 유의한 차이를 보이지 않았다. 따라서 DMSA 신 스캔은 높은 방사선 조사율에도 불구하고 역류가 없거나 낮은 요로감염 환아에서도 조기검사로 시행해야 한다.

부적절한 일차 처치와 연관된 이하선 종양 2례 (Tumors of the Parotidomasseteric Area Associated with Inadequate Primary Treatment: Report of 2 Cases)

  • Purpose: Parotid neoplasia are relatively frequent, representing approximately 3% of all tumors in the head and neck regions. But incomplete resection and misdiagnosis of parotid gland is followed by multiple tumor invasion, tumor recurrence, and other iatrogenic tumor formation. In patients undergoing parotidectomy for confirmed or suspected malignancy, the traditional or modified rhytidectomy incision may prove suboptimal because it does not easily lend itself to a continuous neck dissection. Similarly, patients with tumors of the anterior accessory lobe or patients with large anterior tumors may also require the modified Blair incision for adequate surgical exposure. This report serves to revisit the topic of accessory and parotid gland neoplasms to emphasize proper management, particularly the surgical aspects, so that consequences of recurrence are avoided. Methods: This is a retrospective review of our experience with two cases of parotid tumors; one accessory parotid gland neoplasm and one parotid gland neoplasm. We report the case of parotid tumor and epidermal cyst in a 54-year old male patient and the case of case of recurrent parotid tumor with local invasion in 30-year old male patient. Results: All were removed through a modified Blair incision. Pathologic report notified that One was found pleomorphic adenoma and epidermal cyst, and the other one pleomorphic adenoma with subcutenous invasion. The patients recovered well without any complication such as infection, hematoma, facial nerve palsy, and necrosis of skin flap. Patients were discharge POD#7. Patients were followed up to for 1 year and they have no sign of recurrence. Conclusions: A high index of suspicion, prudent diagnostic skills(including fine-needle aspiration biopsy, CT, US), and meticulous surgical approach are the keys to a successful management of these lesions. We experienced two cases of parotid neoplasia, in the treatment of tumor reccurence & iatrogenic tumor arising from the parotid gland and are presented with the review of literatures.

응급 수술을 시행한 신경외상 환자들에 있어 전염성 바이러스 감염의 유병율에 대한 분석 (Seroprevalence of Viral Infection in Neurotrauma Patients Who Underwent Emergent Surgical Intervention)

  • Purpose: The aim of this study was to estimate the seropositive prevalence of blood-borne infection in neurotrauma patients who underwent emergent surgical intervention, especially patients with hepatitis B virus (HBV), hepatitis C virus (HCV), syphilis and human immunodefIciency virus (HIV). Methods: A retrospective review identified 559 patients with traumatic brain injury and spinal trauma who underwent emergent surgery between 2007 and 2014. We reviewed the medical records and extracted data, including age, sex, location of lesion, result of serologic tests, time interval of admission and surgery after presenting to emergency room. Serologic tests for HBV, HCV, syphilis and HIV were performed and analyzed to determine whether the seropositive results were confirmed by the surgeon before surgery. Results: The majority of the patients were male (74.6%), and the mean age was $55.4{\pm}20.2years$. Most patients underwent surgery due to traumatic brain injury (90.0%). Fifty-three patients (10.0%) showed a positive result on at least one serologic test. Seropositive rates according to pathogens were 0.5% for syphilis, 5.2% for HBV and 3.9% for HCV. No positive results were noted on the serologic tests for HIV. HBV in patients with spinal cord injury and age from 40 to 49 years were associated with high serologic positive rate, and that result was statistically significant. However, no statistically significant differences were found in the other variables. Serologic results could not confirmed before surgery in the majority of the cases (62.1%), and 10.4% of these patients showed seropositive results. Conclusion: The results of this study emphasize the importance of taking precautions and conducting rapid serologic testing in preventing the occupational transmission of blood-borne viruses to health-care workers.

Functional outcome predictors following mandibular reconstruction with osteocutaneous fibula free flaps: correlating early postoperative videofluoroscopic swallow studies with long-term clinical results

  • Background: Advancements in the field of microvascular surgery and the widespread adoption of microvascular surgical techniques have made the use of osteocutaneous fibula free flaps the standard of care in the surgical management of segmental mandibular defects. Although the literature possesses abundant evidence to support the effectiveness of fibula free flaps as a reconstructive method, there are relatively few studies reporting on outcomes as objectively measured by videofluoroscopic swallowing studies (VFSS). The purpose of this study is to explore the potential correlation between early postoperative VFSS and the long-term swallowing outcomes in patients who underwent mandibular reconstruction with fibula free flaps. Methods: We performed a retrospective chart review of 36 patients who underwent mandibular reconstruction with osteocutaneous fibular free flaps between 2009 and 2012. Demographics, clinical variables, VFSS data, and diet information were retrieved. Penetration and aspiration findings on VFSS, long-term oral feeding ability, and the need for gastrostomy tube were statistical endpoints correlated with postoperative clinical outcomes. Results: Thirty-six patients were reviewed (15 females and 21 males) with a mean age of 54 years (7-81). Seventeen cases were treated for malignancy. The size of the bony defect ranged from 3 to 15 cm (mean = 9 cm). The cutaneous paddle, a surrogate for soft tissue defect, ranged from 10 to 125 ㎠ (mean = 52 ㎠). A gastrostomy tube was present in patients preoperatively (n = 8), and postoperatively (n = 14). Seventeen patients had neoadjuvant exposure to radiation. Postoperative VFSS showed penetration in 13 cases (36%) and aspiration in seven (19%). Overall, 29 patients (80.6%) achieved unrestricted diet, and this was statistically correlated with age (p = 0.037), radiation therapy (p = 0.002), and preoperative gastrostomy tube (p = 0.03). The presence of penetration or aspiration on VFSS was a strong predictor for long-term unrestricted oral diet (p < 0.001). Conclusion: Early postoperative VFSS is an excellent predictor for long-term swallowing outcomes in patients undergoing mandibular reconstruction with osteocutaneous fibula free flaps.

