Review of Tumor Dormancy Therapy Using Traditional Oriental Herbal Medicine

  • Objective: Standard cancer therapy prolongs survival, but can be detrimental to the quality of life, compromise the immune system, and leave residual disease that can cause recurrence years or decades in the future. Tumor dormancy therapy is a novel therapeutic approach that may improve these shortcomings, promote quality of life, and prolong survival. The aim of this study was to analyze studies on dormancy therapy, especially studies using traditional Oriental herbal medicine, so as to evaluate the efficacy of dormancy therapy with traditional oriental herbal medicine. Methods: We conducted a systematic literature review using Scientific and Technical Information Integration Services (NDSL), PubMed, and RISS. We searched for clinical reports, papers, and books related to tumor metastasis, recurrence, immunotherapy, tumor dormancy, and traditional oriental herbal medicine with anticancer effects. Seventy-nine (79) experimental and clinical articles in both Korean and English were reviewed. This study was conducted from March 1, 2012 to May 31, 2012. Results: This approach, Tumor dormancy therapy, rather than seeking to remove the tumor, includes combination of low-dose chemotherapy, immunotherapy, immunosurveillance, and other methods to stabilize tumor growth and to enhance the host is immunity against disseminated tumor cells and thus to manage cancer as a chronic disease while maintaining quality of life. In particular, integrative use of Oriental herbal medicine has been shown to induce or maintain tumor dormancy, increase the effectiveness of conventional chemotherapy, improve quality of life, and prolong survival. Conclusion: Tumor dormancy therapy is a promising novel therapeutic approach that may be especially effective with Oriental herbal medicine. Further research is needed to determine its potential mechanisms and therapeutic applications.

Screening of Indigenous Strains of Lactic Acid Bacteria for Development of a Probiotic for Poultry

  • In an attempt to develop a probiotic formulation for poultry feed, a number of lactic acid bacteria (LAB) were isolated from chicken intestinal specimens and a series of in vitro experiments were performed to evaluate their efficacy as a potential probiotic candidate. A total of 650 LAB strains were isolated and screened for their antagonistic potential against each other. Among all the isolates only three isolates (TMU121, 094 and 457) demonstrated a wide spectrum of inhibition and were thus selected for detailed investigations. All three selected isolates were able to inhibit the growth of E. coli and Salmonella species, although to variable extent. The nature of the inhibitory substance produced by the isolates TMU121 and 094 appeared to be associated with bacteriocin, as their activity was completely lost after treatment with proteolytic enzymes, while pH neutralization and catalase enzyme had no effect on the residual activity. In contrast, isolate TMU457 was able to resist the effect of proteolytic enzymes while pH neutralization completely destroyed its activity. Attempts were made to study the acid, bile tolerance and cell surface hydrophobicity of these isolates. TMU121 showed high bile salt tolerance (0.3%) and high cell surface hydrophobicity compared to the other two strains studied, while TMU094 appeared the most pH resistant strain. Based on these results, the three selected LAB isolates were considered as potential ingredients for a chicken probiotic feed formulation and were identified to species level based on their carbohydrate fermentation pattern by using API 50CH test kits. The three strains were identified as Lactobacillus fermentum TMU121, Lactobacillus rhamnosus TMU094, and Pediococcus pentosaceous TMU457.

Role of Neoadjuvant Chemotherapy in the Management of Advanced Ovarian Cancer

  • Objective: To analyze efficacy of neoadjuvant chemotherapy for advanced ovarian cancer. Materials and Methods: A total of 107 patients with advanced ovarian cancer undergoing cytoreductive surgery were divided into a neoadjuvant chemotherapy group (n=61) and a primary debulking group (n=46) and retrospectively analyzed. Platinum-based adjuvant chemotherapy was applied to both groups after cytoreductive surgery ande overall and progression-free survival times were calculated. Results: No significant difference was observed in duration of hospitalization ($20.8{\pm}6.1$ vs. $20.2{\pm}5.4$ days, p>0.05). The operation time of neoadjuvant chemotherapy group was shorter than the initial surgery group ($3.1{\pm}0.7$ vs. $3.4{\pm}0.8$ h, p<0.05). There were no significant differences in median overall survival time between neoadjuvant chemotherapy group and surgery group (42 vs. 55 months, p>0.05). Similarly, there was no difference in median progression-free survival between neoadjuvant chemotherapy group and surgery group (16 vs. 17 months, p>0.05). The surgical residual tumor size demonstrated no significant difference between initial surgery and neoadjuvant chemotherapy groups (p>0.05). Multivariate analysis showed that more than 3 cycles of regimen with neoadjuvant chemotherapy was associated with more resistance to chemotherapy compared with patients without receiving neoadjuvant chemotherapy (OR: 5.962, 95%CI: 1.184-30.030, p<0.05). Conclusions:Neoadjuvant chemotherapy can shorten the operation time. However, it does not improve survival rates of advanced ovarian cancer patients.

