Explosions inside transportation tunnels might result in failure of tunnel structures. This study investigated the failure mechanisms of circular cast-iron tunnels in saturated soil subjected to medium internal blast loading. This issue is crucial to tunnel safety as many transportation tunnels run through saturated soils. At the same time blast loading on saturated soils may induce residual excess pore pressure, which may result in soil liquefaction. A series of numerical simulations were carried out using Finite Element program LS-DYNA. The effect of soil liquefaction was simulated by the Federal Highway soil model. It was found that the failure modes of tunnel lining were differed with different levels of blast loading. The damage and failure of the tunnel lining was progressive in nature and they occurred mainly during lining vibration when the main event of blast loading was over. Soil liquefaction may lead to more severe failure of tunnel lining. Soil deformation and soil liquefaction were determined by the coupling effects of lining damage, lining vibration, and blast loading. The damage of tunnel lining was a result of internal blast loading as well as dynamic interaction between tunnel lining and saturated soil, and stress concentration induced by a ventilation shaft connected to the tunnel might result in more severe lining damage.
Purpose: The occurrence ranks of failure modes can come from the real failure but the severity ranks of failure modes require a highly subjective point of view of users. The severity ranks have to find more objective and scientific values. Methods: We found the optimal values by using the correlation analysis between failure mode effects and the criticality number like RPN (Risk Priority Number) in RCM. Result: This paper shows the result that verified whether the weighted values on each failure effect in criticality number calculation is suitable to the actual failures or not. To get the verification, it used the 5 year data and correlation analysis. Based on the analyzed result, We proposed the more suitable values. Conclusion: This correlation analysis approach can provide guidance of RCM analysis across many industries and situations.
This study aims to quantitatively evaluate failure pressure of wall-thinned elbow under combined load along with internal pressure, by conducting real-scale burst test and finite element analysis together. For quantitative evaluation, failure pressure data was extracted from the real-scale burst test first, and then finite element analysis was carried out to compare with the test result. For the test, the wall-thinning defect of the extrados or intrados inside the center of 90-degree elbow was considered and the loading modes to open or close the specimen maintaining a certain load or displacement were applied. Internal pressure was applied until failure occurred. As a result, when the bending load was applied under the load control condition, the intrados of the defect was more affected by failure pressure than the extrados, and the opening mode was more vulnerable to failure pressure than the closing mode. When the bending load was applied under the displacement control, it was hardly affected by failure pressure though it was slightly different from the defect position. The result of the finite element analysis showed a similar aspect with the test. Moreover, when major factors such as material properties and pipeline thickness were calibrated to accurate values, the analytical results was more similar to the test results.

In this paper, the Time-varying Failure Rates(TFR) of power distribution system components are extracted from the recorded failure data of KEPCO(Korea Electric Power Corporation) and the reliability of power distribution system is evaluated using Mean Failure Rate(MFR) and TFR. The TFR is approximated to bathtub curve using the exponential and Weibull distribution function. In addition, Kaplan-Meier estimation is applied to TFR extraction because of incomplete failure data of KEPCO. Also the reliability of the real power distribution system of Korea is evaluated using the MFR and TFR extracted from real failure data, respectively and the results of each case are compared with each other. As a result, it is proved that the reliability evaluation using the TFR is more realistic than MFR. In addition, it is presented that the application method at power distribution system maintenance and repair using the result of TFR.
For severe accident assessment of reactor pressure vessel (RPV), it is important to develop an accurate model that can predict transient thermo-mechanical behavior of the RPV lower head under the given condition. The present study revisits the lower head failure with two- and three-dimensional finite element models. In particular, we aim to give clear insight regarding the effect of the three-dimensionality present in the distribution of the thickness and thermal load of the lower head. For a rigorous validation of the result, both the OLHF-1 and the OLHF-2 tests are considered in this study. The result suggests that the three-dimensional effect is not negligible as far as the failure location is concerned. The non-uniformity of the thickness distribution is found to affect the failure location and time. The thermal load, which may not be axisymmetric in general, has the most significant effect on the failure assessment. We also observe that the creep property can affect the global deformation of the lower head, depending on the applied mechanical load.
