The clinical effect of Gagamjeonsibaekchul-san on child diarrhea (소아설사(小兒泄瀉)에 대한 가감전씨백출산(加減錢氏白朮散)의 임상효능(臨床效能)에 관한 연구(硏究))

  • Objectives: The objective of this study is to assess the clinical effect of Gagamjeonsibaekchul-san on child diarrhea. Methods: This clinical study was carried out with 42 cases(31male, 11female) of children aged 0 to 14 years old who visited the Department of Pediatrics, Dongguk University Bundang Oriental Hospital and were diagnosed as the diarrhea. Assessments were made with reference to sex, age, the duration of symptoms and physical history. To assess the impact of Gagamjeonsibaekchul-san, the complicated signs, the duration of treatment, the correlation between the use of Gagamjeonsibaekchul- san, san and the duration of symptom were examined before and after the treatment. Results: There were 31 males and 11 females in the incidence by sex. It showed that males were more than females. The most numerous age group was 0-2(19) followed by 3-7 (11) and 9-12 (12). According to the duration of symptom, 12 children have symptom for 0-7 days, 10 for 8-14 days, 11 for 15-20 days, 4 for 22-28 days and 5 over 28 days. With regard to the physical history of the sample, 5 children had atopic dermatitis, 3 children had enteritis, 3 children had rhinitis, 2 children had tonsillitis, 1 child had otitis media, 1 child had asthma and 1 child had pneumonia. The children also had the complicated signs: 22 children had anorexia, 17 children had abdominal pain, 13 children had vomiting and 8 children had nausea in digestive organ. In addition 11 children had dizziness, 7 children had general weakness, 1 child had enuresis and 1 child had sweat in general symptoms. For the duration of the treatment, 14 children were treated for 7 days, 13 for 8-10 days, 8 for 11-14 days, 3 for 15-21 days, 2 for 22-28 days and 2 for 29-40 days. No distinct relationship between the use of Gagamjeonsibaekchul-san and the duration of the symptom was found. Conclusion: Gagamjeonsibaekchul-san is effective in the treatment of intestine mucous membrane and improves the function of digestive system. It is also an effective short-term treatment for child diarrhea.

Obsessive-Compulsive Symptoms in Relation to Duration of Schizophrenia (조현병 환자의 유병기간에 따른 강박증상)

  • Objectives The aim of this study was to evaluate the psychopathological features such as depression and anxiety in schizophrenics with obsessive-compulsive symptoms (OCS) as well as the severity of OCS according to duration of schizophrenia. Methods We randomly selected sixty four inpatients with schizophrenia. We classified the patients into two groups (OCS group, non-OCS group) according to the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS). Clinical and demographic features were evaluated. To assess OCS, Y-BOCS were performed. The Korean version of the Positive and Negative Syndrome Scale(K-PANSS), the Brief Psychiatric Rating Scale (BPRS), the Korean version of the Calgary Depression Scale for Schizophrenia (K-CDSS), the Beck Anxiety Inventory (BAI) and the Satisfaction With Life Scale (SWLS) were conducted. Independent t-test and chi-square test were conducted to compare the two groups. Pearson correlation analysis was performed to examine the relationship between the duration of schizophrenia and the Y-BOCS score. Results The Y-BOCS, K-CDSS, and BAI scores were higher in the OCS group. There was a significant correlation between the duration of schizophrenia and the Y-BOCS score. Conclusions Anxiety and depression symptoms were severe in the OCS group. In addition, the results of this study indicate that the longer duration of schizophrenia, the more severe the OCS. Therefore, the evaluation of OCS in schizophrenics should be accompanied by treatment intervention.

