완전삽입형 인공망막 구현을 위한 인공망막모듈 개발 (Development of Retinal Prosthesis Module for Fully Implantable Retinal Prosthesis)

  • To restore visual sensation of blind patients, we have proposed a fully implantable retinal prosthesis comprising an three dimensionally (3D) stacked retinal chip for transforming optical signal to electrical signal, a flexible cable with stimulus electrode array for stimulating retina cells, and coupling coils for power transmission. The 3D stacked retinal chip is consisted of several LSI chips such as photodetector, signal processing circuit, and stimulus current generator. They are vertically stacked and electrically connected using 3D integration technology. Our retinal prosthesis has a small size and lightweight with high resolution, therefore it could increase the patients` quality of life (QOL). For realizing the fully implantable retinal prosthesis, we developed a retinal prosthesis module comprising a retinal prosthesis chip and a flexible cable with stimulus electrode array for generating optimal stimulus current. In this study, we used a 2D retinal chip as a prototype retinal prosthesis chip. We fabricated the polymide-based flexible cable of $20{\mu}m$ thickness where 16 channels Pt stimulus electrode array was formed in the cable. Pt electrode has an impedance of $9.9k{\Omega}$ at 400Hz frequency. The retinal prosthesis chip was mounted on the flexible cable by an epoxy and electrically connected by Au wire. The retinal prosthesis chip was cappted by a silicone to pretect from corrosive environments in an eyeball. Then, the fabricated retinal prosthesis module was implanted into an eyeball of a rabbit. We successfully recorded electrically evoked potential (EEP) elicited from the rabbit brain by the current stimulation supplied from the implanted retinal prosthesis module. EEP amplitude was increased linearly with illumination intensity and irradiation time of incident light. The retinal prosthesis chip was well functioned after implanting into the eyeball of the rabbit.

Regeneration of the retina: toward stem cell therapy for degenerative retinal diseases

  • Degenerative retinal diseases affect millions of people worldwide, which can lead to the loss of vision. However, therapeutic approaches that can reverse this process are limited. Recent efforts have allowed the possibility of the stem cell-based regeneration of retinal cells and repair of injured retinal tissues. Although the direct differentiation of pluripotent stem cells into terminally differentiated photoreceptor cells comprises one approach, a series of studies revealed the intrinsic regenerative potential of the retina using endogenous retinal stem cells. Muller glial cells, ciliary pigment epithelial cells, and retinal pigment epithelial cells are candidates for such retinal stem cells that can differentiate into multiple types of retinal cells and be integrated into injured or developing retina. In this review, we explore our current understanding of the cellular identity of these candidate retinal stem cells and their therapeutic potential for cell therapy against degenerative retinal diseases. Functional Connectivity Map of Retinal Ganglion Cells for Retinal Prosthesis

  • Retinal prostheses are being developed to restore vision for the blind with retinal diseases such as retinitis pigmentosa (RP) or age-related macular degeneration (AMD). Among the many issues for prosthesis development, stimulation encoding strategy is one of the most essential electrophysiological issues. The more we understand the retinal circuitry how it encodes and processes visual information, the greater it could help decide stimulation encoding strategy for retinal prosthesis. Therefore, we examined how retinal ganglion cells (RGCs) in in-vitro retinal preparation act together to encode a visual scene with multielectrode array (MEA). Simultaneous recording of many RGCs with MEA showed that nearby neurons often fired synchronously, with spike delays mostly within 1 ms range. This synchronized firing - narrow correlation - was blocked by gap junction blocker, heptanol, but not by glutamatergic synapse blocker, kynurenic acid. By tracking down all the RGC pairs which showed narrow correlation, we could harvest 40 functional connectivity maps of RGCs which showed the cell cluster firing together. We suggest that finding functional connectivity map would be useful in stimulation encoding strategy for the retinal prosthesis since stimulating the cluster of RGCs would be more efficient than separately stimulating each individual RGC.

Investigation of the effects of temporomandibular joint arthrocentesis on blood volume of the retinal structures

  • Objective: Arthrocentesis is a minimally invasive surgical procedure that is used to alleviate the symptoms of temporomandibular joint (TMJ) disorders. The aim of this study was to investigate the effect of arthrocentesis on the blood supply to the retinal structures. Materials and Methods: Arthrocentesis was performed on 20 patients with TMJ disorders, and choroidal thickness (CT) in patients was measured to evaluate retinal blood circulation. The blood volume of the retinal structures was evaluated ipsilaterally before and after arthrocentesis, and these measurements were then compared with measurements obtained from the contralateral side. Results: Before arthrocentesis, there were no differences in retinal blood volumes between the ipsilateral and contralateral sides (P = 0.96). When ipsilateral CT measurements taken before and after arthrocentesis were compared, retinal blood supply was found to have significantly decreased after arthrocentesis (P = 0.04). When contralateral CT measurements taken before and after arthrocentesis were compared, retinal blood supply was also found to have decreased after arthrocentesis, but not significantly (P = 0.19). Conclusion: The solution of local anesthesia with epinephrine applied before the arthrocentesis procedure was found to reduce the blood volume of the retinal structures. To the best of our knowledge, this is the first study that has investigated the blood volume of the retinal structures following arthrocentesis.

  • The effect of vesicular L-$\alpha$-phosphatidylcholine on the rate of formation of all-trans-N-retinylidene-n-butylamine (3) and on the regeneration kinetics of bacteriorhodopsin pigment from retinal and bacterio-opsin have been studied. An estimate of the relative positions of retinal and n-butylamine in the vesicles has been made by fluoresence quenching experiments. Partition coefficient of retinal and microviscosity of the retinal-binding region have also been determined. The results are discussed in terms of the nature of chemical interaction between retinal and amine in a lipid environment.

