Analysis of Estimation Technique for Solid Sediments in Combined Sewer Systems (합류식 관거 내 고형물 퇴적량 산정기법 분석)

  • The deposition of sewer solids during dry weather in combined sewer systems results in a loss of flow capacity that may restrict flow and cause a local flooding and enhanced solids deposition. Sewer solid accumulations in drainage systems also create the 'first-flush' phenomena during wet weather runoff periods. In order to solve these problems, measurement of these loadings for a given sewer system for extended period is needed but this task is very difficult and extremely expensive. In this study, generalized procedures for estimating sewer sediment solid during dry weather in combined sewer systems developed by the U. S. Environmental Protection Agency were applied in a drainage system in Korea. As result, the appropriate equation can be selected and applied according to the available data. However, the estimated solid sediment shows considerable difference between methods which classified by model and estimation methods of variable. The estimated values using equations (1) $\sim$ (4) are greater than that of equations (5) $\sim$ (9) and intermediate models show greater values than elaborate or simplest models. The comparison between simulated and measured solid deposition is difficult due to the absent of measurement data, but this estimation method can be used usefully for the management of sewer solid with reduction of cost and effort if the measurement is carried out and the equation is adjusted according to the actual drainage systems in Korea.

Risk Factors for Predicting Hypoxia in Adult Patients Undergoing Bronchoscopy under Sedation

  • Background: Flexible bronchoscopy is one of the essential procedures for the diagnosis and treatment of pulmonary diseases. The purpose of this study was to identify the risk factors associated with the occurrence of hypoxia in adults undergoing flexible bronchoscopy under sedation. Methods: We retrospectively analyzed 2,520 patients who underwent flexible bronchoscopy under sedation at our tertiary care university hospital in South Korea January 1, 2013-December 31, 2014. Hypoxia was defined as more than 5%-point reduction in the baseline percutaneous oxygen saturation (SpO2) or SpO2 <90% for >1 minute during the procedure. Results: The mean age was 64.7±13.5, and 565 patients developed hypoxia during the procedure. The mean sedation duration and midazolam dose for sedation were 31.1 minutes and 3.9 mg, respectively. The bivariate analysis showed that older age, a low forced expiratory volume in one second (FEV1), use of endobronchial ultrasound, the duration of sedation, and the midazolam dose were associated with the occurrence of hypoxia during the procedure, while the multivariate analysis found that age >60 (odds ratio [OR], 1.32), a low FEV1 (OR, 0.99), and a longer duration of sedation (>40 minutes; OR, 1.33) were significant risk factors. Conclusion: The findings suggest that patients older than age 60 and those with a low FEV1 tend to develop hypoxia during the bronchoscopy under sedation. Also, longer duration of sedation (>40 minutes) was a significant risk factor for hypoxia.

REAL-TIME MEASUREMENT OF DENTINAL TUBULAR FLUID FLOW DURING AND AFTER AMALGAM AND COMPOSITE RESTORATIONS (아말감과 복합레진의 수복 과정과 수복 후 발생하는 상아세관액 흐름의 실시간 측정)

  • The aim of this study was to measure the dentinal tubular fluid flow (DFF) during and after amalgam and composite restorations. A newly designed fluid flow measurement instrument was made. A third molar cut at 3 mm apical from the CEJ was connected to the flow measuring device under a hydrostatic pressure of 15 $cmH_2O$. Class I cavity was prepared and restored with either amalgam (Copalite varnish and Bestaloy) or composite (Z-250 with ScotchBond MultiPurpose: MP, Single Bond 2: SB, Clearfil SE Bond: CE and Easy Bond: EB as bonding systems). The DFF was measured from the intact tooth state through restoration procedures to 30 minutes after restoration, and re-measured at 3 and 7days after restoration. Inward fluid flow (IF) during cavity preparation was followed by outward flow (OF) after preparation, In amalgam restoration, the OF changed to IF during amalgam filling and slight OF followed after finishing. In composite restoration, application CE and EB showed a continuous OF and air-dry increased rapidly the OF until light-curing, whereas in MP and SB, rinse and dry caused IF and OF, respectively. Application of hydrophobic bonding resin in MP and CE caused a decrease in flow rate or even slight IF. Light-curing of adhesive and composite showed an abrupt IF. There was no statistically significant difference in the reduction of DFF among the materials at 30 min. 3 and 7 days after restoration (p > 0.05).