Impact of coronavirus disease 2019 on patients with chronic pain: multicenter study in Korea

  • Background: The coronavirus disease 2019 (COVID-19) pandemic has caused significant changes. This study aimed to investigate the impact of COVID-19 on patients with chronic pain. Methods: Patients with chronic pain from 23 university hospitals in South Korea participated in this study. The anonymous survey questionnaire consisted of 25 questions regarding the following: demographic data, diagnosis, hospital visit frequency, exercise duration, time outside, sleep duration, weight change, nervousness and anxiety, depression, interest or pleasure, fatigue, daily life difficulties, and self-harm thoughts. Depression severity was evaluated using the Patient Health Questionnaire-9 (PHQ-9). Logistic regression analysis was used to investigate the relationship between increased pain and patient factors. Results: A total of 914 patients completed the survey, 35.9% of whom had decreased their number of visits to the hospital, mostly due to COVID-19. The pain level of 200 patients has worsened since the COVID-19 outbreak, which was more prominent in complex regional pain syndrome (CRPS). Noticeable post-COVID-19 changes such as exercise duration, time spent outside, sleep patterns, mood, and weight affected patients with chronic pain. Depression severity was more significant in patients with CRPS. The total PHQ-9 average score of patients with CRPS was 15.5, corresponding to major depressive orders. The patients' decreased exercise duration, decreased sleep duration, and increased depression were significantly associated with increased pain. Conclusions: COVID-19 has caused several changes in patients with chronic pain. During the pandemic, decreased exercise and sleep duration and increased depression were associated with patients' increasing pain.

Fever Duration and Renal Scar in Pediatric Urinary Tract Infection (소아 요로감염에서 발열과 신반흔의 관계)

  • Purpose: Urinary tract infections(UTIs) are not uncommon findings in febrile pediatric patients and approximately one third of patients with UTI may have renal scars. This research was intended to establish the relationship between duration of fever and renal scars. Methods: The medical records of 143 patients were reviewed retrospectively. Inclusion criteria were as follows: 1) fever as defined by an axillary temperature $\geq37.5^{\circ}C$, 2) accurate history of fever duration and the use of antibiotics 3) no previous history of UTI and 4) positive urine culture. We observed whether the longer fever duration could be associated with the development of initial renal defects and subsequent renal scars, increased C-reactive protein(CRP), leukocytosis and the presence of vesicoureteral reflux(VUR). Results: 1) Patients with longer fever duration after antibiotics showed more frequent initial renal defects(P=0.014). However, fever duration before antibiotic use was not associated with the development of initial renal defects(P=0.244). 2) Incidence of renal scar increased with fever duration before antibiotic use(P=0.006) and fever duration after antibiotic use(P=0.015). 3) CRP correlated with the fever duration after antibiotic use(r=0.287, P=0.003). 4) There was no relationships between fever duration and VUR(P>0.05). Conclusion: Our data suggest that fever duration before/after antibiotic use is significantly associated with the increased development of renal scars in pediatric UTI.

Association between Sleep duration and Grip strength in Korean adults Using Convergence Survey Data (융복합조사자료를 활용한 수면시간과 악력 간 관련성 연구)

  • The purpose of this study was to investigate the relationship between sleep duration and muscle strength in Korean adults aged 19 years and older. The cross-sectional analysis was conducted using the 2016 National Health and Nutrition Examination Survey data and Chi square test and multiple regression analysis were used. As a result of the analysis, the grip strength of those with more than weekday average sleep duration of 9 hours was found to be -1.267kg compared with those with weekday average sleep duration of 7 hours. The grip strength of those with more than weekend average sleep duration of 9 hours was found to be -0.879kg compared with those with weekend average sleep duration of 7 hours. In model simultaneously adjusting for both the average weekday and weekend average sleep duration, weekday average sleep duration of 9 hours was found to be -1.034kg compared with those with weekday average sleep duration of 7 hours. Therefore, careful observation will be required in light of the fact that both sleep duration and grip strength can predict future health conditions.