Phosphorus Removal from Municipal Wastewater Using Ti-based Coagulants (티타늄계열응집제를 이용한 하수 내 인 제거)

  • This study evaluated the efficacy of Ti-based coagulants on phosphorus (P) removal from municipal wastewater and compared them with Al-based coagulants. Jar test experiments were performed at various chemical doses and OH/Ti molar ratio (B value). The higher the intial phosphate ($PO_4-P$) concentration, the lower the [Ti]/[P] to reach a residual concentration below 0.2 mg P/L. Removal efficiencies of total phosphorus increased with an increased coagulant dose but decreased after the efficiencies reached their maximum value regardless of coagulant or B value. On the other hand, $PO_4-P$ removal showed an increasing trend with an increased coagulant dose, reaching the plateau value under large coagulant dose conditions for both Ti- and Al-based coagulants regardless of B value. The chemical dose of Ti-based coagulants was approximately twice higher than that of Al-based coagulants with the same P-removal efficiency. The coagulation efficiency was influenced by different B values.

Comparison of the bonding strengths of second- and third-generation light-emitting diode light-curing units

  • Objective: With the introduction of third-generation light-emitting diodes (LEDs) in dental practice, it is necessary to compare their bracket-bonding effects, safety, and efficacy with those of the second-generation units. Methods: In this study, 80 extracted human premolars were randomly divided into eight groups of 10 samples each. Metal or polycrystalline ceramic brackets were bonded on the teeth using second- or third-generation LED light-curing units (LCUs), according to the manufacturers' instructions. The shear bond strengths were measured using the universal testing machine, and the adhesive remnant index (ARI) was scored by assessing the residual resin on the surfaces of debonded teeth using a scanning electron microscope. In addition, curing times were also measured. Results: The shear bond strengths in all experimental groups were higher than the acceptable clinical shear bond strengths, regardless of the curing unit used. In both LED LCU groups, all ceramic bracket groups showed significantly higher shear bond strengths than did the metal bracket groups except the plasma emulation group which showed no significant difference. When comparing units within the same bracket type, no differences in shear bond strength were observed between the second- and third-generation unit groups. Additionally, no significant differences were observed among the groups for the ARI. Conclusions: The bracket-bonding effects and ARIs of second- and third-generation LED LCUs showed few differences, and most were without statistical significance; however, the curing time was shorter for the second-generation unit.

The Effects of Electrical Stimulation Therapy with Microchip for the Treatment of Bladder Irritability Symptoms in Stress Urinary Incontinent Women (방광 자극증상을 호소하는 복압성 요실금 환자에서 마이크로칩을 이용한 전기자극치료의 효과)

  • Purpose: This study was carried out to evaluate the efficacy of intravaginal pelvic floor electrical stimulation (FES) therapy on bladder irritability symptoms in stress urinary incontinent women. Materials and Methods: The evaluation before and after treatment included the Blaivas's female Bladder Questionnaire, the quality of life questionnaires and the overall satisfaction with present voiding state using visual analogue test (VAT). All patient were treated for 20 minutes, twice a week for 6 weeks, using a new intravaginal electrical stimulator with microchip (PIC16C74). Results: After the FES therapy, the overall success rate of the SUI was 50.4.% at 9 months. The bladder irritability symptoms such as frequency, nocturia, urgency, residual urine sensation and lower abdominal discomfort were improved. In particular, the symptoms of frequency, urge incontinence, dysuria were significantly improved after the FES therapy at 9 months. The VAT score of the overall satisfaction with the present voiding state was significantly lower after the FES therapy. 73.7% of patients were satisfied with the FES therapy and complications such as hemorrhage, vaginitis, urinary tract infection and pain were not encountered. Conclusion: These results suggest that FES therapy with microchip improves some bladder irritability in SUI women. Therefore, electrical stimulation therapy should be considered as a first line therapy in SUI women with bladder irritability symptoms.

Efficacy of orally administered ginseng stem and leaf in chickens (인삼 지상부를 첨가한 사료 급여가 닭에서 나타내는 효과)

  • Ginseng has been widely used in Korea as a natural medicine due to its saponin contents. Although the total amount of ginseng stem and leaf saponins (GSLS) is 4~5 times higher than that of saponin in the root, the root is mainly used. This is due to two reasons: nervous system-stimulant activity of GSLS and pesticide residues in GSLS. In this study, residual agricultural pesticides were removed from GSLS using two types of bacterial treatments. Two GSLS treatment groups of chickens (GSLS-1 and GSLS-2) were established. The chickens were fed 0.4% GSLS-1 or GSLS-2 mixed with crop. We then evaluated the effects of GSLS on bodyweight and several immune parameters. At the end of the experiments, chickens fed GSLS-1 and red ginseng saponin had significantly higher growth rates (16.6% and 8.0%, respectively) compared to the vaccine control group treated with Noblis Salenvac-T. The group fed GSLS-1 also had the highest IgG titer that was significantly different at the end of experiments compared to the other groups. These findings imply that GSLS-1 is a good candidate feed additive for the chicken industry.