A Case Report of Intravitreal Dexamethasone Implant with Exudative Retinal Detachment for Ocular Toxocariasis Treatment

  • Toxocariasis is one of the most common geohelminth infections in several parts of the world. We describe a rare case of ocular toxocariasis with secondary exudative retinal detachment treated with albendazole and an intravitreal dexamethasone implant. A 13-year-old boy with counting finger vision was diagnosed with retinal vasculitis and exudative retinal detachment in his right eye. Fundoscopic examination revealed retinal hemorrhage, retinal vasculitis, and exudative retinal detachment. Serological test using serum and intraocular aqueous humor were positive for anti-Toxocara specific IgG antibodies. He received repeated doses of intravitreal dexamethasone implants combined with oral albendazole. A sequential follow-up optical coherence tomography revealed that the retina was successfully reattached. His visual acuity subsequently improved to 20/400.

Automatic Segmentation of Retinal Blood Vessels Based on Improved Multiscale Line Detection

  • The appearance of retinal blood vessels is an important diagnostic indicator of serious disease, such as hypertension, diabetes, cardiovascular disease, and stroke. Automatic segmentation of the retinal vasculature is a primary step towards automatic assessment of the retinal blood vessel features. This paper presents an automated method for the enhancement and segmentation of blood vessels in fundus images. To decrease the influence of the optic disk, and emphasize the vessels for each retinal image, a multidirectional morphological top-hat transform with rotating structuring elements is first applied to the background homogenized retinal image. Then, an improved multiscale line detector is presented to produce a vessel response image, and yield the retinal blood vessel tree for each retinal image. Since different line detectors at varying scales have different line responses in the multiscale detector, the line detectors with longer length produce more vessel responses than the ones with shorter length; the improved multiscale detector combines all the responses at different scales by setting different weights for each scale. The methodology is evaluated on two publicly available databases, DRIVE and STARE. Experimental results demonstrate an excellent performance that approximates the average accuracy of a human observer. Moreover, the method is simple, fast, and robust to noise, so it is suitable for being integrated into a computer-assisted diagnostic system for ophthalmic disorders.

  • 개개의 망막신경절세포는 자신이 담당하고 있는 망막의 특정부위에 빛자극이 가해지면 그 빛자극의 특징을 활동전위의 형태로 인코딩하게 된다. 이때 개개의 망막신경절세포가 담당하고 있는 망막의 특정부위를 감수야(receptive field)라 부른다. 그러므로 망막신경절세포의 전기적 특성을 파악하기 위해서는 감수야의 위치를 규정하는 작업이 반드시 필요하다. 그 이유는 감수야의 배열 상태를 알게 되면 신경절세포가 어떻게 시각자극을 인코딩하는지 그 메커니즘에 관한 통찰이 가능하기 때문이다. 본 논문에서는 무작위 바둑판 자극을 MEA의 개개 채널에 독립적으로 인가함과 동시에 여러 망막신경절세포의 흥분파를 기록하였다. 이후 오프라인에서 망막신경절세포의 파형을 추출하고 ON-cell, OFF-cell, ON/OFF-cell로 분류한 후 개개의 망막신경절세포의 감수야를 WATLAB을 이용하여 구현하여 보았다. 이런 방식으로 재구성된 ON-cell과 OFF-cell의 감수야의 예를 제시한다.

Diversity and Function of Retinal-binding Protein in Photosynthetic Microbes

  • Photosynthetic microbes possess a wealth of photoactive proteins including chlorophyll-based pigments, phototropin-related blue light receptors, phytochromes, and cryptochromes. Surprisingly, recent genome sequencing projects discovered additional photoactive proteins, retinal-based rhodopsins, in cyanobacterial and algal genera. Most of these newly found rhodopsin genes and retinal synthase have not been expressed and their functions are unknown. Analysis of the Anabaena and Chlamyrhodopsin with retinal synthase revealed that they have sensory functions, which, based on our work with haloarchaeal rhodopsins, may use a variety of signaling mechanisms. Anabaena rhodopsin is believed to be sensory, shown to interact with a soluble transducer and the putative function is either chromatic adaptation or circadian rhythm. Chlamydomonas rhodopsins are involved in phototaxis and photophobic responses based on electrical measurements by RNAi experiment. In order to analyze the protein, we developed a sensory rhodopsin expression system in E. coli. The opsin in E. coil bound endogenous all-trans retinal to form a pigment and can be observed on the plate. Using this system we could identify retinal synthase in Anabaena PCC 7120. We conclude that Anabaena D475 dioxygenase functions as a retinal synthase to the Anabaena rhodopsin in the cell.

Ocular coherence tomography를 이용한 개의 망막변성증 평가 (Assessment of retinal degeneration with optical coherence tomography in a dog)

  • To report retinal image of in a 6-year-old male castrated poodle dog with a 2-month history of nyctalopia using optical coherence tomography (OCT). Ocular reflexes were present in both eyes and slit lamp examination showed anterior subscapular cataract in the right eye. There were fundus abnormalities in both eyes similar to retinal degeneration. Scotopic electroretinograms (ERGs) revealed significantly subabnormal amplitudes and prolonged implicit time, whereas photopic ERGs were better maintained, although far from normal. OCT of affected dogs revealed generalized retinal thinning much more than a normal age-matched dog. Therefore, OCT scanning is considered to be a useful method for retinal evaluation in dogs with retinal degeneration.