Interpersonal support, Tension in life changes & Life satisfaction in Urban Housewives (도시주부의 대인적 지지, 생활긴장감 및 만족도)

  • The purpose of this study is to investigate interpersonal support, tension in lifechanges & satisfaction. The selected sample is composed of 387 housewives in ChongJoo city. SAS pc program was used for the statistical analysis of the data. Data was analyzed by frequency, F-test, percentage, mean, Duncan's Multiple Range Test, Pearson's correlation coefficient, Regression Analysis. Major findings as follows: 1)At wedding & funeral ceremony, kin networks of her parents & parents-in law side were variables to have influence on tension in life changes. And the number of social organization participated were a variable to have influence on the satisfaction. The age of couple, education of couple, duration of marriage, income, family lifecycle, the number of children, pattern of family were variables to influence tension in life changes, but were not variables to influence on the satisfaction. 2) At usual or wedding & funeral ceremony, kin networks of her parents side were variables to influence on instrumental & companionship support. And the number of friends was a variable to influence on companionship & informational support. The number of neighbors was a variable to influence on instrumental, companionship & informational support. The number of social organization participated was a variable to influence on companionship & emotional support. The age of couple, education of couple,income, duration of marriage, family life cycle, number of children, family size, family type were variables to influence on interpersonal support. 3)The relationship between tension and satisfaction in life changes was negative, and between instrumental support and satisfaction was negative also. But between companionship support and satisfaction was positive relationship and between tension of personal &social life and instrumental support was positive relationship. The relationship between tension of marriage life and companionship support was negative and between tension of family life and information support was negative relationships. The received companionship support was lower tension in life changes than not received it. But the received instrumental support was higher tension of personal & social life. The received companionship & informational support was higher satisfaction than not received them. But the received instrumental support was lower satisfaction than not received it. 4) Instrumental & companionship support, at usual kin network of her parents in taw side, at wedding & funeral ceremony kin network of her parents side,were variables to influence on tension in life changes. Analysis of the Relationship between the Number of Forest Fires and Non-Rainfall Days during the 30-year in South Korea

  • This study examined the relationship between the number of forest fires and days with no rainfall based on the national forest fire statistics data of the Korea Forest Service and meteorological data from the Open MET Data Portal of the Korea Meteorological Administration (KMA; data.kma.go.kr) for the last 30 years (1991-2021). As for the trend in precipitation amount and non-rainfall days, the rainfall and the days with rainfall decreased in 2010 compared to those in 1990s. In terms of the number of forest fires that occurred in February-May accounted for 75% of the total number of forest fires, followed by 29% in April and 25% in March. In 2000s, the total number of forest fires was 5,226, indicating the highest forest fire activity. To analyze the relationship between regional distribution of non-rainfall periods (days) and number of forest fires, the non-rainfall period was categorized into five groups (0 days, 1-10 days, 11-20 days, 21-30 days, and 31 days or longer). During the spring fire danger season, the number of forest fires was the largest when the non-rainfall period was 11-20 days; during the autumn fire precaution period, the number of forest fires was the largest when the non-rainfall period was 1-10 days, 11-20 days, and 21-30 days, showing differences in the duration of forest fire occurrence by region. The 30-year trend indicated that large forest fires occurred only between February and May, and in terms of the relationship with the non-rainfall period groups, large fires occurred when the non-rainfall period was 1-10 days. This signifies that in spring season, the dry period continued throughout the country, indicating that even a short duration of consecutive non-rainfall days poses a high risk of large forest fires.

Orographic Precipitation Analysis with Regional Frequency Analysis and Multiple Linear Regression (지역빈도해석 및 다중회귀분석을 이용한 산악형 강수해석)

  • In this study, single and multiple linear regression model were used to derive the relationship between precipitation and altitude, latitude and longitude in Jejudo. The single linear regression analysis was focused on whether orographic effect was existed in Jejudo by annual average precipitation, and the multiple linear regression analysis on whether orographic effect was applied to each duration and return period of quantile from regional frequency analysis by index flood method. As results of the regression analysis, it shows the relationship between altitude and precipitation strongly form a linear relationship as the length of duration and return period increase. The multiple linear regression precipitation estimates(which used altitude, latitude, and longitude information) were found to be more reasonable than estimates obtained using altitude only or altitude-latitude and altitude-longitude. Especially, as results of spatial distribution analysis by kriging method using GIS, it also provides realistic estimates for precipitation that the precipitation was occurred the southeast region as real climate of Jejudo. However, the accuracy of regression model was decrease which derived a short duration of precipitation or estimated high region precipitation even had long duration. Consequently the other factor caused orographic effect would be needed to estimate precipitation to improve accuracy.
