Estimation of nursing costs for hospitalized patients using the resource-based relative value scale (상대가치(Resource-Based Relative Value)를 이용한 간호행위별 간호원가 산정)

  • A cost analysis for hospitalized patients was performed based on the RBRVS in order to determine an appropriate nursing fee schedule. The study was conducted through three phases as follows: 1) Nursing activities provided for the inpatients currently in Korea were identified and classified using a taxonomy which was developed by our research team through the Delphi process. 2) The resource-based relative points for every nursing activity according to nursing time, mental effort and judgement, technical skill, physical effort and stress were determined through a survey of 300 clinical RNs working at 5 tertiary hospitals from May 25 to July 25. 1998. 3) The nursing cost of every nursing activity for hospitalized patients was estimated based on the RBRVS. As a result, 136 nursing activities were identified and classified by nursing processes and nursing domains. However, our classification system of nursing activities should continue to be refined, and all nursing practices should be standardized. The nursing activities were given resource-based relative points ranging from 100 to 400 points, then each nursing activity was assigned a value for the RBRVS, which was determined by the exponential function of 2resource-based relative point/100. Thus, a value of 2 was calculated for 100 points, 4 for 200 points, 8 for 300 points, and 16 for 400 points. Meanwhile, the unit cost of nursing was calculated as 170 Won. The nursing cost of 136 nursing activities was estimated using the RBRVS as shown in

    . A proper nursing fee schedule for a new reimbursement system based upon the results of the above study should be prepared in the near future.

  • Changes of Electroencephalogram according to Contraction Training Methods of Tibialis Anterior Muscle in Healthy Women (정상 여성에게서 앞 정강근의 수축훈련방법에 따른 뇌파의 변화)

    • Purpose: This study aims to examine the power changes in eletrocenphalogram (EEG) detected from the tibialis anterior muscle, during repetitive contraction exercise in normal female adults. Methods: The subjects of this study were 24 normal adult females, with no musculoskeletal or nervous system disorders. The 24 female subjects were divided into two groups: 12 subjects comprised a voluntary stimulation training group, and the other 12 subjects comprised an electrical stimulation training group. A total of thirty contractions were made repetitively by each woman, with maximal voluntary contraction exercise for six seconds, and a resting time of three seconds. During the experiment, their EEG was measured at eight positions. The eight positions were Fpz, Fz, Cz, CPz, C3, C4, P3, and P4, in accordance with the international 10~20 system. Results: The relative alpha power and beta power showed no statistically significant differences between the two groups. But the relative gamma power of the CPz, C3, C4, P3, and P4 areas showed statistically significant differences between the two groups (p<0.05). The relative theta power of the C4 area showed statistically significant differences between the two groups (p<0.05). Conclusion: Our findings show that tibialis anterior muscle contraction by electrical stimulation and by voluntary repeated contraction differentially affected brain activation. In particular, the CPz, C3, C4, P3 positions of relative gamma power showed brain activation in voluntary contraction. The C4 position of relative theta power showed different brain activation between the two groups.

    • Managers are required to adopt and implement the human resource management practice that fit firm's strategy the most, so that optimize overall performance. However, the time and relative resources that any firm has are limited, which demands managers to understand the relative importance of all sorts of HR practice and promote them in an order of their relative importance. This study follows the universal perspective and contingency perspective(according to firm size and strategy type), try to identify the most effective HR practice on performance as well as their relative importance by "CART Ensemble" analysis. The results are as follows. From universal perspective, firms always need to high level of integration between strategy and HR department, decision making participation, autonomy of speed of working, and autonomy of way of working. Contingency perspective also suggested the importance of integration between HRM and strategy. But others are different case by case. This study suggests useful implications for managers.

    • This study describes a method of finding an optimal path of a soccer robot by using a concentric circle method with different radii of rotation. Comparing with conventional algorithms which try to find the shortest path length, the variable concentric circle method find the shortest moving time. The radius fur the shortest moving time for a given ball location depends on the relative location between a shooting robot and a ball. Practically it is difficult to find an analytical solution due to many unknowns. Assuming a radius of rotation within a possible range, total path moving time can be calculated by adding the times needed for straight path and circular path. Among these times the shortest time is obtained. In this paper, a graphical solution is presented such that the game ground is divided into 3 regions with a minimum, medium, and maximum radius of rotation.

    Analysis on Living Factor and Actual State of Indoor Thermal Environment in Apartment Units during Winter (아파트의 겨울철 실내온열환경 실태와 생활요인 분석)

    • The purposes of this study were to determine the actual state of the indoor thermal environment in apartment units and to analyze the relationship between the living factors and indoor thermal elements. The field surveys consisted of measurements of physical elements and observations of living factors. In addition, the residents of 20 apartment units were interviewed to survey their subjective response. Field surveys were carried out from January to March 2007. Measuring elements were air temperature, globe temperature, and relative humidity. The results showed that the average of indoor temperature for the houses was $21.2{\sim}27.2^{\circ}C$, while 4 houses exceeded the comfort zone. The average of globe temperature for the houses was $21.3{\sim}27.5^{\circ}C$, while 6 houses exceeded the comfort zone. The mean relative humidity was $19.5{\sim}58.8%$, which is a relatively dry condition. The residents' average clothing value was $0.39{\sim}0.89$ clo(average 0.68 clo). The average thermal sensation vote on each room was $4.2{\sim}4.8$, which is 'neutral' to 'slightly warm'. Living factors had significant effect on indoor temperature in regression analysis were ventilation time(outdoor air exchange), opening time of door through balcony, and gas cooker use time.