This study intended to testify the causality among three variables such as accident, depression and cognitive failure of the train drivers. For this purpose, two research models were suggested. Model 1 hypothesized the causality among three variables as 'depression ${\rightarrow}$ cognitive failure ${\rightarrow}$ accident'. On the other hand, model 2 hypothesized the causality among three variables as 'accident ${\rightarrow}$ depression ${\rightarrow}$ cognitive failure'. Results based on AMOS using 416 train drivers' questionnaire showed that model 2 is more valid than model 1. The statistical result of model 1 showed that depression has a positive effect on cognitive failure, however no significant relationship between depression and accident as well as between cognitive failure and accident. In model 2, the result showed that the accident has a positive effect on cognitive failure mediated by depression. This result suggests the necessity for establishment of countermeasures to mitigate mistake and cognitive failure caused by train drivers in a wider context, considering the causality between accident and depression.
This paper quantitatively presents the effects of important factors of the probabilistic safety assessment (PSA) of safety-critical digital systems. The result which is quantified using fault tree analysis methodology shows that these factors remarkably affect the system safety. In this paper we list the factors which should be represented by the model for PSA. Based on the PSA experience, we select three important factors which are expected to dominate the system unavailability. They are the avoidance of common cause failure, the coverage of fault tolerant mechanisms and software failure probability. We Quantitatively demonstrate the effect of these three factors. The broader usage of digital equipment in nuclear power plants gives rise to the safety problems. Even though conventional PSA methods are immature for applying to microprocessor-based digital systems, practical needs force us to apply it because the result of PSA plays an important role in proving the safety of a designed system. We expect the analysis result to provide valuable feedback to the designers of digital safety- critical systems.

Every year in domestic slope failure caused by rainfall is happening frequently. Specially, causable failure accident by localized downpour accompanied when summer rainy season period and produces typhoon gets damage of large scale human life and property. Failure happened at slope of 121 places ranged whole country national highway by No.15 typhoon Rusa that strike whole country during 3 days from August 30, 2002. Slope failure that happen by typhoon are judged for major cause to effect of ground saturation and surface water by localized downpour. In this research, failure characteristic was analyzed to target 20 places attaining site investigation among failure slope. As a result, erosions by surface water was construed for major cause of failure and judged for direct relation in failure slope weathering and topography Also, result that analyze inclination of failure part, in the case of ripping rock, inclination of failure side is forming Incline of the lowest 40$^{\circ}$, because surface failure of depth 4m on or so scale happened, it is require that regulating plan gently design standard inclination of weathered rock and soil layer And it is considered that desirable preparation of design standard about measure that help smooth drainage of surface water and can restrain percolation in ground to reduce failure damage by rainfall.
This study was to compare the major kinematic factors between the success and failure group on performing the back somersault motion in floor exercise. Three gymnasts(height : $167.3{\pm}2.88cm$, age : $22.0{\pm}1.0years$, body weight : $64.4{\pm}2.3kg$) were participated in this study. The kinematic data was recorded at 60Hz with four digital video camera. Two successful motions and failure motions for each subject were selected for three dimensional analysis. 1. Success Trail It was appear that success trail was larger than failure group in projection velocity, but success trail was smaller than failure trail in projection angle. Also it was appear that success trail was longer than failure group in the time required. Hand segment velocity and maximum velocity in success trail were larger than those in failure trail, and this result was increasing the projection velocity and finally increasing the vertical height of center of mass. At the take-off(event 2), flection amount of hip and knee joint angle was contributed to the optimal condition for the take-off and at the peak point, hip and knee joint angle was maximum flexed for reducing the moment of inertia. Also in this point, upper extremities of success trail extended more than those of failure trail. in this base, success trail in upward phase(p3) 2. Failure Trail It was appear that failure trail was smaller than success trail in projection velocity, but failure trail was larger than success trail in projection angle. Also it was appear that failure trail was more short than success trail in the time required. Hand segment velocity and maximum velocity in failure trail were smaller than those in success trail, and this result was reducing the projection velocity and finally reducing the vertical high of center of mass. At the take-off(event 2), flection amount of hip and knee joint angle wasn't contributed to the optimal condition for the take-off and at the peak point, hip and knee joint angle wasn't maximum flexed for reducing the moment of inertia. Web-based analysis programs for reliability assessment of machine tools were developed in this study. First, the reliability data analysis program was developed to search for failure rate using failure data and reliability test data of mechanical part. Second, failure mode analysis was developed through performance tests like circular movement test vibration test for machine tools. This analysis program shows correlation between failure mode and performance test result. Third, tool life was predicted by correlation between flank wear and cutting time, using the extended Taylor tool life equation in turning data and the equivalently converted equation in order to apply ball endmill data to Taylor tool life equation in milling data. All the information related to input and result data can be stored in theses programs.