Immediate breast reconstruction following nipple-sparing mastectomy in an Asian population: Aesthetic outcomes and mitigating nipple-areolar complex necrosis

  • Background Nipple-sparing mastectomies (NSMs) are increasingly performed to obtain the best aesthetic and psychological outcomes in breast cancer treatment. However, merely preserving the nipple-areolar complex (NAC) does not guarantee a good outcome. Darkly pigmented NACs and a tendency for poor scarring outcomes are particular challenges when treating Asian patients. Herein, we review the reconstructive outcomes following NSM at Singapore General Hospital. Methods All breasts reconstructed following NSM over an 11-year period from 2005 to 2015 were reviewed. Information was collected from the patients' records on mastectomy indications, operative details, and complications. Patient satisfaction, breast sensation, and aesthetic outcomes were evaluated in 15 patients. Sensation was quantified using the Semmes-Weinstein monofilament test. Results A total of 142 NSMs were performed in 133 patients for breast cancer (n=122, 85.9%) or risk reduction (n=20, 14.1%). Of the procedures, 114 (80.2%) were autologous reconstructions, while 27 (19.0%) were reconstructions with implants. Complications occurred in 28 breasts (19.7%), with the most common complication being NAC necrosis, which occurred in 17 breasts (12.0%). Four breasts (2.8%) had total NAC necrosis. The overall mean patient satisfaction score was 3.0 (good). The sensation scores were significantly diminished in the skin envelope, areola, and nipple of breasts that had undergone NSM compared to non-operated breasts (P<0.05). Half of the subset of 15 patients in whom aesthetic outcomes were evaluated had reduced nipple projection. Conclusions Immediate reconstruction after NSM was performed with a low complication rate in this series, predominantly through autologous reconstruction. Patients should be informed of potential drawbacks, including NAC necrosis, reduced nipple projection, and diminished sensation.

Recovery of Gallium and Indium from Waste Light Emitting Diodes

  • Recovery of gallium and indium from waste light emitting diodes has been emphasized gradually owing to high content of gallium and indium. This study was established the recovery of gallium (Ga3+) and indium (In3+) from waste gallium nitride was contained in waste light-emitting diodes. The procedure was divided into the following steps; characteristic analysis, alkaline roasting, and leaching. In characteristic analysis part, the results were used as a theoretical basis for the acid leaching part, and the chemical composition of waste light emitting diodes is 70.32% Ga, 5.31% Si, 2.27% Al and 2.07% In. Secondly, with reduction of non-metallic components by alkaline roasting, gallium nitride was reacted into sodium gallium oxide, in this section, the optimal condition of alkaline roasting is that the furnace was soaked at 900℃ for 3 hours with mixing Na2CO3. Next, leaching of waste light emitting diodes was extremely important in the process of recovery of gallium and indium. The result of leaching efficiency was investigated on the optimal condition accounting for the acid agent, concentration of acid, the ratio of liquid and solid, and reaction time. The optimal condition of leaching procedures was carried out for 2.0M of HCl liquid-solid mass ratio of 30 ml/g in 32minutes at 25℃ and about 96.88% Ga and 96.61% In were leached.

  • Intake ratio from river constitutes about 31% (8/26) that beings to "water stress country" as "Medium ~ High" with China, India, Italy, South Africa, etc. Therefore, the present study on a dam that is the most effective and direct for securing water resources has been performed. First of all, climate change scenarios were investigated and analyzed. RCP 4.5 and 8.5 with 12.5 km grid resolution presented in the IPCC (Intergovernmental Panel on Climate Change) 5th Assessment Report (AR5) were applied to study watershed using SWAT (Soil and Water Assessment Tool) and HEC-ResSim models that carried out co-operation. Based on the results of dam simulation, the reduction effects of floods and droughts were quantitatively presented. The procedures of dam projects of the USA, Japan and Korea were investigated. As a result, there are no estimating quantitative criteria, calculating methods or formulas. In the present study, therefore, indexes for selecting suitable dam site through literature investigation and analyzing dam watersheds were determined, Expert questionnaire for various indexes were performed. Based on the above mentioned investigation and expert questionnaire, a methodology assigning weight using AHP method were proposed. The function of suitable dam (FSDS) site was calibrated and verified for four medium-sized watersheds. Finally, automated model for suitable dam site was developed using FSDS and 'Model builder' of GIS tool.