Stabilization of Heavy Metal-Contaminated Mine Tailings Using Phosphate Fertilizers and Red Mud (인산염 비료 및 레드머드를 이용한 중금속 오염 광미의 안정화)

  • The objectives of this study were to investigate the efficiencies of the stabilizers such as mono-potassium phosphate (MKP), phosphate fertilizer and red mud in treating the mine tailings contaminated with heavy metals and to characterize the changes in fractionations of the heavy metals during the stabilization. The TCLP results showed that the stabilization efficiencies of Cd, Pb and Zn increased with the increase in the stabilizer dosage and the reaction times. MKP showed the highest efficiencies for the heavy metals stabilization among the stabilizers tested. When the mine tailings were amended with MKP, the TCLP concentrations of Cd, Pb and Zn were reduced by 79~97%, 61~84%, and 89~99%, respectively. When the composite stabilizers, MKP/phosphate fertilizer or MKP/red mud, were used, the stabilization efficiencies were lower than when MKP was used as a single stabilizer. The sequential extraction results showed that carbonates fraction of Cd and Zn increased generally. Especially, when red mud was used, carbonates fraction of Cd and Zn increased 5 and 18 times, respectively. In the case of Pb, the treatment with MKP increased residual fraction by 10 times. The results showed that MKP was the most effective in stabilizing the heavy metals (Cd, Pb and Zn) to improve the efficacy of the composite binders.

A Study on Feasibility Analysis and Optimum Range Calculation Model by Conversion of Water Supply System (상수도 급수방식 전환의 타당성 분석 및 최적 범위 산정모델 연구)

  • This study concerned the analysis on the efficiency of the conversion of water tank type supply system to direct water supply system to examine the feasibility of the conversion, as well as the calculation of optimal conversion range that enables the supply of safe, high-quality water at stable pressure in accordance with the standards of water supply facility. The results of this research showed that when converting water supply system from water tank type supply system to direct water supply system, more nodal points could be properly converted and more reduction of electricity usage was expected in case water pressure rather than residence time was fixed. This means that higher efficacy can be obtained by fixing water pressure when converting water supply system. However, since the number of the locations that received on-spot inspection was small and the electricity usage measured was not exclusively by water supply facility, it is difficult to judge that such reduction of electricity usage accurately represents reduced electricity usage by water supply facility alone. therefore, after having secured on-spot information about a larger number of locations in apartment complexes that have converted water supply system, and utilizing information about electricity usage exclusively by water supply facility, the proposed method of this research could be applied to accurately deducing expected reduction of electricity usage by water supply facilities of various other apartment complexes. It is also considered possible to deduce an effective operation method of water supply system by finding out an area that shows low pressure or low residual chlorine concentration in the optimal conversion range of water supply, followed by estimating the proper location of pumping station or the proper chlorine dosage at the power purification plant that supply water to the target area.

Arthroscopic Treatment for Meniscal Cyst (관절경을 이용한 반월상연골 낭종의 치료)

  • Recent studies suggest that most meniscal cysts can be treated surgically by arthroscopic management of the meniscal tear and arthroscopic cyst evacuation. But arthroscopic cyst decompression may sacrifice a substantial amount of meniscal tissue that is not torn in order to expose the "stalk" of the cyst. Nowadays, the trend is changing as preserving the involved meniscus to prevent from inevitable degenerative changes after meniscectomy. The purpose of this report is to describe a new surgical technique that minimizes loss of meniscal tissue in hopes of maximizing residual meniscal function. We experienced 10 patients with meniscal cysts that were consisted of four lateral cysts and six medial cysts. Menisci were torn in all cases. Arthroscopic partial meniscectomy and decompression of cysts were performed in 9 cases, and arthroscopic partial meniscectomy and open cystectomy in 1 case. The procedures were consisted of injection of the methylene blue into the cyst, partial meniscectomy of the meniscal tear until the dye was seen in orifice of the cyst, and decompression of cyst through cystic opening. This article serves to confirm the relationship between torn menisci and cysts, to re-evaluate the occurrence ratio of the meniscal cyst on the medial to lateral meniscus, and to assess the efficacy of arthroscopic partial meniscectomy and decompression of cyst as a potentially meniscal sparing procedure.