    • In this study, advanced computational analysis system has been developed in order to investigate flow-induced vibration(FIV) phenomenon for general stator-rotor cascade configurations. Relative movement of the rotor with respect to stator is reflected by modeling Independent two computational domains. Fluid domains are modeled using the unstructured grid system with dynamic moving and local deforming methods. Unsteady, Reynolds-averaged Wavier-stokes equations with one equation Spalart-Allmaras and two-equation SST ${\kappa}-{\varepsilon}$ turbulence models are solved for unsteady flow problems and also relative moving and vibration effects of the rotor cascade are fully considered. A coupled implicit time marching scheme based on the Newmark integration method is used for computing the governing equations of fluid-structure interaction problems. Detailed vibration responses for different flow conditions are presented and then vibration characteristics are physically investigated in the time domain as computational virtual tests.

    • The purposes of this study were to make clear the present condition of indoor thermal environment in apartment units and to analyze the relation of the living factors with indoor thermal elements. The clothing value of residents were $0.39{\sim}0.89$ clo(average 0.68 clo). The average of thermal sensation ratings each room were $4.2{\sim}4.8$, 'neutral'$\sim$'slightly warm'. The results of regression analysis on relations of living factors with the thermal elements are as follow; ventilation time(outdoor air exchange), door opening time with balcony, and gas cooker use time had significant effect on indoor temperature.

    Factors Associated with Use and Types of Multiple Concurrent Care and Education Arrangements in Early Childhood in South Korea

    • This paper explores factors associated with multiple concurrent care and education arrangements in early childhood in South Korea. It draws on a subsample from the Korean Longitudinal Survey of Women and Families. Results show that about one-fifth of the families utilized multiple arrangements for their first preschool child. The primary non-parental option in multiplicity was nurseries or kindergartens. Home care and education options such as home study materials were found to be most prevalent secondary non-parental option, followed by services at private institution as well as relative care. Children's age, care cost, non-parental care time and time constraints were found to be positively related to the incidence of multiplicity for the first preschool child while family income and cost constraints were negatively associated. As a secondary option in the multiplicity, services at private institutions increased with children's age, care cost and when the grandparents live far from the parents' house. Home care and education utilization was found more among mothers with low degree of time constraints. Utilization of relative support decreased with the children's age and meant lower care cost and increased with mother's employment, fathers' education, family income, cost constraints and when grandparents live nearby. This paper, based on the associated factors, suggests how issues of children enrichment and constraints might be related to the multiplicity.

    Predicting the impact of global warming on carbonation of reinforced concrete structures in Zambia and Japan

    • The problem of carbonation-induced corrosion has become a concern in recent times, especially in the 21st century, due to the increase in global temperatures and carbon dioxide (CO2) concentration in the atmosphere possessing a significant threat to the durability of reinforced concrete (RC) structures worldwide, especially in inland tropical regions where carbonation is the most significant concrete degradation mechanism. Therefore, a study was conducted to predict the impact of global warming on the carbonation of RC structures in Lusaka, Zambia, and Tokyo, Japan. The Impact was estimated based on a carbonation meta-model that applies the analytic solution of Fick's 1st law using literature-based concrete mix design data and forecasted local temperature and CO2 concentration data over a 100-year period with relative humidity assumed constant. The results showed that CO2 diffusion increased between 17-31%, effecting a 40-45% rise in carbonation coefficient and a significant reduction in corrosion initiation time of 50-52% in the two cities. Moreover, for the same water-cement ratio, Lusaka showed almost twice higher carbonation coefficient values and one third shorter corrosion initiation time compared to Tokyo, mainly due to its higher temperature and low relative humidity. Additionally, the carbonation propagation depth at the end of 100 years was between 12-22 mm in Tokyo and 18-40 mm in Lusaka. These findings indicate that RC structures in these cities are at risk of rapid deterioration, especially in Lusaka, where they are more vulnerable.

    Effects of Drying Methods Based on Exhaust Cycle and Time on the Quality and Drying of Red Peppers

    • Purpose: The purpose of this study is to develop a system to optimize drying potential energy of the exhausted hot air by changing relative humidity of the air. This study modified the conventional drying method into a drying method changing exhaust cycle and time in order to control the relative humidity of the exhausted hot air during drying process. Method: A valve on the vent was controlled according to a preset time to change the exhaust cycle and time. This study analyzed the influence of the two different types of drying method on the drying characteristics, required energy, and quality of the dried peppers: conventional drying method exhausting hot air continuously and new drying method controlling exhaust cycle and time. Results: Drying characteristics based on exhaust time showed that drying time increased with exhaust time, and specific energy consumption was reduced by 28% from 18.39 MJ/kg (conventional method) to 13.24 MJ/kg when exhaust time was set to one minute. Drying characteristics based on heating time showed that drying time increased with heating time and specific energy consumption was reduced by 30% from 18.39 MJ/kg (conventional method) to 12.87 MJ/kg when exhaust time was set to 22 minutes. Drying characteristics based on exhaust cycle showed that drying time increased with exhaust cycle, and specific energy consumption was reduced by 31% from 18.39 MJ/kg (conventional method) to 12.69 MJ/kg when exhaust time was set to one minute and exhaust cycle was set to 22 minutes before drying and 40 minutes after drying. The quality of the dried red peppers showed that capsaicin, color, and sugar content were high as 34.87 mg/100g, 66.33, and 11.87%, respectively, when exhaust time was set to one minute and exhaust cycle was set to 22 minutes before drying and 40 minutes after drying. Conclusions: In order to utilize the drying potential energy of the exhausted air during drying process, the conventional drying method was modified into the drying method controlling exhaust cycle and time. The results showed that drying with exhaust cycle of one minute was more efficient in terms of drying time, required energy, and quality of the dried peppers than the one with exhaust cycle of 20~40 minutes.