  • Dental sealant is a procedure to prevent dental caries on the occlusal surface of healthy teeth. Raw data from the 6th-7th Korean National Health and Nutritional Examination Survey (KNHANES) were analyzed. This study was conducted to provide basic data which is necessary for the national health care project and for expanding the standards of coverage for the Dental sealant procedure. In this study, subjects were a total of 4,366 children from the age of 6 to 18 and surveyed for the Amount of Received Dental sealant and rate of Received Dental sealant. In 2012, the coverage rate for pit and fissure sealing procedures differed significantly in terms of age, residential area and average monthly income. This coverage varied as per average monthly income in 2014 and as per age and average monthly income in 2017 (P < 0.05). In 2014, average monthly income had a significant influence on the coverage rate, while age and average monthly income had a significant influence on it in 2017 (P < 0.05). These findings suggest that in order to increase the sealing procedure coverage, continuous promotion of dental sealant projects, reduction of copayments, inclusion of varied tooth types and age groups, expansion of coverage to isolated areas and inclusion of socially disadvantaged groups are necessary.

Study on the Nonlinear Analysis Model for Seismic Performance Evaluation of School Buildings Retrofitted with Infilled Steel Frame with Brace (철골 끼움가새골조로 보강된 학교건물의 내진성능평가를 위한 비선형 해석 모델에 관한 연구)

  • Recently, damage to buildings due to earthquakes in Korea occurred mainly in school buildings and Piloti-type multi-family houses, highlighting the need for seismic retrofit for buildings of the same type. In the early days of the seismic retrofit project for school facilities, various patented methods using dampers as a ductile seismic retrofit method were applied without sufficient verification procedures. However, in 「School Facility Seismic Performance Evaluation and Retrofit Manual, 2021」, when the patented method is applied, it must be applied through a separate strict verification procedure, and instead, the strength/stiffness retrofit method was induced as a general method. In practice,when evaluating seismic performance for retrofit by infilled steel frame with brace, the analysis model is constructed by directly connecting only the steel brace to the existing RC member. However, if the frame is removed from the analysis model of the infilled steel frame with brace, the force reduction occurring on the existing RC member near the retrofit is considered to be very large, and this is judged to affect the review of whether to retrofit the foundation or not. Therefore, in this study, preliminary analysis with variables such as whether or not steel frame is taken into account and frame link method for the analysis model of RC school building retrofitted by infilled steel frame with brace and nonlinear analysis for actual 3-story school building was performed, and basic data for rational analysis model setting were presented by comparing preliminary analysis and pushover analysis results for each variable.

A Study on the Hearing Protection Effect of Noise-Filtering Earplugs for Dentists (치과의사를 위한 노이즈 필터링 이어플러그의 청력 보호 효과에 관한 연구)

  • The aim of this study is to evaluate the effect of noise-filtering earplugs on the hearing ability of dentists wearing them during noise-induced dental procedures. Pure tone audiometry and distortion product otoacoustic emission (DPOAE) tests were conducted at the initial visit and 1 year later to evaluate the hearing ability of the participants. As a result of the study, the pure tone average of the group wearing earplugs decreased significantly compared to the group not wearing earplugs, indicating an improvement in hearing. However, the signal-to-noise ratio of the DPOAE tests did not show a significant difference. These findings suggest that noise-filtering earplugs may have some effectiveness in preventing hearing loss. However, since the 1-year observation period may not be adequate to identify changes in hearing, a follow-up study with an extended follow-up period is necessary.

Effects of Single Vessel PCI (Percutaneous Coronary Intervention) using DCR (Dynamic Coronary Road map) on Fluoroscopy Time and Patient Radiation (동적 심혈관 로드맵을 이용한 중재적 시술이 투시 시간 및 환자 피폭에 미치는 영향)

  • Angiography equipment is used to evaluate and treat coronary artery disease. As a common feature of equipment, radiation is used, and function development for dose reduction is being carried out by each company. Therefore, the difference depending on whether DCR installed in angiography equipment is used is analyzed from a radiological point of view to prove the effect. Among 431 patients who underwent coronary artery intervention from March 2021 to February 2023, 250 patients with retrospective data were selected. And than among the 250 subjects obtained, 91 patients used the cardiovascular roadmap function during single-vessel intervention, and 159 patients did not use the roadmap. When DCR was used, total dose area product (34.57 uGy/m2 : 69.15 uGy/m2), total air kerma dose (688.47 mGy : 1640.4 mGy), fluoroscopy dose (23.87 uGy/m2 : 49.91 uGy/m2) and fluoroscopy time (723.55 s : 366.03 s), total number of images (17 : 26) showed lower values and were statistically significant than those not used. The use of DCR function in single vessel coronary intervention is thought to be radiologically safer as single vessel coronary intervention using dynamic cardiovascular DCR showed lower perspective time and perspective dose than procedures performed without the DCR.