Spontaneous bone regeneration after surgical extraction of a horizontally impacted mandibular third molar: a retrospective panoramic radiograph analysis

  • Background: The mandibular third molar (M3) is typically the last permanent tooth to erupt because of insufficient space and thick soft tissues covering its surface. Problems such as alveolar bone loss, development of a periodontal pocket, exposure of cementum, gingival recession, and dental caries can be found in the adjacent second molars (M2) following M3 extraction. The specific aims of the study were to assess the amount and rate of bone regeneration on the distal surface of M2 and to evaluate the aspects of bone regeneration in terms of varying degree of impaction. Methods: Four series of panoramic radiographic images were obtained from the selected cases, including images from the first visit, immediately after extraction, 6 weeks, and 6 months after extraction. ImageJ software® (NIH, USA) was used to measure linear distance from the region of interest to the distal root of the adjacent M2. Radiographic infrabony defect (RID) values were calculated from the measured radiographic bone height and cementoenamel junction with distortion compensation. Repeated measures of analysis of variance and one-way analysis of variance were conducted to analyze the statistical significant difference between RID and time, and a Spearman correlation test was conducted to assess the relationship between Pederson's difficulty index (DI) and RID. Results: A large RID (> 6 mm) can be reduced gradually and consistently over time. More than half of the samples recovered nearly to their normal healthy condition (RID ≤ 3 mm) by the 6-month follow-up. DI affected the first 6 weeks of post-extraction period and only showed a significant positive correlation with respect to the difference between baseline and final RID. Conclusions: Additional treatments on M2 for a minimum of 6 months after an M3 extraction could be recommended. Although DI may affect bone regeneration during the early healing period, further study is required to elucidate any possible factors associated with the healing process. The DI does not cause any long-term adverse effects on bone regeneration after surgical extraction.

Sandblasted, Large-grit and Acid-etched Implant에 대한 후향적 임상 연구 (Retrospective Study of Sandblasted, Large-grit and Acid-etched Implant)

  • Purpose: This study evaluated the prognosis and survival rate of SLA (Sandblasted, Large-grit and Acid-etched) implants and it also evaluated the prosthodontic complications and the associated factors. Methods: Twenty seven patients (14 men and 13 women, mean age: 54.9) who visited Chosun University Hospital Implant Center with the chief desire for placement of an implant in an edentulous area from March, 2008 to December 2008 and who received placement of a SLA implant ($Implantium^{(R)}$, Dentium Co., Korea) were selected for this study. Results: The average follow-up period was 15 months and the study was based on the treatment records, radiographs and clinical examinations. A total of 69 implant cases were retrospectively assessed for the width and length of the implant, the primary and secondary stability, the combined surgery, the employed bone graft material and barrier membrane, the status of the opposing tooth, implant failure and the prosthetic complications. During the follow-up period (average: 15 months), the accumulative survival rate of the 69 implants in 27 patients was 100%. Complications such as infection, sinusitis and fixture exposure after surgery were seen for 5 implants in 4 patients. Complications such as screw loosening, contact loosening and peri-implant gingivitis after prosthodontic treatment occurred in 7 cases (10.14%). Conclusion: This study reports placement of SLA implants may cause various complications, yet the final accumulative survival rate was 100%. The SLA implant ($Implantium^{(R)}$) has an excellent clinical survival rate and outcome.

Elbow Reconstruction Using Island Flap for Burn Patients

  • Background Deep burns of the elbow lead to soft tissue necrosis and infection, with exposure of deep structures. Adequate wound coverage of this area requires thin, pliable, and durable tissue, while optimal functional recovery requires early coverage and functional rehabilitation. We have found 3 types of island flaps that provide reliable coverage for the elbow. Methods A retrospective study was performed on all patients who underwent flap coverage of an elbow defect at our hospital. The patients' data including age, sex, cause of injury, wound dimensions, timing of flap coverage, postoperative elbow motion, and complications were investigated. Results Between 2001 and 2012, 16 patients were treated at our hospital. The mean age was 53.3 years. Three kinds of flaps were performed: 9 latissimus dorsi flaps, 4 lateral arm flaps, and 4 radial forearm flaps. The average defect size was 183.5 $cm^2$ (range, 28 to 670 cm2). Wound coverage was performed at mean duration of 45.9 days (range, 14 to 91 days). The mean postoperative active elbow flexion was $98^{\circ}$ (range, $85^{\circ}$ to $115^{\circ}$). Partial flap failure occurred in 1 latissimus dorsi flap. Minor complications included partial flap loss (11.8%), hematoma (23.5%), seroma (35.3%), and wound infection (5.9%). Conclusions Flap selection for elbow reconstruction is determined by the defect size and the extent of the adjacent tissue injury. Elbow reconstruction using an island flap is a single-staged, reliable, and relatively simple procedure that permits initiation of early rehabilitation, thereby improving a patient's functional outcome.
